2.Non-limiting prosthetic knee replacement for children with knee dysfunction:prognosis and complications

BACKGROUND:Pediatric knee dysfunction is a congenital malformation. Fusion and fixation can effectively eliminate pain, but no joint has active function. Total knee arthroplasty is a safe and effective method to relieve pain and to rebuild its function. With the continuous development of total knee arthroplasty, the type of knee prostheses is more. OBJECTIVE: To study the efficacy of different reconstruction methods for children with stunted knee arthroplasty. METHODS:120 knee stunted children were equaly divided into treatment group and control group depended on the reconstruction, and then al children were given the artificial knee replacement surgery and were completed by the same group of physicians. In the control group, stable fixed platform prosthesis was selected. In the treatment group, the non-limiting prosthetic-posterior cruciate ligament resection of low contact pressure rotating platform prosthesis was given. Operative time, blood loss, postoperative hospital stay, and complications were observed in both groups. Al patients were folowed up for 3 months. Knee function was assessed by Hospital for Special Surgery and American knee society knee score. Multivariate regression analysis was used to analyze the possible factors affecting Hospital for Special Surgery score during folow up. RESULTS AND CONCLUSION: No significant difference in operative time, blood loss and postoperative hospital stay was compared in both groups (P> 0.05). The postoperative overal incidences of inflammatory response, wound healing, periprosthetic infection, and joint stiff were significantly lower in the treatment group than in the control group (P < 0.05). At 3-month folowed-up, Hospital for Special Surgery and American knee society knee score clinical score and functional score were significantly higher in the treatment group than in the control group (P < 0.05). Hospital for Special Surgery score > 80 served as a standard. Multivariate regression analysis results showed that age, type of prosthesis, and varus deformity were the major independent risk factors for affecting joint replacement (P< 0.05). Results indicate that knee arthroplasty for knee dysfunction obtained good prognostic effect. Simultaneously, the non-limiting application of prosthetic reconstruction can effectively reduce postoperative complications and improve knee function.
3.Application of evidence-based nursing combined with KAP model in health education to the parturients from suburbs

Objective To study the effect of evidence-based nursing combined with KAP model on health education to the parturients from suburbs.Methods According to problems in 200 pregnant women from suburban areas, health education was performed to them using evidence-based nursing and KAP model. The rate of access to knowledge, confidence about laboring, spontaneous participation in action to promote delivery, natural birth rate, episiotomy rate, breastfeeding rate and rate for satisfaction with midwives were compared between pre-and post-health education.Results After intervention, their access to knowledge, confidence about laboring, spontaneous participation in action to promote delivery were obviously improved than those before intervention(P<0.05). The rate of natural birth, cesarean delivery, forceps delivery and satisfaction with midwives were 66.0 %, 30.0 %, 1.0% and 94.4 %, respectively.Conclusion Evidence-based nursing and KAP model is effective in the health education to pregnant women from suburbs.
4.Effects of lifestyle intervention on glucose control in people with type 2 diabetes

Lifestyle intervention is basic and background treatment for people with type 2 diabetes.Effective lifestyle intervention contributes to better glucose control and delay of diabetic complications.Dietary and exercise interventions as well as lifestyle advice are major components of lifestyle intervention.Dietary intervention focuses on restriction of total caloric intake and balanced nutritional composition,while exercise intervention focuses on exercise mode,frequency,intensity,timing,and duration.This review introduced several lifestyle intervention measures including exercise and dietary interventions as well as lifestyle advice,and discussed their effects on glucose control in people with type 2 diabetes.
5.Relationships of Activin,Activin A and Brain Injury

Activin is a growth factor belong to the transforming growth factor-? superfamily.Recently,experimental studies have shown that activin and activin A expressed higher in various brain injury models and activin have beneficial roles to neuronal protection.The high correlation of activin and activin A with clinical and biochemical signs of brain injury lead us to suggest that activin and activin A are new possible indicators to predict and treat brain injury.
6.Effects of lactic acid bacteria on P38 signaling in immune response to dust mite in mite-sensitized mice

