Background: The interaction of the non-deletional α+-thalassaemia mutations Haemoglobin
Constant Spring and Haemoglobin Quong Sze with the Southeast Asian double α-globin gene deletion
results in non-deletional Haemoglobin H disease. Accurate detection of non-deletional Haemoglobin
H disease, which is associated with severe phenotypes, is necessary as these mutations have been
confirmed in the Malaysian population.
Methods: DNA from two families with Haemoglobin H disease was extracted from EDTAanticoagulated
whole blood and subjected to molecular analysis for α-thalassaemia. A duplex
polymerase chain reaction was used to detect the Southeast Asian α-globin gene deletion. Polymerase
chain reaction-restriction fragment length polymorphism analysis was then carried out to determine
the presence of Haemoglobin Constant Spring and Haemoglobin Quong Sze. A combine- amplification
refractory mutation system protocol was optimised and implemented for the rapid and specific
molecular characterisation of Haemoglobin Constant Spring and Haemoglobin Quong Sze in a single
polymerase chain reaction.
Results and Conclusions: The combine- amplification refractory mutation system for
Haemoglobin Constant Spring and Haemoglobin Quong Sze, together with the duplex polymerase
chain reaction, provides accurate pre- and postnatal diagnosis of non-deletional Haemoglobin H
disease and allows detailed genotype analyses using minimal quantities of DNA.
2.Evaluation of Domestic Cryostorage Blood Bags.

BACKGROUND: Recent studies have shown that the optimal temperature at which the cord blood and the peripheral blood stem cells need to be cryopreserved is below -160 degrees C in a nitrogen tank. This study evaluated a domestic cryostorage cord blood processing bag (CBP set) made from poly (ethylene-co-vinylacetate) (EVA) recently produced by the Medi-Rution Company in Korea. METHODS: The domestic cryostorage bags was evaluated using 100 bags from Medi-Rution, which were divided into 3 different groups and compared with 20 bags produced by the Thermogenesis company from the United States as a control. In the first group, the mononuclear cells separated from the human blood sample were divided equally into the cryosolution using DMSO, auto plasma and RPMI media. The blood was preserved in 20 domestic bags and 20 bags produced by the Thermogenesis company. In the second group, the cells were stored in the cryosolution with either auto plasma or fetal bovine serum. In the third group, 20 bags were frozen at -70 degrees C before being stored in the nitrogen tank whereas the other 20 bags were frozen in a controlled refrigerator before being stored in the nitrogen tank at -196 degrees C. The bags containing the peripheral blood cells were stored in the nitrogen tank at vapor phase for a period of one year and evaluated for the rate of breakage during thawing, recovery rates and viability of the cell after thawing. RESULTS: None of the bags evaluated in this study showed any signs of breakage or leakage of the products. All three groups tested showed similar results regardless of where the bags were made, the composition of the cryosolution or the early freezing conditions. CONCLUSION: The CBP set cryostorage bags recently produced by Medi-Rution can be used to preserve blood cells at extremely low temperatures for a one year period without any significant breakage.

To examine the clinical features of male patients with eating disorders in Singapore; and determine the differences in clinical features between the patients across the diagnostic categories. Methods: This is a database analysis of all male cases presenting to the Eating Disorders Clinic at Singapore General
Hospital between 2003 and 2012. Results: 72 cases were identified; 36.1% were diagnosed with anorexia nervosa, 33.3% had bulimia nervosa and 30.5% had the
diagnosis of eating disorder not otherwise specified. The mean presenting age was 19.9 years. 63.9% were heterosexual, while 15.3% were homosexual/bisexual.
61.1% had comorbid psychiatric diagnoses, with depression being the most common. 59.7% recorded premorbid obesity, while 66.7% reported excessive exercise. The patients in the various diagnostic categories had more similarities
than differences. Conclusion: With more male cases over the years, it is important to further understand this condition, to better refine prevention, detection and treatment strategies.

4.The Relationship between Media Use and Body Mass Index among Secondary Students in Kuching South City, Sarawak, Malaysia

