The paper is to report the establishment of a method of characteristic figure analysis for the quality control of Panacis Quinquefolii Radix. Application of HPLC-UV-ELSD techniques was connected in series and applied. The separation was carried out on the Agilent Extend-C18 (250 mm x 4.6 mm, 5 microm) column. The mobile phase consisted of water and acetonitrile with gradient elution. The flow rate was 1.0 mL x min(-1) and the wavelength of measurement was 203 nm. The temperature of drift tube was maintained at 106.5 degrees C and the flow rate of air was set at 2.9 L x min(-1). Twenty batches of the Panacis Quinquefolii Radix were determined. Hierarchical cluster analysis (HCA) and principal component analysis (PCA) were applied to study on the HPLC characteristic figure and chemical pattern recognition. The HPLC-UV and HPLC-ELSD characteristic figure of Panacis Quinquefolii Radix was developed, the ginsenosides Rg1, Re, Rb1, Rc, Rb2, Rb3, Rd and the pseudoginsenoside F11 were identified. This method is accurate and reliable, and it can be used to control the quality of the Panacis Quinquefolii Radix.

BACKGROUND:Research on hand braces is relatively backward, which is difficult to be promoted widely. Zone II extensor tendon has a complex structure, and poor recovery is commonly seen after injury. The main treatment for zone II extensor tendon injury is the fixation with plaster slab, plywood and polymer materials, and up to now, there is no ideal brace for zone II extensor tendon injury.
OBJECTIVE:To evaluate the effect of home-made multi-function hand brace on the repair of zone II extensor tendon injury.
METHODS:Seventy-six patients with zone II extensor tendon injury were randomly divided into test group and control group, with 38 in each group. In the test group, patients were given appropriate rehabilitation education and multi-function hand brace based apart from conventional hand surgery on zone II extensor tendon; in the control group, patients underwent conventional hand surgery and guiding functional training. Total active motion of the extensor tendons of fingers were evaluated at 1, 2, 3 months after treatment; and the manual dexterity and fine motor of the hand were assessed at 2 and 3 months after treatment.
RESULTS AND CONCLUSION:The difference in the total active motion of extensor tendons of fingers was of great significance between the two groups at 1, 2, 3 months after treatment (P < 0.05). The Minnesota manual dexterity and fine motor of the hand also showed significant differences between the two groups at 2 and 3 months after treatment (P < 0.05). Patient’s cost-effectiveness analysis results showed that the total cost of the two groups had no statistical significance (P> 0.05), but less cost was used for one point increase in the above-mentioned three functional scores of the test group than the control group (P < 0.05), indicating the implementation of cost-effective treatment. This home-made brace can prevent adhesions and anchylosis during the postoperative auxiliary exercise; meanwhile, it can improve the range of motion, flexibility and fine motor ability of the hand and has a higher cost performance in the postoperative adjuvant exercise therapy of zone II extensor tendon injury.

Background: The quality of life has been greatly influenced and the cost of medical expenses is very high in patients with irritable bowel syndrome (IBS). The etiology and pathogenesis of IBS are still unclear, and the prevention and treatment of this disease still lack of effective methods. Objective: To explore and analyze the effects of Tiaohe Ganpi Hexin Decoction (TGHD), a compound traditional Chinese herbal medicine for regulating the liver and spleen, on IBS patients with diarrhea. Design, setting, participants and interventions: All 40 IBS patients came from the First Hospital of Jinan University, Guangzhou Red Cross Hospital, and the First Affiliated Hospital of Guangzhou University of Chinese Medicine, and were randomly divided into two groups. Patients in the treatment group (n=20) were given TGHD, while those in the control group (n=20) were prescribed oral pinaverium with a four-week treatment period. Main outcome measures: Traditional Chinese medicine (TCM) syndrome score, total obviously effective rate, disappearance rate of symptoms, and clinical symptom score in the two groups were evaluated before and after four-week treatment. Results: After the treatment, TCM syndrome scores in both groups were decreased (P<0.01), and the TCM syndrome score in the treatment group was significantly lower than that in the control group (P<0.01). There was a significant difference in the total obviously effective rate between the two groups (P<0.01), and the total obviously effective rates in the treatment and control groups were 85%(17/20) and 45%(9/20) respectively. The disappearance rates of abdominal pain, abdominal distention, poor stool output, stool frequency, stool character and mucous stool in the treatment group were higher than those in the control group (P<0.05). The symptom scores of abdominal pain, abdominal distention, poor stool output, stool frequency, stool character and mucous stool in the treatment group were lower than those in the control group (P<0.05). Conclusion: TGHD can significantly improve the clinical symptoms in IBS patients with diarrhea.

