1.THE UPPER AIRWAY CT SCAN OF OBSTRUCTIVE SLEEP APNEA PATIENTS

Compared with CT scan results of OSAS patients and normal adults to explore the value of CT diagnosis of upper airway stricture in patients with OSAS, 35 patients with OSAS and 35 normal adults were included in the study. 22, 19, 12, 8 cases had obstruction at palate, uvula, lingua and epiglottis region respectively, the awaked CT scan results of 61 obstructive sites were compared with those of normal adults. Obvious difference was found between patients and normal adults in area, left right dimension and coronal dimension. difference was also found in thickness of lateral pharyngeal tissue of palate, uvula and lingua region and retropharyngeal tissue of palate and uvula. The normal extent was determined by unilateral 95% confidence interval in normal adult group. And 43 sites had abnormal CT scan results, the positive ratio was 72 13%. The results sugget that there are obvious differences between the patients with OSAS and normal adults, the CT scan measure can show the upper airway obstructive site of patients with OSAS.
2.Effect of Dexamethasone on LC3 expression of neurons in cerebral cortex of juvenile rats with sepsis

Objective To investigate the effect of Dexamethasone on microtubule - associated protein 1 light chain 3(LC3)expression of cells and neurons in cerebral cortex of juvenile Wistar rats with sepsis. Methods Models of juvenile Wistar rats with sepsis were established through cecal ligation and puncture(CLP). Totally 60 cases of 30 -day - old juvenile male Wistar rats were randomly divided into sham - operation group(10 cases),non - treated group (25 cases)and Dexamethasone group(25 cases). Twelve hours after CLP,rats in Dexamethasone group were injected with Dexamethasone(1 mg / kg)via tail vein every other day,with a total of 3 times. The same dose of saline was used in the non - treated group. All rats were killed at the age of 40 days. Expressions of LC3 and neuronal nuclei(NeuN)of cells in cerebral cortex of rats were detected by using immunofluorescence assay,and the number of positive cells was calculated by using image analysis system software. Expressions of LC3 - Ⅰ and LC3 - Ⅱ protein were measured by a-dopting Western blot. Results Three hours after CLP,rats appeared to be curled up and showed piloerection and shi-vering and the neurobehavioral score in non - treated group was significantly lower than that in sham - operation group (t = 9. 895,P = 0. 000). Twelve of 25 rats in Dexamethasone group died in 10 days after CLP(48% ),while 8 of 25 rats in non - treated group died(32% ),and the difference was not statistically significant between the 2 groups(χ2 =1. 333,P = 0. 248). The immunofluorescence staining and image analysis showed the percentage of LC3 positive cells in non - treated group was significantly increased(0. 606 7 ± 0. 030 1 vs 0. 353 3 ± 0. 025 8,t = 15. 644,P = 0. 000;0. 606 7 ± 0. 030 1 vs 0. 270 3 ± 0. 019 4,t = 22. 450,P = 0. 000). The LC3 positive neurons in the cerebral cortex of rats were less in sham operation group and Dexamethasone group. Conclusions The LC3 expres-sion of cells in the cerebral cortex of juvenile Wistar rats with sepsis was up - regulated,LC3 - Ⅱ/ LC3 - Ⅰ ratio in-creased,and the number of LC3 positive neurons also increased,while Dexamethasone could have inhibitory effect on them.
3.Construction and identification of wild-type PTEN eukaryotic expression vector

