1.Clinical Applification of Combined Detection of Pencilliosis Marneffei in a Non-HIV-infected Child

Objective To study the diagnostic value of pencilliosis marneffei (PM)in a non-HIV-infected child with the com-bined detection of aspergillosis galactomannan,fungus Glucan(1-3)-β-D and boold culuture.Methods The venous blood specimen from the child was collected for the quantified detection of aspergillosis galactomannan,fungus Glucan(1-3)-β-D. The growth and colonial morphology of fungus was inspected with the positive blood culture and the characteristics of fun-gus smear were observed under microscope.Results The result of aspergillosis galactomannan was 14.45 μg/L and fungus Glucan (1-3)-β-D 77.14 pg/ml.Penicillium marnrffei was identified using blood culture.It was mycelia form under 25℃ and the salouraud medium produced water soluble claret-red pigment produced.It was mycelia form under 35℃ and the colony was gyri creases,the characteristic broom-like hypha and separation hypha could be found under microscope.Conclusion It is effective for the early diagnosis and therapy of PM with the combination detection of aspergillosis galactomannan,fungus Glucan (1-3)-β-D and boold culuture and have better clinical diagnosis value.
3.The clinical effects of Tanreqing in the treatment of radiation pneumonia

Objective To evaluate the clinical effect of Tanreqing in the treatment of radiation pneumonia. Methods 75 patients with radiation pneumonia were randomly divided into treatment group and control group according to the registration of this study.38 cases in the treatment group treated by Tanreqing,antibiotics and glucocorticoid,while 37 cases in the control group was given antibiotics and glucocorticoid.Then,the therapeutic efficacy was compared.Results From the chest routine scan result and the clinical symptom release,the excellence rate of the treatment group was 68.4%,which of the control group was 43.2%,there was statistically significant difference between the two groups (χ2 =4.823,P =0.028 ).Conclusion Treating radiation pneumonia with Tanreqing has good effect,and it is worthy of clinical application.
4.Preliminary Discuss on Identification and Characterization of One Decolorizing Fungus

According to morphological and microscopic characteristics, a high-efficient decolorizing fungus, the strain Asaw117, was identified as Aspergillus awamori. Selecting eight different dyes from Azo dyes, anthraquinones dyes and oxygen Quinones dyes, the decolorizing assays of various dyes showed that the strain Asaw 117 was the highest decolorizing potential to 0.1 g/L Vat Blue RSN, the discoloration rate up to 100 percent. Comparing to different kinds of medium and several of carbon and nitrogen sources, the strain had the best decolorizing efficient although grew slower in the Czapek medium, otherwise, grew quicker and decolorizing efficient lower in the PDF medium. It could use Vat Blue RSN as a nitrogen source, but not as a carbon source. The medium composing of saccharose and ammonium nitrate as carbon and nitrogen sources was decolorizing potential markedly during different combinations of carbon and nitrogen sources. So the strain has good potential for the dyeing wastewater treatment
5.Efficacy evaluation of limited internal fixation with posterior titanium plates for unstable atlas fractures

Objective To determine the clinical efficacy of limited internal fixation with posterior titanium plates for unstable atlas fractures.Methods A retrospective case series analysis was performed for 29 patients with unstable atlas fractures immobilized using the posterior titanium plate from January 2011 to November 2014.There were 22 males and seven females,with a mean age of 41.5 years (range,17-68 years).Ten patients were diagnosed with 1/2 ring Jefferson fractures,12 anterior 1/2 Jefferson fractures and seven patients posterior 3/4 Jefferson fractures.Frankel neurological performance scale was grade E in all patients.Operation time,amount of bleeding during operation and postoperative complications were recorded.X-ray and CT were performed to identify the fusion,internal fixation location and atlantoaxial stability.Visual Analogue Scale (VAS) for craniocervical pain and cervical range of motion were evaluated.Results Mean duration of procedure was (86.6 ± 7.8) min (range,68-122 min) and mean blood loss was (120.0 ± 25.3) ml (range,90-400 ml).No vascular and neurologic complications were noted.All patients were followed up for (22.5 ± 18.0)months (range,12-32 months).CT scans confirmed bony union in all patients 6 months after operation.No plate-related complications,implant lessening and implant breakage occurred during the follow-up.Postoperative VAS [(1.9 ± 1.1) points] was significantly lower than that before operation [(7.2 ± 1.4) points] (P < 0.05).At the last follow-up,the ranges of cervical flexion-extension [(81.3 ± 22.2) °] and rotation [(152.4 ±38.7) °] were close to the normal range.Conclusion Limited internal fixation with posterior titanium plates is a safe and effective surgical method that contributes to good reduction and maximal preservation of cervical physiological motion.
6.Imageology and significances of rotational axes of distal femur on Chinese adults with total knee arthroplasty.