BACKGROUND:Alcohol has become pathogenic factors of avascular necrosis, and the alcohol induced
abnormal lipid metabolism in bone marrow may be the important reason for the onset of avascular necrosis, but the mechanism is not clear yet.
OBJECTIVE:To observe the changes of structure and function of fat cel s under the action of alcohol, in order to analyze the pathogenesis of alcoholic femoral head necrosis.
METHODS:Primary adipocytes in vitro culture technique was used to obtain rabbit femoral head intramedul ary adipose tissue, and then the fat cel s were separated, and the phenotype was identified with oil red O staining. The passaged stable intramedul ary fat cel s were col ected. Coverslip was cut into 1 cm × 1 cm in size, and placed in the 24-wel culture plate before planting. The cel s were randomly divided into alcohol group and control group, 24 holes (each hole for a sample) in each group. The control group was without alcohol, while the alcohol group was added with 0.15 mol/L alcohol. At 4, 6, 8 and 10 days, the culture medium was replaced. Medium was changed and no longer adding alcohol, and then cultured for 10 days. When the culture terminated, the coverslip was removed for oil red O staining. Final y, the morphology and the number of the fat cel s were observed under light
microscope.
RESUTLS AND CONCLUSION:With time prolonging, the number of fat cel s in the alcohol group was significantly more than that in the control group (P<0.001). The lipid droplets in the two groups were gradual y increased and enlarged, but more significant in the alcohol group. The number of intramedul ary fat cel s in the alcohol group after cultured for 4, 6, 8 and 10 days was respectively (200.90±24.60), (1 102.30±76.73), (1 160.30±28.37) and (1 199.70±44.74)/cm2;the
number of intramedul ary fat cel s in the control group was respectively (99.80±10.82), (0.40±94.71), (1 000.20± 41.85) and (1 059.80±26.79)/cm2, the number of fat cel s increased with the time of alcohol influence. Alcohol can promote the intramedul ary fat cel s to increase and enlarge, and this may be the main reason for femoral head necrosis, as long-term alcoholism can lead to bone marrow fat tissue increasing, intraosseous pressure increasing and perfusion reducing, thus resulting ischemia.

ObjectiveTo investigate and analyze the clinical and pathological features of surgical treatment for primary bronchogenic carcinoma in adolescent patients.MethodsA retrospective review is presented of patients less than 30 years with surgical treatment of bronchogenic carcinoma between 1969 and 2008.There were59 patients (36 male and 23 female).Mean age was 23 years ( range 8-29 ) .The ratio of men to women patients was 1.7∶1.Forty-nine cases ( 83.0% ) were symptomatic at presentation and 18 cases(30.5% )were misdiagnosed as other diseases.Surgical procedures included radical resection in 46 cases,palliative resection in 3 cases,thoracotomy only for unresectable disease in 7 cases and VATS biopsy in 3 cases.The histological types were 18 adenocarcinomas,13 carcinoids,9 mucoepidermoid carcinoma,5 squamous cell carcimomas,4 small cell lung cancer,3 adenosquamous carcinoma and 4 others.On TNM staging,8 cases in stage Ⅰa,3 cases in stage Ⅰb,9 cases in stage Ⅱ a,12 cases in stage Ⅱb,15 cases in stage Ⅲa,8 cases in stage Ⅲb,4 cases in stageⅣ.ResultsThere were no operative death in radical group.Post-operative atelectasis in 3 cases.One case died from postoperative respiratory failure in explosive group,the postoperative five year survival rate was 27.0%.radical resection group 5-year survival was 35%.Univariate analysis identified TNM stage and surgical procedures as predictors of survival( P <0.05).factors that had no significant effect on overall survival included gender,histologic sbutype and postoperative chemotherapy (P > 0.05).The 5 year survival in stage Ⅰ,Ⅱ,Ⅲa,Ⅲb + Ⅳ were 75.0%,33.3%,14.3% and 0,respectively.The 5 year survival in lobectomy,pneumonectomy and exporsive were 43.0%,18.2% and O,respectively.On multivariate analysis,TNM stage of disease was the only independent predictor of survival ( P =0.000) .ConclusionWe should pay attention to adolescent lung cancer and improve the diagnosis rate avoiding of delaying surgical treatment.The five year survival rate of radical resection for adolescent lung cancer was good.They should be treated with aggressive multimodality therapy and surgical resection is the first-line treatment for them.

