1.Interaction between serum uric acid and blood glucose and its influence on prehypertension among Chinese Adults

ObjectiveTo explore the relationship between serum uric acid and prehypertension and the effects of age,obesity,fasting glucose and lipids in Chinese adults.Methods14,451 non-hypertensive cases from a community-based health examination survey in Xuzhou,Jiangsu province of China were enrolled in this study.Blood pressure,BMI,and determination of fasting glucose,lipids and serum uric acid were measured in all cases.ResultsThe odds ratios ( OR,95% CI ) of prehypertension across increased serum uric acid after adjusting for age,sex were 1.0,1.20 ( 1.07 - 1.35 ),1.55 ( 1.36 - 1.76),1.82(1.60-2.09),2.33(2.03-2.67) (Pfor trend <0.01).The odds ratios were 1.0,1,04(0.92-1.18),1.21(1.06-1.38),1.26(1.09 - 1.45),1.36(1.17 - 1.58),( Pfor trend <0.01) after adjusting for age,sex,BMI,glucose,and lipids.In addition,fasting glucose significantly interacted with uric acid ( P for interaction < 0.01 ).Conclusions Serum uric acid was associated with prehypertension,which might be an independent metabolic risk factor.Fasting glucose may reinforce the associations.
2.Study on extracting process and formulation of Mashi Granules

Objective: To ascertain the process condition of extraction Mashi Granules(Herba Ephedrae, Gypsum Fibrosum, Radix Puerariae, Herba Methal, Semen Armeniacae Amerum, etc.) Methods: In combination with area under the curve of hypothermy in rabbits, process water, macerating time and boiling times were used as parameters in uniformidy design. And then appropriate excipients were selected on the basis of hygroscopicity and granulation. Results: 20 times amount of water as much as herbs was added, macerating time reached as far as 40 min, and extracted 3 times, 2h a time. Excipient formula consisted of lactose and mannitol(4∶1, w/w). Conclusion: The experimental results provide the basis for the ascertainment of extracting process and formulation of Mashi Granules.
3.Continuous veno-venuous hemodialysis/filtration for the treatment of multiple organ dysfunction syndrome in pediatrics

Objective To investigate the efficacy of continuous veno-venuous hemodialysis/filtration(CVVHD/F) for the treatment of multiple organ dysfunction syndrome(MODS)caused by severe infection and to explore the mechanism in children.Method Nineteen cases of pediatric septic shock with MODS were treated with CVVHD/F in Children's Hospital Affiliated to Shanghai Jiaotong University from December 2002 to November 2007.The clinical data were studied including mortality rate,serum electrolytes,arterial partial pressure of oxygen (PO2),artery partial pressure of carbon dioxide(PCO2),FiO2/PO2,urine output,blood pressure,doses of vasoactive agents,Cr,BUN,etc.Results Cannulation and CVVHD/F were well performed in a total of 19 cases,with median age 33.4±36.5 months(from 3 months to 8 years) ,with their gender ratio of male(13 cases)to female (6 cases) to be 68.4% and 31.6%.The mean pediatric crifcal illness score(PCIS) was 69.1±10.4 and Median Pediatric Risk of Mortality score(PRMS Ⅲ)12.66±7.85,respectively.The duration of CWHD/F was 92 hours(ranged from 16 hours to480 hours).FiO2/PO2,PCO2,and PO2 were iraproved significantly after 12 to 24 hours CVVHD/F in patients with acute respiratory distress syndrome(ARDS) or lung edema (P<0.05).The concentrations of serum kalium,natrinm and HCO3- level resumed to well-balanced in 24 hours (P<0.05).The serum Cr and BUN were decreased to normal range(P<0.05).The mortality rate was 63.2%.Conclusions CVVHD/F was effective for treatment of septic shock with MODS in pediatric by improving oxygenation,maintaining normal serum electrolytes,conecting metabolic acidosis,increasing the tissue perfusion and eliminating the serum Cr and BUN.
4.Histological analysis of nano-hydroxyapatite for repairing defect in rabbit jaw

