1.Difference of upper airway variety with respiration in the sufferer of OSAHS and normal adult

Objective:To inquiry the variety difference of upper airway caliber OSAHS sufferer and normal person in quiet respiration Method:Twenty OSAHS sufferer who were viewed by PSG and 16 normal adults who hadve no chief complaint of sleeping disease were selected.The curves of the subjects in a respiratory cycle were recorded by respiratory monitoring system in PSG,while the morphological changes in the harynx of all subjects were observed by fiberscope in a calm respiratory cycle,and then both of the two processes simultaneously were recorded on the same computer.According to the different stages of respiratory cycle by analyzing respiratory curve the video had been edited into pictures about the various anatomical areas in the upper airway,he cross section area and the dimension of palate and lingua and root of the tongue region upper irway wereas studied by the image tools in computer,and the changes of areas and dimensions at palate,and lingua and root of the tongue region upper airway were calculated.Result:It was found that there wasis a morphological change f the upper airway with the respiratory movement in the both groups.The upper airway caliber decrease with inspiration begin and reach the most narrowing at the end of inspiration,then upper airway caliber enlarges with the expiration begin and reach the most widening at the end of expiration.No matter the normal group or the OSAHS roup has the obvious changes in the palate and lingua region on the diameter,the cross section area and the dimension in respiration.The changes in the palate and lingua region on the diameter,the cross section area and the dimension of OSAHS group were greater than normal group.No matter OSAHS group or normal group on the diameter nd cross section area change in the palate was obviously more than the tongue area and the root of tongue area.The changes of OSAHS group on the dimension in the palate were greater than the tongue area and the root of tongue area.Conclusion:There are periodically changes of upper airway during respiration cycle in normal adults and SAHS patients.The effects of respiration on upper airway caliber of OSAHS patients are more obviously than normal adults.and the increasing effects in OSAHS patients is one of OSAHS etiology.
2.Clinical application of continuous glucose monitoring in children with diabetes mellitus

Blood glucose control is not only the key issue of diabetes management, but also one of the goals of diabetes treatment.Pain associated with self-monitoring of blood glucose(SMBG)results in poor compliance with blood glucose testing.With the advent of continuous glucose monitoring(CGM), it is more convenient to detect blood glucose and realize dynamic glucose monitoring.Continuous glucose monitoring(CGM)is widely used in adults with diabetes, and the usage among children is also increasing.Many studies have conducted clinical trials on the use of CGM in children with diabetes, initially confirming the use of CGM in children with diabetes.However, some studies still have controversies on the use of CGM in children.In this paper, the clinical studies of CGM in children′s diabetes in recent years were summarized to further understand the application of CGM and its combined insulin pump in pediatrics.
3.Determination of total flavonoids,eriodictyol-7-methylether and isosakuranetin in seeds of Artemisia halodendron Turcz

Objective:To establish a method for determination of total flavonoids,eriodictyol-7-methylether and isosakuranetin in Artemisia halodendron seeds.Method:The content of total flavonoids was determined by ultraviolet spectrophotometry at 506 nm,and compared with eriodictyol-7-methylether as a reference substance;the content of eriodictyol-7-methylether and isosakuranetin was determined by RP-HPLC,the analytical column was YMC-Pack ODS-A column(5?m,4.6 mm?250 mm),the mobile phase consisted of acetonitrile-0.2% phosphoric acid aqueous(35:65) at a ow rate of 1.0 ml?min-1,and the detection wavelength was 288 nm.Results:The linear range of total avonoids was 0.1228--0.7368mg(r=0.9991) and the average recovery(n=6) was 100.5%(RSD=1.41%);the linear ranges of eriodictyol-7-methylether and isosakuranetin were 3.016--30.16?g/ml(r=0.9999)and 3.304--33.04?g/ml(r=0.9999),respectively;the average recoveries(n=6) were 98.9%(RSD=1.20%)and 96.8%(RSD=1.38%),respectively.Conclusion:The methods of UV and HPLC are simple,accurate with good reproducibility and suitable for determination of total avonoids,eriodictyol-7-methylether and isosakuranetin.
4.Ameliorative effect of metformin on insulin resistance induced by olanzapine gavage in rats

