1.Content Determination of Chloramphenicol and Salicylic Acid in Acne Tincture by HPLC

OBJECTIVE:To establish a method for the content determination of chloramphenicol and salicylic acid in Acne tinc-ture. METHODS:HPLC method was adopted with the column of Shim-pack CLC-ODS with the mobile phase of methanol-water (50∶50,V/V)at the flow rate of 0.8 ml/min. The detection wavelength was 278 nm,column temperature was 35 ℃,and the vol-ume was 10 μl. RESULTS:The linear range was 2.0-10.0 μg/ml(r=0.999 2)for chloramphenicol and 5.0-25.0 μg/ml(r=0.999 1) for salicylic acid;the RSDs of precision,stability and repeatability tests were lower than 1%;the average recoveries chlorampheni-col and salicylic acid were respectively 99.77%(RSD=0.35%,n=9) and 99.29%(RSD=0.48%,n=9). CONCLUSIONS:The method is simple,rapid,accurate,specific,precise,and can be used for quality control of Acne tincture.
2.Investigation of Off-label Drug Use in Pediatric Outpatient Department of Our Hospital

OBJECTIVE:To provide baseline data of safe and rational drug use in pediatrics. METHODS:The pediatrics outpatient prescriptions were randomly selected from pediatric department of The Second Affiliated Hospital of Xinjiang Medi-cal University during Mar. 2014-Feb. 2015. The behavior of off-label drug use was determined according to the content of drug instruction. The type of off-label drug use,the relationship of different age groups with various drugs were analyzed. RE-SULTS:A total of 2400 outpatient prescriptions were collected from pediatric department,5115 medication records were ana-lyzed,involving 171 kinds of drugs. By the number of prescriptions,medication records and drug types,the incidence of off-la-bel drug use were 59.08%,40.87% and 80.70%,respectively. The main categories of drug off-label use were off-label dosing frequency(43.03%),off-label dosage(29.55%)and off-label indication(18.83%). The incidence of off-label drug use in in-fants and adolescents ranked the top 2 places,being 67.58%,63.01%. The top-four drugs in the list of the incidence of off-la-bel drug use were drugs for external use(55.56%),drugs for nerve system(54.12%),drugs for respiratory system(48.95%) and nutritional agents(48.09%). CONCLUSIONS:The phenomenon of drug off-label use is widespread in pediatric outpatient prescriptions.
3.Effect of modified Tongyou decoction combined with FOLFOX4 chemotherapy on pre-thrombus state of patients with advanced prostatic cancer

Objective To analyze the effect of modified Tongyou decoction combined with FOLFOX4 chemotherapy on pre-thrombus state and curative effect of patients with advanced prostatic cancer.Methods 142 patients with advanced prostatic cancer were collected,all patients were randomly divided into study group and control group,71 cases in each group.The control group was given FOLFOX4 chemotherapy and the study group was given modified Tongyou decoction on the baseis of the control group.After treatment,the serum levels of homocysteine(Hcy),D-two dimer(D-D),fibrinogen(FIB),the level of hemorrheology,clinical effect,the incidence of venous thrombosis in one years,the six month survival rate,the one year survival rate were detected in all patients.Resutls After treatment,compared with control group,the serum levels of Hcy,D-D,FIB,the whole blood high,middle and low-shear viscosities,plasma viscosity and the incidence of venous thrombosis in one years were higher in the study group and the difference was statistically significant(P<0.05);the erythrocyte deformability index,effective rate and one year survival rate were higher were higher in the study group and the difference was statistically significant(P<0.05).Conclusion The modified Tongyou decoction combined with FOLFOX4 chemotherapy in treatment of patients with advanced prostatic cancer can improve the prethrombotic state,improve the therapeutic effect,reduce the incidence of venous thrombosis and increase the one year survival rate,and have a guiding significance for clinic.
4.Risk and Precautions against Single Dispensing in Emergency Pharmacy

