OBJECTIVE To study the characteristics and management of fungal infection in severely burn patients.METHODS The clinical data of 51 severely burn patients with fungal infection in our department from 2000 to 2004 were analyzed.RESULTS The infection rate in total 213 severely burn patients was 23.72%.Candidiasis was the most frequent type in fungal infection accounting for 93.98%.The risk factors for fungal infection were the(severity) of burn,tracheal intubations,body catheterizations,long-term use of broad-spectrum antibiotic and(hospitalization).CONCLUSIONS Development and the types of fungi in severely burn patients with fungal infection have their own characterizations and inducing factors.it is important to eluminate the susceptible factors and provide early diagnosis and treatment.

Objective: To explore the protective roll of ifbroblast growth factor 21 (FGF21) in endoplasmic reticulum stress (ERS) induced rat's H9c2 cardiomyocyte apoptosis with its mechanism. Methods: pcDNA4 was used as gene vector, pcDNA4-FGF21 plasmid was constructed and transfected into rat's H9c2 myocardiocytes for 48 h. ERS model was established by 10 μM tunicamycin (TM) induction for 24 h. The experiment was conducted in 4 groups:①Control group,②TM group, the cells were treated by TM,③pcDNA4-FGF21+TM group,④pcDNA4+TM group. The expressions of FGF21, protein kinase R-like ER kinase (PERK) and c-Jun N-terminal kinases (JNK) mediated apoptosis pathway related protein were measured by Western blot analysis; cell survival rate was examined by CCK-8 method and apoptosis rate was detected by TUNEL technique. Results: pcDNA4-FGF21 vector was successfully constructed and overexpressed in H9c2 myocardiocytes. Compared with Control group, TM group and pcDNA4+TM group had up-regulated endogenous FGF21 expression, increased PERK and JNK mediated apoptosis pathway related protein expression; reduced cell survival rate and elevated apoptosis rate. Compared with TM group and pcDNA4+TM group, pcDNA4-FGF21+TM group had down-regulated PERK and JNK mediated apoptosis pathway related protein expression; increased cell survival rate and decreased apoptosis rate. Conclusion: FGF21 overexpression can reduce ERS induced apoptosis rat's H9c2 myocardiocytes which might be partly related for inhibiting PERK and JNK mediated signal transduction of apoptosis pathway.

The index components contents of different time and different stubbles in honeysuckle were measured by HPLC, and were analysis by using the method of SPSS. Results showed that the content of index ingredients of different time had differences, and firstly decreased, then increased with time. The content of index ingredients of different stubbles had significantly differences, and firstly decreased, then increased with time. The chlorogenic acid contents were 2.059%-3.593%. The luteolosid contents were 0.110%-0.171%. Results indicated that the best picking buds time is before seven o'clock in the morning and evening at before and after seven o'clock, the index component content is higher. Picking buds in spring and at autumn index component content is higher; Picking buds in summer index component content is low. The experiment provides theoretical support for quality control in the whole process of the honeysuckle harvested and comprehensive utilization of honeysuckle.

Objective To discuss the value of MR T2 mapping in the research of the biomechanics and function of cartilage of knee joint.Methods Knees of 20 healthy adults before and after jogging and 19 osteoarthritis patients were examined with sagittal 8-echo SE sequence.The T2 value of cartilage was selected and calculated.The T2 values in the superficial and deep cartilage of femoral and tibial joint before and after jogging were compared,so did between the osteoarthritis patients and healthy adults.The source images were sent to the workstation to get T2 mappings.The T2 value of cartilage between before and after jogging was compared with paired-samples t test.The T2 value between superficial and deep cartilage before jogging was compared with independent-samples t test,so did between the osteoarthritis patients and healthy adults.Results The T2 values in the superficial and the deep tibial cartilage were(48.8±6.3)ms,(44.3±5.7)ms before jogging and(43.4±5.0)ms,(40.3±6.1)ms after jogging.The T2 values were significantly different between before and after jogging(t=6.004 and t=5.037,P＜0.05).There was a significant difference between superficial and deep tibial cartilage before jogging(t=3.148,P＜0.01).The T2 Values in the superficial and deep femoral cartilage were(52.1±5.7)ms,(47.7±5.3)ms before jogging and(47.2±4.5)ms,(43.6±4.1)ms after jogging.The T2 values were significantly different between before and after jogging(t=6.169 and t=5.957,P＜0.05).There was a significant difference between superficial and deep femoral cartilage before jogging(t=3.384,P＜0.01).The T2 mapping showed those changes.The mean T2 value in the tibial cartilage of osteoarthritis patients was(56.0±9.1)ms and was higher than that of healthy adults.There was a significent difference between osteoarthritis patients and healthy adults(t=-3.446,P＜0.01).Conclusion T2 mapping can be used in the research of biomechanics and function of cartilage and has a prelimilary value in the diagnosis of cartilage degeneration.

