1.Effects of Methimazole Combined with Acupoint Application on Hormone Levels and Bone Metabolism In-dexes of Patients with Hyperthyroidism

OBJECTIVE:To investigate the effects of methimazole combined with acupoint application on hormone levels and bone metabolism indexes of patients with hyperthyroidism. METHODS:150 hyperthyroidism patients were randomly divided into group A(50 cases),group B(50 cases)and group C(50 cases). Based on routine treatment,group A was given Methimazole tab-lets orally,with initial dose of 20-40 mg,1-2 times a day;2-6 weeks later,reducing and maintaining at 2.5-10 mg if the disease condition was improved;maintaining 20-40 mg if no improvement. Group B was acupoint application,one piece,pasting on Zu-sanli,Sanyinjiao,Shenmen,Taichong,Neiguan points 1 h after breakfast. Group C received Methimazole tablets(same usage and dosage as group A)+acupoint application(same usage and dosage as group B). Treatment courses of 3 groups lasted for 2 months. The levels of TSH,FT3,FT4,T3,T4,T,E2,LH,FSH,CT,BGP and PTH were observed in 3 groups before and after treat-ment,and the occurrence of ADR was also observed. RESULTS:Before treatment,there was no statistical significance in the lev-els of above indexes among 3 groups(P>0.05). After treatment,the levels of TSH and PTH in 3 groups were significantly higher than before,and the group C was significantly higher than the group A and B. The levels of FT3,FT4,T3,T4,T,E2,LH, FSH,CT and BGP in 2 groups were significantly lower than before,and the group C was significantly lower than the group A and B;there was statistical significance (P<0.05). There was no statistical significance in each index between group A and B (P>0.05). There was no statistical significance in the incidence of ADR among 3 groups (P>0.05). CONCLUSIONS:Based on rou-tine treatment,methimazole combined with acupoint application can significantly improve the thyroid function,correct the disorder of sex hormones and abnormal bone metabolism in hyperthyroidism patient,without increasing the incidence of ADR.
2.Inhibitory effect of selective cyclooxygenase-2 inhibitor NS-398 on proliferation and invasion of human colorectal cancer cells

Objective:To investigate the effect of NS-398,a selective cyclooxygenase-2(COX-2) inhibitor,on the proliferation and invasion of human colon carcinoma cells in vitro,so as to determine the possibility of COX-2 as a new target for treatment of colon carcinoma.Methods: The expression of COX-2 in colorectal cancer cells(CW-2,COLO-320) was detected by RT-PCR and Western blotting.COLO-320 cell proliferation was measured by MTT after treatment with NS-398.Cell invasion ability was measured using migration and invasion chamber systems.Western blotting assay was used to examine the influence of NS-398 on MMP-2 expression.Results: Our results showed that CW-2,COLO-320 cells expressed COX-2 mRNA and protein.NS-398 inhibited the proliferation of COLO-320 cells in a time-and concentration-dependent manner.Invasion test showed that NS-398 inhibited the migration and invasion of COLO-320 cells.Western blotting revealed that NS-398 inhibited the expression of MMP-2 in COLO-320 cells.Conclusion: The selective COX-2 inhibitor NS-398 can inhibit COLO-320 cell proliferation and invasion,indicating COX-2 may serve as a new target for colon carcinoma treatment.
3.Effect of obstructive jaundice on recovery from sevoflurane anesthesia in pediatric patients

Objective To evaluate the effect of obstructive jaundice on recovery from sevoflurane anesthesia in pediatric patients. Methods A total of 80 pediatric patients scheduled for elective surgery were included, 42 pediatric patients with biliary atresia scheduled for Kasai operation served as obstructive jaundice group ( group OJ ) , and 38 pediatric patients scheduled for other operations served as control group ( group C) . Pediatric patients were 1-4 months old and full?term infants, and weighed 3.2-8.0 kg. Anesthesia was maintained with inhalation of 2%-4% sevoflurane during surgery, and pediatric patients inhaled 4% sevoflurane staring from peritoneum closure until the end of surgery. The duration from closing sevoflurane vaporizer to BIS value reaching 60, 70, 80 and 90 was recorded. The duration from stop of sevoflurane inhalation to BIS value returning to 60, 70, 80 and 90 was recorded. The duration from termination of sevoflurane inhalation to the time for tidal volume returning to 6 ml∕kg, to the time for muscle strength recovering to grade Ⅲ, to spontaneous eye opening and to tracheal extubation, and the corresponding BIS values were recorded. BIS value while entering the operating room, BIS value at the end of surgery, and the highest BIS value during recovery from anesthesia were recorded. The occurrence of delayed emergence from anesthesia was recorded. Results Compared with group C, the duration from termination of sevoflurane inhalation to spontaneous eye opening and to tracheal extubation were significantly prolonged, and BIS value at the end of surgery was decreased, and no significant change was found in the other parameters in group OJ. No pediatric patients developed delayed emergence from anesthesia in the two groups. Conclusion When only sevoflurane is used for inhalation anesthesia, although the time for recovery from anesthesia is prolonged, it shows no significant difference clinically in pediatric patients with obstructive jaundice.
4.Study on HLA alleles and haplotypes of 572 patients with acute lymphoblastic leukemia in southern Chinese Han

