1.Diagnosis and treatment of pouchitis and pouch dysfunction.

Restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) has become the surgical treatment of choice for patients with medically refractory ulcerative colitis (UC) or UC with dysplasia and for the majority of patients with familial adenomatous polyposis. However, UC patients with IPAA are susceptible to a number of inflammatory and non-inflammatory sequelae, such as pouchitis, Crohn disease(CD) of the pouch, cuffitis, and irritable pouch syndrome, in addition to common surgery-associated complications, which adversely affect the surgical outcome and compromise patient's health-related quality of life. Pouchitis is the most frequent long-term complication of IPAA in patients with UC, with a cumulative prevalence of up to 50%. Pouchitis may be classified based on the etiopathogenesis into "idiopathic" and "secondary" types and the management is often different. Pouchoscopy is the most important tool for the diagnosis and differential diagnosis in patients with pouch dysfunction. Antibiotic therapy is the main stay of treatment for active pouchitis. Some patients may develop dependency on antibiotics, requiring long-term maintenance therapy. While management of antibiotic-dependent or antibiotic-refractory pouchitis has been challenging, secondary etiology for pouchitis should be evaluated and modified, if possible.

3.Effect of p-nitrophenol Shock on Sludge Activity and Microbial Populations and in UASB Reactor

Effect of p-nitrophenol shock on microbial populations and sludge activity in UASB reactor was studied by DGGE-PCR of 16S rDNA fragments and detection of COD removing and biogas yield.The results showed that p-nitrophenol seriously inhibited the sludge activity,resulting in the drop of biogas and COD removing rate.The 40mg/L p-nitrophenol had more inhibition than 20mg/L p-nitrophenol.It would take 27 and 16 days respectively for reactor to recover after 40mg/L and 20mg/L p-nitrophenol shock.The diversity of eubacteria and methanogens were also effected by the p-nitrophenol shock.The variation of eubacteria was more than that of methanogens after p-nitrophenol shock.The drop of biogas was mainly related to the variation of Methanosaeta sp.and Methanomicrobia sp.after p-nitrophenol shock.Among the eubacteria the population of Chloroflexi sp.、Bacteroide sp.and Anaerovibrio sp.decreased greatly after p-nitrophenol shock.And more,the Rheinheimera sp disappeared after 40mg/L p-NP treatment.But the Flavobacteria sp.appeared after p-nitrophenol shock,which was probably related to the degradation of p-NP.
4.Vasectomy does not increase the risk of prostate cancer in Chinese men: A meta-analysis.

OBJECTIVETo investigate the correlation of vasectomy with the risk of prostate cancer in Chinese men.
METHODSWe systematically searched the databases CNKI, VIP, Wanfang, PubMed, Embase, and Cochrane Library for the literature relating the relationship between vasectomy and the risk of prostate cancer in Chinese males up to December 2014. According to the inclusion and exclusion criteria, two investigators independently selected the eligible publications, evaluated their quality, and extracted relevant information, followed by a meta-analysis with the software STATA 12.0.
RESULTSNine studies were included in the analysis involving 1 202 cases of prostate cancer and 4,496 controls. Random-effect model analysis revealed no statistically significant correlation between vasectomy and the risk of prostate cancer (OR = 1.05; 95% CI 0.62-1.79), with an obvious heterogeneity (P < 0.001, I2 = 85.7%). No significant publication bias was found among the included studies (Egger, P = 0.824; Begg, P = 0.348).
CONCLUSIONThe results of our meta-analysis do not support the association of vasectomy with the increased risk of prostate cancer in Chinese population.

