1.Investigation of Tryptophan in Gastric Juice by High Performance Liquid Chromatography Coupling with Ultraviolet,Fluorescence Detection and Mass Spectrometric Identification

The HPLC-UV-Fluorescence technique and HPLC-UV-MS technique were used to determine fluorescence molecule in gastric juice of patients with malignant gastric cancer,which is identified as tryptophan. Lidocaine was detected in gastric juice of patients who were examined by gastric endoscopy. A method was developed to determine concentration of tryptophan and lidocaine in gastric juice using HPLC-UV technique,in which Kromasil C_(18) column(150 mm×4.6 mm,5μm) and methanol(containing 0.1% trifluoroacetic acid (TFA))-water(containing 0. 1% TFA) as gradient mobile phase were used. The linear relationships were obtained between the concentration of tryptophan in the range of 0.5 - 200 mg/L and its peak area at 278 run with correlation coefficient of 0.9994,and between the concentration of lidocaine in the range of 20 -5000 mg/L and its peak area at 254 nm with correlation coefficient of 0.9992. The limit of detection is 0.15 mg/L for tryptophan and 5 mg/L for lidocaine. The recovery is 70. 8% - 110. 4% for tryptophan and 87. 1% -116.2% for lidocaine. 38 gastric juice samples of patients with malignant gastric disease and 48 gastric juice samples of patients with benign gastric disease were tested and the concentration of tryptophan in those two groups of gastric juice samples was apparently different.
2.Design and application of quality improvement checklist of catheter-related blood stream infection in the intensive care units

Objective To design quality improvement checklist of catheter-related blood stream infection (CRBSI),in order to improve the nursing quality of ICU and ensure nursing safety.Methods A new ICU quality improvement checklist of CRBSI was designed and applied for the quality control of the patients with central venous catheter from April to June 2012.The using rate of central venous catheter and incidence of CRBSI were observed.Results After application of the checklists,the using rate of central venous catheter was decreased and the incidence of CRBSI was decreased.Conclusions The checklist may help improve the prevention and control of CRBSI in ICU.
3.Auditory tests in 14 patients(16 ears)with acoustic neuroma

Objective:To investigate the relationship between hearing loss and acoustic neuroma(AN),the tests of pure tone audiometry,acoustic emissions impedance audiometry,audiometry brainstem response(ABR)and evoked otoacoustic emissions(EOAE)wee measured in 14 patients (16 ears)from March 1999 to December 2000.Methods:Fourteen patients (16 ears)with acoustic neuroma (8 males and 6 females,ranging in age from 21 to 72 years old)were diagnosed by CT or MRI scaning,and final confirmed by surgery and pathology.In the auditory tests ,efferent suppression test was curried out only in 4 ears with recordable emissions,promontory stimulation test (PST)was examined only in 5 ears with severe or profound deafness (hearing loss≥80dB SPL)who have no both measurable ABR and recordable EOAE.Results:It was found that 2 ears (12.5%,2/16)of the AN ears showed neural impairment,6 ears (37.5%,6/16)were cochlear impairment and 8 ears (50.0%,8/16)were cochlear-retrocochlear impairment.All of 4 tumors ears with EOAE emission have a disorders of efferent function.Conclusion:EOAE test had significant value for evaluation of the status of cochlear function (at the level of outer hair cells)in AN patients.The retrocochlear auditory nerve function of AN patients were evaluated by the tests of ABR combined PST which showed significant value.Results showed that the hearing impairment of AN have different levels of the peripheral auditory system according to auditory tests,including cochlear,eighth cranial nerve and efferent nerve level at the same or independently.
4.Distinguish brain glioma recurrence for postoperative radiotherapy patients using perfusion weighted magnetic resonance imaging

ObjectiveTo evaluate the role of perfusion weighted magnetic resonance imaging ( PWMRI ) in distinguishing brain glioma recurrence for postoperative radiotherapy patients.Methods Thirty-five patients with suggested recurrent brain glioma (20 true,15 false) were examined by conventional MRI and PWMRI.Regions of interests (ROI) were manually drawn in contrast-enhancing lesion,surround edema region and normal white matter of contra-lateral brain on regional cerebral blood volume (rCBV) images.rCBV values of ROI and rCBVmax were measured,the standardized rCBV ratios were calculated,The mean rCBVmax ratio were compared between two lesion types with group t-test.The rCBVmax ratios of contrastenhancing regions was analyzed by the receiver operating characteristic curve.ResultsThe mean rCBVmax ratio of contrast-enhancing lesion in tumor recurrence was higher than those in non tumor recurrence (4.36vs 1.28,t =3.00,P =0.005 ).The mean rCBVmax of edema regions in tumor recurrence was also higher than those in non tumor recurrence ( 1.79 vs 0.85,t =2.41,P =0.026).Four patients with recurrent tumor and three with non tumor recurrence were wrong diagnosed when set the recurrent tumor rCBVmax ratio >2.15.The diagnostic sensitivity,specificity and accuracy for recurrent tumor by PWMRI were 80%respectively.Conclusion PWMRI examination is a useful method for distinguishing brain glioma recurrence.

