1.Compliance guarantee in acupuncture clinical research.

Through review of the major databases in English and Chinese, it is found that there is an absence of articles on special coverage of compliance of research fellows and testees in acupuncture clinical trials. Therefore, the author proposed (1) The compliance of research fellows should be guaranteed through proper work division with reasonable, standard and strict research designation as well as comprehensive systems of selection, supervision and estimation. (2) The compliance of testees should be guaranteed through enhancing of the right to know, improving of the medical environment and constructing a better doctor-patient relationship. Therefore, the compliance of clinical trials can be improved, the quality of clinical research enhanced and the reliability of the research results can be guaranteed.

2.Investigation of health of 2560 female workers in a electronic enterprise

Objective To investigate the occupational health situation of female workers in the electronic enterprise. Methods A cluster sampling method was used to investigate the occupational health situation of 2560 female workers who were working in the workshops by questionnaire. The major occupational hazardous factors in the workplaces were monitored, and the health examination was conducted in 2560 female workers exposed to the occupational hazardous factors. Results The average age of 2560 female workers was (19.4±2.9) years, the average working days a month was (26.2±3.1) days, he average working time a day was (9.6±1.4) hours with work overtime at times. All hazard factors in the workplace are controlled within safe standard except for tin oxide concentration of air and noise level in workshop, 18.1% of female workers exposed to the low concentration of benzene suffered from the decreased number of leukocytes. The heavy workload (such as, working overtime and moving heavy objects) and irrational ergonomic design induced obviously the muscular-skeletal system symptoms, such as backache, neurological symptoms, occupational stress, visual fatigue and accident injury as well. However, the enough interval rest could reduce significantly the occurrence of above symptoms (P<0.05). Conclusion For the labor-intensive electronic enterprises, the occupational hazard prevention and control measures should be strengthened. The labor load regulation and rational ergonomic design can reduce the effects of the occupational hazard on the health of workers.

4.Thalassemia Genotypes and HbA2 levels of Children in Wuzhou,Guangxi

Objective:To analyze thalassemia genotypes and distribution of children in Wuzhou Guangxi,and evaluate the diagnostic value of HbA2 in children's thalassemia screening,so as to provide scientific evidence for the prevention and control strategies of thalassemia.Methods:Four hundred and fifty-eight children suspected with thalassemia in Wuzhou were enrolled from March 2017 to June 2022.The level of HbA2 was detected using Bio-Rad VARIANT Ⅱ Hb analysis system.The deletion of α-thalassemia was measured with gap-PCR assay,and the point mutation of α-and β-thalassemia was tested with DNA reverse dot blot hybridization assay.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of HbA2 for children's thalassemia.Results:A total of 304 thalassemia carriers were detected in 458 children,accounting for 66.38%.One hundred and seventy-five cases were defined to be α-thalassemia,with the main type of--SEA/αα(54.86%).Thirty-six cases were defined to be intermediate α-thalassemia,with the main type of-α3.7/--SEA(9.72%).In 108 cases with β-thalassemia,βCD41-42/βN was the main type,accounting for 49.07%,followed by βIVS-Ⅱ-654/βN(14.81%).Seven cases were moderate/severe β-thalassemia(predominantly β-28/β-28 and βCD41-42/βCD17)Twenty-one genotypes of α-and β-thalassemia were found in the children.There was significant difference of HbA2 level between the children with different types of thalassemia and healthy controls(all P<0.001).ROC curve analysis showed that the sensitivities of HbA2 for α-thalassemia,β-thalassemia and αβ-thalassemia were 74.3%,82.4%and 85.7%,with the optimal cut-off values of 2.60%,3.60%and 3.70%,respectively,the specificities were 64.3%,96.1%and 96.8%,and the area under the curve were 0.690,0.887 and 0.916,respectively.Conclusion:The thalassemia genotypes of children in Wuzhou are diverse.It is necessary to further strengthen the prevention and control measure of thalassemia to reduce birth defects and improve birth quality.
5.Correlation of resistance to peer pressure and risky decision-making with adolescent health risk behaviors.

