2.Risk factors for recurrence after surgery for biliary pancreatitis

Objective To investigate the risk factors for recurrence after surgery for biliary pancreatitis.Methods A total of 284 patients with biliary pancreatitis who were treated in The Fourth Hospital of PLA from January 2008 to December 2014 were followed up,and the risk factors for postoperative recurrence were analyzed.The chi-square test was used for categorical data,and the unconditional logistic regression model was used for multivariate analysis.Results The follow-up period ranged from 6 to 27 months (mean 36.4 ± 8.4 months).Of all patients,27 experienced recurrence,and the recurrence rate was 9.51%.The univariate analysis showed that postoperative recurrence was associated with a family history of gallstone disease,high-fat diet,sand-like stones,intrahepatic bile duct stones,biliopancreatic duct opening stenosis,and diverticulum around the ampullar region (x2 =8.721,5.979,8.641,15.996,33.833,and 27.203,all P <0.05).The multivariate logistic analysis showed that high-fat diet (OR =2.296,P =0.012),biliopancreatic duct opening stenosis (OR =2.280,P =0.007),and diverticulum around the ampullar region (OR =2.522,P =0.009) were independent risk factors for recurrence after surgery for biliary pancreatitis.Conclusion Biliary pancreatitis patients with high-fat diet,biliopancreatic duct opening stenosis,or diverticulum around the ampullar region tend to experience recurrence after surgery.Intervention and close follow-up should be performed for these patients to prevent recurrence.

Parkinson's disease(PD)is a chronic neurodegenerative disease and commonly seen in middle-aged and elderly people.Combined with age-related loss of bone mass, inadequate intake of vitamin D and calcium due to dysphagia and reduced gastrointestinal motility in the late stages of the disease makes PD patients more susceptible to osteoporosis(OP). Some drugs used to treat PD can also induce and even aggravate osteoporosis in PD patients.Moreover, osteoporosis further impairs mobility in PD patients, causing pain and even fractures.This article reviews the risk factors, pathogenesis, prevention and treatment of PD combined with osteoporosis.

7.Screening of Flocculant-Producing Bacteria and Application in Treatment of Azo Dye Wastewater

Objective To screen the flocculatnt-producing strain from activated sludge that can treat azo dyeing wastewater effectively.Methods Screening and rescreening were used to acquire the strain which possesses high efficiency of flocculatant production and the strain was identified.The character of microbial flocculant(MBF) and the ability to treat azo dye wastewater were studied.Results A strain of high-efficiency bioflocculant-producting bacteria(AJ-6),initially identified as Alcaligenes sp,was screened.The bioflocculant produced by the strain was able to flocculate kaolin suspension with 94.4% and fly ash suspension with 98.9% respectively.The flocculation activity distribution tests showed that the active components of the bioflocculant existed in the supernatant fluid after centrifugation.It had good treatment efficiency in treating azo dyes methyl orange wastewater.The maximal efficiencies of removing CODCr and chroma from the wastewater were 81.3% and 94.2%.Compared with the other flocculants,the performance of MBF was better than that of polyacrylamide(PAM),aluminum sulfate,ferrous sulfate.The MBF was more thermostable when treated with 100 ℃ for 30 min,and the animal toxicity test showed that the MBF had no acute toxicity to mice.Conclusion The bioflocculant produced by the strain AJ-6 is applicable to treat azo dyeing wastewater and can be also used for the other wastewater.
8.Vegetative state treated with acupoint injection combined with plum-blossom needle in children: a randomized controlled trial.

