1.The outcome study of rheumatic disease patients with chronic hepatitis B infection

Objective To assess the outcomes of chronic hepatitis B (CHB) infection following immunosuppressant and corticosteroid treatment in patients with rheumatic disuses.Methods The medical records of patients with positive HBsAg and rheumatic diseases from 1 Jan 2004 to 31 Dec 2007 were retros pectively reviewed and analyzed for the types of rheumatic diseases,hepatitis B seroiogies,name and dosage of immunosuppressive agents used,anti-viral therapies and outcomes of CHB infection.Results Twenty one patients were included.There were 14 female and 7 male patients.The mean age of all patients was (45±16)years.All patients were positive for hepatitis B surface antigen (HBsAg) and hepatitis B core antibody (HbcAb),and had alanine aminotransferase (ALT) less than 60 U/L except one patient with dermatomyositis.Twelve(57%) patients treated with immunosuppressant only.Among them,rheumatoid arthritis (75%) was the most commonly diagnosed rheumatic diseases.These patients were treated with methotrexate (no more than 10 mg per week) or leflunomide ( 10 mg/d),combined with suffasalazine or hydroxychloroquine.Three patients received antivirus drugs because of the elevation of HBV-DNA.During the follow-up period (7 to 47 months with a median of 25 months),four (33%) developed ALT elevation,but none had developed HBV reactivation.Nine (43%) patients were treated with prednisolone and /or immunosuppressants.Among them,5 (56%)patients were diagnosed as systemic lupus erythematosus,others were adult onset of Still's disease and dermatomyositis,and 3 patients had elevation of HBV-DNA copies.These patients were treated with predni-soione (0.8~1.2 mg·kg-1·d-1) only or combined with immunosuppressants (methotrexate or cyclophosphamide),and all patients received antivirus drugs.During the follow-up period (3 to 50 months with a median of 13 months),two developed ALT elevation,but none had developed HBV reactivation.Conclusion In patients with rheumatic disease complicated with chronic HBV infection,methotrexate (no more than 10 mg per week) and leflunomide (10 rag/d) may be safe for patients with negative HBV-DNA.Prednisolone and immunosuppressants (methotrexate or cyclophosphamide) may be used safely with prophylactic antivims drugs.
2.Myositis in primary Sj(o)gren's syndrome

This study was to identify the clinical features of myositis complicated with primary Sj(o)gren's syndrome (pSS). A total of 202 patients with pSS were investigated. Myositis was diagnosed according to the clinical findings, muscle enzyme levels, electromyographic results, and muscle biopsy, and compared with 15 polymyositis (PM) patients. Myositis was identified in 4 of 202 pSS patients (2.0%). They developed myositis 5 to 20 years after the onset of SS. Two patients showed no myalgia and muscular weakness. Creative kinase (CK) was increased from 480 to 2702 IU/L. Anti-Jo-1 antibody was negative. All patients responded well to prednisone and had a median serum CK decrease by 48.9%. No patients had myositis recurrence. Compared with the PM group, the percentage of myalgia, peak of CK, descending rate of CK, and positive rate of anti-Jo-1 antibody were all significantly different. Myositis with Sj(o)gren's syndrome is not common, show relatively moderate symptoms, and respond well to prednisone.
4.Health status of migrant workers in cities and policy suggestions

The paper analyzes the age structure, educational background, occupation, income, migration and other sociological features of migrant workers in cities in present China and discusses the general health status of the group, including the incidence and spread of contagious diseases, workplace accidents and occupational hazards, reproductive and psychological health, and social adaptation. It also puts forward policy suggestions for improving the health status of migrant workers and raising their health level from the perspectives of perfecting the social security system, giving expression to impartial health services, showing concern for the health status of vulnerable populations, enhancing health education and promoting health.
5.Relationship between the Apoptosis of MNCS and the Expression of Cytokines and Adhesion Molecules in Patients with SLE