Objective To investigate the efficacy and safety of oxcarbazepine (OXC) suspension for treating the 2 to 4 years old patients newly diagnosed as partial epilepsy.Methods A total of 62 patients between 2 to 4 years old diagnosed as partial epilepsy,selected from the outpatients of the pediatric neurology in the hospital from October 2009 to March 2011,were randomly divided into the experimental group of 32 patients and the control group of 30 patients.Experimental group:OXC suspension,the initial dose was 8-10 mg· kg-1 d-1,orally taking 2 times every day,increasing the dose by 10 mg · kg-1 d 1 once every 7 days to the complete control of the seizure,the target dose was 20-40 mg· kg-1 · d-1 Control group:oral administration of carbamazepine (CBZ) group,the initial dose was 5 mg· kg 1 · d-1,increasing the dose once every 5-7 days up to 10-15 mg · kg-1 · d-1,if necessary,the dose could be 20 mg · kg 1 d-1 to maintain.The observation period was 26 weeks.Results The rate of valid cases in OXC group after 13 weeks and 26 weeks were both 78.1％ (25/32),and the rate of non-attack cases were 53.l％ (17/32) and 50.0％ (16/32),respectively; the rate of valid cases in CBZ group after 13 weeks and 26 weeks were 76.7％ (23/30) and 70.0％ (21/30),respectively,and the rate of non-attack cases were 50.0％ (15/30) and 40.0％ (12/30) (x2 =0.022,0.004 ; P =0.883,0.947 respectively.).In the 26th week,the quit rate of OXC group was 6.2％,while the quit rate of CBZ group was 13.4％.The rates of adverse reactions of OXC and CBZ were 15.6％ and 26.7％,respectively,with no significant difference.Conclusion OXC suspension monotherapy for 2 to 4 years old patients with partial epilepsy was significant effective,and no significant difference when compared with carbamazepine group.The rate of the adverse reactions in the OXC group was relatively fewer,and the extent was slight.

ObjectiveTo evaluate the changes in the expression of purinergic P2X4 receptor (P2X4R) in spinal cord dorsal horn and dorsal root ganglion in a rat model of acute morphine tolerance or inflammatory pain.MethodsThirty male SD rats were randomly divided into 3 groups:normal saline group (group NS,n =5);acute morphine tolerance group (group M,n =5) and inflammatory pain group (group F,n =20).Inflammatory pain was induced by subcutaneous injection of 4％ formalin 50 μl into the plantar surface of left hindpaw in group F.The animals received intrathecal morphine 10 μg ( 10 μl) once every 2 h for 6 times (T1-6) in group M,while the equal volume of normal saline was given instead in group NS.The rats were then sacrificed 2 h after the last time administration.Paw withdrawal latency (PWL) to a thermal nociceptive stimulus was measured at T1-6 in groups M and NS,or on day 4,7,10 and 14 after establishing the model of inflammatory pain in group F,The rats were sacrificed after measurement of PWL and spinal cord dorsal horn and dorsal root ganglion were removed to detect the expression of P2X4 R by immuno-histochemisty.ResultsCompared with the baseline value,PWL was significantly decreased on day 4-14 after intlammatory pain in group F,and PWL was significantly increased at T1-5,while no significant change was found at T6 in group M ( P ＞ 0.05).Compared with group NS,the expression of P2X4 R was up-regulated in group M,and the expression of P2X4 R was up-regulated on day 4 after inflammatory pain,peaked on day 7 after inflammatory pain,and then was down-regulated gradually on day 10 and 14 after inflammatory pain in group F ( P ＜ 0.01).ConclusionThe expression of purinergic P2Y4 R is up-regulated in spinal cord dorsal horn and dorsal root ganglion in rats with acute morphine tolerance or inflammatory pain,and the change may be related to the development of acute morphine tolerance or inflammatory pain.

Objective:To study the effect of interleukin-18 on transdifferentiation in renal tubular epithelial cells(TECs).Methods:Human proximal tubular epithelial cell line(HK-2) was cultured in vitro.TECs were exposed to different concentrations(0,0.1,1,10 and 100 ng/ml) of IL-18 for 24,48 and 72 hours.At the end of each incubation,the expressions of the ?-SMA and TGF-?_1 mRNA were assessed by RT-PCR,the rate of ?-SMA expressing TECs was assessed by immunocytochemistry,and the expression of the ?-SMA protein was assessed by Western blotting.Results:(1)The expressions of ?-SMA and TGF-?_1 mRNA were increased significantly by a dose- and time-dependent manner when TECs were exposed to IL-18(P

0.05).Conclusion:TH1 cell might not complicate in the pathogenesis of RSV bronchiolitis. The IL-2 levels showed a heterogenous behavior.