BACKGROUND: To repair bone defect, histocompatibility, growing characteristics, biodegradation and repairing mechanism of nanometer need to be further studied in clinic. OBJECTIVE: To observe the growing characteristics and histocompatibility of nano-hydroxyapatite (Nano-HA) for repairing jaw defect of rabbits.DESIGN: Randomized grouping animal study.SETTING: Beijing Jishuitan Hospital and Stomatology College of Jiamusi University.MATERIALS: A total of 24 New Zealand rabbits, either gender, weighing 2.5-3.5 kg, were provided by Animal Experimental Center of Jiamusi University. The animal experiment had got confirmed consent from local ethic committee. Nano-HA was provided by Material Engineering College of Jiamusi University and dealt with routine hyperthermia/hypertension sterilization. In addition, hydroxyapatite was provided by Wuhan Industry University, and the diameter was 1.0-2.0 μm.METHODS: The experiment was carried out in the Experimental Animal Center of Jiamusi University from November 2001 to May 2006. All rabbits were randomly divided into experimental group and control group with 12 in each group. Bone defect in the diameter of 1.0 cm was produced on body of mandible. Nano-HA was used to repair the bone defect of rabbits in the experimental group, while hydroxyapatite was used to repair the bone defect of rabbits in the control group. At 1, 4, 8 and 12 weeks after operation, all rabbits were sacrificed. In addition, medical imaging analysis system was used to analyze generative quantity of tissue in the two groups; meanwhile, histological quality and quantity were also analyzed so as to observe histocompatibility and newborn osteogenesis. MAIN OUTCOME MEASURES: Histocompatibility and newborn osteogenesis.RESULTS: With the time passing by, the amount of repairing materials was decreased because of the combination with newborn tissue into bone in bone defect-repaired region in the experimental group. When it was closed to normal bone, the amount was stable. However, bony callus was not able to grow in materials in the control group. Results of correlation analysis demonstrated that materials were negatively straight-line correlation with newborn bone (r = -0.912 0, P < 0.01). During the repairing procedure of bone defect, newborn bone was closely correlative with Nano-HA; while, with the increase of newborn bone, the amount of repairing materials was decreased because of the combination with newborn tissue into bone.CONCLUSION: Nano-HA can combine with newborn bone tissue so as to rapidly generate bone, while it has an excellent biocompatibility.
5.Expression of bone morphogenetic protein and its receptors in different stages of hypertrophic scar

BACKGROUND:Bone morphogenetic proteins are associated with the formation and development of scars. Fibroblasts are closely related to the proliferation and maturation of scars. However, the expression of bone morphogenetic proteins in fibroblasts in different stage of hypertrophic scars remains poorly understood.
OBJECTIVE:To observe the expression of bone morphogenetic protein 2/4, bone morphogenetic protein 7, bone morphogenetic protein receptor IA in fibroblasts in different stage of hypertrophic scars.
METHODS:The immunohistochemical SP method was employed to detect the expression of bone morphogenetic protein 2/4, bone morphogenetic protein 7, bone morphogenetic protein receptor IA in fibroblasts in 20 cases of proliferative stage hypertrophic scar and 20 cases of maturation stage hypertrophic scar. Al samples were obtained from the Department of Plastic and Aesthetic Surgery, the People’s Hospital of Guangxi Zhuang Autonomous Region, China.
RESULTS AND CONCLUSION:The expression of bone morphogenetic protein 2/4 in fibroblasts in proliferative hypertrophic scars significantly elevated compared with mature hypertrophic scars (P<0.05). In various stages of hypertrophic scars, the expression of bone morphogenetic protein 7 and bone morphogenetic protein receptor IA in fibroblasts was not obviously different (P>0.05). Results demonstrated that bone morphogenetic protein 2/4 expression down-regulated during the development from proliferative stage to maturation stages in fibroblasts in hypertrophic scars. However, there was no change in the expression of bone morphogenetic protein 7 and bone morphogenetic protein receptor IA.
6.Relativity among starch quantity, polysaccharides content and total alkaloid content of Dendrobium loddigesii.

