OBJECTIVE:To discuss the flow-sheet mode of drugs under digitized management.METHODS:Rational medicine flow-sheet was made based on the outfitting and functional authority of each drug storeroom and dispensary as well as the concrete prescription-filling method.RESULTS&CONCLUSION:The key for the success of digitized drug man?agement in hospital is to clarify a suitable relationship between receiving and dispatching of the medicine thus to keep the drugs in a due logistic network and facilitate the receiving and dispatching and accounting.
2.Epigenetic modification in human leukemia.

Epigenetic modification, which involve DNA methylation, RNA-associated silencing and histone modification, is implicated in cell proliferation, differentiation, survival, apoptosis and malignant transformation. Some leukemogenesis has been shown to be aberrance of epigenetic modification. This paper discussed the potential causes of some of leukemias correlating with the methylation of cell cycle regulation genes, small interference RNA and modification abnormality of histone after translation. The study on epigenetic modification abnormality of leukemia cells provides a new strategy for treatment of leukemia.

3.Comparison of anterior chamber maintainer and viscoelastic agent on corneal astigmatism and endothelial cells after phacoemulsification

AIM:To compare the influence of anterior chamber maintainer and viscoelastic agent on corneal astigmatism and endothelial cells after phacoemulsification.METHODS:Totally 70 patients(70 eyes) of cataract from April 2013 to April 2015 were randomly divided into the study group and the control group, with 35 cases in each group.The study group were treated with anterior chamber maintainer during phacoemulsification with support system approach, and the control group were treated with phacoemulsification under viscoelastic agent.RESULTS:The age (t=0.215, P=0.831), the density of corneal endothelial cells (t=-0.352, P=0.726) and corneal luminosity (t=-0.162, P=0.872) of two groups had no significant difference before surgery;there were no significant difference in preoperative visual acuity (t=0.463, P=0.599) and visual acuity (t=1.616, P=0.124) at 1mo after operation.And patients in the study group (t=-21.129, P<0.01) and the control group (t=-12.780, P<0.01) before surgery and 1mo postoperative when compared with the naked eye eyesight showed significant difference.The visual acuity after operations improved significantly.There were significant differences of corneal endothelial cells density (t=8.489, P< 0.01) and corneal astigmatism (t=-2.032, P=0.046) in the study group before surgery and 1mo after surgery;corneal endothelial cell density (t=8.999, P<0.01) and corneal astigmatism (t=-2.167, P=0.034) in the control group before surgery and 1mo after surgery also had significant differences.There was no significant difference in the rate of corneal endothelial cell loss between the two groups (t=0.410, P=0.683).CONCLUSION:Compared with viscoelastic agent, anterior chamber maintainer during phacoemulsification in patients with cataract won't increase the damage of postoperative surgically induced astigmatism and corneal endothelial cells, which mean the method of anterior chamber maintainer during phacoemulsification in the treatment of cataract is safe and effective.

5.Clinical efficacy for ptosis patients with severe undercorrection after frontal muscle flap suspension surgery

AIM:To observe the treatment for ptosis patients with severe undercorrection after frontal muscle flap suspension surgery. · METHODS: Totally 46 patients ( 67 eyes ) were randomly divided into 2 groups: 23 cases ( 33 eyes ) in Group A were given conjoint fascial sheath ( CFS ) suspension surgery; 23 cases ( 34 eyes ) in Group B Whitnall ligament suspension. Postoperative follow-up was 6mo. Corrected rate, recurrence rate and postoperative adverse reactions were compared and analyzed. ·RESULTS:The corrected rates of the two groups were both satisfied without significant difference between the two groups (P>0. 05). the Whitnall ligament suspension surgery had less recurrence than CFS suspension surgery, but the difference was not statistically significant (P>0. 05). The rate of hypophasis after CFS suspension surgery was significantly less than that after Whitnall ligament suspension (P<0. 05). ·CONCLUSION:The two operations both have satisfied corrected rate and low recurrence rate, the Whitnall ligament suspension surgery is less damaging and more easy to operate, while CFS suspension surgery is repeatable and less rate of hypophasis.
6.The efficacy and safety of high-dose irbesartan in treatment of clinical proteinuria in patients with chronic kidney disease

