1.Application of Quality Risk Management in Drug Withdrawal Links of Hospital Pharmacy

OBJECTIVE:To control the drug quality risk in drug withdrawal links of hospital pharmacy,and provide reference for the application of quality risk management(QRM)in hospital pharmacy. METHODS:Drug QRM was developed in drug with-drawal links through risk identification,risk assessment,risk control,risk assessment and other steps. Using the incidence of risk factors and reusable rate of withdrawal drug as indexes,related data of before(Jul.-Dec. 2015)and after(Jan.-Jun. 2016)devel-oping QRM in our hospital were compared to evaluate the effect of drug QRM. RESULTS:Determining whether specific storage drugs kept the required storage conditions,whether the numbers of withdrawal drugs counted to minimal packaging and other 5 fac-tors were high-risk factors(the risk score of each factor>4 points);control measures were taken separately for high risk factors, then high-risk factors were reduced to acceptable levels(risk score<4 points). After developing QRM,the incidence of risk factors were reduced than before(reduce 1.35%-6.19%),reusable rate of withdrawal drug was increased(98.64% vs. 86.32%)(all P<0.05). CONCLUSIONS:Developing QRM in drug withdrawal links of hospital pharmacy can reduce the drug quality risk.
2.Effects of navigation and robot technology on percutaneous ultrasound-guided liver tumor ablation: Accuracy and feasibility of clinical application

OBJECTIVE: To assess the effects of navigation and robot technology on percutaneous ultrasound-guided liver tumor ablation. METHODS:The liver model tumors were placed inside excised calf livers, and the needle was guided into the tumor using either conventional ultrasound-guided method or two new methods-3D navigation and a method based on a robotic assistant. Totally 40 experiments were performed, and the distance (all methods) from the tip of the needle to the planning point in the tumors was measured.RESULTS: Based on the measurements, the robotic assistant performed better than both the 3D navigation and the conventional ultrasound-guided method. While, the 3D navigation performed better than the conventional ultrasound-guided method.CONCLUSTON: This study demonstrates navigation combined with robot technique is feasible to improve the precision of deploying ablation needles, future studies are warranted to optimize the system to meet requirements in clinical practice.
3.Minimally invasive techniques in the treatment of benign prostatic hyperplasia: An update.

Lower urinary tract symptoms (LUTS) resulting from benign prostatic hyperplasia (BPH) obviously impair the quality of life of middle-aged and elderly men. Current management of BPH includes wait-and-watch, medical therapy, and conventional surgery. As a new approach, minimally invasive surgery has been playing an increasingly important role in the management of BPH, with potential advantages of less operative trauma, quicker recovery, lower risk of postoperative complications and higher quality of life. This review mainly discusses prostatic urethral lift (Urolift® System), transurethral water vapor therapy (Rezūm® System) and robot-guided high-energy water ablation (PROCEPT Aquablation™ System).

4.Disinfection and Isolation in Basic Medical and Health Institutions:Current Situation Issues and Countermeasures

OBJECTIVE To develop the countermeasures how to strengthen disinfection and isolation in the basic(medical) and health institutions.METHODS Through investigation,to analyze the current situation and problem of disinfection and isolation in the basic medical and health institutions.RESULTS There were some problems of the(disinfection) and isolation in the basic medical and health institutions and some possibility for patient catching(nosocomial) infection.CONCLUSIONS It is very important for strictly observing Medical Instrument Surveillance and Management Regulations published by the State Council and Hospital Infection Management Standards,Disinfection Technology Standards and Disinfection Management Methods published by Ministry of Health,and strengthening the management of disinfection and isolation in the basic medical and health institutions.
5.Current Advances in Laparoscopic Splenectomy

Objective To evaluate the operation of laparoscopic splenectomy(LS),its safety and clinical effects.Methods Literature of the advances of LS were reviewed and analyzed.Results With the development of technology and surgical technique,the indications for LS were widened,the size of spleen plays an important role in the clinical outcome of LS.Conclusion LS has all the advantages of minimally invasive surgery.The application of hand-assisted technique is safe and feasible for giant spleen.With the accumulation of surgical experiences and technique development,LS will be extensively used in clinic.
6.Research on the physical components stimulated by rotating grating using functional magnetic resonance imaging

AIM: Using functional magnetic resonance imaging, to inspect v sual cortex physical reactions stimulated by rotating grating, and to its components. METHODS: On 1.5T MR scanner, GRE-EPI imaging sequence was carried on 9 h discover ealthy volunteers, the visual cortex response data were processed after delinearation by SPM99.RESULTS: Different components of rotating grating excited different areas of the visual cortex. Dramatic response in the central part of the occipital lobe, which was related to white light stimuli, located at primary visual cortex. Response area in bilateral Broadaman 19 areas was related to vision-motion function. And weak response area in the central part of the occipital lobe was related to shape perception.CONCLUSION: Rotating grating conclude plenty of visual information, and it excites different areas of the optic center as a stimuli. fMRI is a valuable equipment to study the physiology of visual cortex.
7.A robot for image-guided percutaneous intervention: Decoupled motion capability correlated to the positioning and orientation steps of the percutaneous intervention