OBJECTIVETo find out the relativity among starch quantity, polysaccharides content and total alkaloid content of Dendrobium loddigesii.
METHODMicroscopy-counting process was applied to starch quantity statistics, sulfuric acid-anthrone colorimetry was used to assay polysaccharides content and bromocresol green colorimetry was used to assay alkaloid content. Pearson product moment correlation analysis, Kendall's rank correlation analysis and Spearman's concordance coefficient analysis were applied to study their relativity.
RESULT AND CONCLUSIONExtremely significant positive correlation was found between starch quantity and polysaccharides content, and significant negative correlation between alkaloid content and starch quantity was discovered, as well was between alkaloid content and polysaccharides content.

7.Serum HMGB1 Levels and Its Clinical Significance in Patients with Idiopathic Pulmonary Arterial Hypertension

[Objective]To estimate the clinical value of serum high mobility group box 1(HMGB1) as a biomarker of idiopathic pulmonary arterial hypertension (IPAH).[Methods]This study included 33 patients with IPAH that were confirmed by right heart catheter in the Second Xiangya Hospital, Central South University from May 2011 to April 2015. 8 patients with IPAH were followed up for 6 months during treating with PAH-specific pharmacotherapies. All the subjects ' clinical data were collected,HMGB1 levels were determined by enzyme linked immunosorbent assay(ELISA).[Results]Serum HMGB1 levels (ng/mL) were significantly increased in patients with IPAH compared with the control group(14.8 ± 2.4 vs. 3.8 ± 1.2, P<0.001);The serum HMGB1 levels were significant?ly positive correlation with mean pulmonary arterial pressure(MPAP) and pulmonary vascular resistauce (PVR) ( r=0.864, P<0.001; r=0.460,P=0.002) in the patients with IPAH. After treating with PAH-specific pharmacotherapies for 6 month, HMGB1 levels(ng/mL) were significantly decreased(15.9±5.3 vs 11.1±2.5,P=0.021)along with the patients'MPAP(62.3±9.7 vs 54.0±8.7,mmHg)and 6-min walk distances(m)improved(368±69 vs 401±55,P<0.001).[Conclusions]Our study suggested that serum HMGB1 may be used as a biomarker of treatment response to targeted therapy, and it will be used as a biomarker in the follow-up evaluation of patients with IPAH.
8.Analysis of clinical characteristics of autoimmune encephalitis with antibodies against contactin-associated protein-like 2

