Background and purpose:A chemokine receptor,CXCR4,and its endogenous ligand,stromal cell-derived factor-1(SDF-1),have been recognized to be involved in the invasion and metastasis of cancer.Inhibition of CXCR4 may be a new therapeutic target.We studied whether shRNA-CXCR4 could inhibit the CXCR4 gene expression and the proliferation in MCF-7,MDA-MB-231 and MDA-MB-435s breast cancer cells.Methods:Through knockdown CXCR4 by shRNA,the CXCR4 mRNA expression was detected by RT-PCR and the CXCR4 protein expression was mesured by Western blot.The proliferation of breast cancer cells was evaluated by MTT and flow cytometry.Results:The CXCR4mRNA and its protein expression decreased significantly in MCF-7(the average level of CXCR4 mRNA and protein expression were 0.089,0.177 in PG-CXCR4 group,compared with 0.327 and 0.313 for mRNA expression,0.911,0.874 for protein expression in control and PG-HK group),MDA-MB-231(the average level of CXCR4 mRNA and protein expression were 0.152 and 0.153 in PG-CXCR4 group,compared with 0.40 and 0.45 for mRNA expression,0.829 and 0.878 for protein expression in control and PG-HK group)and MDA-MB-435s(the average level of CXCR4 mRNA and protein expression were 0.198 and 0.173 in PG-CXCR4 group,compared with 0.69 and 0.77 for mRNA expression,0.877 and 0.906 for protein expression in control and PG-HK group)breast cancer cells(P

Objective To study the expression of chemokine receptor-4(CXCR4) in human breast cancer and its correlation with the prognostic factors.Methods Forty-four samples of breast cancer were collected from Sep.2004 to Jan.2006.The expressions of CXCR4 mRNA and protein were evaluated by RT-PCR and Western blotting respectively.The expressions of CXCR4,estrogen receptor(ER),progesterone receptor(PR) and C-erbB-2 were examined by immunohistochemical methods.The clinicopathological features,such as ages,menstrual status,lymph node metastasis,tumor size and histological grading were recorded and analyzed.The relationship between CXCR4 and the clinicopathological features was analyzed.Results The immunohistochemical staining showed that CXCR4 expressed mainly in cytoplasm and little in nuclei.CXCR4 mRNA and protein expressed differently in 44 samples of breast cancer,and the expressive level increased with the increase in metastatic lymph nodes number.Compared with axillary lymph nodes metastasis negative group,the expression of CXCR4 mRNA and protein increased significantly in ≥4 and 1-3 nodes metastasis groups(P0.05).Conclusion CXCR4 might play an important role in promoting the metastasis of breast cancer.
4.ANALYSIS OF PYROLYSIS GAS CHROMATOGRAPHY OF SALMONELLA TYPHI

The chromatograms of pyrolysis gas chromatography (PGC) of fifteen strains of Salmonella typhi were reported to see if there is any possible relation of the graphs to serological typing and drug-resistance spectra of the bact. In the fifteen strains there were fourteen clinical strains and one reference strain (Hg01). The results indicated that the clinical strains might be divided into group A and B (7 strains each) by PGC. The graphs drawn from group A organisms were entirely the same in shape while those of group B showed only basic similarity. According to the diversity of graphs of group B organisms, the group B organisms, may be subdivided into group B1 (4 strains), B2 (2 strains) and B3 (1 strain). The graphs within each subgroup were completely the same and those of subgroup B1 exhibited close similarity to that of the reference strain. Albeit no regular connection of chromatograms with serological types and drug-resistance could be seen, the graphs yet provided useful reference for founding a bank of chromatograms, compaed with which salmonella typhi graph could then be identified.
5.Study on the cognition status of implementing case-based payment policy among healthcare providers:A case in Chengdu, Sichuan Province

Objective:To understand the implementation status of case-based payment among healthcare provid-ers and their cognition on the matter. Methods:Semi-structured interview was conducted on 30 purposely selected staff from 9 hospitals in Chengdu. Results:After one-year implementation of case-based payment, hospitals at different level carried out the policy vigorously:executed corresponding expense control measures and management. Neverthe-less, the proportion of cases that were paid with case-based payment was low, moreover the inclusion criteria for case was of disunity and the formulation of the expense standard was ambiguous to some extent. Conclusion:Certain achievements were accompanied with problems, so it is essential to refine reimbursement standards, improve case-based payment, make clear the inclusion criteria for case and extend the covering range of case-based payment in Chengdu. In addition, the medical insurance agency should strengthen the supervision of healthcare providers,and guide them to set up effective incentive mechanism.
6.Epidemiological study of children with Kawasaki disease in Nanjing of Jiangsu Province from 2012 to 2014