OBJECTIVETo explore possible interrelationships among resistance to peer pressure, risky decision-making and health risk behaviors among young adolescents.
METHODSBased on the cluster sampling method, the participants who were recruited from 5 junior middle schools in Guangzhou and 3 junior middle schools in Shenyang city on October, 2010, were administered to complete the questionnaire concerned with their experiences with drinking and smoking during the past 30 days preceding the survey, and the hours using computer daily both in weekdays and in weekend. The level of resistance to peer influence and risky decision-making were assessed by Resistance to peer influence scale (RPIS) and Youth decision-making questionnaire (YDMQ). Logistic regression was used to explore possible interrelationships among resistance to peer influence, risky decision-making and health risk behaviors among young adolescents.
RESULTSA total of 1985 questionnaires were valid, including 1001(50.4%) boys and 984 (49.6%) girls. About 27.1% (537/1985) junior middle school students reported having health risk behaviors, boys' (30.7%, 307/1001) was higher than girls' (23.4%, 230/984) with significant gender difference (P < 0.05). The prevalence of smoking, drinking during the past 30 days before the survey and using computer over 3 hours daily in weekdays and in weekend were 5.1% (102/1985), 14.3% (284/1985), 3.5% (70/1985) and 13.7% (272/1985), respectively. The rate of drinking, using computer over 3 hours daily in weekdays and in weekend were higher in males (16.4% (164/1001), 4.5% (45/1001), 16.2% (162/1001)) than those in females (12.2% (120/984), 2.5% (25/984), 11.2% (110/984)) (P < 0.05). The scores of RPIS and YDMQ of the two cities adolescents were 2.82 ± 0.39 and 1.68 ± 0.62. The students reported smoking, drinking during the past 30 days before the survey and using computer over 3 hours daily in weekend gained lower RPIS scores (2.43 ± 0.40, 2.61 ± 0.41, 2.77 ± 0.40) than their counterparts who didn't report these kind of health risk behaviors (2.84 ± 0.38, 2.85 ± 0.38, 2.82 ± 0.39)(P < 0.05). And those reported smoking, drinking during the past 30 days before the survey and using computer over 3 hours daily in weekdays and in weekend gained higher YDMQ scores (2.38 ± 0.66, 2.06 ± 0.66, 1.97 ± 0.72, 1.84 ± 0.64, respectively) than their counterparts who didn't report these kind of health risk behaviors (1.64 ± 0.38, 1.61 ± 0.58, 1.67 ± 0.61, 1.65 ± 0.61, respectively) (P < 0.05). After adjusting gender, area, parental education degree, self-reported family economic condition, multi-variant logistic regression analysis indicated that the low and middle level of resistance to peer influence (low and middle level vs high level, had odds ratios of 2.97 (1.96 - 4.50) and 1.51 (1.05 - 2.16)), and also the middle and high level of risky decision-making (middle and high level vs low level, had odds ratios of 1.62 (1.19 - 2.22) and 3.43 (2.39 - 4.90)) were all the risk factors of adolescent health risk behaviors.
CONCLUSIONAdolescents with poor ability of resistance to peer pressure and high risky decision-making were both the risk factors of adolescent health risk behaviors.

6.Experimental study on promoting the engraftment of hematopoietic stem/progenitor cell by all-trans retinoic acid.

OBJECTIVETo explore the effect of all-trans retinoic acid (RA) on the engraftment of unrelated umbilical cord blood stem/progenitor cell transplantation (UCBT) in murine model.
METHODS1 x 10(6) and 0.5 x 10(6) nucleated cells (NC) from C57BL/6 (H-2(b)) fetal and neonatal peripheral blood (FNPB) were separately transfused into lethally cyclophosphamide (380 mg/kg, ip) treated BALB/C (H-2(d)) recipients, 15 mg.kg(-1).d(-1) and 5 mg.kg(-1).d(-1) RA (15 mg and 5 mg RA) were administrated respectively 2 days before and after UCBT. Hematopoiesis and immune recovery, graft versus host disease (GVHD), engraftment and survival rates were then observed.
RESULTSHematopoiesis and immune recovery occurred faster in RA treated than in untreated mice (P < 0.05). Acute GVHD was absent. The levels of engraftment were higher in both 15 mg and 5 mg RA treated mice than those in untreated controls (P < 0.05). In 1 x 10(6) NC transfused mice, 15 mg and 5 mg RA could significantly increased the 30 and 60 days survival rates from 41.67% (without RA) to 72.23% and 70.83%, respectively (P < 0.05). In 0.5 x 10(6) cells transfused mice, 15 mg and 5 mg RA increased the survival rate from 14.29% (without RA) to 42.86% and 43.48%, respectively (P < 0.05), which were comparable to that of being transfused 1 x 10(6) cells without RA treatment (P > 0.05).
CONCLUSIONRA enhances the engraftment of umbilical cord blood stem/progenitor cells in murine model for UCBT. This might provide an experimental evidence of RA in clinical UCBT.