Objective To construct the recombinant plasmid that highly expressed human subcellular PTEN,in order to provide a basis for further study on its anti-tumor effect. Methods PTEN cDNA was amplified by RT-PCR based on mRNA of placenta.The PCR product was ligated into T-vector,and transfected into E.coli;the obtained T-PTEN plasmid was identified with restrictive digestion and sequencing.PCR was used to incorporte nuclear signal of localization(NSL) into PTEN when T-PTEN was used as template.Then the PCR product was ligated into T-vector,and transfected into E.coli,and T-NSL-PTEN plasmid was obtained.pcDNA3.1 and T-NSL-PTEN were ligated after digested with EcoRⅠand BamHⅠ,and transfected into E.coli,the recombinant vector pcDNA3.1-NSL-PTEN was obtained,and identified with digestion and sequcncing.Results The recombinant expression vector DUM-PTEN and PUM-NSL PTEN were identified by restrictive digestion and DNA sequencing.As expected,by EcoRⅠ and BamHⅠ digestion,it showed the band of 1 200 bp.The sequencing result showed the NSL was incorporated successfully.The recombinant pcDNA3.1-PTEN was obtained with 1 200 bp,the sequencing result showed that its sequence was same as target gene;the recombinant pcDNA3.1-NSL-PTEN was comfirmed by restrictive digestion and sequencing,and the NSL was incorporated successfully. Conclusion The recombinant expression plasmid pcDNA3.1-NSL-PTEN is constructed successfully which can highly express human subcellular PTEN.
4.Mucoraceae infection lead to the pseudoaneurysms following renal transplantation: a report of 2 recipients with the same donor after cardiac death

Objective To analyze the diagnosis and treatment of iliac pseudoaneurysms following renal transplantation.Method The data of two patients with pseudoaneurysm who underwent kidney transplantation were retrospective analysied.Result One case with pseudoaneurysm received transplant nephrectomy,and pathological examination diagnosed mucoraceae infection; the other patient received endovascular treatment and amphotericin B therapy,endly died of hemorrhagic shock.Conclusion The development of pseudoaneurysms at the kidney transplantation recipients with the same donor results in high rates of mucormycosis.They should take anti-fungus therapy and operation as early as possible.
5.Correlation of tacrolimus blood concentration and the levels of blood lipid and fasting glucose after renal transplantation

Objective To study the implact of tacrolimus blood concentration (Tac) after renal transplantation on blood lipid and fasting glucose (FBG) in recipients.Method The recipients who regularly given Tac + mycophenolate mofetil + prednisone after renal transplantation were chosen and those had normal preoperative levels of blood lipid and fasting glucose as subjects.The recipients were classied into high concentration group,normal concentration group and low concentration group by comparing Tac blood concentration of different postoperative periods with corresponding normal concentration range.The changes of biochemical indexes such as steady Tac valley blood concentration,blood lipid and FBG were monitored,and the differences in blood lipid and FBG levels at different periods among three concentration groups were compared.Result TG level was significantly higher (P<0.05),and HDL-C level was significantly lower in high concentration group than in normal and low concentration groups one month after operation (P<0.05).Three months after operation,TG and FBG levels were significantly higher in high concentration group than in normal and low concentration groups (P<0.05 and 0.01,respectively).Six months after operation,TC,TG and FBG levels were significantly higher in high concentration group than in normal and low concentration groups (P<0.05,0.01 and 0.05,respectively).There was no significant difference between normal and low concentration groups in various biochemical indexes at any time point (P>0.05).Conclusion The higher blood concentration of Tac and the longer used after renal transplantation,the easier it might cause drug-induced hyperlipidemia and diabetes.

Melanin produced by T4 was investigated, and T4 was identified as Proteus mirabilis primarily.

8.Alzheimer’ s disease-like lesions may occur in and accompany the high-sugar high-fat diet-induced rat models of metabolic syndrome

