Currently,research and practice in the of hospital management in China are at low level.This paper agrees that the innovation of hospital research management should be based on scientific knowledge management,knowledge communities is a core component of knowledge management implementation in hospitals.This paper descripts the method that how to buide the knowledge community,for example,the types of knowledge community,hospital knowledge system,and the job title based knowledge map and process-oriented knowledge map.
2.Expression of ANGPTL3 in kidney and its relationship to proteinuria and hyperlipidemia in adriamycin-induced nephrotic rats

Objective To observe the expression and distribution of angiopoietin-like protein 3(ANGPTL3) in kidney of adriamycin(ADR)-induced nephrotic rats, and to explore the possible association of ANGPTL3 with the development of proteinuria and hyperlipidemia. Methods Adriamycininduced nephrotic rat models were established by a single injection of adriamycin via the tail vein. Immunohistochemical staining (IHS)and dot-ELISA were used to examine the expression and distribution of ANGPTL3 in kidney and the level of ANGPTL3 in serum of ADR rats at day 7,14, 21 and 28 after adriamycin injection. 3.Impact of transplant nephrectomy on panel reactive antibodies

Objective To observe the impact of transplant nephrectomy on panel reactive antibodies and a secondary renal transplantation.Methods Panel reactive antibodies in 15 patients with a failed renal transplant admitted in our hospital between 2004 to 2007 were measured before transplantation,before and 1 month,6 months,12 months after transplant nephrectomy,and the pathological changes were observed after transplant nephrectomy.Results Panel reactive antibodies was increasing after renal transplantation,and reached the highest level one month after transplant nephrectomy,then grandually got down.New HLA allosensitization sites were discovered after transplant nephrectomy.Large amount of C4d was stained in failed transplant.Conclusion Serum PRA increased after transplant nephrectomy.New HLA allosensitization sites were found,which may be useful in HLA matching.
4.Inhibitory effect of SIN-1 on ICAM-1 expression in rat kidney after ischemia/reperfusion injury

Objective To explore the therapeutic mechanisms of nitric oxide donor (SIN 1) in ischemic acute renal failure (IARF). Methods The effects of SIN 1 on intercellular adhesion molecule 1 (ICAM 1) expression and inflammatory cell infiltration in rat kidney after ischemia/reperfusion (I/R) injury were examined using ICAM 1, ?2 integrin polyclonal antibody, and immunohistochemistry. The renal functions were measured simultaneously. Results Progressive increased expression of ICAM 1 was found in kidneys, especially in vasa recta, after IR injury. ?2 integrin positive cells increased simultaneously in the outer medulla. The two indexes reached the peak values at 24 h after I/R injury. SIN 1 infusion at the beginning of reperfusion could remarkably inhibit the enhanced ICAM 1 expression, reduce the local infiltration of inflammatory cells, and ameliorate the renal functions. Conclusion NO donor can inhibit the renal expression of ICAM 1 and the infiltration of leukocytes after ischemia reperfusion injury, which may play an important role in the treatment of ischemic acute renal failure.
5.The correlative study of detecting depths of arteries with transcranial doppler and head circumference in children

Objective To evaluate the correlation of the depths of detecting arteries vessel in transcranial dopplor(TCD) and child head circumference.Methods Five hundreds and eighty-eight health children were selected at random for the study. TCD ultrasonography was used to detect the optimal depths of arteries, middle and anterior and posterior cerebral arteries for each child, together with the head circumference.Results The data showed that the depths of detecting cerebral arteries significantly changed in different age groups, probably due to the growth of the head circumference.Conclusion There was close relationship between the depths of detecting cerebral arteries and child head circumference, which is of clinical significance.

