1.Discussion of Effect that Management of Scientific Projects in Hospital Act on Supporting from National Natural Science Foundation of China

The amount of the funds supported by National Natural Science Foundation of China (NSFC) played as an important role in judging the capabilities and tendency of scientific research.It is believed that the supporting rate of NSFC is promoted via scientific management policies.To explore the tendency and potential of scientific management plan in the first affiliated hospital of Xi'an Jiaotong University,we analyze the projects supported by NSFC of our hospital from 2002 to 2011.Accordingly,our outcomes show the supporting rates of variable programs from NSFC were similar to the average for whole nation.The inspiring policies encourage our staff to put their passion on the scientific research and funding application.The quality of bidding documents for NSFC is partially improved by the suggestions and lecture of invited experts who took the charge of evaluation of funds from NSFC for actively guide,so as to increase goal average.Of note,a cohort of the researchers at the age below 45 did the main contribution to scientific research developing and programs application from NS-FC.Furthermore,we will pay more attention to reach 3 point following:1.the policy of matching fund to strengthen the competition of the researchers all over the nation.2.the policy of Integrated research resources for the major programs.3.further improvement of projects supervision.
2.The clinical use of interventional chemotherapy and intravesical instillation for preventing recurrence of superficial bladder cancer

Objective To study the therapeutic efficacy of combined interventional chemotherapy and intravesical instillation of mitomycin on preventing bladder cancers from recurring after local ablation. Methods 28 patients with superficial bladder cancers were randomized into combined interventional chemotherapy and intravesical instillation of mitomycin or intravesical instillation of mitomycin alone for preventing recurrence after local ablation.The result was assessed by ? 2 test. Results The patients have been followed up for 12~26 months (mean 21 months).1 case has had tumor recurrence in the combined modality therapy group and 4 in the intravesical instillation alone group,the tumor recurrence rate being 7% (1/14) and 29% (4/14) respectively(P

AIM: To establish the methods for determining ephedrina hydrochloridum,pseudoephedrine hydrochloride,puerarin,baicalin in Ganmao Soft Capsule (Herba Ephedrae,Radix Puerariae lobatae,Radix Scutellariae,Ramulus Cinnamomi,Radix Angelicae sinensis,Radix Angelicae dahuricae,etc.) METHODS: Part 1(determination of ephedrina hydrochloridum and pseudoephedrine hydrochloride):The HPLC was carried out on kromasilTMC18 column(4.6 mm ? 250 mm,5 ?m) with acetonitrile-0.02 mol/L phosphoric acid solution(pH2.7)-triethylamine(2 ∶ 98 ∶ 0.1)as mobile phase,gradient elution.The detection wavelength was set at 207 nm.Part 2(determination of pueranin and baicalin):The HPLC was carried out on kromasilTMC18 column(4.6 mm ? 150 mm,5 ?m)with acetonitrile-water(11 ∶ 89)as mobile phase,gradient elution.The detection wavelength was set at 250 nm.RESULTS: The average recoveries were 99.96% with RSD 1.25% ,98.41% with RSD 1.46% ,98.67% with RSD 0.91% ,99.30% with RSD 0.88% ,respectively.CONCLUSION: The assay results demonstrate that the methods are simple,accurate and reproducible for quantification of four active components in Ganmao Soft Capsule.
5.Radiosensitization of IGF-1R inhibitor AG1024 on the esophageal cancer xenografts

Objective To investigate the effect of IGF-1R inhibitor AG1024 on the esophageal cancer xenografts and the underlying mechanisms.Methods The mouse model was established by injecting EC9706 cells subcutaneous in nude mice.When the tumors were 100 mm3 in size,the mice were divided into 4 groups randomly withcontrol group with no treatment; irradiation group with 8 Gy 6 MV Xrays at 1 and 8 d each time; AG1024-treatment group with 30 μg/(kg-d) AG1024 injected intraperitoneally (ip) 5 times a week for two weeks ; combination group:receiving both 30 μg/(kg· d)-1 AG1024 ip and irradiation of 8 Gy X-rays.The diameters of the tumors were measured every 3 days.The mice were sacrificed and the weights of tumors were measured at 15 d after treatment.Tumor inhibition rate was calculated.The cell cycle was examined by flow cytometry.The expression of cell cycle protein D1 (CyclinDl) of the tumors were detected by immunohistochemical staining.Terminal deoxynucleotidyl transferase biotin-dUTP nick end labeling (TUNEL) assay was used to detect the cell apoptosis in the tumor tissue.Results The tumor weight in the irradiation group,AG1024-treatment group and combination group were significantly decreased compared with the control group,and the inhibition rate were 39.16%,18.73%,57.04%,respectively (F =13.566,P < 0.05).After treatment with AG1024 and irradiation,tumor tissue cells were significantly accumulatedin the G0/G1 and G2/M phases and decreased in S phase compared to the irradiation group (t =-6.654,-16.738,12.871,P < 0.05).The CyclinD1 expression of the combination group was significantly decreased compared with the control group.In the combination group,the apoptotic cells were detected by TUNEL assay.Conclusions IGF-1R inhibitor AG1024 could change the cell cycle and induce cell apoptosis,which might result in the enhancement of radiosensitivity on the esophageal cancer xenografts.
6.Evaluation on biocompatibility of carbon-fiber-reinforced polyether-ether-ketone after implantation in rabbit models with single bone cortex defect of mandibula