Background: Overweight and obesity rates among adolescents have increased substantially
over the years. This study aimed to determine the body mass index (BMI) of students and parents
and the relationship among media use, BMI, socio-demographic profiles, and snacking behaviour
during television watching of secondary school students in Kuching South City.
Methods: In accordance with the two-stage sampling method, a total of 316 adolescents aged
13–17 years from 7 secondary schools participated. Data were collected using questionnaire and
anthropometric measurement. Independent t test, one-way ANOVA, Mann–Whitney U test, and chisquare
test were performed.
Results: The mean BMI was 20.56 kg/m2 (SD 4.33) for boys and 20.15 kg/m2 (SD 3.91) for
girls. No significant difference in terms of z score for BMI-for-age or socio-demographic factors was
found. The mean duration of time devoted to media use was 4.69 hours (SD 2.93) on weekdays and
5.69 hours (SD 2.87) on weekends. Boys were found to spend more hours on media use than did the
girls (t = 4.293, P < 0.01). Respondents were reported to consume more cereal compared with soft
drinks and junk foods. Respondents whose fathers worked in the private sector devoted the fewest
hours to media use, whereas those with self-employed fathers devoted the most time to media use.
Respondents with mothers who were employed spent more time on media use than did respondents
whose mothers were housewives (F = 4.067, P < 0.01). No significant difference was found between
BMI and media time or snacking habits.
Conclusion: This finding indicated that media time has no effect on body weight, because
respondents were found to have normal weight and to consume less unhealthy food.

The molecular basis of variable phenotypes in P-thalassaemia patients with identical genotypes has been associated with co-inheritance of alpha-thalassaemia and persistence of HbF production in adult life. The Xmn I restriction site at -158 position of the Ggamma-gene is associated with increased expression of the Ggamma-globin gene and higher production of HbF This study aims to determine the frequency of the digammaferent genotypes of the Ggamma Xmn I polymorphism in P-thalassaemia patients in two ethnic groups in Malaysia. Molecular characterisation and frequency of the Ggamma Xmn I polymorphism were studied in fifty-eight Chinese and forty-nine beta-thalassaemia Malay patients by Xmn I digestion after DNA amplification of a 650 bp sequence. The in-house developed technique did not require further purification or concentration of amplified DNA before restriction enzyme digestion. The cheaper Seakem LE agarose was used instead of Nusieve agarose and distinct well separated bands were observed. Genotyping showed that the most frequent genotype observed in the Malaysian Chinese was homozygosity for the absence of the Xmn I site (-/-) (89.7%). In the Malays, heterozygosity of the Xmn I site (+/-) was most common (63.3%). Homozygosity for the Xmn I site (+/+) was absent in the Chinese, but was confirmed in 8.2% of the Malays. The ratio of the (+) allele (presence of the Xmn I site) to the (-) allele (absence of the Xmn I site)) was higher in the Malays (0.66) compared to the Chinese (0.05). The (+/-) and (+/+) genotypes are more commonly observed in the Malays than the Chinese in Malaysia.

6.Relation Between Vascular Resistance and Intimal Thickness in Patients with Coronary Artery Disease.

BACKGROUND: The intimal thickening of coronary artery was understood as an early process in the beginning of atherosclerosis. However, the implication of intimal thickening as a morphologic change of coronary artery disease, on the coronary vascular hemodynamics has not been explored. METHODS: To evaluate the effect of intimal thickening on the coronary vascular hemodynamics, we studied the extent of intimal thickening on intravascular ultrasound(IVUS) and the coronary vascular flow and resistance by using Doppler wire in left anterior descending coronary arteries after successful intervention in 40 patients(29 males, mean age 55+/-9 years) with coronary artery disease. Mean intimal index and mean lumen cross sectional area were determined by IVUS and coronary flow average peak velocity was obtained by using Doppler wire before and after intracoronary adenosine in distal artery to the stenosis. Coronary flow velocity reserve(CFR), volumetric coronary blood flow(CBF) and coronary vascular resistance index(CVRI) were calculated. Hyperemic pressure-to-flow ratio(hyperemic mean aortic pressure/hyperemic volumetric coronary blood flow), ie, an index of minimal coronary vascular resistance(mCVR), was further derived. RESULTS: Intimal index(mean 20.0+/-8.3%) was significantly correlated with CVRI(mean 0.33+/-0.14, r=.37, p=.02) and mCVR(mean 0.81+/-0.40mmHg/ml/min, r=.36, p=.02). However, there was no correlation with CBF and CFR. CONCLUSION: The CVRI and mCVR(hyperemic pressure to flow ratio), the indices coronary microvascular resistance, were significantly related to the degree of intimal thickness of coronary artery. The extent of intimal thickness of coronary artery on IVUS study may be related to microvascular integrity.