Objective To evaluate the incidence and epidemiologic characteristics of Kawasaki disease (KD)in Nanjing.Methods We reviewed the medical records and reports of all patients admitted to Nanjing Chil-dren′Hospital with KD diagnosed during 3 -year periods from 2012 to 2014.Results We studied 1 446 inpatients diagnosed with KD from 2012 to 2014.There were 1 135 cases(78.5%)with complete KD.The ratio of male to female was 1.7241.The disease occurred in all of the seasons,but the peak was from spring to summer.Age at onset ranged from 1 month to 14 years old,and the peak age group was 1 year old.Fever(99.45%)was the most common clinical symptom,followed by conjunctive congestion(93.15%),rash(73.32%),oral changes(51.76%), extremities desquamate(75.08%)and lymphadenopathy(45.89%).There were 131(9.05%)patients with coronary aneurysm,the ratio of male to female was 3.02 1.14 patients (0.97%)developed recurrent KD.Conclusion Patients with KD has become more and more in Nanjing,and clinical doctors should improve the understanding of KD. Early diagnosis and treatment is very important to reduce the sequelae and reduce the mortality of KD.
7.Impact of Antiepileptic Drugs on Cognitive Function of Patients with Epilepsy

Objective To explore the impact of the different antiepileptic drugs on cognitive function in patients with epilepsy.Methods There are 280 cases of patients with epilepsy from Department of Neurology in our hospital out-patient and hospitalization, while 32 cases are the normal control group. The patients were randomly divided into groups of topiramate, carbamazepine and sodium valproate group and topiramate plus valproate combined therapy group. Wechsler Memory Scale was used by the patients before and after treatment to assess cognitive function, and compared with the normal group. Results Carbamazepine monotherapy group and the valproate group were no significant difference in areas of cognitive function belong to the normal level; Topiramate monotherapy group cognitive function was significantly lower than normal, the joint drug group was the most of the lower level of cognitive function. Conclusion Carbamazepine and valproate monotherapy were applied had the minor cognitive impairment, combination therapy of epilepsy in patients had severe cognitive impairment.
8.Studying and practicing of ultrasound anatomy teaching sample of fetus heart with complex abnormity

ObjectiveTo make ultrasound anatomy teaching samples of odynopoeia fetus heart with complex abnormity consistent with echocardiographic view,and utilize them in echo teaching.MethodsTen odynopoeia foetus hearts were cutted comparing to different echocardiographic imaging respectively.Results Dissections of fetus heart were obtained including 5 cases of single cardiac ventricle(2 case with interruption of aortic arch),4 cases of double outlet of right ventricular,and 1 case of hypoplastic left heart syndrome with serious coarctation of the aorta.The apical long axis view,the three blood vessel view,as well as the long axis view of the aortic arch were typical views of ultrasound diagnosis and teaching.ConclusionsIt is profit to raise and accurate rate of fetus echocardiography diagnosis by studying dissection of fetus heart anatomic structure with complex abnormity,which would make ultrasound imaging visualization.
9.Effects of isoflurane or sevoflurane combined with remifentanil anesthesia on blood amyloid beta protein in elderly patients undergoing abdominal surgery

Objective To compare the effects of isoflurane or sevoflurane in combination with remifentanil anesthesia on blood amyloid beta protein (Aβ) in the elderly patients undergoing abdominal surgery.Methods Two hundred patients of both sexes,aged 65-75 yr,weighing 51-76 kg,of ASA physical status Ⅰ or Ⅱ,scheduled for elective abdominal surgery under general anesthesia,were randomly divided into 2 groups (n =100 each) using a random number table:isoflurane combined with remifentanil anesthesia group (IR group) and sevoflurane combined with remifentanil anesthesia group (SR group).Fifty healthy elderly subjects served as control group (group C).After anesthesia was induced with iv penehyclidine,sufentanil,propofol and vecuronium,the patients were endotracheally intubated and mechanically ventilated.In group IR,anesthesia was maintained with inhalation of isoflurane (end-tidal concentration 1.68 %,in IR group) or sevoflurane (end-tidal concentration 1.71%,in SR group),and target-controlled infusion of remifentanil (target plasma concentration 2-6 ng/ml).At l day before surgery and 3 days after surgery,the patients' cognitive function was assessed using Mini-Mental State Examination (MMSE),the development of postoperative cognitive dysfunction (POCD) was recorded,and blood samples were taken for determination of serum Aβ40 and Aβ42 concentrations.Results The incidence of POCD was 5% (in C group),56% (in IR group) or 22% (in SR group),and there was no significant difference among the three groups.There were no significant differences in the serum Aβ42 and Aβ40 concentrations after surgery among the three groups.Conclusion The mechanism by which sevoflurane or isoflurane in combination with remifentanil anesthesia results in POCD is not related to the levels of blood Aβ40 or Aβ42 in the elderly patients undergoing abdominal surgery.
10.Detection of MPL exon 10 mutations in myeloproliferative neoplasms V617F-negative patients

Objective To explore the frequencies of MPL exon 10 mutations in JAK2 V617F-negative myeloproliferative neoplasms patients. Methods MPLW515K/L was processed through allele-specific PGR combined with direct sequence analysis. The mutations of others MPL exon 10 were detected by single strand conformation polymorphism PGR (PCR-SSCP) combined with direct sequencing. Results Of the 103 patients with JAK2 V617F-negtive myeloproliferative neoplasms patients, 1 carried MPLW515K mutation (TGG→AAG)in PMF; 1 was found to have new mutation (CTGGTGATCGCT insert) in ET and have homozygous mutation. Conclusion JAK2 V617F-negtive myeloproliferative neoplasms patients carried additional mutations in addition to W515K/L mutations in MPL exon 10, but its frequency of mutation is low.