Objective To investigate the mechanical properties of carbon-fiber-reinforced polyether-ether-ketone (CF/PEEK)composite materials and to evaluate the biocompatibility and bioactivity of CF/PEEK after implanted into mandibula.Methods 27 adult Japanese white rabbits were randomly divided into experimental group (n=9) and control group (n=18),CF/PEEK was implanted into bilateral mandibula of the rabbits in experimental group, autologous bone was implanted into left mandibula of the rabbits in positive control group (n=9 )while the right sides were taken as blank control group (n=9 )without implants. The materials-bone interface of samples were collected to make the observation of gross,imaging and histology and to analyze the histological score after 8,12, and 16 weeks.Results All the animals and no infection had secondary fractures.Obviously callus formation was observed on the edge of the defect in both experimental group and positive control group in the imaging observation, while the rabbits in blank control group had no obvious change.Compared with 8 weeks after operation, the score evaluation of local cellular immune response showed significant decreasing in 12 weeks after operation(P<0.01), the data was the minimum in 16 weeks(P<0.05).At the same time,the score evaluation of local cellular immune response in three groups showed no significant difference (P> 0.05).Trabecular bone and osteoblasts could be observed in the bone defect of both experimental group and positive control group after 16 weeks,and blank control group was filled with fibrous connective tissue.The osteogenesis rates in experimental group and positive control group were increased with the prolongation of time, and they were better than that in blank control group (P<0.01).Conclusion CF/PEEK material has good mechanical property,biocompatibility and bioactivity;and it has light immune response after implantation.
7.Analysis of Volatile Components in Cladia aggregata by SPME-GC-MS

OBJECTIVE:To analyze the volatile components of Cladia aggregate by SPME-GC-MS. METHODS:The column was ZB-5 MSI 5% benzene-95% dimethylpolysiloxane fused silica capillary column,programmed temperature was heated from 5 ℃/min to 220 ℃(maintaining 2 min),detector was flame ionization detector,carrier gas was high purity He(99.999%),the column flow rate was 1.0 ml/min,vaporization room temperature was 250 ℃,splitless injection. MS conditions:ion source was electron ionization source,ionizing voltage was 70 eV, interface temperature was 280 ℃,ion source temperature was 230 ℃, quadrupole temperature was 150℃,emission current was 34.6μA,multiplier voltage was 1 502 V,solvent delay time was 1.5 min, and quality range was 30-500 amu. RESULTS:19 volatile chemical components were separated and identified from C. aggregate, mainly olefins an aldehydes,alcohols,ketones,furans oxygenated volatile chemical components,relative contents accounted for 63.55% to all chromatographic peaks in total ion chromatogram,in which,2,4-octadiene(29.66%),hexanal(5.93%),3-meth-yl-1,4-heptadiene(5.45%)and furan(4.65%)showed the highest contents. CONCLUSIONS:The study has basically made clear the main volatile components in C. aggregate,which provide certain scientific basis for further exploring C. aggregate.
8.Pancreatic neuroendocrine carcinoma: clinicopathological characteristics and prognosis

Objective To analyze the clinicopathological characteristics and the related factors influencing the prognosis of pancreatic neuroendocrine carcinoma.Methods The clinicpathological and follow-up data of 21 patients with pancreatic neuroendocrine carcinoma admitted between April 2000 and August 2013 in Zhongshan Hospital of Fudan University were analyzed retrospectively.The influence facts on patients' prognosis were analyzed statistically.Results Univariate analysis showed that age,stage,tumor size,degree of tumor differentiation and lymph node metastasis were factors separately influenced patients' survival.Multivariate analysis (Cox regression) revealed that age (P =0.019) was the only independent factor affecting the prognosis.Conclusions Patients with pancreatic neuroendocrine carcinoma were mostly non-functional and presented no specific clinical features.Tumors tended to metastasize and the prognosis was poor.Age was the independent factor affecting the prognosis.
9.Diagnosis and treatment of functional delayed gastric emptying after subtotal gastrectomy:a report of 53 cases

ObjectiveTo determine the possible causes for functional delayed gastric emptying (FDGE) and its diagnosis and treatment. MethodsThe clinical data of 53 FDGE patients after subtotal gastrectomy from 1987 to 2001 were retrospectively analysed. ResultsAll the 53 patients were recovered and discharged. Among them, 11 were misdiagnosised as mechanical ileus and were reoperated on. ConclusionsThe main cause of FDGE may be the disturbance of gastrointestinal motility which may be caused by vegetative nerve injury during the operation. The main therapy is non-surgical treatment and reoperation should be avoided at the early stage.
10.Effects of inhaled iloprost on pulmonary hypertension after cardiopulmonary bypass in patients with congenital heart disease undergoing surgical correction of anomaly

Objective To evaluate the effects of inhaled iloprost on pulmonary hypertension after cardiopulmonary bypass in patients with congenital heart disease undergoing surgical correction of anomaly. Methods Fifty-eight patients with congenital heart disease aged 14-60 yr undergoing surgical correction of anomaly under cardiopulmonary bypass (CPB) were enrolled in this study. Radial artery was cannulated before induction of anesthesia. A 6-lumen pulmonary catheter was placed via right internal jugular vein after tracheal intubation. Their mean pulmonary arterial pressure was still > 25 mm Hg after operation. ECG, HR, BP, CO, PAP and SpO_2 were continuously monitored. Aerosolized iloprost 10 fig was inhaled via nebulizer after CPB. Hemodynamics were measured before iloprost inhalation (baseline) and at 0, 5, 15, 30 and 60 min after the end of iloprost inhalation. Results There were 28 patients with pulmonary hypertension after CPB among the 58 patients with congenital heart disease. Inhalation of iloprost 10μg significantly decreased mPAP, pulmonary vascular resistance and intrapulmonary shunt as compared with the baseline. Iloprost inhalation could also improve significantly CO and the mixed venous oxygen saturation (SvO_2 ). Conclusion Iloprost inhaled after CPB can effectively reduce pulmonary arterial pressure and pulmonary vascular resistance in patients with congenital heart disease undergoing surgical correction of anomaly and help them wean from CPB.