OBJECTIVE:To reduce the risk of single dispensing,and to improve the quality of pharmaceutical care in emergen-cy pharmacy. METHODS:Single dispensing environmental factors,human factors and social factors of emergency pharmacy in our hospital were analyzed;the suggestions and measures were put forward,risk intervention mechanism was established and manage-ment effects were evaluated. RESULTS:Through improving the pharmacy hardware and software,carrying out the relevant sys-tem,enhancing training,improving the process of dispensing and window order,promoting the drug management and carrying out near error management. Compared with before management(Oct.-Dec. 2014),the times of near error and disputes significantly re-duced after management(Feb.-Apr. 2015)(P<0.05),and medication error reduced to zero. CONCLUSIONS:As single dispens-ing,risk management and the improvement of prescription audit ability can improve pharmaceutical care in emergency pharmacy.

6.Study on Stability of Qingrejiedu Granules

Objective: To evaluated the granules stabilities in four conditions by taking the change of Baricalin content and granules surface color as stability test markers. Methods: The content of baicalin was determined by dual wave length iso absortion method and the granules surface color was determined by the diffuse reflectrance spectroscopy. ① The granules were tested in a simulated summer environment (30 ?C , RH92.5%, 1month) and in a high intensity light condition (3000LX, 10dyas). ② The influence of humidity on the stability of the granules was investigated. ③ The expiry date of the granules was predicted by the mutivariate linear regression analysis and compared with the result of the classical constant temperature method. ④ The influences of temperature and humidity on the granules stability were etudied by orthogonal test. Results: ① There was hardly any change in baicalin content of samples in a simulated summer environment and in a high intensity light condition. ② The results showed that the decrease of baicalin content and charge of granules color were obvious when the relative humidity of environment was high and the granules color was more easily influenced by the humidity. ③ The expiry date was about 2.87 years. ④ The results indicated that both temperature and humidity had effects on the change of baicalin content. Conclusion: We tried to estimate the stability of the granules by two indexes. The results showed that Qingrejedu granules was stable in different conditions. It satisfied the stability requirements of solid dosage form.
7.The impact of lymph node metastatic rate on clinical outcome following131I therapy in patients with papillary thyroid carcinoma

Background and purpose:This study aimed to investigate the relationship between lymph node metastatic rate (LR) and response to radioiodine therapy in patients with papillary thyroid carcinoma (PTC).Methods:A total number of 143 PTC patients after radioiodine therapy were included and classified into 4 groups [Ⅰ(0%-10%),Ⅱ(>10%-25%),Ⅲ(>25%-50%),Ⅳ(>50%)] according to the lymph node metastatic rate, and the responses to initial radioiodine therapy after a median follow-up period of 20.7 months were evaluated. They were classiifed into 4 groups [excellent response (ER), indeterminate response (IDR), biochemical incomplete response (BIR), and struc-tural incomplete response (SIR)] according to the guideline proposed by 2015 American Thyroid Association. One-Way analysis of variance,χ2 test and Kruskal-Wallis test were used to evaluate the differences in basic clinicopathological features and clinical responses among the 4 groups. The ROC curve was analyzed to evaluate the clinical value of lymph node metastatic rate for predicting ER and optimal cut-off point.Results:There were no signiifcant differences in gender and T-stage among 4 groups (P>0.05). However,Ⅰ group was signiifcantly older than the other 3 groups (P=0.001). With the increase of lymph node metastatic rate, the number of ER cases decreased, while cases of BIR and SIR generally increased. Compared with the other 3 groups, less cases of ER (27.8%), while more BIR (27.8%) or SIR (11.1%) were observed in groupⅣ (H=18.816,P=0.000). Cut-off value of lymph node involved rate was 52.27%, with a better speciifcity of predicting ER. Area under the ROC curve was 0.668.Conclusion:The higher lymph node metastatic rate in patients with PTC, the worse clinical outcome it could be. A cut-off value of lymph node metastatic rate 52.27% is a speciifc independent predictor for the clinical outcome in PTC patients treated with radioiodine therapy.
8.Study on Bioequivalence of Levodopa Micro-capsule Floating Tablets in Beagle Dogs after Multi-dose Ad-ministration

