1.Influence of lung suctioning method on mechanical ventilation related pulmonary infection of patients after cardiac valve replacement

Objective To observe the effect of lung suctioning method on mechanical ventilation related pulmonary infection of patients after cardiac valve replacement. Methods 102 patients of mechanical ventilation related pulmonary infection after cardiac valve replacement were randomly divided into the treatment group( 50 patients )and the control group( 52 patients ). The treatment group received intervention of lung suctioning method and the control group adopted routine sputum aspiration. The pulmonary infection rate and blood gas analysis before and after the treatment were observed and the cure rate and the positive rate of specimen of sputum were recorded. Results The pulmonary infection rate of the treatment group was lower than the control group;PaO2 and SaO2 were obviously different after treatment in the control group, but no difference in the treatment group. The cure rate of pulmonary infection patients in the treatment group was higher than those in the control group. Conclusions The intervention of lung suctioning method was a safe and effective method to improve hypoxemia and abate the rate of pulmonary infection.
2.Inhibitory effect of 5-FU and cisplatinmediated by miR-449b on the migration of HCC cells

Objective To investigate the impact of 5-fluorouracil (5-FU) and cisplatin on miR-449b expression in human hepatocellular carcinoma (HCC) and elucidate the molecular mechanism of 5-FU and cisplatin inhibiting the migration of HCC cells.Methods Real-time qPCR analysis was conducted to determine the expression of miR-449b in 50 HCC tissues.RT-PCR assay was performed to detect the expression of miR-449b in HCC cells with 5-FU and cisplatin treatment.The migration of HCC cells with the overexpression of miR-449b was determined by wound-healing assay;Rescue assay was employed to investigate the correlation between 5-FU & cisplatin, miR-449b and the migration capacity of HCC cells;The putative targets of miR-449b were predicted and validated using target prediction programs and immunoblots.Results The expression of miR-449b decreased in HCC tissues (P<0.0001).miR-449b expression increased in HCC cells upon the treatment of 5-FU and cisplatin (P<0.001).The overexpression of miR-449b inhibited the migration of HCC cells (P<0.001).Rescue assay revealed that inhibition of miR-449b to prevent 5-FU and cisplatin induction resulted in suppressed migration in SMMC7721 cells(P<0.05).Catenin-δ was a functional target of miR-449b.Conclusions 5-FU and cisplatin inhibit the migration of HCC cells at least partly via inducing the expression of miR-449b.
3.Effects of benzene and toluene on serum immunological indexes and miRNA-146a,-155 expression in pet-rochemical workers

Objective To investigate the effects of immune system and immuno-related miRNAs expression in low-dose benzene and its homologue mixed-exposed workers in a short time. Methods A total of 40 workers were recruited from a petrochemical factory ,and their blood samples were collected before and after work to detect the levels of IgA,IgG,IgM,IFN-γ,IL-1β,IL-2,IL-8,TNF-αin serum and miRNA-146a,-155 expression in the peripheral blood mononuclear cell. In the meanwhile ,we measured the individual benzene and its homologue exposure level of recruited workers. Results According to the concentration of benzene ,the subjects were divided into benzene-exposed group(20 workers)and control group(20 workers). And toluene exposure level in benzene-exposed group was significantly higher than that in control group(P<0.05). Significant decrease of IgG was found in benzene-exposed workers after one work shift,compared with control group(P<0.05). Moreover,the interaction between benzene and toluene was significant correlated with the decrease of IL-1β and TNF-α(P < 0.05). Additionally,the interaction between miRNA-155 and miRNA-146a was significant correlated with the decrease of TNF-α(P<0.05). Conclusions:Occupational exposure to low-dose benzene and toluene in a short time could decrease immune function. And there may be an effect of miRNAs in the regulation of cytokine.
4.Effect of tea polyphenols on the mRNA and nuclear protein expression of Nrf2/Bach1 in human skin fibroblasts