Objective To explore relationship between the changes of mononuclear cells (MNCS) apoptosis and the levels of inflammatory cytokines and adhesion molecules in patients with systemic lupus erythematosus(SLE). Methods The apoptotic rates of MNCS in 32 patients with SLE and 10 healthy subjects were detected with flow cytometry, and the plasma levels of inflammatory cytokines(IL-8, IL-6,TNF-? and NO) and adhesion molecules(P-Sel, ICAM-1) were assayed with ELISA. Results The percentage of MNCS apoptosis in the patients with active stage of SLE was obviously higher than that in healthy subjects and patients with remission stage of SLE, and there was no significant difference in the percentage of MNCS apoptosis between healthy subjects and patients with remission stage of SLE. The levels of plasma IL-8, TNF-?, IL-6, NO, P-sel and ICAM-1 in patients with active stage of SLE were significantly higher than those in healthy subjects and the patients with remission stage of SLE, and were positively correlated with the percentage of MNCS apoptosis and the severity of SLE.There was no significant difference in the levels of plasma inflammatory cytokines and adhesion molecules between patients with remission stage of SLE and healthy subjects. Conclusion The apoptotic rate of MNCS increased in the patients with active SLE, and was closely associated with the severity and efficacy of SLE. The high expressions of inflammatory cytokines and adhesion molecules may be the reason of MNCS apoptotic increase. Appropriately regulating the expression of cytokines and adhesion molecules, and moderately controlling the apoptosis of MNCS may improve the prognosis of SLE.
6.Efficacy of transanal local excision in the treatment of low rectal cancer

Objective To evaluate the therapeutic effect of transanal local excision in the treatment of low rectal cancer.Methods The data of 49 cases of low rectal cancer patients treated by transanal local excision (group A,37 cases of T1 patients and 12 cases of T2 patients) were reviewed retrospectively,and compared with the group of 45 cases treated by radical resection( group B,11 cases of T1 patients and 34 cases of T2 patients).The 5-year survival rates,recurrence rates and postoperative complications between the two groups were compared.Results The incidence of postoperative complications in group A was significantly lower than that in group B(4.1% vs 15.6%,x2 =4.69,P <0.05 ).The 5-year survival rate of group A was 97.9% and the 5-year survival rate of group B was 95.6%.There was no significantly statistical difference on the 5-year survival rate between the two groups( x2 =0.439,P > 0.05 ).But for the T2 patients,the recurrence rates in group A was 33.3%,which was significantly higher than the T2 patients in group B(5.9% ) ( x2 =5.89,P <0.05 ).All patients with low histopathological grade tumor in group A showed postoperative recurrence and metastasis.Conclusion Transanal local excision with a rigid criteria can get good curative effect in the T1 patients,but not suitable for the T2 patients or patients with low histopathological grade tumor.
7.The prevalence of metabolic syndrome in Shanghai adults

Objective To assess the prevalence of the metabolic syndrome (MS) in Shanghai adults according to the criteria of the US National Cholesterol Education Programme Adult Treatment PanelⅢ ( NCEP ATP Ⅲ) definition. Methods Randomly selected adults were studied by means of stratified sampling. A cross-sectional study was conducted in Shanghai with a representative sample of 7 414 Chinese adults aged 35-74 years.Results Defined by the ATPⅢ criterion, the preliminary prevalence of MS was 19.81%. The prevalence of MS was 16.21% (95%CI 15.37%-17.05% ) after age was adjusted. The prevalence of MS was higher in female than in male (22.59% vs 16.64% ,P<0.01 ), and higher in urban than rural residents( 17.43% vs 17.16% ,P<0.01 ). The prevalence of MS increased with age in both male and female, especially in females aged above 55 years. There were 19.81% subjects with over 3 risk factors. The most common combination of four components in MS were central obesity, high triglyceridemia, low high-density lipoprotein-cholesterol and hypertension. Conclusions There is a high prevalence of MS in Shanghai adults. MS is increasingly becoming a noteworthy health problem. Early-diagnosis and integrated-methods of prevention and treatment of MS are mandatory.
8.Clinical research of ureteral replacement using the Yang-Monti principle

