1.Diagnosis and treatment of sepsis associated encephalopathy

Sepsis associated encephalopathy (SAE) features are the diffuse cerebral dysfunction induced by the systemic response to the infection,without clinical or laboratory evidence of direct brain infection or other types of encephalopathy.The core feature of SAE is a diffuse disturbance in cerebral function free of any lateralizing signs.The primary clinical feature of SAE is a change in mental status,especially that of consciousness and cognition.The key prerequisite for the diagnosis of SAE is presence of extracranial infection.There is no precise criteria for the diagnosis of SAE or exclusion of it,and no well-established clinical or biological markers are available.But,neural electrophysiological examination,cranial imaging and biomarkers have been found with implications for the diagnosis of SAE.At present,treatment options for SAE are limited.Things of significant importance are early identification of change in mental status,searching for pathogens and lesions,as well as prompt anti-infectious treatment,maintaining hemodynamic stability and supporting therapy.
2.The influence of vasectomy and vasovasostomy on cellular function of rabbits' testis

31 male rabbits were divided into a control group (C), a vasectomy group (V), a vasovasostomy fertility group (VaF) and a vasovasostomy infertility group (VaI). The results showed that: (1) The serum testosterone levels in C, VaF, VaI and V were 7.1?4.2, 11.3.4?4.7, 4.3.4?2.3 and 4.9?2.7 nmol/L respectively, the nuclear androgen receptor concentrations in testis were 61.1?17.8, 66.2?38.2, 44.5?26.8 and 68.9?22.5 fmol/mg DNA respectively, those in VaF, VaI and V were not significanly different from that in C (P
3.Effect of vasectomy on testosterone receptor in rabbit prostate

14 Japanese white male rabbits were used in this experiment, 7 of themwere sham operation group (SOG), 7 were vasectomized group (VG). The changes of testo-sterone receptor in prostate and serum testosterone were measured 16 months after vasec-tomy. The results showed that the Kd values in cytosol in VG and SOG were 10.21 nMand 3.42 nM and the Bmax were 33.68 and 10.42 fmol/mgpro. respectively; the Kd va-lues in nucleus were 9.36 and 4.81 nM and Bmax were 440.90 and 203.66 fmol/mg DNArespectively. By single point analysis, the cytosolic testosterone receptor concentrationswere found to be 26.13?5.64 and 8.40?1.66 fmol/mgpro. respectively, being significan-tly higher in VG as compared with SOG (P
4.The correlation analysis between insulin resistance,inflammation factors and cognitive dysfunction in patients with metabolic syndrome

Objective To explore the correlation between insulin resistance,inflammation factors and cognitive dysfunction in patients with metabolic syndrome (MS).Methods 150 cases of patients with MS were chosen,and they were divided into group A1 (n=68) and group A2 (n=82) according to whether they were with mild cognitive dysfunction (MCI).50 healthy cases chosen from physical examination were set as group B.The general data,laboratory examination results and inflammatory factor levels were compared between each group.MCI was set as dependent variable,insulin sensitivity index(ISI),high sensitive C-reactive protein(hs-CRP),and tumor necrosis factor-α(TNF-α) were set as independent variables.The relationship between inflammatory factor,insulin resistance and MCI was analyzed with multiple factors logistic regression analysis.Results The difference of body mass index(BMI),total cholesterol (TC),triglyceride (TG),low density lipoprotein (LDL),highdensity lipoprotein (HDL),fasting blood glucose(FBG),hemoglobin A1c (HbA1c),fasting insulin (FINS),and ISI between the three groups had statistical significance (P<0.05).The levels of HbAlc in group A1 and group A2 were significantly higher than those in group B (P<0.05).FBG and FINS showed a trend of reduce among the three groups,while ISI showed a trend of increase.The difference had statistical significance between each group (P<0.05).The difference of hs-CRP,TNF-α between the three groups had statistical significance (P<0.05).Levels of hs-CRP and TNF-α in group A1 were significantly higher than those in group A2 and group B,while the levels of hs-CRP and TNF-α in A2 group was significantly higher than those in group B (P<0.05).Point biserial correlation analysis showed that the risk of MCI for MS patients had positive correlation with FBG,FINS,hs-CRP and TNF-α(r=0.219,0.231,0.309 and 0.287,P<0.05),while it had negative correlation(r=-0.294,P<0.05) with ISI.Multiariable logistic regression analysis showed that the more severe of insulin resistance (the lower ISI),the higher of inflammatory factor hs-CRP and TN-o,the greater the risk of MCI in patients with MS.Conclusion Insulin resistance and inflammation degree are more obvious in patients with MS combined with MCI,which can predict risk of cognitive dysfunction in patients with MS.

