1.Comparative Study on Different Methods for Measurement of Diameter of Patent Ductus Arteriosus in Transcatheter Interventions

Objective To evaluate the value of transthoracic echocardionraphy(TTE)and angiographic appearance for measurement of the diameter of patent ductus arteriosus(PDA)in transcatheter interventions.Methods In 52 patients,the position,diameter and length of ductus arteriosus between the descending aorta and the pulmonary artery were observed and the narrowest diameter of PDA was measured by TTE.In angiographic appearance,pig catheter was selected to perform left lateral aortography and measure the narrowest diameter of PDA.Comparative studies were performed on the numbers measured by 2 methods.Results Aortography showed that 45 patients had funnel shape PDA and 5 patients had tubular shape PDA,while 1 patient had funnel plus tubular shape PDA and 1 patient had residual shunt after operation.TTE showed that 46 patients had funnel shape PDA,4 patients had tubular shape PDA and 1 patient had window shape PDA,1 patient had residual shunt after operation.The mean diameter of PDA measured by angiographic appearance was(3.72?2.51)mm,which was statistically lower than those by TTE(4.47?2.31)mm(t=5.17 P

4.Clinical analysis of anaemia and nutritional situation in the elderly population

Objective To observe the incidence rate of anaemia and analyze the nutritional situation in the elderly population.Methods From April to August 2010,a total of 2052 elderly persons from out-department of Tianjin First Center Hospital were divided into four age groups:65-69years group (n=528),70-74 years group (n=593),75-79 years group (n=423) and 80 years and over group (n= 508).The body mass index (BMI),haemoglobin (Hb),mean corpuscular volume (MCV),triglyceride (TG),creatinine (Cr),serum albumin (ALB) and ferritin (Fer) were tested and evaluated using K-W test.Results (1) Anaemia was found in 90 subjects (4.4 %),4.0% in men and 6.l% in women,9.5% in 80 years and over group.The subjects with Hb level less than 90g/L were all 80 years and over.(2)There were significant differences among the groups in BMI,TG,Cr,ALB,Hb,MCV and Fer levels (all P<0.01).There were significant differences among four groups in Hb and ALB levels (P<0.01 ),and partially differences in TG, Cr and MCV levels (P<0.05).(3)The Hb level was significantly correlated with TG, ALB, Fer and MCV levels (r=0.115,0.211, 0.114 and 0.089, all P<0.01 ) from Pearson analysis.Conclusions Anemia incidence in elderly population is increased along with aging and is higher in women than in men, especially higher in 80 years and over group.Nutritional situation gets worse with aging.
5.Therapeutic efficacy of gemcitabine combined with cisplatin for advanced non-small cell lung cancer

Objective To evaluate the efficacy and toxicity of the combination of gemcitabine and cisplatin in the treatment of patients with advanced non-small cell lung cancer (NSCLC). Methods A total of 30 cases of non-small cell lung cancer (stages Ⅲ and Ⅳ) were treated with gemcitabine (1 200 mg/m 2, iv infusion, at 1 and 8 d) and cisplatin (100 mg/m 2, iv infusion, at 1 d or cisplatin at 30 mg/m 2, iv infusion, at 1 and 8 d). The administration course was 28 d. Results An objective response was obtained in 46.67% of patients (2 complete and 12 partial responses), whereas 11 patients had no change and 5 patients were progressive. The response rate was 52.38% in patients with no prior chemotherapy and the response rate of 33.33% was achieved in patients who had been given prior treatment. Objective To study the mechanism of multicellular drug resistance of pulmonary adenocarcinoma cell line A549 mediated by cell-cell adhesion. Methods We compared the sensitivity of monolayer cells (MCs) to adriamycin (ADM) with that of multicellular spheroids (MCSs) which was employed as a three dimensional cell culture model. Transmission electron microscopy was applied to observe the cellular ultrastructure. Bcl-2, Bcl-xL, and Bax expression levels were detected by flow cytometry and indirect immunofluorescent staining. Results Compared with MCs, MCSs had more than two layers of cells, more extensive and compact cell adhesion, and inlay junctions were found within them. MCSs were more resistant to ADM. At the same time, Bcl-2 and Bcl-xL expressions in MCSs were much higher. After treatment with ADM, expressions Bcl-2 and Bcl-xL increased markedly in MCSs. Conclusion MCSs could simulate the solid tumors in vivo and has multicellular drug resistance mediated by cell-cell adhesion. The possible mechanisms may be associated with the upregulation of Bcl-2 and Bcl-xL.
7.Effect of sapindoside on primary hypertension rat

