2.Research progress of exosome delivery vehicles in tumor phototherapy

Exosome is a self-secreted phospholipid bilayer nanovesicles, and has shown great potential in drug delivery field due to the important advantages of low immunogenicity and homologous targeting. Phototherapy, mainly includes photodynamic therapy (PDT) and photothermal therapy (PTT), utilize light to activate photoactive drug for tumor cell killing. The advanced therapeutic strategy shows low toxic side-effect and non-invasion precise advantages, and thus has made great progress in tumor treatment over the past few years. Therefore, using exosomes as a drug delivery system to deliver phototherapeutic agents can improve therapeutic performances with a reduced side-effect, and further enhance their application potential for clinical tumor therapy. This review focus on the rising cross-subjects field involving exosomes and phototherapy, and mainly introduce the research progress and relative case of exosomes-based delivery system for cancer phototherapy. Additionally, the advantages and challenges of exosome-based phototherapy are also discussed and proposed.
3.Glutathione S-Transferase π Expression in Transitional Cell Carcinoma of Bladder

Objective: The current study was designed to investigate the expression of glutathione s-transferase π ( GST-π ) and its clinical significance in human primary transitional cell carcinoma(TCCs) of bladder. Methods: A total of 107 human primary bladder TCCs of the previously untreated patients were analyzed for expression of GST-π and P-glycoprotein (Pgp) by SP immunohistochemistry (IHC). Results: The GST-π positive staining were divided into three groups: single cytoplastic staining, single nuclear staining, both cytoplastic and nuclear staining. GST-π were present in 72.9% generally, GST-π cytoplastic staining (single cytoplastic, both cytoplastic and nuclear staining) were found in 58.9% of the samples, 46.7% of them were positive nuclear staining (single nuclear, both cytoplastic and nuclear staining). GST-π general positive staining and cytoplastic positive staining were significantly correlated with grade, stage and recurrence after post-operative intravesical chemotherapy. Both of them were also strongly related to the expression of Pgp. While GST-π nuclear positive staining was related to stage but not with grade and recurrence. Conclusions: Overexpression of GST-π can be used as a predictive marker of post-operative intravesical chemotherapy in TCCs. Furthermore increased cytoplastic staining is a better predictor of drug resistance while increased nuclear staining might be associated with progression in TCCs.

Mesenchymal stem cells are a kind of non-hematopoietic adult stem cells with selfrenewal and multilineage differentiation potential, which have special biological characteristics, such as secreting hematopoietic growth factors, reconstructing hematopoietic microenvironment, low immunogenicity, and can be transfected and expressed by exogenous gene. This article summarizes the biological characteristics of MSCs and their models of application to acute radiation disease in animals.

5.Preparation and preliminary evaluation of KGDS-targeted ultrasound contrast agent

Objective To prepare a thrombus-targeted ultrasonic contrast agent and to investigate its targeted ability to fresh blood clots. Methods We first synthesized FITC-KGDS-Palm compound, and then prepared thrombus-targeted microbubbles using "ultrasound & high speed shearing method".Fluorescence labeling thrombus-specific peptides and KGDS,directed at the activated glycoprotein(GP)Ⅱb/Ⅲa receptor of platelets were attached to the surface of lipid microbubbles. The concentration and size of TUCA were measured by Malvern Zeta Sizer Nano-ZS590 and Coulter counter.Immunofluorescence was applied to confirm the conjugation.The conjunct ratio was assessed by flow cytometer (FCM).Results The KGDS-TUCA was straw yellow turbid liquor,and the concentration was 1.5×10~9/mL,and the average size was 1.5 μm. The targeted microbubbles conjugated with the thrombus-specific peptides showed bright green rings by fluorescence microscope.FCM demonstrated that the wavelength of shell of KGDS-TUCA changed greatly,and the conjunct ratio was 90.04%.In vitro study showed KGDS-TUCA remained stable for 48 h at 4 ℃ and target-attached to blood clots and showed good stability.Conclusion The ultrasound & high speed shearing method to prepare TUCA is easy and in favor of purification.KGDS-TUCA has high specific biological activity.The conjunct ratio and stability of KGDS-TUCA are excellent.
6.Arterial plasty and reconstruction of variant hepatic arteries in live donor liver transplantation