OBJECTIVE:To study the bioequivalence of Levodopa micro-capsule floating tablets in Beagle dogs after multi-dose administration. METHODS:6 dogs were collected and divided into Levodopa micro-capsule floating tablets group and Com-pound levodopa preparation group (Benserazide tablet,reference preparation). They were given levodopa 200 mg intragastrically, every 8 h,for consecutive 4 day. In two-period crossover test,HPLC method was established to determine the concentration of le-vodopa in dog. The pharmacokinetic parameter,bioequivalence and plasma concentration fluctuation of steady state were calculated. RESULTS:The main pharmacokinetic parameters of Levodopa micro-capsule floating tablets and reference preparation were as that cmax were(4.23±0.75)and(8.47±1.18)μg/ml;AUC0-∞ were(12.18±1.16)and(13.81±2.12)μg·h/ml;tmax were(1.83±0.26) and(0.67±0.13)h,respectively. 90% confidence intervals for the geometric mean ratio of AUC0-∞ for test and reference prepara-tion were 80.61%-97.90%,and that for cmax were 42.75%-57.63%,respectively. There was statistical significance in tmax between test and reference preparation. Degree of fluctuation of test and reference preparation at steady state were(283.914±43.217)% and (506.489±78.965)%,and fluctuation coefficient were(177.463±7.873)% and(187.405±1.650)%,respectively. The degree of fluctuation of test preparation was significantly less than that of reference preparation. CONCLUSIONS:Levodopa micro-capsule floating tablets show good sustained-release property,and are bioequivalent with reference preparation in absorption after multiple dose administration. It also has lesser fluctuation of blood concentration.
9.The relationship between the number of dissected central lymph nodes and clinical outcome in pN1a papillary thyroid carcinoma

Background and purpose: Neck lymph node metastasis, most of which presents in central neck compartment, is common in patients with papillary thyroid carcinoma (PTC). The objective of this study was to investigate the relationship between the number of dissected central neck lymph nodes and clinical outcome after radioactive iodine (RAI) ablation in pN1a PTC with no more than 5 lymph nodes involvement. Methods: A total of 167 PTC patients who had 1-5 proven metastatic lymph nodes according to postoperative pathological diagnosis were retrospectively analyzed, all of whom underwent total or near total thyroidectomy and central lymph node dissection. After a median follow-up period of 26 months, the clinical outcome of each patient was evaluated as excellent response (ER), indeterminate response (IDR), bio-chemical incomplete response (BIR), or structural incomplete response (SIR) according to the new American Thyroid As-sociation guidelines. The accumulative ER rate (ERn) was calculated in patients with different numbers of dissected lymph nodes (ERn was defined as the proportion of patients who achieved ER with the dissected lymph node number of ≤n). The relationship between the number of dissected central neck lymph nodes and ERn were investigated. Results: As the increase in the number of dissected central neck lymph nodes,there was also an overall increase in ERn, especially when n rose from 1 to 10. The values of ER1, ER5, ER10 and ER30 were 25.0%, 66.7%, 74.7% and 79.1%, respectively. Besides, the proportion of patients who achieved ER was higher in those with 10 or more dissected lymph nodes than in those with less than 10 (85.7% vs 73.3%, P=0.05). In the multivariate logistic regression analysis, both the dissected central lymph node number of ≥10 (OR=2.720, 95%CI: 1.052-7.033, P=0.039) and the level of preablation stimulated thyroglobulin (OR=0.955, 95%CI: 0.926-0.984, P=0.003) were shown to contribute independently to ER. Conclusion: As the increas-ing number of dissected central neck lymph nodes, the percentage of pN1a PTC patients that achieved ER after RAI ablation generally rises. In pN1a PTC patients with no more than 5 lymph nodes involvement, a central compartment dissection with 10 or more lymph nodes might help them achieve ER after RAI ablation.
