1.The expression and clinical significance of hyaluronan synthase in nasal polyps.

OBJECTIVE:
To study the expression of hyaluronan synthasel-3 (HAS1-3) in nasal polyps, and discuss their clinical significance.
METHOD:
The expression of HA, HAS1, HAS2 and HAS3 in nasal polyps group (25 cases) and inferior turbinate group (15 cases) was detected by immunohistochemistry and semi-quantitative RT-PCR.
RESULT:
Compared with the control, the expression of HA, HAS1 and HAS3 was higher in nasal polyps (P<0. 05), while there was no significant difference in HAS2 expression (P>0. 05).
CONCLUSION
The increase of HASI and HAS3 expression may be the main cause of the excessive deposition of HA in nasal polyps, which may play an important role in the pathogenic process of nasal polyps, and become a potential target for therapy of nasal polyps.

3.Effect of isorhynchophylline on head-shakes behavior and levels of monoamine neurotransmitter in model rats with Tourette syndrome

Objective To explore the effect of rhynchophylline and isorhynchophylline on headshakes behavior and levels of monoamine neurotransmitter in model rats with Tourette syndrome.Methods 40 Wistar rats were randomly divided into DOI-induced head-shakes rats (HSR group),haloperidol group,rhynchophylline group and isorhynchophylline group with 10 in each group.The inhibitory effects of rhynchophylline and isorhynchophylline were estimated by observing the HSR behavior.Dopamine (DA) and 5-hydroxytryptamine(5-HT) in the rat striatum were detected by the enzyme-linked immunosorbent assay (ELISA) method.The 5-HT2A receptor mRNA expression in prefrontal lobe cortex of the rats was measured by real-time PCR.Results Compared with HSR group,the head shakes of the rats in haloperidol group and isorhynchophylline group were significantly decreased(P<0.01),and no change of head-shakes number was observed in rhynchophylline group (P>0.05).There was no significant difference of head-shakes number between the haloperidol group and isorhynchophylline group(P>0.05).Compared with HSR group,DA levels in the rat striatum were significantly decreased in isorhynchophylline group and haloperidol group((152.35± 5.80) μ~L vs (111.19±4.30) μg/L,(152.35±5.80) μg/L vs (126.42±3.17) μg/L,P<0.01),while DA levels in the rat striatum in rhynchophylline group were not changed ((152.35±5.80) μg/L vs (142.71±5.51) μg/L,P>0.05).There was no significant change of 5-HT2A receptor mRNA expression in rat prefrontal lobe cortex in every group(P>0.05).Conclusion Isorhynchophylline may have an inhibitory effect on rats with DOI-induced HSR.Isorhynchophylline may decrease the DA levels in the rat stratum with DOI-induced HSR.Rhynchophylline has no significant inhibitory effect on head-shakes behavior and DA levels in the rat stratum with DOI-induced HSR.
4.Advances in the Research on the Brucella Intracellular Life

Brucella organisms are facultative intracellular bacteria capable of surviving inside professional and non-professional phagocytes.Upon cell contact the bacteria is internalized via receptor molecules.Once inside cells,Brucella localizes in early phagosomes,where it avoids fusion with late endosomes and lysosomes.Then,the bacterium redirects its trafficking to autophagosomes and finally reaches the endoplasmic reticulum,the replicating niche.Once inside the endoplasmic reticulum,Brucella extensively replicates without restricting basic cellular functions or inducing damage to cells.Invasion,intracellular trafficking and replication of Brucella organisms in professional and non-professional phagocytes and the molecular determinants involving Brucella intracellular life are reviewed in this article.
5.Effects of hydrogen gas on myocardial injury in rats with endotoxemia

Objective To investigate the effects of hydrogen gas on the myocardial injury in rats with endotoxemia.Methods Forty-eight healthy male Sprague-Dawley rats,weighing 200-220 g,were randomly divided into 4 groups (n =12 each ):control group (group C ),hydrogen gas control group (group HC ),endotoxemia group (group E) and hydrogen gas + endotoxemia group (group LH).Endotoxemia was induced by intraperitoneal lipopolysaccharide (LPS) 20 mg/kg in groups E and LH,while the equal volume of normal saline was given in groups C and HC.After LPS administration,the rats were exposed to the air containing 2% hydrogen gas for 6 h in groups HC and LH,and the rats were exposed to the air for 6 h in groups C and E.Blood samples were taken from the abdominal aorta after 6 h inhalation of hydrogen gas to determine the serum level of cTnI.The hearts were then removed to determine the content of TNF-α and IL-6 and expression of phosphorylated nuclear factor κB ( NF-κB) inhibitory protein (p-IκB-α) and p38 mitogen-activated protein kinase (p-p38MAPK) in the myocardial tissues.Results Compared with group C,no significant change was found in the levels of cTnI,TNF-α and IL-6 and expression of p-IκB-α and p-p38MAPK in group HC ( P > 0.05),and the levels of cTnI,TNF-α and IL-6 were significantly increased and the expression of p-IκB-α and p-p38MAPK was up-regulated in groups E and LH ( P < 0.05).Compared with group E,the levels of cTnI,TNF-α and IL-6 were significantly decreased and the expression of p-IκB-α and p-p38MAPK was down-regulated in group LH ( P < 0.05).Conclusion Hydrogen gas can reduce the myocardial injury in rats with endotoxemia,and inhibition of the p38MAPK and NF-κB pro-inflammatory pathway and reduction of the inflammatory response of myocardial tissues are involved in the mechanism.
7.Biodegradation and mechanical characteristics of poly(lactic-co-glycolic acid) biliary stents in vitro