ObjectiveTo share the experience of arterial plasty and reconstruction of variant arteries in living donor liver transplantation. MethodsFrom September 2006 to May 2010, 73 living donor liver grafts (64 cases using the right lobe,9 cases using left lobe) were used in patients with end-stage liver disease. The hepatic arteries were evaluated preoperatively with computed tomography and magnetic resonance angiography. Back-table arterial plasty was performed under a microscope or a loupe according to arterial variation. We described technical points based on anatomic variations. There were 13 (17. 8 %) liver grafts with anatomic hepatic arterial variations and all of these cases were subjected to back-table reconstruction with interrupted 8-0 or 9-0 nonabsorbable nylon monofilament sutures according to the diameter of artery. ResultsIn 3 cases, the associate right hepatic arteries that were arisen from superior esenteric arteris (SMA) were reconstructed to cystic arteries. In 2 cases with the associate right hepatic arteries arisen from the abdominal trunk, the right hepatic arteries and associate right hepatic arteries of donors were anastomosed with right hepatic arteries and left hepatic arteries in recipients respectively. In 2 donors, hepatic arteries had branches, which were reconstructed. All of the arterial plasty were conducted on a back table. No arterial thrombosis was found during a postoperative follow-up period of 6 months. ConclusionLive donor liver transplantation using the right lobe with hepatic artery variation can be performed safely, but there is a potential operative risk of severe complication after transplantation. Tominimize operative difficulties and complications, back-table reconstruction should be applied and proper treatment is given according to individual situations to ensure a safe and satisfactory outcome
7.Koyanagi technique for repairing proximal hypospadias with penoscrotal transposition.

OBJECTIVETo investigate the application of Koyanagi technique for repairing proximal hypospadias with penoscrotal transposition.
METHODSKoyanagi procedure was used for repairing proximal hypospadias with penoscrotal transposition in 26 boys (mean age 4.2 years) between July 2005 and June 2010.
RESULTSPrimary healing was achieved in 22 cases. Three patients suffered from urinary fistula and the external urethral orifice in one patient was retracted to penis coronary sulcus, who were re-operated successfully. Two cases showed slight penile ventral curvature. Slight stricture of the urethral external orifice was noted in one case, which relieved after one to two times of urethral sounding every week for eight months. 20 patients were followed up for 6-36 months (mean 10.5 months) with good cosmetic result and normal urethra function. The penoscrotal transposition was also corrected.
CONCLUSIONSThe original Koyanagi procedure might be one of the simple and effective method for repairing proximal hypospadias with penoscrotal transposition.

9.Clinical Study on Reversing Hepatic Fibrosis with Handan Ganle (汉丹肝乐) Capsule

Objective: To observe the therapeutic effect on hepatic fibrosis of chronic hepatitis B by Chinese medicine Handan Ganle capsule (HDGLC).Methods: A total of 104 patients with chronic hepatitis B has been treated by HDGLC for 6 months, liver fibrosis indexes and the serum biochemical indexes were detected before treatment, during the curative period and by the end of treatment. Hepatic biopsy was performed before or after the treatment.Results: The improvement rate of clinical symptoms was 79.0%-90.6%, and the recovery rate of ALT was 72.6%, serum fibrosis indexes such as HA, type Ⅳ collagen and LN were significantly decreased along with the extending course of disease (P<0.05). The pathohistological score of liver was decreased from 7.82±6.22 before treatment to 5.16±3.75 after treatment (P<0.05) and the score of hepatic fibrosis was decreased from 7.49±5.45 before treatment to 5.16±4.26 after treatment (P<0.05).Conclusion: HDGLC has remarkable therapeutic and reversing effect on chronic hepatitis B induced hepatic fibrosis.
