1.WBC Increasing Medicine Usage Analysis in Cancer Patients in a Hospital

Objective:To investigate the application of leukogenic medicine in cancer patients and standardize the use of WBC increasing medicine in a hospital. Methods:A retrospective analysis of 579 patient records from January 2014 to December in a hospital diagnosed as malignant tumor and issued WBC increasing medicine . The kinds of drugs,timing of administration, dosage,frequency of administration,the use of medication,drug combination and the choice of solvent were analysed by collecting the patients’ essential information, chemotherapy, myelosuppression etc. Results: The l white blood drugs mainly oral preparations;There were 414 rational drug use medication orders,accounting for 71. 50 percent;irrational drug use medical record 165,accounting for 28. 50% . The dosage and duration of treatment in line with requirements,and unreasonable cases mainly suitable varieties and timing of administration based,42. 42% and 27. 88% ,respectively;followed by repeated dosing and combination therapy is not suitable,13. 33% 1and 2. 73,respectively;a lower incidence of solvent choice unsuitable,is 3. 64% . Conclusion:The use of WBC increasing medicine need to be standardized in cancer patients in the hospital. Clinicians need to strengthen studying the knowledge of the application of WBC increasing medicine and use WBC increasing medicine rationally.
2.Surface Enhanced Raman Spectroscopy Sensor Based on Magnetic Beads-induced Nanoparticles Aggregation for Detection of Bacterial Deoxyribonucleic Acid

A novel method based on the separation and enrichment effect of magnetic beads and the fully complementary hybridization of two DNA strands was developed for highly sensitive detection of bacterial DNA using a surface-enhanced Raman spectroscopy (SERS) with 5,5’-dithio-bis(2-nitrobenzoic acid) (DTNB)-modified gold nanoparticles as reporter probes. Capture probe was immobilized onto the surface of streptavidin-enwrapped magnetic beads ( SA-MB ) through high affinity between biotin and avidin, by which the target bacterial DNA sequences that connected with the reported probe decorated AuNPs with DTNB and SH-DNA ( AuNPs@DTNB@DNA) were captured and loaded onto the magnetic beads by the hybridization reaction with the capture probe. Compared with previous methods, this design shortened the distance between particles by the ways that the magnetic beads tempted to nanoparticles aggregation, and produced the plasma resonance coupling effect, which increased the SERS signal significantly. The results showed that, under the optimized conditions and in the concentration range from 5 pmol/L to 5 nmol/L, the method performed a good linear relationship between Raman intensity and DNA concentration. The limit of detection ( LOD) of bacterial DNA was estimated to be 5 pmol/L. The method is simple and low cost, and can be used in the sensitive and selective detection of bacterial DNA.

4.Surgical treatment of acute biliary pancreatitis

Objective To investigate the timing of operation and the selection of surgical procedure for acute biliary pancreatitis.Methods The clinical data of 247 cases of acute biliary pancreatitis were retrospectively analysed.Results Among 10 non-operation patients,4 died;12 patients underwent emergency operation,5 had postoperative complications and 2 patients died after operation.169 patients underwent delayed operation after effective conservative therapy and 1 patient had postoperative complication;56 patients underwent(selective) operation without any postoperative complication or death.Conclusions Early emergency operation should be performed for relief of biliary obstruction in acute biliary obstructive pancreatitis.The patients with acute non-obstructive biliary pancreatitis should undergo conservative therapy at first and then,after(pancreatitis) has been controlled,delayed operation or selective operation to treat the biliary pathologic condition should be performed.

6.Determination of Paeonol in Yuxianling Granules by HPLC

OBJECTIVE: To establish a HPLC method for the determination of Paeonol in Yuxianling granules. METHODS: The determination was performed on Phenomenex C18-ODS with mobile phase consisted of methanol-water (60∶40) at a flow rate of 1.0mL?min-1. The detection wavelength was set at 274nm. RESULTS: The linear range of Paeonol was 8.24~41.20?g?mL-1(r=0.999 7). The recovery was 99.50% (RSD=1.12%). CONCLUSION: The established method is simple, rapid, sensitive, accurate, stable and reproducible, and suitable for the determination of the content of paeonol in Yuxianling granules.
7.The surgical treatment and prognosis of bilateral sporadic renal tumors.

OBJECTIVETo evaluate the surgical management and prognostic characteristics of bilateral sporadic renal tumors (BSRTs).
METHODSTwenty-three cases of BSRTs were identified and retrospectively reviewed. The presenting clinical findings, surgical treatment, and prognosis were evaluated.
RESULTSTwelve patients had synchronous tumors and 11 had asynchronous tumors. BSRTs comprised 1.8% of all RCC cases with a male-to-female ratio of 6.67:1 in the same term. The mean age of patients was 56 years. Seventeen patients were preformed bilateral operation (10 had synchronous and 7 had asynchronous tumors), 4 patients were preformed unilateral operation (1 had synchronous and 3 had asynchronous tumors), and 2 patients were not performed operation (asynchronous tumors). Twenty-one patients were followed up (11 had synchronous tumors and 10 had asynchronous tumors, mean 27 months). One and 5-years cancer specific survival rate of BSRTs were 84.6% and 33.5% respectively. The rate of renal failure of synchronous BSRTs (18%) was lower than that of asynchronous BSRTs (70%) after operation (P=0.030).
CONCLUSIONSPrognosis of BSRTs is poor. The key in the treatment of BSRTs is to detect and resect tumor earlier. Our results indicate that nephron sparing surgery should be performed in order to obtain optimal functional preservation of the renal remnant with complete local excision of tumor.