BACKGROUND: The biliary stents commonly used in clinic are made of plastics and metal, both of which have some irresolvable defects. New type of material characterizing by great biocompatibility and degradation for the making of biliary stents needs to be found and studied.OBJECTIVE: To evaluate the biodegradation law and the change in radical force of five different mole-ratios of biliary stents made of poly (lactic-co-glycolic acid) (PLGA) in human bile in vitro.DESIGN, TIME AND SETTING: A Completely random design was performed at the Laboratory of Hepatobiliary Surgery, the First Affiliated Hospital of Medical College, Xi'an Jiaotong University between July and December 2006.MATERIALS: Five different mole-ratios of PLGA hiliary stents (90L/10G, 80L/20G, 70L/30G, 60L/40G, and 50L/50G) were provided by Dalgang Biological Science and Technology Co., Ltd., Jinan; Human fresh bile was obtained from the patients who underwent external drainage of bile in the Department of Hepatobiliary Surgery, the First Affiliated Hospital of Medical College, Xi'an Jiaotong University.METHODS: Thirty-two PLGA biliary stents of each proportion were respectively divided into eight groups, with four stents in each group. All of them were independently submerged into test tubes with 10 mL fresh human bile which were then placed in constant temperature oscillator. The temperature was set at 37 ℃. The bile was changed every day. After incubated for one week to eight weeks, the PLGA biliary stents were respectively taken out of test tubes from the first to the eighth group each week. They were washed with distilled water and dried in calorstat for 24 hours.MAIN OUTCOME MEASURES: ① Gross morphology, color and integrality of PLGA stents; ② Scanning electron microscope; ③ the mass and the radical force of PLGA stents.RESULTS: ① Five different mole-ratios of PLGA stents followed a similar changing law as to gross morphology: stents remained intact → stents became soft → the surface ofstents became coarse and cracked → stents collapsed → stents were degraded into pieces → stents were completely degraded. ② Before degradation: limpid stereo reticular structure of PLGA; One week after degradation: PLGA was corroded and the reticular structure was decreased; Three weeks after degradation: airspace was gradually disappeared and the reticular structure was further decreased; Five weeks after degradation: airspace and the reticular structure were completely damaged, which was in an indefinite form. ③ The mass loss of each proportion of PLGA stents gradually speeded up along with the time extension. Furthermore, with the content of PGA increasing, the mass loss of the stents became faster. The initial radical force was different among five different mole-ratios of hiliary stents. With the content of PLA increasing, the radical force of the stents became stronger.CONCLUSION: The material of PLGA possesses adjustable biodegradation and good mechanical feature, so it is suitable for preparation of biliary stents.

9.A comparison of endoscopic papillary balloon dilation and endoscopic sphincterotomy for the removal of common bile duct stones

Objective To explore the effectiveness and safety of endoscopic papillary balloon dilation( EPBD ) for the removal of common bile duct stones. Methods Three hundred and twenty consecutive patients with common bile duct stones on endoscopic retrograde cholangiopancreatography (ERCP) who met all eligibility criteria were randomly assigned endoscopic sphincterotomy (EST) or EPBD.Complications were classified by an expert panel unaware of treatment allocation and outcome. Results After a single ERCP, all stones were removed from 112 patients ( 70% ) assigned EST and 104 ( 65% )assigned EPBD. Mechanical lithotripsy was used to fragment stones in 36 (22. 5% ) EPBD procedures and 32 ( 20. 0% ) EST procedures. Early complications occurred in 5.6% EST patients and 8.1% EPBD patients. No patient died. Gallstone disease recurrence, which is a long-term complication, is 7. 5% ( 12/160) in EST patients and 2.5% (4/160) in EPBD patients, P <0. 05. Conclusions The success rate of EPBD was similar to that of EST. We found no evidence of previously suggested higher risk of pancreatitis with EPBD, and suggest that EPBD is preferred in patients who are not suitable for EST, such as those with high risk of bleeding. This procedure is a valuable alternative to EST in patients with bile duct stones.
10.Efficacy of dexmedetomidine versus midazolam for sedation in critically ill patients: a Meta-analysis

Objective To systematically review the efficacy of dexmedetomidine or midazolam for sedation in critically ill patients. Methods We searched the PubMed, EMBaes, Cochrane Library, Wanfang Database,CNKI and VIP for all randomized controlled trials (RCTs) about the efficacy of dexmedetomidine versus midazolam for sedation in severe cases. The quality of the studies was evaluated by the method recommended by Cochrane Collaboration. Meta-analysis was conducted using the Cochrane Collaboration's RevMan 5.0 software. Results Six RCTs involving 613 patients were included in our Meta-analysis. The results of Meta-analysis showed that the length of ICU stay was significantly shorter in group dexmedetomidine than in group midazolam. There were no significant differences in the duration of mechanical ventilation, incidences of bradycardia, hypotension and delirium and mortality rate between the two groups. Conclusion Dexmedetomidin can shorten the length of ICU stay and is beneficial for the outcome in critically ill patients.