7.Immunomodulatory effects of lactobacillus on dust mite sensitized murine model

Objective: To compare the immunomodulatory capability to allergic asthma between orally taken Lactococcus lactis and Lactobacillus Lactobacillus plantarum in dust mite-sensitized mice. Methods: C57BL/6 mice were divided into four groups;Group N,M,L and P.Mice in M,Land P groups were sensitized with 2 % mite extract on day 0,7and 14 and challenged with 4% mite extract on day 21,22,23 and 24.Meanwhile we fed Lactococcus lactis and Lactobacillus plantarum to mice in L and P group respectively from each day of sensitization on as well as from the first day of challenge on,continuing for five days.Mice in N and M group were treated with saline as control.Histopathologic examination in the lung tissues of the mice was performed at day 3 after the last challenge.Cell count and classification,and Th1/Th2 cytokine levels in BALF were measured simultaneously.Isolated splenocytes from the mice were subjected to the detection of cell proliferation by BrdU incorporation in response to the mite extract.Using ELISA,the levels of Th1/Th2 cytokines in the supernatant of mite-stimulated splenocytes were measured. Results: Pulmonary allergic inflammation with eosinophilic infiltration as well as increased levels of IL-4 and IL-5 in BALF were observed in mice sensitized/challenged with mite extract(Group M).Pulmonary inflammation was decreased significantly in group L concomitant with down regulated levels of IL-4 and IL-5,total cell count and the percentage of eosinophils in BALF(P
8.A retrospective analysis of 33 primary intraspinal tumors treated with postoperative radiotherapy

Objective To describe the clinical characteristics,outcome and prognostic factors of primary intraspinal tumor treated with postoperative radiotherapy.Methods Thirty-three patients diagnosed as suffering from intraspinal tumor treated with postoperative radiotherapy were analyzed.There were eighteen male and fifteen female,with a median age of 37 years(range 7-64 years).Two patients were treated with grossly total resection,25 subtotal resection,2 biopsy only.These patients were treated either with(~(60)Co)(n =18,before 1998) or 6?MV-X ray(n =15,after 1998) to the total dose of 45-68?Gy.The overall response and survival rates were calculated by Kaplan-Meier method and the difference between groups was tested by the log-rank method.The prognostic factors related to the characteristics of patient,tumor,and treatment were determined.Results The overall response,five-year and disease-free survival rate was 81.3%,59.4% and 56.3%,respectively.By multivariate analysis,the histological type(P= 0.039) and gender(P= 0.049) had independent impact on the 5-year survival rate.Ependymoma had a better prognosis compared to other histological subsets and female gained more survival advantage over male.There was no close relationship between survival and other factors,such as age,extent of surgery,initial site,irradiation dose,or the interval between surgery and radiotherapy.Conclusions Postoperative radiotherapy is safe and effective for primary intraspinal tumor.The histological type and gender are important prognostic factors of survival in primary intraspinal tumors.
9.Percutaneous vertebroplasty in the treatment of osteoporotic thoracolumbar vertebral compression fractures

[Objective]To observe the clinical results of percutaneous vertebroplasty in the treatment of painful osteoporotic thoracolumbar vertebral body compression fractures in old people.[Method]Six cases of PVP and 2 cases of kyphoplasty were performed with polymethylmethacrylate(PMMA) through unipedicular or bipedicular under C-arm fluoroscopy.[Result]No leakage of PMMA was found in the operation,the pain was clearly relieved or disappeared postoperatively,no severe complication occurred.[Conclusion]PVP is safe,effective and enconomical in the treatment of painful osteoporotic thoracolumbar vertebral body compression fracture.
10.Peroneal nerve injury after total knee arthroplasty:assessment of causing and prevention factors

[Objective] To explore the causing and prevention factors of peroneal nerve injury after total knee arthroplasty(TKA).[Method] Nine postoperative peroneal-nerve injury in 9 patients were documented in a retrospective review of 2000 consecutive total knee arthroplasties performed at one institution from Jan.1996 to Jun.2007.Six fresh cadaver knees were executed TKA to observe the causing factors of peroneal nerve injury in operation.[Result]The causing factors of the 9(0.45%)patients were not clear.Cadaver knees TKA suggested the dangerous handlings by turns include the belows:①lifting distal femur by encircle dragging may enhance lesion probability of common peroneal nerve.②risk for common peroneal nerve to set Hoffman crook not paralleling longitudinal axis of tibia or posterior the lateral collateral ligament.③excessive straightening knee joint after the prosthesis had been fixed.[Conclusion]Lifting distal femur by encircle dragging should be avoided while peeling posterolateral joint caps and caput laterale musculi gastrocnemii.The location,inserting orientation and depth of Hoffman crook should be taken care of.Hyperextension of the knee should be avoided.