8.Research in the replacement of heated humidifiers with heat moisture exchangers during mechanical ventilation

Objective To study the feasibility of using heat and moisture exchangers (HME)as an alternative to heated humidifiers (HH) in patients undergoing mechanical ventilation. Methods 266 pa-tients with mechanical ventilation admitted to our ICU over the recent 3 years were allocated to the experi-mental group (humidification with a heat and moisture exchanger) and the control group (with heated hu-midifier), and the effect of humidification, the reserved time of artificial airway, the time on mechanical yen-tilation, the time of stay in ICU, the ineidenee of ventilator-associated pneumonia (VAP) and the mor-tality rate were comparatively studied and analyzed. Results Significant differences were found between the experimental and the control group in effect of humidification, insufficiency of humidification or excessive hu-midification, airway spasm and time on mechanical ventilation and time of stay in ICU. The incidence of VAP in the control group was significantly higher than that in the experimental group. There were no significant dif-ference between the two groups in the reserved time of artificial airway and the mortality rate. There were no accident of humidification occurred in the experimental group while there were one case complicated with air-way burn and 11 eases complicated with choking with water in the control group. Conclusions We conclude that HH can be replaced by HME on mechanical ventilation while disease evolution and effect of humidification should be monitored closely and keep HME unobstructed.
9.Differential transcription of Bcl-2 and Bax through the cell cycle in prostate cancer cell line.

OBJECTIVETo investigate the differential expression of apoptosis associated gene Bcl-2 and Bax through cell cycle and its possible clinical meaning.
METHODSThe prostate cancer cell line PC-3 was synchronized in M, G1, S and G2 phase using modified thymine deoxyriboside blockage and high pressure N2O technique. The efficiency of synchronization was detected by flow-cytometry. RT-PCR and Western blot methods were used to examine the expression of Bcl-2 and Bax in mRNA and protein level.
RESULTSThe synchronized rate of M, G1, S and G2 phase were 92.1%, 87.0%, 80.2% and 75.9% respectively. Bcl-2 was constitutively expressed through the cell cycle, but both the mRNA and protein expression level of Bcl-2 were very high in the G1 phase, dramatically decreased in M, S and G2 phase. The expression level of Bax had no change through the cell cycle.
CONCLUSIONSCell cycle could influence the expression level of Bcl-2 significantly but not Bax, these might have some clinical relevance.

10.Effects of chemotherapy on circulating angiogenic factor levels in patients with breast cancer.

OBJECTIVETo study the changes in circulating VEGF and endostatin (ES) levels during chemotherapy for patients with breast cancer, and their correlation with efficacy of chemotherapy.
METHODS40 breast cancer patients with metastases were included in this study. They received TAC/TEC, CAF/CEF, NP, CAP, CMF, TFP, TA or TC regime chemotherapy, respectively. Totally 120 serum samples were collected from the patients at three time points: before chemotherapy, the end of 1 and 5-6 chemotherapy cycles, and analyzed for VEGF and ES levels using ELISA. Tumor agiogenesis activity was evaluated by serum soluble vascular cell adhesion molecule (VCAM - 1) measured by ELISA as a surrogate marker.
RESULTS(1) Before chemotherapy, the median level of VEGF in patients with breast cancer was 496.6 pg/ml, 4.7 times higher than that of healthy controls (P <0.001). The median level of ES was 95.5 ng/ml, 18.3% lower than that of healthy controls (P = 0.183). VCAM-1 was 1077.1 ng/ml and higher than that of controls (P <0.001). The serum VEGF levels correlated with VCAM-1 levels, tumor staging and metastatic sites (P <0.05). (2) At the end of 1 cycle of chemotherapy, the serum VEGF level (median 524.8 pg/ml) was higher than the pretreatment values (P = 0.047), whereas the levels of ES and VCAM-1 were not significantly altered (110.5 ng/ml, P = 0.055; and 975.6 ng/ml, P = 0.27). (3) At the end of 5-6 cycles, the changes in VEGF correlated with the response to chemotherapy. Serum VEGF levels in 27 patients with chemotherapy-responsive and stable disease showed a significant decrease (median 287.4 pg/ml) , but not observed in 13 patients with progressive disease. VCAM-1 also showed a treatment-related change like VEGF. However, chemotherapy might only have a minor effect on ES, because there was no significant difference in the ES levels among 5-6 cycle patients, 1 cycle patients and healthy controls, and neither between therapy-responsive patients.
CONCLUSIONIntensive chemotherapy for breast cancer results in a significant decrease of serum VEGF level, which might be an indicator of the controlled disease status, and following the treatment-induced response or stabilization, the tumor angiogenesis seems to change into an anti-angiogenesis direction.