Objective To evaluate effects of tea polyphenols on the mRNA and nucleoprotein expression of Nrf2/Bach1 in human skin fibroblasts (HSFs).Methods Some HSFs were incubated with tea polyphenols at different concentrations of 0,2.5,5,10,20 and 40 mg/L for 24 hours.Methyl thiazolyl tetrazolium (MTT) assay was conducted to evaluate the proliferative activity of HSFs to screen the optimal concentration of tea polyphenols.Then,some other HSFs were treated with tea polyphenols at this optimal concentration for 24 hours.Real-time quantitative PCR (RT-qPCR) was performed to determine mRNA expression of Nrf2 and Bach1,Western blot analysis to measure nuclear expression of Nrf2 and Bach1 proteins,and immunofluorescence assay to determine the distribution of Nrf2 and Bach1 protein in the cell nucleus.Results MTT assay showed that 5 mg/L tea polyphenols had no obvious effects on the proliferation of HSFs,so 5 mg/L was chosen as the optimal concentration of tea polyphenols for subsequent experiments.HSFs cultured without tea polyphenols served as control group.After the treatment,the 5-mg/L tea polyphenol group showed significantly decreased mRNA and nuclear protein expression of Bach 1 (mRNA:0.629 ± 0.077 vs.0.940 ± 0.033,t =6.397,P < 0.05;protein:1.424 ± 0.171 vs.16.966 ± 1.702,t =15.730,P < 0.05),but significantly increased mRNA and nuclear protein expression of Nrf2 (mRNA:1.467 ± 0.076 vs.0.977 ± 0.091,t =7.133,P < 0.05;protein:6.929 ± 0.121 vs.3.537 ± 0.126,t =33.636,P < 0.05) compared with the control group.Immunofluorescence assay showed increased accumulation of Nrf2 protein,but decreased accumulation of Bach1 protein in the nucleus.Conclusion Tea polyphenols can promote the mRNA and nuclear protein expression as well as nuclear distribution of Nrf2,but suppress the mRNA and nuclear protein expression as well as nuclear distribution of Bach 1,finally exerting antioxidative effects.
5.Study of polymorphisms of genes related to folic acid metabolism among women of child-bearing age from Shanxi.

OBJECTIVETo analyze polymorphisms of genes related to folic acid metabolism among women of child-bearing age from Shanxi.
METHODSBuccal smears were collected from 1070 women of child bearing age with cotton swabs. Sequences of MTHFR C667T and A1298C, MTRR A66G, and SLC19A1 A80G were determined by DNA sequencing. The results were compared with data from other regions of China.
RESULTSFor MTHFR C667T, the wild type homozygote, heterozygous mutants, and homozygous mutants have respectively accounted for 20.5%, 50.3%, and 29.2% of the study group, with the frequency of the mutant T allele being 54.4%. For MTHFR A1298C, these were 68.7%, 29.3%, and 2.0%, with the frequency of mutant C allele being 16.6%. For MTRR A66G, the above frequencies were 51.5%, 41.8%, and 6.7%, with the frequency of the mutant G allele being 27.6%. For SLC19A1 A80G, these were 29.2%, 48.0%, 22.8%, with the frequency of mutation G allele being 46.8%. Compared with other regions of China, women of child-bearing age from Shanxi has shown a significant difference in allelic distribution of MTRR A66G and SLC19A1 A80G (P<0.05).
CONCLUSIONThe polymorphisms of genes related to folic acid metabolism showed significant regional difference. Over half of women from Shanxi have carried high-risk alleles for folic acid insufficiency and should have individualized folic acid supplement.

6.Infantile DiGeorge syndrome: autopsy diagnosis and clinicopathologic analysis in 5 cases.

OBJECTIVETo investigate clinicopathological features of DiGeorge syndrome (DGS).
METHODThe clinical features, histological and immunohistochemical findings were analyzed in 5 cases of DGS by autopsy.
RESULTSFive cases of DGS in male infants aged 4 days, 1 month, 7 months, 10 months, and 13 months respectively. Gross and microscopic observations revealed that thymic cortex was depleted of lymphocytes or showed few, dispersed lymphocytes. The thymic medulla showed predominantly epithelial cells with calcified Hassall bodies as well as lymphocyte depletion. T lymphocytes were also scarce in the tonsils, lymph nodes, spleen, and mucosa-associated lymphatic tissue of ileum. In addition, 3 of the 5 patients also showed parathyroid aplasia or dysplasia, and congenital hypertrophy of the ventricular septum.
CONCLUSIONSThe pathological changes indicate that clinicians should be aware of defects of immune system if the infants suffer from severe infections. Pathologists should recognize the importance of abnormalities of lymphohematopoietic tissues in the diagnosis of primary immunodeficiency diseases such as DGS.

7.Association of MK2 gene polymorphisms with low-density lipoprotein cholesterol and tumor necrosis factor-alpha in Uygur population from Hetian area of Xinjiang.

OBJECTIVETo investigate the association of MK2 gene with low density lipoprotein cholesterol (LDL-C) and tumor necrosis factor-alpha (TNF-Α) between different gender in Xinjiang Uygur population.
METHODSA total of 350 Uygur males and 595 females were recruited randomly from Hetian area. Two single nucleotide polymorphisms (44890c/t, rs 45514798) in MK2 gene were selected and genotyped by Taqman-PCR in these subjects. All subjects underwent questionnaire-based survey, physical examination, measurement of lipid profiles and plasma TNF-Α determination.
RESULTSAmong the male subjects, the concentration of total cholesterol (TC) [TT vs. CT vs. CC: (4.35±1.20) mmol/L vs. (4.69±1.34) mmol/L vs. (4.83±1.44) mmol/L, P=0.033]and TNF-Α [TT vs.CT vs.CC: (106.63±62.39) ng/dL vs. (128.44±86.15) ng/dL vs. (153.06±82.99) ng/dL, P=0.001]were significantly different in 3 genotypes of 44890c/t. However, the LDL-C levels in TT, CT, and CC genotypes of 44890c/t were not different neither in males nor in females [males: (2.64±1.16) mmol/L vs. (2.81±1.28) mmol/L vs. (3.04±1.32) mmol/L, P>0.05; females: (2.42±1.11) mmol/L vs. (2.36±0.99) mmol/L vs. (2.43±1.05) mmol/L, P>0.05]. None of the allele and genotype frequencies of 44890c/tand rs 45514798 were different between high LDL-C group and control group. Linear regression analysis indicated that body mass index (BMI) (beta=0.089) and TNF-Α (beta=0.092) were significantly associated with LDL-C levels in males (P<0.05), while the age, BMI, and waist/hip ratio with LDL-C levels in females (P<0.05).
CONCLUSIONThe nucleotide polymorphisms (44890c/t and rs 45514798) in MK2 gene may not be associated with LDL-C in both males and females in the Uygur population in Hetian, Xinjiang.