7.Pretreatment Serum Amyloid A and C-reactive Protein Comparing with Epstein-Barr Virus DNA as Prognostic Indicators in Patients with Nasopharyngeal Carcinoma: A Prospective Study.

PURPOSE: The measuring Epstein-Barr virus (EBV) DNA is an important predictor of nasopharyngeal carcinoma (NPC). This study evaluated the predictive value of pretreatment serum amyloid A (SAA) and C-reactive protein (CRP) comparing with EBV DNA in patients with NPC. MATERIALS AND METHODS: In an observational study of 419 non-metastatic NPC patients, we prospectively evaluated the prognostic effects of pretreatment SAA, CRP, and EBV DNA on survival. The primary end-point was progress-free survival (PFS). RESULTS: The median level of SAA and CRP was 4.28 mg/L and 1.88 mg/L, respectively. For the high-SAA group (> 4.28 mg/L) versus the low-SAA (≤ 4.28 mg/L) group and the high-CRP group (> 1.88 mg/L) versus the low-CRP (≤ 1.88 mg/L) group, the 5-year PFS was 64.5% versus 73.1% (p=0.013) and 65.2% versus 73.3% (p=0.064), respectively. EBV DNA detection showed a superior predictive result, the 5-year PFS in the EBV DNA ≥ 1,500 copies/mL group was obviously different than the EBV DNA < 1,500 copies/mL group (62.2% versus 77.8%, p < 0.001). Multifactorial Cox regression analysis confirmed that in the PFS, the independent prognostic factors were including EBV DNA (hazard ratio [HR], 1.788; p=0.009), tumour stage (HR, 1.903; p=0.021), and node stage (HR, 1.498; p=0.049), but the SAA and CRP were not included in the independent prognostic factors. CONCLUSION: The results of SAA and CRP had a certain relationship with the prognosis of NPC, and the prognosis of patients with high level of SAA and CRP were poor. However, the predictive ability of SAA and CRP was lower than that of EBV DNA.

8. Targeting immunometabolism by active ingredients derived from traditional Chinese medicines for treatment of rheumatoid arthritis

Rheumatoid arthritis (RA), the most common inflammatory arthropathy word wild, is a systemic autoimmune disease that mainly affects the synovium of joints with a high disability rate. Metabolic mis-regulation has emerged as a fundamental pathogenesis of RA linked to immune cell dysfunction, while targeting immunometabolism provides a new and effective approach to regulate the immune responses and thus alleviate the symptom of RA. Recently, natural active compounds from traditional Chinese medicines (TCMs) have potential therapeutic effects on RA and regulating immunometabolism. In this review, in addition to updating the connection between cellular metabolism and cell function in immune cells of RA, we summarized that the anti-inflammatory mechanisms of the potential natural compounds from TCM by targeting metabolic reprogramming of immune cells, and discusses them as a rich resource for providing the new potential paradigm for the treatment of RA.
9.Role of Diabetes Mellitus on Treatment Effects in Drug-susceptible Initial Pulmonary Tuberculosis Patients in China.

We assessed the role of diabetes mellitus (DM) on treatment effects in drug-susceptible initial pulmonary tuberculosis (PTB) patients. A prospective study was conducted in eight provinces of China from October 2008 to December 2010. We enrolled 1,313 confirmed drug-susceptible initial PTB patients, and all subjects received the treatment regimen (2H3R3E3Z3/4H3R3) as recommended by the national guidelines. Of the 1,313 PTB patients, 157 (11.9%) had DM; these patients had more sputum smear-positive rates at the end of the second month [adjusted odds ratios (aOR) 2.829, 95% confidence intervals (CI) 1.783-4.490], and higher treatment failure (aOR 2.120, 95% CI 1.565-3.477) and death rates (aOR 1.536, 95% CI 1.011-2.628). DM was a contributing factor for culture-positive rates at the end of the second month and treatment failure and death of PTB patients, thus playing an unfavorable role in treatment effects of PTB.