Objective:To explore the clinical characteristics of patients with antibodies against contactin-associated protein-like 2 (CASPR2).Methods:The clinical data of 24 patients with anti-CASPR2 encephalitis diagnosed at the First Affiliated Hospital of Zhengzhou University from January 2018 to December 2022 were retrospectively analyzed. According to the age of first onset, the patients were divided into early onset group (10 cases, onset age<45 years) and late onset group (14 cases, onset age≥45 years). The clinical data including clinical manifestations, auxiliary examinations, and treatment response between these 2 groups were compared.Results:Among the 24 patients, there were 13 cases with epilepsy, 13 cases with cognitive decline, 13 cases with mental disorders, 14 cases with autonomic dysfunction, 8 cases with peripheral nerve hyperexcitability, 5 cases with Morvan syndrome, 5 cases with unstable walking, and 8 cases with sleep disorders. Among the 10 cases of the early onset group, 7 cases are females, and 8 cases showed epilepsy. The incidence rate of epilepsy in the early onset group was higher than that in the late onset group (5/14, Fisher exact probability, P=0.047). Among the 14 cases of the late onset group, 6 cases are females, 9 cases showed cognitive impairment and 8 cases presented with mental disorders. There were 6 cases with abnormal brain magnetic resonance imaging (MRI). The cerebrospinal fluid protein of the late onset group [0.37 (0.29, 0.58) g/L] was higher than that in the early onset group [0.22 (0.16, 0.30) g/L; Z=-2.667, P=0.008]. The modified Rankin Scale (mRS) scores before and after treatment were 3.29±0.83 and 1.50 (0.75, 2.25), which were higher than those in the early onset group [mRS scores before and after treatment were 2.10±0.99 and 0 (0, 1.00), t=-3.188, P=0.004; Z=-2.335, P=0.020]. Conclusions:There are various symptoms in patients with anti-CASPR2 encephalitis. The early onset patients are common in women, with a higher incidence of epilepsy. The late onset patients are common in males, with prominent manifestations of cognitive impairment and mental disorders, which have a greater impact on daily living abilities. And abnormal MRI findings are common, and the cerebrospinal fluid protein is higher in late onset patients. Anti-CASPR2 antibody may cause more severe immune damage to the nervous system in elderly patients.
9.Effect of 4-hydroxytamoxifen on the expression of pituitary tumor transforming gene in GH3 prolactinonm cells

Objective To investigate the effect of 4-hydroxytamoxifen on the expression of pituitary tumor transforming gene (PTTG) in GH3 prolactinoma cells. Methods RT-PCR and Western blotting were employed to detect the expressions of PTTG mRNA and protein in human GH3 prolaetinoma cells. Different concentrations of estradiol (E2) or 4-hydroxytamoxifen (OHTam) were addedl into the hormone-depleted medium, and the viable cell number and expression levels of PTTG mRNA and protein were measured. Results The growth of OH3 prolaetinoma cells was significantly inhibited in hormone-depleted medium. E2 at a concentration of 1×10<'-8> mol/L obviously promoted the cell growth, the effect of which was inhibited by the application of OHTam (1×10<'-6> mol/L) to cause slowed cell growth. The expressions of PTTG at both the mRNA and protein levels were detected in detected in untreated GH3 prolactinoma cells, and OHTam at the concentration of 1×10<'-6> mol/L significantly inhibited their expressions. Conclusion The growth of GH3 cells is estrogen-dependant and can be inhibited by estrogen antagonist OHTam, which also results in reduced expression of PTTG gene in the cells.
10.3D-printing rapid prototyping used in classification of acetabular fractures and education of young surgeons

Objective To explore the application of 3D-printing rapid prototyping in classification of acetabular fractures and education of young surgeons.Methods The data of 20 patients with acetabular fracture were reviewed in this study who had been treated between January and June 2016.Three junior orthopedic surgeons and 3 senior ones were chosen as observers.The conventional radiographs (X-ray films of the pelvis and acetabulum,CT scans and 3D reconstruction images of the pelvis) and 3D-printing rapid prototyping models of the 20 patients(1∶ 1) were randomly numbered.All the observers were asked to make Letournel-Judet classification of each radiograph and 3D-printing model independently in the first assessment.Four weeks later in the second assessment,all the observers were asked to make the same classifications after all the conventional radiographs and 3D-printing models were randomly numbered again.The kappa statistics was used to evaluate inter-and intra-observer agreements among the recorded results.Results At the first assessment,the inter-observer agreement was 0.887 and 0.962 respectively for conventional radiographs and 3D-printing models in senior surgeons while 0.659 and 0.849 in junior surgeons.The second assessment showed the intra-observer agreement was 0.906 for radiographs and 0.925 for 3D-printing models in senior surgeons while 0.696 and 0.849 in the junior ones.Conclusions Compared with conventional radiographs,since 3D-printing models can effectively enhance the reliability of acetabular fracture classification,they may be more helpful for young surgeons in understanding acetabular fractures.