AIM: To observe the effects of sapindoside on blood pressure,AngⅡ,Ald,ET in the blood plasma and NO in the serum in primary hypertension rat. METHODS: The primary hypertension rats were divided into 5 groups(high,middle and low sapindoside group,control group and captopril group,10 each) in a random fashion and drugs had been given by ig.for 32 days.The blood pressure was messured in the 1,3,7,32 day after administration.At the end of the 32 nd days,AngⅡ,Ald,ET in the blood plasma and NO in the serum were measured. RESULTS: Sapindoside could significantly lower the blood pressure,increase the levels of NO in serum,reduce the concentration of AngⅡ,Ald,ET in the blood plasma. CONCLUSION: Sapindoside plays an important role in decreasing the blood pressure of primary hypertension rat.

9.Construction of three dimensional in vitro culture model of pulmonary adenocarcinoma and detection of its drug resistance

Objective To explore an optimal model of three dimensional in vitro cell culture for simulating solid tumors in vivo . Methods The model of three dimensional cell culture was constructed under the conditions of inhibiting the cell wall attachment and stirring the medium. Multicellular spheroids (MCS) were cultured using microcarriers (CutiSpher). Drug sensitivity of monolayer cells (MC) and MCS was tested by MTT staining and cytometry, respectively. Ultrastructures of the MC and MCS were observed by transmission electron microscopy. Results Cells in three dimensional cell culture model without microcarriers were compacted into mass at 4 d while cells in MCS were found to attach to the microcarriers at 0.5 h. MCS had more than two layers of cells growing within it at 5 d. Compared with MC, MCS was more resistant to the anticancer drug, and had more plenty of organell and microvilli with more extensive and compact cell adhesion. Conclusion MCS has strong developmental properties and can simulate the cell cell interactions in vivo , especially cell adhesion, which may contribute to the drug resistance of MCS.
10.Uniportal video-assisted thoracoscopic anatomic segmentectomy for lung diseases: 52 cases report

Objective To evaluate the feasibility and safety of uniportal Video-assisted thoracoscopicsurgery(VATS) anatomic segmentectomy for lung diseases.Methods We performed a retrospective review of 52 patients undergoing uniportal VATS anatomic segmentectomy from Mar 2015 to Dec 2015.There were 16 males and 36 females with a mean age of 52.7 years (32-82 years).The incision,about 4 to 5 cm long,is performed at the fourth or the fifth intercostal space.The camera and the instruments are all through the single incision.Results Fifty-one patients underwent uniportal VATS segmentectomy successfully.1 patient with extensive pleural adhesion was needed for auxiliary incision.The median operative time was 125 min (60-240 min),the median blood loss in operation was 60 ml (10-300 ml),the median hospital stay after operation was 4.6 days(2-14 days).There was no perioperative mortality.Major morbidity occurred in 7 patients(13.5%).Pathological examination showed that there were 10 cases of benign diseases and 42 cases of non-small cell lung cancer(26 cases of carcinoma in situ and micro invasive adenocarcinoma,16 cases of infiltrating adenocarcinoma,mucinous adenocarcinoma and carcinoid).Conclusion Uniportal VATS segmentectomy has the advantage of less intercostal nerve injury and good operative perspective.It is a safe and feasible procedure after surgery practice.
