1.Advances of neonatal hypoglycemia and hypoglycemic brain damage
International Journal of Pediatrics 2013;40(5):477-480
Hypoglycemia is a common and early metabolic disease of neonate.Serious and persistent hypoglycemia could result in brain damage. So far the pathogenesis of the disease is on research, but the definition of hypoglycemia, the glucose threshold of intervention, and diagnostic criteria of brain damage are still unclear. The pathogenesis of hypoglycemia is complicated.Brain damage may be related to the specific excitory amino acids receptors, reduced blood flow in occipital lobe, and cerebral hypoxia and ischemia.Diffusion weighted imaging of brain MRI and magnetic resonance spectroscopy are strongly recommended for the diagnosis of brain damage.Besides, clinical manifestation and cerebral function monitor are also included in the diagnosis. The threshold of intervention is suggested as serum glucose concentration with 2.2 ~ 2.6 mmol/L.

Objective To investigate the change of lung morphology and the mechanism in the brain-dead state. Methods Ten Ba-Ma mini pigs were randomized into 2 groups: brain-dead group (group B, n=5) and control group (group C, n=5). Brain-dead model was established in group B by increasing intracranial pressure in a modified, slow and intermittent way, and brain-dead state maintained for 24 h by respiration and circulation support. The serum TNF-?, IL-1?, and IL-6 were determined at 3, 6 12, 18 and 24 h after the initial confirmation of brain death. Lung tissues were taken at 24 h. The alterations of lung tissues were observed by HE staining, and the expression of PKC-? detected by immunohistochemistry. PKC-? mRNA at each time point was detected by RT-PCR. The microstructure of hepatic tissues was observed under an electron microscope. Results (1) Under the light microscopy, broadened lung alveolar septum, edematous, extravasate in the alveolar congestive capillary vessel, infiltration of lymphocyte were observed. Under the election microscopy, cytoplasm edema, swollen mitochondria of the type-Ⅱ epithelial cells, partial membrane dissolution of mitochondria were observed and the microtomentum disappeared. No obvious morphological injury was observed under light microscopy and election microscopy. (2) Compared with control group, the serum IL-1?, IL-6 and TNF-? in brain-dead group began to increase at 3 h after brain death, and become more and more higher when the maintained state was prolonged. The expression of PKC-? and PKC-? mRNA in brain-dead group was increased at 24 h in the lung tissue. Conclusion Brain death may evoke lung morphological injury and increase the level of inflammatory mediators; After brain death the levels of PKC-? mRNA transcription and protein translation is increased. The activation of PKC-? and the alteration of inflammatory mediators are supposed to be one of the mechanisms of the lung injury.

Objective: To express glycosyl-phosphatidylinositol (GPI) modified Met- RANTES fusion protein on Chinese hamster ovary (CHO) cells and to develop a novel immunosuppressant GPI anchored form of Met-RANTES. Methods: The eukaryotic expression vector PEF/GPI-Met-RANTES were constructed and transfected into CHO cells by electroporation. The transfectants were selected with methotrexate (MTX). Expression of the recombinant protein was assessed by flow cytometric analysis, cell immunofluorescence staining and immunogold electron microscopy. Results: The chimeric molecules of GPI anchored form of Met-RANTES including the whole reading frame were constructed, and the sequence was identical to the designed sequence. GPI anchored form of Met-RANTES was stably expressed on CHO- DHFR- cells. Conclusion: A large amount of GPI modified Met-RANTES fusion protein was expressed on CHO cells. GPI anchored form of Met-RANTES may be used as novel immunosuppressant for suppressing reaction in graft rejection.

Chinese Pediatric Emergency Medicine 2015;22(2):100-103
Objective To study the role of Ginsenoside Re and Rb3 on mice with viral myocarditis (VMC) and explore the mechanisms. Methods BALB/C mice were infected by coxsackievirus B3 ( CVB3) to establish VMC model. The mice were divided into control group, virus group and Ginsenoside Re and Rb3 treatment group(treatment group). On day 5,day 10,day 20 after infection, the level of serum crea-tine phosphokinase-MB( CK-MB) was detected. Then myocardial sections stained with Masson′s trichrome were used to observe the distribution of mice myocardial collagen fibers, quantify collagen volume fraction (CVF),and detect the levels of superoxide dismutase(SOD). Results (1)The level of CK-MB peaked on day 5,and decreased afterwards[day 5:(463. 68 ± 47. 62) U/L; day 10:(588. 81 ± 56. 09) U/L; day 20:(340. 48 ± 58. 22) U/L,respectively]. While the levels of CK-MB in treatment group were lower than those in virus group[day 5:(378. 69 ± 56. 02) U/L;day 10:(452. 56 ± 67. 78) U/L; day 20:(327. 13 ± 47. 20) U/L,respectively] in the same point. There were significant differences between groups on day 5 and day 10 (P<0. 01). (2) In viral group, the blue staining degree gradually increased with time in myocardial sections stained with Masson′s trichrome, especially in myocardial necrosis area. Compared with treatment group,CVF increased significantly in virus group on day 10 and day 20 ( day 10:6. 52% ± 2. 34% vs. 8. 94% ± 1. 67%;day 20:7. 00% ± 1. 53% vs. 10. 46% ± 1. 74%,P<0. 01). The levels of SOD in myocardial sections in virus group were lower than those of control group[day 5:(48. 83 ± 17. 74) U/L;day 10:(61. 41 ± 14. 58) U/L;day 20:(66. 26 ± 18. 97) U/L,respectively,P<0. 05],but in treatment group, the level of SOD could beimprovedsignificantly[day5:(72. 07 ±24. 85)U/L;day10:(83. 22 ±19. 52)U/L;day20:(92. 00 ± 20. 46) U/L, respectively, P < 0. 05]. Conclusion Because of the inhibition of oxygen radicals and oxidative stress, Ginsenoside Re and Rb3 can protect myocardial tissue. Ginsenoside Re and Rb3 can effectively reduce the extent of myocardial fibrosis. The mechanism may be related with the reduced peroxide level in vivo.