7.Lifestyle and risk of Alzheimer's disease: a case-control study

Objective To evaluate the relationship between lifestyle and development of Alzheimer's disease(AD).Methods Two hundred and thirty-eight AD patients(102 males and 136 females) and 476 healthy controls(204 males and 272 females) were recruited from Ningbo communities into this 1 ∶ 2 matched case-control study.Mini-Mental State Examination(MMSE) and Clinical Dementia Rating(CDR) were required to fill in.Chi-square test and conditional logistic regression were used for data analysis.We adopted Epidata 3.1 to establish the database and did statistical analysis by SPSS15.0.The count data were analysis by Chi-square test,meanwhile multiple factors analysis by conditional Logistic regression analysis.Results Through single factor analysis we found thatcigarette ≥20/day(F=8.687,P=0.003),children visiting(F=22.721,P<0.05),friendship(F=16.784,P<0.05),family gathering(≥1 times/week)(F=8.198,P=0.004),working after retirement(F=33.099,P<0.05),travel(F=16.784,P<0.05),social activities(F=24.919,P<0.05),physical exercises(F=24.404,P<0.05),eating pickles or pickled products(F=6.662,P=0.01),saturated fatty acid intake(F=23.069,P<0.05),daily consumption of fruits and vegetables(F=8.401,P=0.004),chess(F=17.365,P<0.05),reading(≥30 min/d)(F=36.390,P<0.05),using computer(F=8.688,P=0.003) were related AD,the difference was statistically significant(P<0.05).In multiple factors analysis,social activities,chess games or joke,travelling,working after retirement,reading,physical exercises,friendship,family gathering,saturated fatty acid intake and daily consumption of fruits and vegetables were risk factors of AD(odds ratio(OR) values were 0.571(0.342-0.753),0.623(0.343-0.889),0.686(0.461-0.942),0.534(0.326-0.714),0.276(0.175-0.438),0.538(0.336-0.738),0.585(0.385-0.765),0.466(0.316-0.745),0.527 (0.368-0.787) and 0.482(0.316-0.665),respectively; constant terms:OR=0.526).Conclusion A positive and leisure lifestyle and health reasonable diet could effectively reduce the risk of AD.

The expression of endothelial nitric oxide synthase traffic inducer (NOSTRIN) was examined in the umbilical vessels of the patients with pre-eclampsia (PE) to explore its possible role in the pathogenesis of PE. The NOSTRIN mRNA in umbilical tissues was determined by RT-PCR. The eNOS activity in umbilical vessels was spectrophotometrically detected. NO2-/NO3-, the stable metabolic end products of NO, was measured by using nitrate reductase. RT-PCR showed that the expression level of NOSTRIN was significantly higher in women with PE than in the normal group (P<0.01). The activity of eNOS was significantly decreased in PE group [(12.83+/-3.61) U/mg] than in normal group [(21.72+/-3.83) U/mg] (P<0.01). The level of NO2-/NO3- in PE patients (27.53+/-7.48) micromol/mg was significantly lower than that of normal group (54.27+/-9.53) micromol/mg (P<0.01). The significant negative correlation existed between the expression of NOSTRIN and the activity of eNOS in umbilical vessels of women with PE (r=-0.58, P<0.01). It was concluded that the level of NOSTRIN expression was increased in umbilical vessel of women with PE, indicating that it may be involved in the pathogenesis of PE.

9.Expression of apoiipoprotein H in childhood primary nephrotic syndrome

Objective To study the expression of apolipoprotein H (ApoH) in childhood primary nephrotic syndrome (PNS) and to discuss its role in PNS. Methods Immunohistochemistry staining and real-time quantitative polymerase chain reaction(RT-PCR) were performed to evaluate the expression of ApoH in renal tissues of 78 patients with PNS and 14 cases of normal controls. Serum albumin, serum lipid, proteinuria and urinary retinol binding protein (RBP) were tested before renal biopsy in all patients. Tubulointerstitial lesions were investigated. Results (l)There was positive expression of ApoH in renal tissues of PNS patients and normal controls,mainly in the proximal tubules by immunohistochemistry staining. ApoH mRNA was also shown in all renal tissues by RT-PCR. ApoH protein expression was positively correlaed with its mRNA expression(r=0.264, P 0.05) whereas these data displayed no significant difference between two groups.