Objective To study the distributive characteristics of HLA-A,B,DRBI alleles and haplotypes patients with ALL in southern Chinese Han.Methods The frequencies of HLA-A,B,DRB1alleles and haplotypes were estimated by Expectation-Maximization method based on the genotypes of 572patients with ALL and 5645 unrelated health donors,and then compared by chi-square test.Results The frequencies of HLA-A33(7.15%vs 9.3%,OR=0.73,P<0.05),B58(5.93%vs 8.75%,OR=0.64,P<0.05),DRB1*17(5.15%vs 6.30%,OR=0.82,P<0.05)alleles and HLA-A33-B58-DRB1*17(2.46%vs 4.14%,OB=0.35,P<0.05)haplotype were significantly lower in ALL patient groups than that in controls.The frequencies of HLA-A3(2.1%vs 1.26%,OR=1.7,P<0.05),B51(7.25%vs 5.78%,OR=1.3,P<0.05)and DRB*12 (16.13%vs 12.99%,OR=1.35,P<0.05)alleles and A2-B51-DRB1*12(1.24%vs.0.89%,OR=1.66,P<0.05)haplotype were significantly higher in ALL patient groups than that in controls.Conclusion These results indieated that HLA-A33-B58-DRB1*17 haplotype was a associated with a diminished incidence of ALL.and HLA-A3 auele or A2-B51-DRB1*12 haplotype was weakly associated with ALL.
5.Research on the quality standard of Baoyun pill

Objective To improve the quality standard of Baoyun pill. Methods The natures of Psoralea corylifolia, Radix Dipsaci and Loranthus parasiticus were detected by thin layer chromatography (TLC); the content of Hesperidine were measured by high performance liquid chromatography (HPLC). The operating conditions by HPLC were InertSustain C18 (4.6 mm×250 mm, 5 μm ), the mixture of methanol-acetic acid-water (35:4:61) as mobile phase, 1.0 ml/min as flow speed and wavelength 283 nm. Results The results showed that the TLC spots of Psoralea corylifolia, Radix Dipsaci and Loranthus parasiticus were fairly clear, and the according blank test showed no interference. Hesperidine showed a good linear relationship within the range of 0.820-4.920 μg(r=1.000 0)with an average recovery of 99.85%(RSD=0.21%).Conclusions The TLC and HPLC are handy,safe and accurate for Baoyun pills to determine the quality.
6.Effect of lung-protective ventilation strategy on postoperative pulmonary complications in pediatric patients undergoing scoliosis surgery: a retrospective propensity score-matched cohort study

The data of pediatric patients who underwent scoliosis surgery from July 2016 to July 2019 were collected retrospectively.The pediatric patients were divided into traditional ventilation group (T group) and lung-protective ventilation group (P group) based on the ventilator settings.Standardized tidal volume(V T)was obtained by V T/ideal body weight (IBW). Patients with standardized V T ≥ 8 ml/kg and without positive end-expiratory pressure (PEEP) were included in group T, and patients with V T <8 ml/kg and PEEP 4-8 cmH 2O were included in group P. The propensity score was used to match the baseline information and intraoperative variables in the two groups.A total of 415 pediatric patients accepted screening, and 171 cases were successfully matched, including 92 cases in group T and 79 cases in group P. Compared with group T, the incidence and grade of pulmonary complications were significantly decreased at day 30 after operation, postanesthesia care unit stay time was shortened, body temperature was decreased at 24 h after operation, V T, standardized V T and oxygenation index were decreased, P ETCO 2, PEEP, ventilation frequency and I∶ E were increased ( P<0.05), and no significant change was found in IBW, PaCO 2, incidence of postoperative surgical complications and anesthesia-related complications, plasma C-reactive protein concentration and white blood cell count at 24 h after operation, hospitalization time and hospitalization cost in group P ( P>0.05). In conclusion, the lung-protective ventilation strategy low V T combined with PEEP can decrease the risk of postoperation lung complications and is helpful in improving prognosis in the pediatric patients undergoing scoliosis surgery as compared with traditional mechanical ventilation strategy.
7.Anaplastic lymphoma kinase-positive adenocarcinoma of lung:a cytopathologic analysis

Objective To study the cytomorphologic features of anaplastic lymphoma kinase ( ALK )-rearranged pulmonary adenocarcinoma.Methods The morphologic features in 153 pulmonary adenocarcinoma cytology specimens encountered during the period from September, 2011 to April, 2015 in Shanghai Cancer Hospital were retrospectively reviewed.Fluorescence in-situ hybridization ( FISH) and/or immunohistochemistry ( Ventana D5F3) for ALK gene rearrangement were carried out.The samples studied included 34 pleural effusion specimens, 40 endobronchial ultrasound-guided transbronchial needle aspirates ( EBUS-TBNA) and 79 fine needle aspirates of palpable masses on body surface.Results Thirty-nine cases (25.5%) of ALK-rearranged samples were identified by FISH and/or immunohistochemistry, including 3 cases diagnosed by FISH and 36 cases by both technologies.The median age of the ALK-positive group was 50 years, significantly younger than that of the ALK-negative group (60 years old,P=0.002) .Only 4 of the ALK-positive patients were smokers, which was significantly less than that of the ALK-negative group (P<0.01).In ALK-positive group, 3 cases showed cribriform pattern with prominent nucleoli, 3 cases showed cribriform pattern with mucin-rich cells and 8 cases showed extracellular mucus with mucin-rich cells.The above cytomorphologic patterns were significantly less common in ALK-negative tumors ( P <0.01 ) . Conclusions ALK-rearranged lung adenocarcinoma is associated with certain distinctive morphologic patterns, including cribriform architecture, presence of prominent nucleoli, mucin-rich cells and extracellular mucus, which can be observed in cytology specimens ( including conventional smears and cell block sections) .These findings, when combined with clinical features, may give clues to detection of ALK-positive cases.