6.Factors affecting therapeutic effect of ADHD treated by optimal dose of methylphenidate

Objective:To explore the therapeutic effect,security and influencing factors of methylphenidate titration program,try to find the potential predicting index of therapeutic effect.Methods:This study was openly designed.The patients who were fit for DSM-Ⅳ diagnosis criteria of attention deficit hyperactivity disorder(ADHD) underwent systematic methylphenidate titration treatment until they attained the most optimal dose with best therapeutic effect and minimal side effects.The primary evaluative index of therapeutic effect was ADHD rating scale filled by teachers(ADHD-RS-Ⅳ),and secondary index included abbreviated Conners questionnaire filled by teachers,ADHD rating scale and Conners questionnaire filled by parents.The matching sample t analyses were used,then the differences of rating scale between pre and post treatment were compared.The potential influencing factors information of therapeutic effects including symptom severity degree when visiting,IQ,sensory integration capability and achievement of school etc.were collected.Finally,the correlation analysis was used for continuous variables andvariance analysis for dichotomization variables to analyze and check the factors' influence of declining scores of behavior scale.Results:The final prescribed daily dose for patients who completed dose titration was(17.3?7.7) mg on average,computed by kilo bodyweight was(0.45?0.11) mg/(kg?d).All the scale scores between pre and post treatment were significantly different(the pre-treatment score of ADHD rating scale completed by teachers was 30.25?9.16,and post-treatment score were 19.87?10.03,P

Netilmicin n etabolic characteristics of shaking-flask fermentation were investigated. In 12L glass fermentor, it was showed that phosphate and methionine stimulated growth of mycelia and increased the production of antibiotic.On this basis, netilmicin fermentation had been scaled up successfully according to the standard of the same agitated power per unit volume from 12L fermentor to 5t fermentor.

A series of activities on popularization of health sciences including nutrition and food safety, healthy lifestyle promotion, prevention and control of major infectious diseases, and decipher-ing the mystery of life have been carried out by Center for Laboratory teaching and management of Chongqing Medical University, using quality resources of teaching and scientific research, in order to explore the effective mode and long lasting mechanisms of work in popularization of sciences in higher education institutions. A combined method of quantification and qualification has been applied to sci-entifically evaluate the effect of the activities, and the results indicated that huge potentials and adva-ntages of higher education institutions existed in the field of popularization of sciences through coherent management system of the institution, as well as arousing the enthusiasm of teachers and students to take part in the activities.
9.Clinical analysis of 27 patients with philadelphia positive acute lymphoblastic leukemia

Objective To study the clinical effects of imatinib and hematopoietic stem cell transplantation in Ph+acute lym‐phocytic leukemia(ALL) .Methods Collecting 27 new diagnosed patients with Ph+ ALL in which 19 were assigned to induction treatment with imatinib combined with chemotherapy(8 of IVD ,8 of VDLP and imatinib ,3 of VDP and imatinib) ,the other 8 cases were treated with conventional VDLP chemotherapy .22 patients after complete remission(CR) had maintenance therapy combined with imtinib ,3 patients had maintenance therapy without imtinib .Followed‐up the blood routine examination ,bone marrow aspira‐tion and ABL fusion gene ,6 patients had hematopoietic stem cell transplantation(one was auto‐HSCT ) .Results The CR rate of imatinib combined was higher than the patients without imatinib(89 .5% vs .50 .0% ,P<0 .05) ,the induction mortality rate was al‐so higher (0 vs .12 .5% ,P<0 .05) .The median remission duration of imatinib combined and without imatinib were (10 .0 ± 1 .4) months and 2 .0 months (P<0 .05) .The disease‐free survival(DFS) was significantly longer in patients received allo‐HSCT than in those received chemotherapy only (83 .3% vs .12 .6% ,P<0 .05) ,but the 2 year overall survival(OS) rate was not significantly dif‐ferent(75 .0% vs .14 .0% ,P>0 .05) .Conclusion Imatinib is effective for the induction therapy of Ph+ ALL .The remission dura‐tion of patients who received HSCT is obviously longer than those who received chemotherapy only .
10.Concentration of hyaluronic acid in peritoneal fluid in patients with gastric carcinoma and its clinical significance

Objective: To study the relationship between the level of hyaluronic acid(HA) in peritoneal fluid in patients with gastric cancer and the invasion or diffusion of the cancer. Methods: The concentration of peritoneal fluid hyaluronic acid (PFHA) was determined by RIA in 48 patients with gastric cancer prior to and after operation. The PFHA from 23 non carcinoma patients were taken as control. The peritoneal fluid was obtained by the implantable Infuse Port. Results: The average PFHA level of gastric cancer group[(913.01?170.69) ?g/?g] was significantly higher than that of the non carcinoma control [(362.39?168.82) ?g/?g]. The PFHA concentration were significantly higher in groups of pTNM Ⅲ+Ⅳ [(1 094.42?127.65) ?g/?g] than that in groups of pTNM Ⅰ+Ⅱ [(814.03?232.22) ?g/?g]. The PFHA level in the radical resection group was significantly reduced a month after operation, and there were no significantly changes in patients received palliative resection. Conclusion: These findings suggest that the elevation of PFHA may be related to the peritoneal invasion and diffusion potential of gastric cancer. The PFHA level shows a decreasing tendency in patients received radical resection.