The purpose of this study was to develop a robotic system for image-guided percutaneous interventions using ultrasound (US) or computed tomography (CT) for needle biopsy, ablation, cryotherapy, and other needle procedures. The conventional operation of puncture was analyzed firstly, and a compact robot was then designed for manipulating a needle or other slender surgical instrument in the confined space. Its distinctive characteristic is the decoupled motion capability correlated to the positioning and orientation steps of the percutaneous intervention. This approach allowed each step of the intervention to be performed using a separate mechanism of the robot. One major advantage of this kinematic approach was patient safety.
8.Effect of Sanqi Oral Liquid on Podocytes and Slit Diaphragm-associated Molecules Expression in Diabetes Mellitus Rats After Unilateral Nephrectomy

Objective To observe the effect of Sanqi Oral Liquid on the podocytes and the expression of slit diaphragm-associated molecules (including Nephrin, Podocin and CD2AP proteins) of diabetes mellitus (DM) rats after unilateral nephrectomy, so as to explore its mechanism for protecting renal function. Methods SD male rats were randomized into sham operation group, model group and Chinese medicine group. The experimental diabetes mellitus ( DM) model was given unilateral nephrectomy and intraperitoneal injection of 35 mg/kg of streptozocin (STZ). And then the model rats were given intragastric administration of Sanqi Oral Liquid (2.5 g· kg-1·d-1) and the same volume distilled water respectively for 8 weeks. After treatment, the density of podocytes and the podocytic foot process width of different groups were measured. Immunohistochemistry was used to observe the changes of the expression of Nephrin, Podocin and CD2AP in renal tissues of different groups. Results After treatment with Sanqi Oral Liquid, the density of podocytes was increased, the foot process lesion was relieved, and the expression levels of Nephrin, Podocin and CD2AP proteins were increased (P<0.05 or P<0.01 as compared with those in the model group). Conclusion The protective mechanism of Sanqi Oral Liquid for renal function of unilateral nephrectomy DM rats is possibly related with the alleviation of podocyte injury and with the regulation of the expression of Nephrin, Podocin and CD2AP proteins in podocytes.
9.Effect Analysis of the Optimization of Initial Drug Placement in the Automated Drug Dispensing Machine by Minimum Time Algorithm

OBJECTIVE:To optimize the drug placement in the automated drug dispensing machine to improve work efficien-cy. METHODS:Based on the principle of the minimum time algorithm,the drug which would be used at a high frequency was placed in the drug storage tank nearest to the drug outlet. Meanwhile,the rule of drug use was drawn from the information on a large number of prescriptions,based on which the drugs correlated with each other were placed in the drug storage tanks that were adjacent. With daily time it takes to add drugs,average time it takes to make up a prescription and the maximum number of drugs stored as the evaluated indexes,the initial drug placement in the automated drug dispensing machine was optimized. The changes in the indexes within 3 months before and after the above-mentioned optimization were statically analyzed. RESULTS:After calculat-ing the three-dimensional sizes of the packages of drugs and the dispensing frequency data of the previous year,354 drugs were se-lected and placed in the nearer or farther storage tanks in the automated dispensing machine according to the dispensing frequency and the correlation among them. After the optimization of the placement,daily time it takes to add drugs reduced by 54 min(218 vs. 165 min)on average,average time it takes to make up a prescription reduced by 8 s(24 vs.16 s)and the maximum number of drugs stored increased by 1 333 boxes(13 113 vs. 14 446 boxes)on average. There was statistical significance in differences(P<0.05). CONCLUSIONS:The initial drug placement in the automated drug dispensing machine that was optimized by minimum time algorithm has reduced daily time it takes to add drugs and average time it takes to make up a prescription and increased the maximum number of drugs stored and thus improved work efficiency.
10.Study of physiology of visual cortex activated by rotating grating with functional MRI

Objective To research the physiology of visual cortex activated by rotating grating with functional-MRI(fMRI), and to identify the components of the activation. Methods Functional MRI was performed in 9 healthy volunteers by using GRE-EPI sequences on a 1.5 T MR scanner. In the block designing, rotating grating, static grating, and luminance were plotted as task states, while static grating, luminance, and darkness were set as control states. The stimuli tasks included six steps. Imaging processing and statistical analysis was carried out off-line using SPM99 in single-subject method. Results Some respective areas of visual cortex were activated by the various stimuli information supplied by rotating grating. The strong activation in the middle of occipital lobe located at primary vision area was related to the stimuli of white luminance. Its average maximum points were at 13,-98,-2 and 11, -100,-4. The bilateral activations of Brodmann 19th area located at MT area were related to visual motion perception. Its average maximum points were at 46,-72,-2 and -44,-74, 0. The mild activation in the middle of occipital lobe was related to form perception. Its average maximum points were at -12,-98,-6 and -16,-96, (-6.) Conclusion The plotting of control state is important in bock design. The effective visual information of rotating grating includes components of luminance, visual motion perception, and form perception. FMRI has potential as a tool for studying the basic physiology of visual cortex.
