Neonatal bacterial meningitis is difficult to diagnose at the onset,with atypical symptoms and high mortality,those who survive would have neurological complications.Early diagnosis and standard treatment are very crucial.European Society for Clinical Microbiology and Infectious Diseases published a guideline to the diagnosis and treatment of acute bacterial meningitis.In combination with the latest research results and the guideline,this article aims to provide guidance in diagnosis and treatment of neonatal bacterial meningitis.
4.Electroencephalogram monitoring of premature

As the development of technology in Neonatal Intensive Care Unit,the neurology outcome of premature has become a critical issue for their future life quality.Early electroencephalogram (EEG) test is a noninvasive and convenient method to monitor brain function.Nowadays,EEG monitoring has been used to evaluate the maturity of background activity,to detect subclinical seizures and to predict future neurological outcomes.The guideline of American clinical neurophysiology society in 2011,and the guideline of French clinical neurophysiology society in 2015 have made recommendations for the indication,timing and duration of EEG monitoring.Interpret EEG needs both skill and experience,it is hard for clinicians to master the skill in a short time.Hence,despite the advantages of EEG monitoring,the application of EEG among premature is limited.The amplitude-integrated EEG and other quantitative EEG monitoring method have become the latest research hotspot for evaluation of preterm brain function.In order to improve the recognition of EEG in the area of preterm neurologic evaluation,now the latest researches are reviewed.
5.Prognosis after fertility-sparing management with oralprogestin for women with complex endometrial hyperplasia and endometrial cancer

Objective To investigate the oncologic and reproductive outcomes after progestin treatment of complex endometrial hyperplasia(CEH) and grade 1 endometrial carcinoma(EC).Methods In a retrospective study, data were obtained for patients with CEH or grade 1 EC at presumed stage IA(without myometrial invasion) who wished to preserve fertility and were treated at the Peking Union Medical College Hospital, China, between January 1, 2000 and December 31, 2011.Patients had received oral medroxyprogesterone acetate(250-500 mg/d) or megestrol acetate(160-480 mg/d) for at least 6 months.Results Among 55 included patients, median age was 32 years(range 21-41 years).41(75%) achieved complete response after a median period of 6(3-24) months.Complete response was less frequent among obese than nonobese patients(4/12 [33%] vs 37/43 [86%];P=0.001).Disease recurrence was recorded in 10(24%) patients with complete response;the 5-year recurrence-freesurvival rate was 71%.Among the 33 patients who retained a desire to conceive, 17(52%) became pregnant.Conclusions Fertility-sparing management with oral progestin is effective and safe.Obesity is associated with a lower probability of long-term success.
7.Spindle and chromosome configuration of human in vitro matured oocytes after slow freezing-fast thawing

Objective To evaluate the development of immature oocytes after freezing-thawing by conventional cryopreservation method for mature oocytes.Methods Immature oocytes were collected from stimulated ovaries of intracytoplasmic sperm injection(ICSI)cycles.Immature oocytes were in vitro matured directly or after slow freezing-fast thawing and immunostained for tubulin and chromatin and at last visualized by confocal microscopy.Results No statistical difference was found in maturity rate between freezing groups and the controls.There was a statistically significant increase in abnormalities of chromosome(23.7% vs. 50%)and spindle(28.9% vs.53.9%)in the GV freezing group compared with the GV control(P
8.Comparison of cognition and attitude towards the elderly family nursing among college students and nurses

Objective To investigate the cognition and attitude towards the elderly family nursing among nurses and college students in Chengde, and analyze its influencing factors. Methods A total of 1000 college students and nurses in Chengde were investigated by self-designed questionnaire about their cognition and attitude to elderly family nursing. Results Nurses' cognition and attitude towards the elderly family nursing was better than nursing college students, medical students were better than non-medical students; and there was no statistical significance about their expectation for government's senior citizen service among different groups. Conclusions More attention should be paid to family members' attitudetowards the elderly home care, and should strengthen the elderly family care through various way.
9.Preparation of andrographolide-loaded solid dispersions

AIM To prepare the andrographolide-loaded solid dispersions.METHODS Differential scanning calorimetry (DSC) and X-ray powder diffraction (XRPD) were used for the analysis of solid dispersions previously prepared from the mixture of andrographolide and PVPK30 (1 ∶ 9) in autoclave.In addition to the evaluation index of in vitro dissolution rate,the influencing factors of pressure,temperature and reaction time were taken into consideration for the preparation optimization by Box-Behnken method.RESULTS Andrographolide was totally dispersed in solid dispersions in an amorphous state,manifesting an inhibited crystal diffraction peaks' formulation.Under the optimal conditions of 21.26 MPa for pressure,40.76 ℃ for temperature,and 1.13 h for reaction time,the in vitro dissolution rate was 85.49%.CONCLUSION After andrographolide is made into solid dispersions,its in vitro dissolution rate is obviously increased.
10.The diagnostic value of ProGRP for small cell lung cancer

Objective To explore the clinical significance of serum ProGRP in diagnosis of SCLC.Methods Serum ProGRP were detected in twenty-three SCLC patients,forty NSCLC patients and forty-three patients with benign pulmonary disease whose were definite diagnosis collected from April 15,2016 to July 19,2016,and a contemporary cohort of 40 healthy controls were while recruited.Results The levels of ProGRP in SCLC group were significantly higher than those in NSCLC group,benign pulmonary disease group and healthy control group(P <0.05).The levels of ProGRP in extensive disease group were significantly higher than those in 1imited disease group(P <0.05).There was no significant difference in the positive rates of ProGRP between the sex group and age group (P > 0.05 ).The boundary value of diagnosis of SCLC through ProGRP identified through ROC curve was 65.66 ng/L.The sensitivity and specificity of the ProGRP for the diagnosis of SCLC were respectively 90.9% and 89.9%.Conclu-sion Serum ProGRP can be use as a sensitive and specific index for diagnosis of SCLC,and the level of ProGRP can also be used for the clinical staging of SCLC.