8.Effect of curcumin on expression of cannabinoid receptor 1 and NR2B in spinal cord dorsal horn and dorsal root ganglion neurons in a rat model of neuropathic pain

Objective To investigate the effect of curcumin on the expression of cannabinoid receptor 1 (CBR1) and NR2B subunit-containing NMDA receptor in spinal cord dorsal horn and dorsal root ganglion (DRG) neurons in a rat model of neuropathic pain.Methods Seventy-two male Sprague-Dawley rats,weighing 200-230 g,were randomly divided into 4 groups ( n =18 each):sham operation group (S group),chronic constrictive injury (CCI) group,curcumin group (Cur group) and solvent control group (SC group).Neuropathic pain was induced by CCI.The sciatic nerve was exposed and 4 loose ligatures were placed on the sciatic nerve at 1 mm intervals in groups CCI,Cur and SC.Curcumin was injected intraperitoneally at 100 mg· kg- 1· d-1 for 14 consecutive days after operation in Cur group,while the equal volume of dimethyl sulfoxide was given instead of curcumin in SC group.Mechanical withdrawal threshold (MWT) and thermal withdrawal latency (TWL) were measured on 2 days before operation and on 1,3,7,10 and 14 days after operation.Six rats in each group were sacrificed on 3,7 and 14 days after operation and the lumbar segments of the spinal cord ( L4,5 ) and DRGs were removed to determine the expression of CBR1 and NR2B by immuno-histochemistry.Results Compared with S group,MWT was significantly decreased,TWL was significantly shortened,and the expression of CBR1 and NR2B in spinal cord dorsal horn and DRG neurons was up-regulated after operation in the other three groups (P < 0.05 ).Compared with CCI group,MWT was significantly increased,TWL was significantly prolonged,the expression of CBR1 inspinal cord dorsal horn and DRG neurons was up-regulated,and the expression of NR2B in spinal cord dorsal horn and DRG neurons was down-regulated after operation in group Cur (P < 0.05 ),and no significant change in the parameters mentioned above was found in group SC ( P > 0.05 ).Conclusion Curcumin can alleviate neuropathic pain in rats,and up-regulation of CBR1 expression and down-regulation of NR2B expression in spinal cord dorsal horn and DRG neurons are involved in the mechanism.
9.Effects of curcumin on type 2 diabetic neuropathic pain and expression of inositol-requiring enzyme 1α in spinal dorsal horn and dorsal root ganglia in rats

ObjectiveTo investigate the effects of curcumin on type 2 diabetic neuropathic pain (DNP)and expression of inositol-requiring enzyme 1α (IRE1α) in spinal dorsal horn and dorsal root ganglia (DRG) in rats.MethodsType 2 diabetes mellitus was induced by high-fat and high-sucrose diet and intraperitoneal streptozotocin (STZ) 35 mg/kg,and confirmed by fasting blood glucose level > 16.7 mmol/L in male SD rats.Type 2 DNP was confirmed by the mechanical withdrawal threshold (MWT) and thermal withdrawal latency (TWI.) measured on day 14 after STZ administration < 80% of the baseline value.The rats were then randomly divided into 3 groups ( n =27 each):DNP group,curcumin group (group Cur) and solvent control group (group SC).Curcumin and corn oil 100 mg/kg (25 mg/ml) were given intraperitoneally once a day for 14 consecutive days starting from 14 days after administration of streptozocin in Cur and SC groups respectively.Another 27 rats were chosen and served as control group (group C) and fed with common fodders.MWT and TWL were measured at 3,7 and 14 day after curcumin administration.The expression of IRE1α in spinal dorsal horn and DRG was detected by Western blot.ResultsCompared with group C,MWT was significantly decreased and TWL was significantly shortened,and the expression of IRE1α was up-regulated in DNP,Cur,and SC groups (P < 0,05),Compared with group DNP,MWT were significantly increased,TWL was significantly prolonged,and the expression of IRE1α in spinal dorsal horn and DRG was down-regulated in group Cur (P < 0.05).There was no significantdifference in the parameters mentioned above between DNP and SC groups (P > 0.05).ConclusionCurcumin can attenuate type 2 1)NP and inhibition of the expression of IRE1α in spinal dorsal horn and DRG is involved in the mechanism.