OBJECTIVETo explore clinical significance of rotational axis of distal femur on Chinese adults in total knee arthroplasty (TKA).
METHODSThere were 86 Chinese adults (106 normal knees) including 47 males (53 knees) and 39 females (53 knees), 54 knees were on left and 52 on right. The CT scan was employed in the distal femur. The scan direction was aligned to be on the plane perpendicular to the mechanical axis of the femoral. The CT images of cross sections across lateral and medial femoral epicondyle were moved to personal computer,lateral angle between anterior posterior line (APL) and surgical transepicondylar axis (STEA) (ATA),lateral angle between posterior condylar line (PCL) and APL (APA), angle between perpendicularity of APL and PCL (A-PA), posterior condylar angle (PCA), condylar twist angle (CTA), angle between clinical transepicondylar axis (CTEA) and STEA (CSA) were measured. These values were divided into different groups according to gender and side, the values of CTA, PCA, A-PA, angle PT (varus of tibia plateau), constant 3, ATA, APA and constant 90° were compared by statistically. A-PA and PCA, and CTA were analysed statistically with the liner regression, the relationship among CTEA, STEA ,PCL, APL and PLP were performed to assess by liner regression.
RESULTSATA was (89.79 ± 1.22)°, APA was (84.84 ± 1.83)°, A-PA was (5.16 ± 1.83)°, PCA was (4.80 ± 1.23)°, CTA was (8.23 ± 1.40), CSA was (3.45 ± 0.68)°. All the parameters had no differences on sex and side,but CSA had difference on male and female. There was no difference among angle PT, PCA, A-PA. There was significant difference in CAT, constant 30 and angle PT, PCA,A-PA. There was no difference between ATA and constant 90°, but there was difference between APA and constant 90°. There was relativity between PCA and CTA, and also PCA and A-PA, CTA and A-PA. There was significant relativity between STEA and CTEA, between STEA and APL, between STEA and PCL, and also between APLP, APL and PCL, but there was no significant relativity between PCL and CTEA.
CONCLUSIONTKA for Chinese, the section of femoral posterior condyle should be external 5° to obtain the optimum rotational orientation. The property is different entirely between STEA and CTEA, the rotational alignment is not performed according to parallel to the CTEA in distal femur. Among STEA, APL, PCL, the STEA is the most reliable mark ofrotational alignment of the distal femur, and the PCL is the less reliable mark.

7.New progress on three-dimensional movement measurement analysis of human spine.

Spinal biomechanics, especially the range of spine motion,has close connection with spinal surgery. The change of the range of motion (ROM) is an important indicator of diseases and injuries of spine, and the essential evaluating standards of effect of surgeries and therapies to spine. The analysis of ROM can be dated to the time of the invention of X-ray and even that before it. With the development of science and technology as well as the optimization of various types of calculation methods, diverse measuring methods have emerged, from imaging methods to non-imaging methods, from two-dimensional to three-dimensional, from measuring directly on the X-ray films to calculating automatically by computer. Analysis of ROM has made great progress, but there are some older methods cannot meet the needs of the times and disappear, some classical methods such as X-ray still have vitality. Combining different methods, three dimensions and more vivo spine research are the trend of analysis of ROM. And more and more researchers began to focus on vivo spine research. In this paper, the advantages and disadvantages of the methods utilized recently are presented through viewing recent literatures, providing reference and help for the movement analysis of spine.

8.The changes of monocarboxylate transporter-2 in spinal cord horn in a rat model of chronic inflammatory pain.

OBJECTIVETo investigate the changes in the levels of monocarboxylate transporter-2 in spinal cord horn in a rat model of chronic inflammatory pain.
METHODSMale SD rats weighting 180 - 220 g were randomly divided into two groups(n = 48): normal saline group (NS group), complete Freund's adjuvant group (CFA group). Rats were given injections of CFA 100 µl in left hind paw in group CFA, and an equal volume of saline was given injection in group NS. Mechanical withdraw threshold(MWT) and thermal withdraw latency(TWL) were measured at before injection(T0 and 3 h, 1 d, 3 d, 7 d, 14 d, and 21 d after injection(T1-7). Four rats were chosen from each group at T0-7 and sacrificed, and L4-5 segments of the spinal cord horn were removed for measurement of the expression of monocarboxylate transporter-2 by Western blot analysis.
RESULTSIn CFA group, mechanical hyperalgesia and allodynia appeared on the 3 h after CFA injection, then until the day 14. The expression of monocarboxylate transporter-2 in the spinal dorsal horn of rats in CFA group was significantly higher than that in normal control group at T1-6(P <0.05). The protein level of monocarboxylate transporter-2 was apparently correlated with MWT and TWL(P <0.01 and P <0.05) in CFA group.
CONCLUSIONThe level of monocarboxylate transporter-2 in spinal dorsal horn is significantly increased in a rat model of chronic inflammatory pain and the change may involve in the formation and maintenance of central sensitization in spinal cord of chronic inflammatory uain.

