Objective To study the effects of Xiaofeng Powder on IL-4,IFN-? and sIL-2R in delayed hypersensitivity and to explore the mechanism of treatment. Methods The mice model of allergic contact dermatitis was formed by DNCB. The effect of Xiaofeng Powder on ear swelling and leukocyte counts of the model mice were observed. The serum levels of interleukin-4 (IL-4),interferon-gamma (IFN-?) and soluble interleukin-2 receptor (sIL-2R) of the mice were detected by enzyme-linked immunosorbent assay (ELISA). Results Xiaofeng Powder could inhibit ear swellings and obviously reduced the total amount of leucocytes. Xiaofeng Powder could heighten IL-4 level and reduce the level of IFN-?,sIL-2R. Conclusion Xiaofeng Powder has the function of inhibiting delayed hypersensitivity. The mechanism of treatment may be related to inhibiting the total amount of leucocytes,regulating cell factor and receptor.

Objective To investigate the clinical value of joint detection of procalcitoninl (PCT ) ,C-reactive protein (CRP) ,and myocardial enzymes in patients with neonatal hyperbilirubinemia .Methods 30 cases of each cause were included in the study ,which were neonatal hyperbilirubinemia caused by sepsis ,pneumonia ,ABO hemolytic and breast milk jaundice .20 cases of each cause were selected including neonatal hyperbilirubinemia caused by neonatal hepatitis and low birth weight infants .All the cases involved in the study were diagnosed .30 healthy full-term newborns in the same period were recruited as control group .PCT ,CRP and myocar-dial enzymes (AST ,CK ,CK-MB ,and LDH) concentration in serum were determined .Results Compared with the control group , CRP and PCT concentration increased significantly in bacterial infected group (P< 0 .05) .Myocardial enzyme concentration in-creased significantly both in infected group and non-infected group(P<0 .05) .Among myocardial enzymes ,CK-MB concentration increased significantly in bacterial infected group ,low birth weight infants group and ABO hemolytic group(P<0 .01) and increased significantly in breast milk jaundice group and hepatitis group (P<0 .05) .AST and LDH concentration increased very significantly in hepatitis group(P<0 .01) .The specificity of PCT in bacterial infected group was significantly higher than that of CRP (P<0 .05) ,while its sensitivity was significantly lower than that of CRP in pneumonia group (P<0 .01) .Conclusion Changes of PCT , CRP and myocardial enzymes concentration are related to the occurrence and development of neonatal hyperbilirubinemia ,determi-nation of these indicators can be complementary .

Objective To assess the diagnostic accuracy of NT-proBNP in hypertension patients by observing the effect of decreased GFR on N-terminal pro-brain natriuretic peptide (NT-proBNP) concentration in patients with different cardiac function.Methods Eighty-nine hypertension patients were divided into two groups based on the results of UCG.Forty-three patients had normal left ventricular function and 46 patients had dysfunction.GFR,NT-proBNP and other biochemical markers of cardiac and renal function were measured.The factors affecting the NT-proBNP concentration under normal left ventricular function were analyzed,and the diagnostic value of NT-proBNP affected by normal or decreased GFR(＞ 80 ml/min or ≤80 ml/min) under different left ventricular functions were further assessed.The two-sample t test,rank sum test and multiple regression analysis were used to analyze their statistical difference and relationship.Results In patients with normal left ventricular function,GFR (β =-0.361,P ＜ 0.05) and left ventricular end-diastolic diameter (LVEDD,β =0.385,P ＜ 0.05) were significant factors to NT-proBNP level.They were both meaningful compared with LVEF (β =0.189,P ＞0.05) and septal thickness (β =0.003,P ＞ 0.05).The median concentration of NT-proBNP was 13.18 and 24.14 μg/L in patients with normal left ventricular function and dysfunction,respectively (Z =-3.19,P ＜ 0.01).While in patients with decreased GFR,6 cases with normal left ventricular function and 19 cases with dysfunction had a median concentration of NT-proBNP of 38.45 and 44.20 μg/L,respectively (Z =-0.45,P ＞ 0.05).In patients with normal GFR,37 cases with normal left ventricular function and 27 cases with dysfunction had a median concentration of NT-proBNP of 12.51 and 20.31 μg/L,which was lower than that of patients with decreased GFR (Z =-2.76,both P ＜ 0.05).The NT-proBNP concentration had no significant difference between patients of normal left ventricular function with decreased GFR and patients of dysfunction with normal GFR (38.45 and20.31 μg/L,Z=-2.18,P＞0.05).Conclusion In hypertension patients,the effect of decreased GFR on assessing the diagnostic accuracy of NT-proBNP for left ventricular dysfunction should be considered.