5.Identification of a Thermoacidophilic Sulfolobus sp. Isolated from a Hot Spring in Tengchong Rehai

An extremely thermoacidophilic isolate K4-1 was obtained from an acidic hot spring in Teng- chong Rehai, Yunnan province. Morphology, growth characteristics, utilization of carbon compounds, en- ergy sources and 16S rRNA gene sequence of K4-1 were studied. Cells of K4-1 are irregular cocci with monotrichous flagella. The strain grew aerobically in either a lithotrophic or a heterotrophic mode. Growth on elemental sulfur occurred through oxidation of sulfur. It grew optimally at 75?C and pH 3.5. On the basis of 16S rRNA gene sequence similarity, strain K4-1 was shown to belong to genus Sulfolobus, being related to the type strains of genus Sulfolobus (86.6%~94.3% similarity), and being most closely related to strain Sulfolobus tengchongensis RT8-4 (98.9% similarity). The GenBank accession number of strain K4-1 16S rRNA gene sequence is EU729124.
6.Alteration of ATP-sensitive K~+ channel expression in hippocampal neurons after severe chronic hypoxia

Objective To explore the alteration of ATP-sensitive K~+(K_(ATP))channel expression in hippocampal neurons after severe chronic hypoxia. Methods The hippocampal neurons from 1-week-old rat were incubated and divided into normal control(incubated under 5%CO_2 and 95%air for 8h),hypoxia(incubated under 5%CO2 and 95%N_2 for 8h),hypoxia combined with diazoxide-treated (incubated with 100 μmol/L diazoxide under 5%CO_2 and 95%N_2 for 8h)and hypoxia combined with tolbutamide-treated groups(incubated with 100 μmol/L tolbutamide under 5%CO_2 and 95%N_2 for 8h).Cell apopmsis was identified by MTT.And the mRNA and protein expressions of K_(ATP) channel were estimated by RT-PCR and Western blot analysis.respectively. Results Hypoxia combined with diazoxide-treated group showed a significantly decreased apoptosis rate of neuron as compare with the normal control group 8h after hypoxia(P<0.05);while hypoxia combined with tolbutamide-treated group showed a significantly increased apoptosis rate of neuron compared with the normal control group(P<0.05).The expression of SUR1 in the three hypoxia groups significantly increased as compared with that in the normal control group(P<0.05);however,the expression of Kir6.2 in the three hypoxia groups did not change as compared with that in the normal control group(P>0.05).Conclusion K(ATP),channels can protect the hippocampal neurons under severe chronic hypoxia through the activation of KATP channels and upregulation of expression of KATP channels SUP1 subunit.
7.Studies on chemical constituents in leaves of Mallotus furetianus I.

OBJECTIVETo investigate the chemical compounds in leaves of Mallotus furetianus.
METHODThe chemical components were isolated by solvent extraction and chromatography. The structures were identified on the basis of physico-chemical constant and spectral data.
RESULTEight compounds were isolated and identified as 3-Hydroxy-4, 5 (R)-dimethyl-2(5H)-furanone (I), gallic acid (II), (6S, 9R)-roseoside (III), (Z)-3-Hexenyl-beta-D-glucopyranoside (IV), 3, 4, 8, 9, 10-Pentahydroxy-dibenzo [b, d] pyran-6-one (V), friedelinol (VI), beta- sitosterol (VII), friedelin (VIII).
CONCLUSIONCompounds I - VIII were obtained for the first time from this plant.

8.Analysis of median-term therapeutic effect of coronary artery bypass grafting in the elderly patients

Objective To summarize the clinical experiences and early-to median effect of coronary artery bypass grafting(CABG)in patients aged 70 years or above.Methods From Dec.2006 to Dec.2007,86 patients aged 70 or above underwent CABG,of whom 71 cases underwent off-pump coronary artery bypass grafting(OPCAB),15 cases had CABG with cardiopulmonary bypass(CPB).5 cases underwent mitral valve replacement,3 underwent aortic valve replacement besides CABG,and one case underwent removal of ventricular aneurysm.Their clinical data were retrospectively analyzed.Results There was one case who died of enterobrosis after operation.The average ICU stay time was(77.4±49.6)h,the average postoperative stay time was 15 d,and the average drainage was 530 ml.22 postoperative complications were found.76 cases were followed up for 10-22 months.Follow up effect was better showing cardiac classification I-II and no angina pectoris,myocardial infarction and late death.Conclusion Patients with coronary heart disease in good condition will have not remarkable improvement in mortality as compared with ordinary patients but attention should be paid to their preoperative pulmonary function and angina pectoffs.OPCABG would have encouraging result in reducing the rate of postoperative complications and hospital death,which should be recommended as the first choice for the elderly patients.
9.Clinical analysis of 7 cases of Patch-skill for controlling refractory bleeding in open heart surgery