BACKGROUND AND OBJECTIVES: It is well known that anti-platlet agents decrease the rate of subacute thrombosis after intracoronary stenting significantly. The aim of this study is to assess the antithrombotic effect and safety of 2-month combined regimen of cilostazol and aspirin on intracoronary stenting. METHODS: The study population consisted of 78 lesions of 57 patients (age: 58.1+/-10.3, male 47, female 10) with ischemic heart disease who were underwent successful intracoronary stenting. They were received cilostazol(200mg/day) and aspirin(100mg/day) two days before intracoronary stenting and continued for 8 weeks, and then aspirin was medicated continuously during the study. The laboratory and clinical findings were evaluated before cilostazol administration, 4 weeks, 8 weeks and 6 months after intervention. The excercise treadmill test was done at 6 months after intervention. RESULTS: Subacute thrombosis occurred in 2 patients(3.5%). Target lesion revascularization(TLR) was done in 4 patients(7.3%). Clinical restenosis (symptomatic or positive stress test, subacute thrombosis and TLR) occurred in 15 patients(26.3%). There was no granulocytopenia, or severe liver dysfunction. HDL-cholesterol was increased significantly at 2 months(36.6+/-7.4 mg/dl versus 41.6+/-9.3 mg/dl. p<0.01) and 6 months(36.6+/-7.4 mg/dl versus 42.4+/-10.6 mg/dl. p<0.01) follow up. CONCLUSION: Two-month combined regimen of cilostazol and aspirin was effective and safe after intracoronary stenting. Subacute thrombosis and clinical restenosis rate were comparable with pervious reports. Further large randomized trials are needed for the evaluation of favorable effect of cilostazol on lipid metabolism.

9.Antiallergic Function of KR62980, a Peroxisome Proliferator-Activated Receptor-gamma Agonist, in a Mouse Allergic Rhinitis Model.

PURPOSE: Peroxisome proliferator-activated receptor gamma (PPAR-gamma) has been shown to play an important role in the control of inflammatory responses acting on macrophages, mast cells, T cells and eosinophils. A novel PPAR-gamma ligand, KR62980 have been recently focused on due to the lower undesirable effects than other PPAR-gamma ligands such as rosiglitazone and pioglitazone. The present study was aimed to investigate the effects of KR62980 on nasal symptoms and immunopathological profiles in allergic nasal mucosa in murine allergic rhinitis model. METHODS: BALB/c mice were sensitized and challenged intranasally with ovalbumin (OVA). KR62980 was administered intraperitoneally or orally 3 hours before each intranasal OVA challenge. RESULTS: Administration of KR62980 significantly decreased the number of nasal rubbing, nasal sneezing, ova-specific IgE and total IgE in serum, secretion of Interleukin (IL)-4, IL-5, and IL-17 from the spleen and eosinophilic infiltration in the nasal mucosa. KR62980 decreased the expression of IL-4, IL-5 and IL-10 mRNAs in the nasal mucosal tissue, while, it elevated the level of IL-10 and IFN-gamma in splenocyte culture. KR62980 seemed to decrease IL-17 level in local and systemic level even though it did not reach to statistical significance. The anti-inflammatory effect was more definite when the KR62980 was administered intraorally than intraperitoneally. CONCLUSIONS: A novel PPAR-gamma ligand, KR62980 can attenuate OVA-induced allergic inflammation in mice mainly through modulation of Th2 cytokines. This finding suggests that PPAR-gamma might have a role in the treatment of allergic rhinitis.

10.Inhibition of Allergic Response by Intranasal Selective NF-κB Decoy Oligodeoxynucleotides in a Murine Model of Allergic Rhinitis.

PURPOSE: It remains unknown whether local inhibition of Nuclear factor-kappa B (NF-κB) could have therapeutic value in the treatment of allergic rhinitis (AR). This study aimed to evaluate the effect of selective NF-κB inhibition using NF-κB decoy oligodeoxynucleotides (ODNs) for the local treatment of AR in ovalbumin (OVA)-sensitized wild-type mice. METHODS: BALB/c mice were sensitized with OVA and alum, and then challenged intranasally with OVA. NF-κB decoy ODNs were given intranasally to the treatment group, and NF-κB scrambled ODNs were given to the sham treatment group. Allergic symptom scores, eosinophil infiltration, cytokine levels in the nasal mucosa, nasal lavage fluid, and spleen cell culture, serum total and OVA-specific immunoglobulins, as well as intercellular adhesion molecure-1 (ICAM-1) in the nasal mucosa, were analyzed. RESULTS: NF-κB decoy ODNs significantly reduced allergic symptoms and eosinophil infiltration in the nasal mucosa. They also suppressed serum levels of total IgE, OVA-specific IgE, and IgG1. IL-5 and TNF-α levels and the expression of ICAM-1 were decreased in the nasal mucosa of the treatment group compared to the positive control and sham treatment groups. In addition, IL-6 levels were significantly decreased in the nasal lavage fluid of the treatment group. Furthermore, NF-κB decoy ODNs significantly reduced expression of the systemic Th2 cytokines, IL-4 and IL-5 in spleen cell culture. CONCLUSIONS: This study demonstrates for the first time that local NF-κB inhibition using NF-κB decoy ODNs suppressed the allergic response in a murine AR model. This shows the therapeutic potential of local NF-κB inhibition in the control of AR.