ObjectiveTo evaluate the efficacy and safety of high-dose irbesartan in the treatment of mild and moderate proteinuria in patients with chronic kidney disease (CKD).MethodsA single center,prospective,observational study was performed.A total of 96 subjects were given irbesartan 150 mg/d for 4 weeks.Twenty-six were divided into single-dose ( 150 mg/d) irbesartan group when their clinical efficacy were eligible for improvement criteria and 70 were divided into high-dose (300-600 mg/d) irbesartan group when there were no effect for single-dose treatment.Both groups received treatment for 48 weeks.Then 24-hour quantitative urine protein,systolic pressure,diastolic pressure,TC,LDL-C,plasma albumin,serum creatinine,blood urea nitrogen,blood uric acid,serum potassium and ALT were determined.Results The proteinuria level after treatment in the single-dose irbesartan group was decreased by 68.3% with a statistically significant difference( P <0.001 ).In the high-dose group,the dose of irbesartan was increased based on the ineffectiveness when treating with single-dose,and the proteinuria was decreased by 63.4% (P <0.001 ).Total effective rate in treating proteinuria in high-dose group was 72.9% (51/70).Among the blood pressure sub-groups,the effective rates for the normal blood pressure group and hypertension group in treating proteinuria were 68.2% and 76.9% respectively ( P > 0.05 ).However,in the normal blood pressure group and hypertension group,the proteinuria was decreased by 61.9% and 67.5% respectively after treatment (P<0.001,P<0.01),while without difference between the two groups (P>0.05).The effective rates of high doses of 300,450 and 600 mg/d of irbesartan in treating proteinuria were 70.8%,63.6% and 66.7%,respectively.The difference in effective rates of treating proteinuria among different doses had no statistical significance ( P > 0.05 ).No obvious increase of SCr value before and after treatment in high-dose group ( P =0.583 ).The increasing level of serum potassium in high-dose group after treatment was higher than that in the single-dose group ( P < 0.05 ),but the highest concentration (4.8 mmol/L) was still within the normal range.The blood pressure of 3 cases who quit observation because of low blood pressure in high-dose group returned to normal after drug withdrawal.ConclusionHigh-dose irbesartan can effectively lower the mild and moderate proteinuria in CKD patients with a good safety and tolerance and the efficacy is independent of lowering blood pressure.
7.Rehabilitation of Sensory Ataxic Acute Guillain-Barre Syndrome: A Case Study

Objective To observe the effect of comprehensive rehabilitation based on motor control on sensory-ataxic acute Guillain-Barre syndrome. Methods A patient with sensory-ataxic acute Guillain-Barre syndrome was treated with kinesitherapy based on motor control,included proprioceptive neuromuscular facilitation, coordination training, functional training, balance training, as well as sensory stimulation on the end of arms and legs, recumbent cross trainer therapy and cycle therapy for 2 months. Results After treatment, the score of International Cooperative Ataxia Rating Scale (ICARS) decreased 36 points, activities of daily living increased 9 points, Berg Balance Scale increased 9 points, and modified Barthel Index increased 50 points. Conclusion The comprehensive rehabilitation based on motor control is effective on this patient, but more cases and systematic curative effect evaluation are needed.
8.Clinical analysis of otogenic Mouret abscess: a case report.

Mouret abscess is a rare extracranial complication of suppurative otitis media. It is generally believed to be a deep neck abscess caused by inflammation leading to the rupture of the bony tip of the mastoid tip. The location of Mouret abscess is deep. The symptoms are insidious at the onset, but may eventually spread to the surrounding tissue, and even lead to mediastinal abscess, cavernous sinus thrombosis, meningitis, dyspnea and other serious complications. At present, with the popularization of antibiotics, the occurrence rate of Mouret abscess is very low, and only sporadic cases have been reported.In this paper, a case of Mouret abscess caused by cholesteatoma was analyzed to explore Mouret abscess in terms of the route of infection, clinical manifestations, imaging features, diagnosis and treatment.

10.Analysis of clinical features and its influencing factors of depression patients with and without psy-chotic symptoms

Objective To analyze the clinical features and its influencing factors of depression pa?tients with and without psychotic symptoms. Methods According to the presence of psychotic symptoms, 374 patients with major depression were divided into psychotic major depression ( PMD group,79 case) and nonpsychotic major depression ( NMD group,268 case) . All patients completed social demographic question?naire ( including gender,age,marital status,educational level,family history,positive family history of psychi?atric disorder,disease onset age,frequency of attack,times of hospitalization,etc.) and Hamilton's depression scale (HAMD?24). Results Positive family history(PMD group 47/79,NMD group 104/268, P<0.01), psychiatric disorder positive family history(PMD group 19/79,NMD group 26/268, P<0.01) and times of hospitalization(PMD group(2.52±1.29),NMD group(1.39±1.31), P<0.01) in the PMD group were signifi?cantly higher than the NMD group.HAMD total scores(PMD total score(47.85±8.69),NMD total score (43.44±8.51), P<0.01),the factor score of anxiety/somatization(PMD group(11.40±3.28),NMD group (9.19±3.54), P<0.01) and cognitive impairment(PMD group(12.24±3.77),NMD group(9.45±3.68), P<0.01) in the PMD group were significantly higher than the NMD group.Disease onset age(PMD group(30.09 ±5.17),NMD group(35.95±9.06), P<0.01),the course of the disease(PMD group(1.58±0.76),NMD group(3.02±2.87), P<0.01),the factor score of day and night change(PMD group(0.76±0.46),NMD group(1.01±0.51), P<0.01) in PMD group were significantly lower than the NMD. Logistic regression anal?ysis showed the major depression with positive family history of mental disorders( β=3.278) ,attack in early age( β=-2.524),seriously cognitive dysfunction in patients( β=3.836) may indicate the psychotic symp? toms( P<0.05).Conclusion PMD patients more positive family history,psychiatric disorder positive family history,likely to be hospitalization,severe symptoms and attack in early age compared with NMD. With posi?tive family history of mental disorders,attack in early age,seriously cognitive dysfunction in patients with ma?jor depression may indicate the psychotic symptoms.