Objective To compare the effects of dexmedetomidine on the respiration and circulation with midazolam in mechanically ventilated patients.Methods A total of 62 patients supported with mechanical ventilation in medical intensive care unit were randomly (random number) divided into two groups:midazolam group ( group A,n =38 ) and dexmedetomidine group ( B group,n =24).The patients in group A were intravenously administered with midazolam in bolus dose of 0.05 mg/kg given over 30 - 60 s,and then maintained with 0.05 -0.15 mg/(kg ·h) by venous pump.The patients in group B were intravenously administered dexmedetomidine in bolus dose of 1.0 μg/kg given in more than 10 min,and then maintained with 0.2 - 0.7 μg/( kg · h) by venous pump.When the patients' sedation degree were required to reach Ⅲ - Ⅳ Ramsay Grade,the heart rate,mean arterial pressure,respiratory rate and SpO2 before sedation were compared with those 10 min,30 min and 2 hours after sedation between two groups.Results Respiratory rate or heart rate after sedation significantly decreased compared with those of presedation ( P ＜ 0.05 ).Blood pressure after sedation did not decrease significantly compared with that of presedation in Dexmedetomidine group.In midazolam group,blood pressure was (83.11 ± 12.95 ) mm Hg before sedation,and there was a significant decrease to (74.13 + 12.50) mm Hg in 10 min after sedation (P ＜ 0.05 ),while there were no significant difference between ( 80.53 + 12.93 ) mm Hg 30 min after sedaton and (82.47 ± 12. 15 ) mm Hg 2 hours after sedation.Conclusions There is no difference in effects on respiration of mechanically ventilated patients between dexmedetomidine and midazolam,and the effect of dexmedetomidine on circulation was less than that of midazolam.

Abstract?AIM:To compare the loss of peripapillary retinal nerve fiber layer ( RNFL ) thickness measurements by spectral-domain optical coherence tomography ( SD -OCT ) in healthy individuals and glaucoma patients with or without progression.? METHODS: A total of 60 eyes, comprising 36 glaucomatous eyes with primary open angle glaucoma (POAG) and 24 healthy controls, were included in the study over a 2-year period.All eyes underwent at least 4 serial RNFL measurements performed by Cirrus OCT every half a year over a period of 2a.Visual field ( VF) testing was performed by using the Swedish interactive threshold algorithm( SITA) Standard 30-2 program of the Humphrey field analyzer within the same week as the optic disc/RNFL photography. By masked comparative analysis of VF test results and optic disc/RNFL photographs, the eyes were classified into non -progressive and progressive glaucoma cases. Longitudinal loss of RNFL thickness was compared.?RESULTS: The mean follow-up time was 2.1 ±0.3a. Seventeen from 36 subjects were identified as progressors. Mean rates of change in average RNFL thickness were significantly higher for progressors compared with nonprogressors (2.46μm/a vs.1.21μm/a;P <0.001 ). Inferior quadrant RNFL thickness were significantly correlated with MD reduction in glaucoma eyes with progression(r=0.423, P=0.03).? CONCLUSION: Longitudinal measurements of RNFL thickness using SD-OCT show a pronounced reduction in patients with progression compared with patients without progression.Inferior RNFL thickness parameters might be more important in discriminating eyes with progressive glaucomatous optic nerve damage.