8.Epidemiologic characteristics of hand-foot-mouth disease in Guiyang between 2008 and 2010.

OBJECTIVETo study the epidemiologic characteristics of hand-foot-mouth disease (HFMD) in Guiyang between 2008 and 2010.
METHODSThe epidemiologic characteristics of HFMD were analyzed by descriptive statistical methods based on the data from the China Information System for Disease Control and Prevention.
RESULTSA total of 27383 cases of HFMD were recorded in Guiyang between 2008 and 2010. The incidence of HFMD increased from 66.4439/100000 in 2008 to 163.9276/100000 in 2009 and 471.5515/100000 in 2010 (P<0.01). The mortality rate was 0.1026/100000 in 2010, which was significantly lower than in 2009 (0.2821/100000) (P<0.05). HFMD occurrence showed seasonality and reached a peak between April and June. HFMD cases were commonly noted in children under 5 years old, and especially in children under 3 years old. The main detected pathogen was human enterovirus 71 (EV17) in 2009. Whereas in 2010 the disease was mainly caused by CoxA16 and other intestinal viruses.
CONCLUSIONSThe incidence of HFMD in Guiyang increased year by year from 2008 to 2010, but the mortality rate decreased year by year. HFMD occurrence showed an obvious seasonality. HFMD was common in children under the age of five. The main pathogens of this disease included EV17, CoxA16 and other intestinal viruses.

9. Relationship of job stressors,the coping styles and social supports among medical workers in primary and secondary schools in Guangzhou City

OBJECTIVE: To explore the relationship of job stressors,the coping styles and social supports among medical workers in primary and secondary schools in Guangzhou City. METHODS: A total of 211 medical workers from 168 primary and secondary schools in Guangzhou City were selected as study subjects by random sampling method. The relationship of job stressors,the coping styles and social supports were investigated by the Scale for Occupational Stressors in Clinical Medical Workers,the Ways of Coping Questionnaire and Social Support Questionnaire. RESULTS: The total scale score of job stressors was( 90. 5 ± 16. 7),medium level and above of stress scale( total score≥95. 0) accounted for 41. 2%( 87 /211). The median scores of positive coping dimension score and negative coping dimension score were 2. 0 and 1. 0,the total score of social support was( 41. 2 ± 9. 8). The medium level and above of social support scale( total score ≥32. 0)accounted for 86. 3%( 182 /211). The multiple stepwise regression analysis results showed that the clinicians and the parttime school doctors had higher job stress than those medical workers who were not majored in clinic and full-time school doctors( P < 0. 05). The medical workers with higher monthly income and higher score of social support showed higher job stress( P < 0. 05). The medical workers with higher social support had lower job stress( P < 0. 05). CONCLUSION: High job stress was found in the medical workers in primary and secondary schools in Guangzhou City. Positive coping styles and social supports has important role in relieving job stress of school medical workers.
10.Investigation of training intention and satisfaction of nurses in grade A class three comprehensive hospital of Shenzhen city

Objective To investigate the training intention and satisfaction of nurses in grade A class three comprehensive hospital of Shenzhen city , and explore the influencing factors .Methods Six hundred and fifty-four nurses in grade A class three comprehensive hospital of Shenzhen city were surveyed by the questionnaire about the training intention and satisfaction , and the SNK-q test was used to compare the training intention and satisfaction of different demographic characteristics of nurses .Results The total scores of the training intention and satisfaction of 654 nurses were respectively (2.10 ±0.67) and (2.72 ±0.49), and the training intention was high but low in training satisfaction .The young and junior nurses had higher training intention, and head nurses and above duty of head nurse had highest .The difference in training intention was found in the different department , and the training intention of out-patient clinic nurse was significantly lower than those in other department .Conclusions Nursing administrators should establish an open , flexible, efficient, personalized and interactive training mode , and provide more abundant learning resources and the convenient learning platform for nurses so as to improve the training satisfaction and effectiveness .