OBJECTIVETo evaluate the outcome of Titanium elastic intramedullary nailing (TEN) for the treatment of displaced midclavicular fractures.

METHODSBetween February 2010 and February 2013, 62 patients with displaced midclavicular fractures were treated by TEN, including 27 males and 35 females with an average age of 37.6 years old ranging from 15 to 67 years. The course of disease was from 1 to 9 days (means 2.7 days). Thirty-nine cases were treated by closed reduction and 23 cases by assistant small incision. Based on OTA (the Orthopaedic Trauma Association classification) classification, 31 cases of simple fractures involved 5 cases of 06-A1,15 cases of 06-A2,11 cases of 06-A3; 31 cases of wedge fractures involved 4 cases of 06-B1,12 cases of 06-B2,15 cases of 06-B3. Postoperative pain relief were evaluated by VAS score,and operation time,fracture healing time were recorded. After 6 weeks and after removing internal fixation shoulder joint function was evaluated by Constant score, and shoulder joint function were assessed by Herscovici score after 6 weeks.

RESULTSExcept 2 cases were lost to follow-up, 2 cases did not remove internal fixation, 3 cases' fractures were nonunion, the remaining 55 patients received follow-up for an average time of 11.4 months. The average preoperative VAS score was 5.20±0.71, and it turned to be 1.550.59 at 3 days after operation. The average operative duration was 40 min (15 to 65 min). The average bone healing time was (2.71±0.54) months (2 to 5 months). No difference about bone healing time was found between simple fractures and wedge fractures,and between the patients treated by closed reduction and assistant small incision. According to Herscovici standard, the shoulder function was excellent in 49 cases, good in 4, fair in 1, and poor in 1. Simple fractures achieved better Constant score than wedge fractures at 6 weeks postoperatively, whereas no difference was found at 4 weeks after fixation removal.

CONCLUSIONTitanium elastic nails (TEN) for treatment of displaced midclavicular fractures has advantages of good clinical effect,rapid pain relief,fast recovery of shoulder joint function. For OTA classification type B patients with comminuted fracture can replace steel to achieve very good effect, but to grasp the appropriate.

OBJECTIVETo explore the effect of 5-HT2A-receptors coupled with superoxide anion (O2-) on respiratory regulation signal transductionin passageway in the medial area of nucleus retrofacialis (mNRF).

METHODSmNRF island was prepared from medullary slices of neonatal SD rats according to Johnson's method and transferred separately into 24-well culture plates with reagents according to protocol, followed by incubation for 60 min at 37 degrees C in a humidified incubator with 5% CO2. Absorbance of 100 microl supernatant was measured by spectrophotometry at 550 nm and the effect of 5-HT and 2,5-dimethoxy-4-iodoamphetamine hydrochloride (DOI, agonist of 5-HT2A-receptors) on O2- generation in the mNRF was observed, along with the inhibition of this effect by ketanserin (antagonist of 5-HT2A-receptors) and alpha-lipoic acid (alpha-LA, a antioxidant).

RESULTS5-HT concentration-response curve demonstrated that absorbance peak occurred at 1 micromol/L without further increment with higher concentration. DOI concentration-response curve showed the absorbance peak at 20 micromol/L without further increment. 5-HT and DOI significantly increased the absorbance with comparable effects. Ketanserin and alpha-LA significantly decreased the absorbance generated by 5-HT and DOI.

CONCLUSIONActivation of 5-HT2A receptors results in obvious O2- production in mNRF, suggesting that 5-HT2A receptors regulate respiratory function in association with O2-.
