2.Comparison of Motor Unit Number Estimates and Conventional Electromyogram in the Detection of Neuromuscular Diseases

: ObjectiveTo compare the clinical significance of motor unit number estimates (MUNE) and conventional electromyogram (EMG) in the evaluation of neuromuscular disorders. MethodsUnder the model of MUNE or quantity of motor unit potential (QMUP), 53 patients with various neurogenic disorders were tested with EMG at extensor digitorum brevis, thenar, or hypothenar eminence. ResultsFor 8 patients with amyotrophic lateralizing sclerosis, large and long-duration polyphasic potentials were detected in needle electrode EMG tests, and the motor unit numbers reduced. For 45 patients with peripheral neuropathies, few distinctive features could not be detected by needle electrode EMG, but motor unit numbers reduced in 2 patients; spontaneous activities were the only abnormality in the other 2 patients, and there were no obviously abnormal changes in the configuration and size of motor unit potentials and in motor unit numbers; EMG tests revealed neurogenic features and motor unit numbers significantly reduced in the remaining 41 patients. ConclusionConventional EMG and MUNE can work for the diagnosis of neuromuscular disorders, and do better in combination.
3.Updated molecular genetic mechanisms of globozoospermia.

Globozoospermia is a rare and serious teratozoospermia, which is one of the important causes contributing to human male infertility. The assisted reproductive technique remains the only means for such patients to produce offspring. However, the pathogenesis of globozoospermia is not yet clear. In recent years, related studies have shown that some genes are connected with the onset of globozoospermia. This paper outlines the progress in the studies of pathogenicity genes, aiming to contribute to the molecular diagnosis and mechanism investigation of the disease.

4.Interaction between CYP450 enzymes and metabolism of traditional Chinese medicine as well as enzyme activity assay.

Drugs are exogenous compounds for human bodies, and will be metabolized by many enzymes after administration. CYP450 enzyme, as a major metabolic enzyme, is an important phase I drug metabolizing enzyme. In human bodies, about 75% of drug metabolism is conducted by CYP450 enzymes, and CYP450 enzymes is the key factor for drug interactions between traditional Chinese medicine( TCM) -TCM, TCM-medicine and other drug combination. In order to make clear the interaction between metabolic enzymes and TCM metabolism, we generally chose the enzymatic activity as an evaluation index. That is to say, the enhancement or reduction of CYP450 enzyme activity was used to infer the inducing or inhibitory effect of active ingredients and extracts of traditional Chinese medicine on enzymes. At present, the common method for measuring metabolic enzyme activity is Cocktail probe drugs, and it is the key to select the suitable probe substrates. This is of great significance for study drug's absorption, distribution, metabolism and excretion (ADME) process in organisms. The study focuses on the interaction between TCMs, active ingredients, herbal extracts, cocktail probe substrates as well as CYP450 enzymes, in order to guide future studies.

5.Individual titanium mesh for repair of large-area skull defects in the fronto-temporo-parietal lobes in 16 cases A computer-aided design

BACKGROUND: More recently,repair of skull defect with computer-designed prosthesis contributes to the revolutionary development of skull reconstruction technique. OBJECTIVE: To individually molded titanium mesh by computer-aided design (CAD) technique,and to observe the clinical application value of the titanium mesh in the repair of large-area skull defects in the fronto- temporo-parietal lobes. DESIGN,TIME AND SETTING: A retrospective case analysis was performed at the Department of Neurosurgery,Liuzhou People's Hospital between January 2006 and August 2007.PARTICIPANTS: A total of 16 patients comprising 12 males and 4 females,aged 16-52 years,suffered from skull defects in the fronto-temporo-parietai lobes following standard large trauma craniotomy and were recruited into this stud Two of these patients were complicated by hydrocephalus and received ventriculoperitoneal shunt. Skull defect area ranged between 9. 2 cm ×11.2 cm and 12.2 cm×14.6 cm. Skull defect neoplasty was performed in all patients 3-8months following standard large trauma craniotomy. METHODS: Titanium mesh patches were individually modeled by CAD,computer-aided manufacturing (CAM) and rapid shaping techniques and implanted into skull defect region. In addition,defect edge was fastened with titanium nails. MAIN OUTCOME MEASURES: Moulding effects and complications following skull defect neoplasty. RESULTS: A small amount of subcutaneous effusion was found in one patient and disappeared after liquid extraction and pressure dressing. Titanium mesh was firmly fixed with no loosening. Patients exhibited left-right symmetry,appropriate lateral curvature,no irregular umbilication or chewing dysfunction. All patients were followed for 3-18 months postoperatively and were satisfied with good resuRs,Le.,no complications,infection,material exposure,loosening,or collapse. CONCLUSION: CAD technique used for repair of skull defects is convenient,effective,and safe. This method can. reduce postoperative complications and improve repair effects.

7.Efficacy and safety of CT-guided percutaneous catheter drainage in treatment of uniloculated deep neck abscesses

Twenty-three patients with uniloculated deep neck abscesses (UDNA),in whom the antibiotic therapy failed and CT-guided percutaneous catheter drainage (PCD) was performed from January 2005 to June 2015,were included in the study.Catheter placement was carried out using Trocar technique in all cases.Open surgical drainage was performed when PCD procedures failed.The abscess was completely drained and open surgical drainage was avoided in 19 cases (83%);the surgical drainage was performed because of muhiple internal septation in 3 (13%) case,and 1 (4%) case died from uremia.In this series the technical success rate and clinical success rate of PCD were 96% (22/23) and 83% (19/23),respectively.All patients were followed-up by CT scan.No other complications and no mortality occurred during the procedure,while postoperative pneumatosis developed in 1 case.CT-guided PDC is a safe and highly effective low-cost procedure for the treatment of patients with UDNA who failed medical therapy,it may be considered as an alternative to open surgery.

9.The clinical and pathological features of 85 cases with positive cerebrospinal fluid cytology by thin-layer centrifugal cytological test

Objective To evaluate cytological test of cerebrospinal fluid in the diagnosis of meningeal dissemination of tumor cells.Methods The clinical and imaging features of 85 cases with tumor cells diagnosed by thin-layer centrifugal cytological test of cerebrospinal fluid were retrospectively reviewed.The characteristics of cellular morphology and immunocytochemical staining were analyzed.Results The main presentations of all the patinets was meningeal irritation and neurological dysfunction.The features of the brain MRI were meningeal thicking and enhancement,intracranial abnormal signals and intracranial space occupying lesion in part of the patients.Atypical cells were found in 84 cases (98.8%) with the first sample test and immunocytochemical staining was conducted in 48 cases to identify the tissue origin.Meningeal carcinomatosis was shown to be the majority with lung cancer as the dominated tissue type and adenocarcinoma as the most common histological type.Others were lymphatic hematopoietic system (13 cases),melanomas (5 cases),primitive neuroectodermal tumor (3 cases) and glioma (1 case).In addition,12 cases were only proved to be cancer by cytological test of cerebrospinal fluid.Conclusion The thin-layer centrifugal cytological test of cerebrospinal fluid has a relatively high accuracy for detecting disseminated tumor cells of meninges and could be of great help to identify the source and type of lesion with immunocytochemical staining.
