Objective To investigate the therapeutic effects of the vacularized iliac graft for ischemic necrosis of the femoral head in Niger young patients with sickle cell disease. Methods From November 1998 to Apirl 2001, 12 patients(5 males and 7 females, aging 11-22 years) with sickle cell disease suffered from ischemic necrosis of the femoral head in 14 hips. The lesion was on one hip in 10 patients and on bilateral hip in 2 patients. Necrosis was classified as Ficat Stage Ⅲ-Ⅳ in all patients. Twelve hips in 12 patients were treated with vascularized iliac graft in Maradi Province Hospital of Niger. The effects were evaluated on the basis of the Harris hip score and radiological examination. Results The patients were followed up 24-30 months after operation(27.4 months on average). The clinical results were satisfactory though no radiological evidence of improvement was noted. The mean Harris hip score increased from 75 points to 90 points. Pain relief and hip function improvement were achieved in all patients. No patients needed revision during short-term follow-up of 24-30 months. Conclusion The method is recommendation for treatment of ischemic necrosis of the femoral head in the Ficat Stages Ⅲ- Ⅳ in young patients with sickle cell disease for pain relief and restoration of hip joint function. This procedure may prolong the timing and acting of total hip arthroplasty.

Objective To study the changes in gene expression profiles of dendritic cells(DCs) during their maturation processes using cDNA microarray.Method DCs were generated from human peripheral blood monocytes induced by GM-CSF and IL-4,and lipopolysaccharide(LPS) was used to induce the maturation of DCs.After inducing for 48h,cells were collected to extract the general RNA.Genes expressions of mature DCs(maDC) and immature DCs(imDC) were analyzed using cDNA microarray, and immature DCs without induction served as control. Results DCs expressed highly surface markers of mature DCs after LPS induction. The studied data showed that among those 165 target genes,a total of 50 genes(30.3%) exhibited differential levels of expression in LPS-induced mature DCs.Thirty-five genes were up-regulated and fifteen genes were down-regulated.Maturation-dependent up-regulation,defined by a differential expression ratio of ≥2,included twelve cytokine and chemokine genes, ten antigen uptake and presentation genes and thirteen signal transduction molecule genes.Reciprocally,maturation-dependent down-regulation,defined by a differential expression ratio of ≤0.5,occurred with two cytokine, five antigen uptake and presentation genes and eight signal transduction molecule genes.Semi-quantitative reverse transcription polymerase chain reaction(RT-PCR) was employed to further confirm the results of cDNA microarray. Conclusion It is suggested that LPS has significant influence on many genes of DCs in their maturing processes. Those changed genes include cytokines, chemokines and their receptors, antigen uptake and presentation molecules and signal transduction molecules.Analysis on these differentially expressed genes is helpful in understanding the maturing processes of DCs.

Objective:To develop a new model of evaluating histocompatibility between donor and recipient.Methods:①We harvested 15 couples of blood samples of donor and recipient in human BMT and detected histocompatibility between donor and recipient using the aboved model. The time when GVHR began was recorded.②Skin grafts of KunMing hybride mice were respectively placed on inbred mice,BALB/c and C57BL/6.And then histocompatibility between donor and recipient was detected using the model.Survival time of skin allografts was recorded. Results:The smaller differences of histocompatibility evaluated by means of the model were, the later GVHR in human BMT would happen and the longer survival time of skin allografts in mice would become. Conclusion:The model could be used to detect correctly histocompatibility between donor and recipient.

Objective To analyse protein tyrosine phosphatase 1B (PTP1B) gene mutation in myeloproliferative neoplasms (MPN).Methods DNA sequencing technology was used to detect DNA sequences of PTP1B in MPN patients (n =84) and normal controls (n =37).Results For Exon1-6,Exon9 and Exon10,84 cases of MPN patients and 37 cases of control group were not detected mutation. For EXON 8,18 of 84 MPN patients had Exon8 C/T heterozygous mutation and 10 of 37 normal controls were detected C/T heterozygous mutation. There was no significant difference between MPN patients and normal controls (x2 =0.453,P =0.501).Exon7 was detected in 38 MPN patients and 2 cases of patients were found C/T heterozygous mutation, while in the control group,1 case with G/C heterozygous mutation. All of the cases were not detected homozygous mutation. Conclusion Using DNA sequencing technology to detect gene mutations of PTP1B,there is no significant difference between MPN patients and normal controls.

Objective To evaluate the efficacy and safety of dezocine versus sufentanil for postoperative patient-controlled epidural analgesia (PCEA).Methods PubMed,EMBASE,Cochrane Library,ISI Web of knowledge,Chinese Biomedical Database,Chinese Science-Technology Journal Database,China Journal Full-text Database and Wanfang Database were searched for randomized controlled trials involving the efficacy and safety of dezocine and sufentanil for PCEA from the date of database establishment up to April 2014.Randomized controlled trials met the inclusion criteria were included, and the data were extracted. The quality of the trials was evaluated according to Cochrane Handbook 5.1.0 criteria.Meta-analysis was conducted using RevMan 5.1 software. Results Seven studies involving 760 patients were included in this meta-analysis.The results of meta-analyses showed that there was no significant difference between dezocine group and sufentanil group in VAS scores at 4,8,12,16,24 and 48 h after surgery and in Ramsay sedation scores at 4,12,24 and 48 h after surgery, and the incidence of adverse reactions (postoperative nausea and vomiting, pruritus, urinary retention and somnolence) was significantly lower in dezocine group than in sufentanil group, and there was no significant difference in the incidence of respiratory depression and dizziness between dezocine group and sufentanil group. Conclusion Dezocine provides better efficacy and safety for postoperative PCEA than sufentanil.

To investigate the role of Egr-1 in the carcinogcnestic process of hepatocellular carcinoma (H-CC). Methods Expression of Egr-1 gene in HCC tissues were detected by in situ hybridization and immunohistochemistry. Human breast andmouse liver and brain tissues were used for control. ResultsLittle or no Egr-1 transcription was detected both in HCC tissues and in their normal counterparts. High transcription of Egr-1 was detected in the LCD and atrophic-like liver plate of HCC tissues. Protein expression of Egr-1 gene was consistent with mRNA transcription. High expression of Egr-1 protein was also detected in normal breast and mouse brain tissues. ConclusionsLittle or no expression of Egr-1 may play a role in the deregulation of normal growth in the carcinogenestic process of HCC. The differences of Egr-1 expression among liver cells, breast epithelia and mouse brain tissues might be associated with their different ways of proliferation and differentiation in different cell types.

Objective:To measure the salary satisfaction level of medical staff in county-level public hospitals and provide reference for the design of policy reforms. Method:Eight county-level public hospitals were investigated. All analyses were performed using SPSS 19. 0 via statistical description and binary logistic regression. Results: The total salary satisfaction of the medical staff was 48 . 8%, and the average score was 54 . 05 ± 18 . 04 . The welfare satis-faction is the lowest (38. 8%), and the average score is 7. 98 ± 3. 51. Logistic regression analysis shows that the ac-ademic degree, department category, working hours per week, acceptance of bribes, and officially budgeted posts are the main factors affecting overall compensation satisfaction. Conclusion:The overall salary satisfaction level of medi-cal staff in county-level public hospitals is very low, and the welfare satisfaction is the lowest. Establishing the rea-sonable level of compensation, the rise of compensation, and the welfare mechanism are feasible choices to improve salary satisfaction.