Objective To evaluate the effects of 'patch-skill'in the management of refractory bleeding during the process of open heart surgery.Methods Between May 2007 to Oetober 2008,7 patients undergoing open-heart surgery experienced active bleeding.Autologous perieardial patches were used in 6 patients, and polyester patch in the other one.The patches were sutured continuously by 5-0 Prolene suture.Results The 24 hour chest tube output was less than 500 ml in all 7 cases after operation.There were no death, secondly active bleeding,or mediastinal in-faction.Conclusion The'patch-skill' can effectively control the refractory bleeding during open heart surgery.Au-tologous perieardial patehe is the first choice because it has multiple advantages.The polyester patch ean be used al-ternatively when autologous pereardiurn cannot be used (for various reasons such as edema and infection).
10.Effect of SIRT3 overexpression on hypoxia-reoxygenation injury to hippocampal neurons of mice exposed to high glucose: relationship with SOD2

Objective:To evaluate the effect of sirtuin 3 (SIRT3) overexpression on hypoxia-reoxygenation (H/R) injury to hippocampal neurons of mice exposed to high glucose and its relationship with SOD2.Methods:The normally cultured HT22 neurons at the logarithmic phase were selected and divided into 3 groups ( n=12 each) using a random number table method: high-glucose normoxia group (HG group), high glucose+ H/R group (HHR group) and high glucose+ H/R+ SIRT3 overexpression group (HHR+ SIRT3 group). To establish high glucose model, the neurons in 3 groups were cultured in high-glucose culture medium (glucose concentration of 50 mmol/L) for 8 h. In HHR and HHR+ SIRT3 groups, the cells were exposed to glucose-free and hypoxia for 6 h and then cultured in the high-glucose normoxic environment for 24 h to establish the high glucose and HR injury model.In HHR+ SIRT3 group, the neurons were transfected with SIRT3 overexpressed lentivirus.The cell viability was recorded by the cell counting kit-8 assay, reactive oxygen species (ROS) content was detected by flow cytometry, mitochondrial malonaldehyde (MDA) content, superoxide dismutase (SOD) activity, catalase (CAT) activity and adenosine triphosphate (ATP) content were determined by colorimetry, mitochondrial membrane potential (MMP) was detected by JC-1 probe, and the expression of nuclear respiratory factor 1 (NRF1), mitochondrial transcription factor A (TFAM), SIRT3, SOD2 and acetylated SOD2 (ac-SOD2) was detected by Western blot. Results:Compared with HG group, cell viability, SOD activity, CAT activity, ATP content, MMP, NRF1 and the expression of TFAM and SIRT3 were significantly decreased, and ROS content, MDA content and ac-SOD2/SOD2 ratio were increased in group HHR and group HHR+ SIRT3 ( P<0.05). Compared with HHR group, cell viability, SOD activity, CAT activity, ATP content, MMP, NRF1 and the expression of TFAM and SIRT3 were significantly increased, and ROS content, MDA content and ac-SOD2 /SOD2 ratio were decreased in HHR+ SIRT3 group ( P<0.05). Conclusion:SIRT3 overexpression can alleviate hypoxia-reoxygenation (H/R) injury to hippocampal neurons of mice incubated in high glucose medium, and the mechanism is related to activation of SOD2 deacetylation.