Objective We summarized the clinical experience of modified ileal ureter substitution for treating long segment ureteral defection.Methods We retrospectively analyze the clinical data of 2 patients with long segment ureteral defect who treated with Yang-Monti ileal ureter substitution between March 2015 and November 2015.One 75 years old male patient was diagnosed as upper ureteral malignance and solitary kidney.The length of defection from renal pelvis to bladder was 22 em.His serum creatinine was 100 μmol/L,blood urea nitrogen was 5.7 mmol/L,serum chloride was 98 mmol/L.Another one 41 years old female patient was diagnosed as middle and lower ureteral iatrogenic injury.The traumatic length was 15 cm.Her serum creatinine was 70 μmol/L,blood urea nitrogen was 5.1 mmol/L,serum chloride was 100 mmol/L.they were both treated by Yang-Monti ileal ureter substitution.The ileal intestinal segment was used for the ureteral replacement,which were more than 15 cm to the ilealcecum.The length of intestine was 10.0 cm and 7.5 cm,respectively.The ileal mesentery was preserved.After closing the mesangial hiatus,the ileal segment was pull into the retroperitoneal space and pulling out via descending colonic mesangial window.The ileal segment was divided into three parts,which was 2.5 to 3.0 cm in each part.Each part was opened via long axis and then rotated 90 degree.The 4-0 absorable suture was used to suture the edge of each intestinal part continuously.The sutured intestine was re-tubularized,using 4-0 absorable suture and the F16 catheter was used as the tube model.The length of reconstructed ureter was 22 cm and 18 cm,respectively.The neo-ureter was re-anastomosed with renal pelvis and bladder wall.Two F6 double J stents were placed in the neo-ureter.Results The operative time was 160 min and blood loss was 200 ml in the first case.In the second case,the operative time was 180 min and blood loss was 220 ml.No significant complications were noticed intra-operation and post-operation.Six months after operation,the male patient's serum creatinine was 112 pmol/L,blood urea nitrogen was 6.1 mmol/L,serum chloride was 106.0 mmol/L and electrolytes were normal.In another patient,serum creatinine was 79 μmol/L,blood urea nitrogen was 5.9 mmol/L and serum chloride was 103.0 mmol/L.The GFR was 24.9 ml/min and 22.1 m]/min 3 and 6 months after operation,respectively.Ureteral obstruction wasn't detected on IVU images 3 months after operation.Conclusions For patient with long ureteral defect,which cannot be replaced by other urinary tissue,YangMonti ileal ureter substitution is one of the optional modalities.As a new technique of ureteral substitution,Yang-Monti ileal ureter substitution is simple and fewer complications and can improve the quality of life in patient compared with traditional ureteral substitution.
9.A Comparative Study of Drug Recall System Between China and Australia

10.Haemodynamic change and associated factors analysis in maintenance hemodialysis patients with different cardiac function

Objective To investigate haemodynamic change during hemodialysis and analyze effects of cardiac index (CI) on hemodynamic parameters and associated influencing factors in maintenance hemodialysis (MHD) patients.Methods Seventy-five patients bearing an arteriovenous fistula (AVF) entered the study.Cardiac output (CO),cardiac index (CI),central blood volume (CBV) and peripheral vascular resistance (PR) were determined by ultrasound dilution technique at the end of 1 hour,2 hours and 3 hours of dialysis.AVF blood flow (Qa) was also measured with the same device before haemodynamic parameters investigation.Results Mean age of patients was (55.84 ±12.39) years old (range 21-81 years) and 43 patients (57.3%) were female.Systolic arterial pressure,SV,CO,CI and CBV were significantly declined and no significant change for diastolic arterial pressure and heart rates at the end of 2 hour and 3 hour hemodialysis,whereas PR was increased gradually during hemodialysis.Patients were divided into there groups with CI less than 2.5 L·min 1·(m2)-1,2.5-4.2 L·min-1·(m2)-1 and more than 4.2 L·min-1 ·(m2)-1 at the end of 1 hour dialysis.Statistically significant decreasing SV,CO and increasing PR were detected in patients with CI<2.5 L·min-1· (m2)-1 and 2.5-4.2 L· min-1· (m2)-1,compared with CI>4.2 L·min-1· (m2)-1 group (P<0.01).The hemodynamic change was the most obvious in the group of CI less than 2.5 L· min-1· (m2) 1,and no significant changes happened in CI>4.2 L·min-1 ·(m2)-1 group.Some factors were found to be associated to CI values.Qa and systolic arterial pressure had positive relationship with CI,while age and diabetes had negative relationship with CI.Conclusions Systolic arterial pressure,CO,CI and CBV decrease and PR increases during hemodialysis.Obvious change occurs when CI is less than 2.5 L·min-1· (m2)-1.CI is associated with Qa,systolic arterial pressure,age and diabetes.