10.Safety and effectiveness of esophagojejunostomy through extracorporeal versus intracorporeal methods after laparoscopic total gastrectomy.

Objective: To compare the safety and effectiveness of esophagojejunostomy (EJS) through extracorporeal and intracorporeal methods after laparoscopic total gastrectomy (LTG). Methods: A retrospective cohort study was carried out. Clinicopathological data of 261 gastric cancer patients who underwent LTG, D2 lymphadenectomy, and Roux-en-Y EJS with complete postoperative 6-month follow-up data at the General Surgery Department of Nanfang Hospital from October 2018 to June 2021 were collected. Among these 261 patients, 139 underwent EJS with a circular stapler via mini-laparotomy (extracorporeal group), while 122 underwent intracorporeal EJS (intracorporeal group), including 43 with OrVil(TM) anastomosis (OrVil(TM) subgroup) and 79 with Overlap anastomosis (Overlap subgroup). Compared with the extracorporeal group, the intracorporeal group had higher body mass index, smaller tumor size, earlier T stage and M stage (all P<0.05). Compared with the Overlap subgroup, the Orvil(TM) subgroup had higher proportions of upper gastrointestinal obstruction and esophagus involvement, and more advanced T stage (all P<0.05). No other significant differences in the baseline data were found (all P>0.05). The primary outcome was complications at postoperative 6-month. The secondary outcomes were operative status, intraoperative complication and postoperative recovery. Continuous variables with a skewed distribution are expressed as the median (interquartile range), and were compared using Mann-Whitney U test. Categorical variables are expressed as the number and percentage and were compared with the Pearson chi-square, continuity correction or Fisher's exact test. Results: Compared with the extracorporeal group, the intracorporeal group had smaller incision [5.0 (1.0) cm vs. 8.0 (1.0) cm, Z=-10.931, P=0.001], lower rate of combined organ resection [0.8% (1/122) vs. 7.9% (11/139), χ(2)=7.454, P=0.006] and higher rate of R0 resection [94.3% (115/122) vs. 84.9 (118/139), χ(2)=5.957, P=0.015]. The morbidity of intraoperative complication in the extracorporeal group and intracorporeal group was 2.9% (4/139) and 4.1% (5/122), respectively (χ(2)=0.040, P=0.842). In terms of postoperative recovery, the extracorporeal group had shorter time to liquid diet [(5.1±2.4) days vs. (5.9±3.6) days, t=-2.268, P=0.024] and soft diet [(7.3±3.7) days vs. (8.8±6.5) days, t=-2.227, P=0.027], and shorter postoperative hospital stay [(10.5±5.1) days vs. (12.2±7.7) days, t=-2.108, P=0.036]. The morbidity of postoperative complication within 6 months in the extracorporeal group and intracorporeal group was 25.9% (36/139) and 31.1%, (38/122) respectively (P=0.348). Furthermore, there was also no significant difference in the morbidity of postoperative EJS complications [extracorporeal group vs. intracorporeal group: 5.0% (7/139) vs. 82.% (10/122), P=0.302]. The severity of postoperative complications between the two groups was not statistically significant (P=0.289). In the intracorporeal group, the Orvil(TM) subgroup had more estimated blood loss [100.0 (100.0) ml vs.50.0 (50.0) ml, Z=-2.992, P=0.003] and larger incision [6.0 (1.0) cm vs. 5.0 (1.0) cm, Z=-3.428, P=0.001] than the Overlap subgroup, seemed to have higher morbidity of intraoperative complication [7.0% (3/43) vs. 2.5% (2/79),P=0.480] and postoperative complications [37.2% (16/43) vs. 27.8% (22/79), P=0.286], and more severe classification of complication (P=0.289). Conclusions: The intracorporeal EJS after LTG has similar safety to extracorporeal EJS. As for intracorporeal EJS, the Overlap method is safer and has more potential advantages than Orvil(TM) method, and is worthy of further exploration and optimization.