Objectives To explore the relativity between Alzheimer ’ s disease ( AD)-like lesions and metabolic syndrome models induced by high-sugar high-fat diet in rats.Methods Forty-eight Sprague Dawley rats were randomly divided into 2 groups.The control group (fed with normal diet, 12 rats) and high sugar and high fat group (fed with high-sucrose and high-fat diet, 12 rats) continuously for 12 months.At the end of 6, 9 and 12 months of the experiment , we observed the animal body weight and visceral fat weight .The blood lipid levels , blood glucose and MS-related biochemical parameters were determined . The brain tissues were examined by histopathology . The characteristic AD molecules hippocampus Aβand Tau were detected using ELISA and Western blotting to confirm the presence of AD lesions in the brain.Results Compared with the normal control group , the body weight and visceral fat weight of the rats in the high-sugar high-fat groups were significantly increased; the levels of TG , FPG, LDL, HOMA-IR and hippocampus Aβ,phosphorylated Tau (p-Tau) were higher, but the level of HDL was decreased (P<0.05 for all).The histopathological examination revealed inflammatory cell infiltration in the brain tissues .Conclusions Characteristic AD-like lesions may occur and accompany the rat models of metabolic syndrome , induced by high-sugar high-fat diet, and provide a new idea for the construction of Alzheimer ’ s disease animal models .
9.Analysis on variation tendency of serum thyroid hormone level in the healthy elderly aged over 80 years

Objective To explore the variation tendency of serum thyroid hormone level in the elderly aged over 80 years.Methods The 602 healthy volunteers were divided into 3 groups by age:young group (20-59 years of age,n= 226),elderly group (60-79 years of age,n= 195),and advanced age group (80-102 years of age,n=181).Fasting blood of all persons was harvested,then the levels of serum total triiodothyroxine (TT3),total thyroxine (TT4),free tri-iodothyronine (FT3),free thyroxine (FT4),thyroid-stimulating hormone (TSH) and reverse tri-iodothyronine (rT3) were determined by chemistry luminescence technique and radioimmunoassay.Statistical analysis was made by the software SPSS 13.0.Results The levels of serum FT3 and TT3 were lower in elderly group than in young group (t=2.793,3.627,P=0.005,0.000).There were significant differences in the levels of serum TT3,TT4,FT3,TSH and rT3 between young group and advanced-age group (t =10.930,6.065,15.398,- 2.933,- 5.643,all P = 0.000),also between elderly group and advanced-age group (t= 8.382,4.298,11.573,-3.383,-5.148,all P<0.001).The levels of serum FT3,TT3 and TT4 were negatively correlated with age (r=- 0.51,-0.39 and -0.25,respectively,all P<0.01).And the levels of serum rT3 and TSH showed positive relationships with age (r=0.32,0.12,all P<0.01).There were no relationships between the level of serum FT4 and age.The positive rate of serum TT3,TT4,FT3,FT4,TSH and rT3 concentration beyond the reference value was 0,0,13.8%,0,6.6% and 21% in advanced-age group,respectively.Conclusions The levels of serum thyroid hormone and thyroid-stimulating hormone change with age.The levels of FT3,rT3 and TSH change obviously in the elderly aged over 80 years.It could reduce the false positive rate in clinical practice if normal reference range for serum thyroid hormone levels in different aged elderly is established.
10. Clinical analysis of five elderly patients with non-convulsive status epilepticus

Objective: The non-convulsive status epilepticus (NCSE) is easy misdiagnosed due to its complex etiology, clinical manifestations. It is more difficult to diagnose and treat for elderly patients with multiple basic diseases. The purpose of this study is to investigate the clinical characteristics of elderly patients with NCSE, therapy, diagnosis, and the value of long-range video electroencephalography (LV-EEG). Methods: The clinical and electroencephalography (EEG) data of the five elderly patients with NCSE in my hospital were analyzed retrospectively.Results: Five patients showed episodes of abnormal behavior during seizure (5/5). There were 3 patients with previous history of epilepsy after cerebral infarction (3/5), 1 patient diagnosed intracranial infection because of NCSE episodes to consultation (1/5), and 1 case with previous history of epilepsy (1/5). The V-EEG of the 5 patients showed persistent epileptic form discharges during seizure. Intravenous diazepam had good effect to the 5 patients during seizure. Seizure of the 5 cases was controlled acceptably with different antiepileptic drugs according to different epilepsy forms.Conclusions: The abnormal mind and behavior is still main performance in elderly patients with NCSE. The LV-EEG provides reliable foundation for diagnosis of the disease. The rational use of antiepileptic drugs is effective. Intravenous diazepam slow for elderly patients with NCSE is applied as well.