9.Prevalence of coal-burning-fluorosis by factor analysis in Guizhou Province

Objective To investigate the epidemic situation of coal-burning-fluorosis,and to explore key areas for implementing prevention and control measures in Guizhou Province.Methods Survey data of dental fluorosis of children aged 8-10,anti-fluoride knowledge and health-related behaviors of target students and their families before implementing the comprehensive disease control measures in 2010,were analyzed and evaluated using the method of factor analysis in 23 diseased districts.Results ①The three main factors (F) related to the endemic of coal-burning-fluorosis were economic factor (F 1),knowledge factor (F2) and behavior factor (F3):F1 included the detection rate of 8 to 12 years old children's dental fluorosis,iron stove ownership,corn correct drying rate and chili correct drying rate; F2 included stove correct utilization rate,students awareness and head of the household awareness; F3 included corn food washing rate before cooking and chili food washing rate.②In the economic factor score arrangement,the first three were Zunyi,Xiuwen and Qingzhen.In the F2 score arrangement,the first three were Zunyi,Xishui and Xixiu.In the F3 score arrangement,the first three was Liuzhi,Bijie and Xixiu.③After comprehensive score,Zunyi,Qingzhen,Xishui,Xixiu,Puding and Liuzhi Counties were found to be relatively stronger in control of the epidemic of coal-burning-fluorosis; Xiuwen,Renhuai,Guanling and so on 13 counties were found to be relatively moderate,and Tongzi,Hezhang,Shuicheng and Nayong Counties were found to be relatively weak.Conclusions ①The endemic of coal-burning-fluorosis is related with many factors,and the indexes we selected in this study have a strong correlation and sensitivity,which may be used in evaluating the prevention effect of control measures to coal-buming-fluorosis.②The endemic of coal-burning-fluorosis in Guizhou Province is quite different,and key prevention and control measures can be applied according to a comprehensive factor analysis.
10.Causes and strategies of reoperation in cervico-occipital diseases

Objective To summarize the causes and strategies of reoperation in cervico-occipital diseases,so as to minimize the incidence of reoperation.Methods Between January 2000 and January 2012,23 patients who underwent reoperation in our hospital because of severe complications of cervico-occipital diseases were retrospectively analyzed.There were 14 males and 9 females with the age at primary surgery range from 18-52 years.The interval between reoperation to primary surgery was 3 h-11 years.The reasons of reoperation included pseudarthrosis in 15 cases,aggravated neurological deficit in 3 cases,internal fixation failure in 2 cases,cerebrospinal fluid leakage in 1 case and tumor recurrence in 2 cases.All patients presented with continuous neck pain,numbness of the limbs or pyramidal tract signs.11 patients suffered from neurological deficit,with the average JOA score of 7.8.All patients underwent excision of posterior arch or decompression of foramen magnum,iliac bone grafting,atlantoaxial or occipital-cervical fusion.JOA score and radiograph were used to evaluate the surgical outcome during follow-up.Results The operation was completed successfully in all cases.The average surgical time was 160 min (130-310 min),and the average blood loss of 800 ml (range 300 to 2000 ml).No intraoperative vascular,nerve and spinal cord injury.There were no postoperative neurological deterioration,pharyngeal wall and intraspinal infection.Two cases of postoperative wound infection,healing by dressing change and prolong the use of antibiotics.No severe complications were noted.All patients were followed up for an average of 21 months (6 months-3 years).At the final follow-up,the JOA score was significantly better as compared with preoperation,with an average of 13.2.Based on Macnab criteria,there were 11 excellence,8 good,4middle.Bone fusion was achieved in all patients without instrumentation failure at final follow-up.Conclusion The main reasons of reoperation in cervico-occipital diseases are pseudoarthrosis and remnant symptoms,effective fusion and decompression are the key points to reduce reoperation rate in cervicooccipital operation.