OBJECTIVETo observe the clinical efficacy difference on vegetative state in children between acupoint injection combined with plum-blossom needle and western medication based on basic treatment.
METHODSForty-eight children of vegetative state were randomized into an observation group and a control group, 24 cases in each one. On the basis of the treatment of transcranial magnetic stimulation apparatus, balancing treatment apparatus and massage, the acupoint injection and tapping method with plum-blossom needle were adopted in the observation group, in which Xingnaojing injection, mouse nerve growth factor (mNGF) injection, monosialotetrahexosylganglioside sodium injection (MSI), compound Danshen injection were divided in 6 pairs and were injected respectively in Baihui (GV 20), Yongquan (KI 1), Fengfu (GV 16), Yamen (GV 15) and the others, 0.5 mL in each acupoint, once a day for continuous 10 days. Additionally, the tapping method with plum-blossom needle was used on the Governor Vessel and Jiaji (EX-B 2) on the back. In the control group, the intravenous infusion was adopted with citicoline sodium injection, mannitol injection and dexamethasone injection. The treatment was given once a day, 20 days of treatment made one session and totally 3 sessions were required in the two groups. The clinical efficacy, the vegetative state score and the mean curing time were observed after 20 days, 40 days and 60 days of treatment between the two groups.
RESULTSThe effective rates were 58.3% (14/24), 70.8% (17/24) and 79.2% (19/24) in 20 days, 40 days and 60 days of treatment in the observation group and 20.8% (5/24), 45.8% (11/24) and 58.3% (14/24) in the control group respectively. The efficacy in the observation group was superior to those in the control group (P < 0.01, P < 0.05). The vegetative state score was improved apparently after 20 days, 40 days and 60 days of treatment as compared with those before treatment separately (all P < 0.05). It was improved obviously at the each time point after treatment in the observation group as compared with that in the control group (3.34 +/- 2.41 vs 2.64 +/- 11.56, 6.20 +/- 1.46 vs 4.34 +/- 1.64, 11.26 +/- 2.63 vs 8.75 +/- 2.18, all P < 0.05). The mean curing time was (45.67 +/- 16.24) days in the observation group, which was shorter apparently than that of (55.34 +/- 4.57) days in the control group (P < 0.05).
CONCLUSIONBased on basic treatment acupoint injection combined with tapping method of plum-blossom needle achieve the reliable efficacy on vegetative state in children.

9.Infective endocarditis in children:analysis of risk factors and pathogenic feature

Objective To investigate pediatric infective endocarditis(IE) risk factors and the feature of pathogen.Methods We conducted a retrospective study of 46 cases who were diagnosed as IE from January 2000 to June 2007.Sixteen variables such as sex,primary disease,infectious site,complication,culture result and therapeutic measure were selected for logistic regression analysis.Results Forty-six IE patients accounted for 0.78‰ of all the hospitalized patients during the same period.Of 46 cases,40 had heart diseases.The incidence rate of left-sided endocarditis was higher than that of right-sided endocarditis.Twenty-four had positive blood culture result,one patient had positive culture from vegetation sample.The total positive culture rate was 54%.Gram-positive bacteria were found in 15 cases,and the most common organisms were α-hemolytic streptococcus.Gram-negative bacteria were in 7 cases and fungus infection was present in 3 cases.After antiboitic treatment,40 were cured,6 died.In the logistic regression model,the variables significantly associated with death included negative blood culture (OR=25.127,95%CI=1.110-363.236),hematological system disease (OR=28.620,95%CI=2.261-354.448) and multiple organ failure (OR=19.843,95%CI=0.786-221.754).Conclusion Streptococcus viridans is the most common pathogen in pediatric IE in our hospital.Higher incidence in the left heart than that in the right heart is found.Negative blood culture,hematological system disease and MOF are the risk factors in IE patients.
10.Effects of ketamine on glutamate-induced apoptosis of cultured rat spinal dorsal horn neurons and astrocytes

Objective To investigate the effects of different concentrations of ketamine on the apoptosis of cultured spinal dorsal horn neurons and astrocytes induced by glutamate. Methods Newborn Wistar rats (1-3 days old) weighing 10-15 g were used in this study. The animals were anesthetized and the dorsal hom of T11-L5 segment of the spinal cord was separated under sterile condition. The neurons and astrocytes were obtained by trypsin digestion of the tissue and mixed and cultured for 2 weeks. The cells were then randomly divided into 6 groups ( n = 8 each) : control group (C) in which Hank solution 50 ?l was added; glutamate group (G) glutamate was added (the final concentration was 100 ?mol?L-1); ketamine group (K) ketamine was added (final concentration = 1 mmol?L-1) and glutamate-ketamine groups (GK1 , GK2, GK3) glutamate was added first (final concentration was 100 ?mol?L-1) and 30 minutes later ketamine was added (the final concentration was 0.1, 1 or 10 mmol?L-1). After being incubated for 48 h the supernatant was harvested for determination of IL-1? and TNF-? concentrations. The morphological changes of the cells were examined by Wright staining. The apoptotic neurons and astrocytes were detected by flow cytometry.Results The number of apoptotic cells was significantly greater and the concentrations of IL-1? and TNF-? were significantly higher in group G and GK1 than in group C (P