Objective · To investigate the effect of metformin on insulin resistance (IR)-related metabolic parameters in olanzapine-bearing rats.Methods · Thirty female SD rats were randomly divided into metformin intervention group, olanzapine group and control group. During the first 6 weeks,5 mg /(kg·d) olanzapine was given to the two test groups . The control group was given the same amount of saline. From the 7th week, the intervention group began to combine with metformin 500 mg / (kg·d), while the olanzapine group combined with the same amount of saline, continuing for 4 weeks.At the end of 6th week and 10th week, intraperitoneal glucose tolerance and homeostasis model assessment IR index were assessed. Results · The area under the glucose tolerance curve (P=0.040) and the IR index (P=0.000) were significantly higher for the intervention group and the olanzapine group than the control group at the end of 6th week. At the 10th weekend, the glucose tolerance (P=0.015) and IR index in the intervention group were significantly lower than those in the olanzapine group (P=0.001). Conclusion · Metformin may rectify the impaired glucose tolerance and improve IR induced by olanzapine partly.
5.Semiquantitative assessment of wash out parameter in dynamic contrast enhanced MRI for evaluating therapeutic effect of locally advanced breast cancer after neoadjuvant chemotherapy

Objective To study the value of the semiquantitative-parameter analysis of wash out index of time-intensity curve (Swash-out) in evaluating the therapeutic effect of neoadjuvant chemotherapy for locally advanced breast cancer (LABC).Methods Fifty-nine women with LABC underwent dynamic contrast enhancedt MRI examination before chemotherapy,after the 2nd cycle and the 4th cycle of chemotherapy.All patients were divided into major histological response group (MHR) and non-major histological response group (NMHR) according to the final pathologic response.Swash-out and the variancetrends of Swash-out before NAC,after the 2nd cycle of NAC and after the 4th cycle of NAC were compared in each group and between the two groups.According to the gold standard of Miller & Payne criterion,Receiver operating characteristic curve (ROC) analysis was performed to evaluate the predicting effect of Swash-out for NAC response,and to compare it with Semi-quantitative TIC curve indicators Smax (steepest slope) and PPE (peak percent enhancement).Results Fifty-nine patients of LABC patients were divided into a MHR group of 34 patients and a NMHR group of 25 patients.Swash before NAC of MHR group was-16.99 (-56.72-41.20),Swash-out after the 2nd cycle of NAC was 5.66(-69.45-53.08),Swash-out after 4th cycle of NAC was 15.95 (-7.80-54.23).Swash-out before NAC of NMHR group was-23.08 (-64.24-34.39),Swash-out after the 2nd cycle of NAC of NMHR group was-23.01 (-52.72-28.70),Swash-out after 4th cycle of NAC of NMHR group was-11.45 (-50.49-50.93).Swash-out variance rate of MHR group after the 2nd and the 4th cycle of NAC were-1.18 (-31.32-60.86) and 1.50 (-86.27-3.61),respectively.Swash-out variance rate of NMHR group after the 2nd and the 4th cycle of NAC were-0.28(-3.24-9.46) and 0.27 (-5.34-3.11),respectively.Swash-out was not significantly different between the two groups before NAC (Z =-0.97,P >0.05).Swash-out and Swash-out variance rate of MHR group after the 2nd cycle of NAC were significant higher than that of NMHR group (Z =-3.97 and-3.02,P <0.01).Swash-out and Swash-out variance rate of MHR group after the 4th cycle of NAC were significant higher than that of NMHR group (Z =-3.96 and-3.16,P < 0.01).Area under curve (Az) after the 2nd and the 4th cycle of NAC were 0.805 and 0.804,respectively,and no significant difference was found between them (Z =0.019,P >0.05).Diagnostic cut-off points were-8.670 for the 2nd cycle of NAC and 4.105 for the 4th cycle of NAC.Diagnostic sensitivity was 79.42%,specificity was 76.00% and Youden index was 0.554,for after the 2nd and the 4th cycle of NAC.Conclusion Swash-out of TIC curve before NAC cannot predict the response of NAC,Swash-out of TIC curve after the 2nd cycle of NAC and after the 4th cycle of NAC are efficient in predicting the response of NAC.
6.Simultaneous Identification and Quantification of 9 Drugs of Opiates, Ketamine and Amphetamines in Human Plasma Using UPLC-MS/MS for Doping Control

Objective To develop the method for simultaneous identification and quantification of morphine,6-monoactylmorphine,codeine,heroin,ketamine,3,4-Methylenedioxyamphetamine (MDA),3,4-methylenedioxymethamphetamine (MDMA),amphetamine and methylamphetamine in human plasma.Methods UPLC-MS/MS was adopted to analyze plasma with protein precipitated using 10% trichloroacetic acid.Plasma samples were separated on ACQUITY UPLC HSS C18 column with aqueous solution (0.01% formic acid)-methanol (0.01% formic acid) as the mobile phase,and at a flow rate of 0.3 mL·min-1.The multiple reaction monitoring (MRM) mode was performed combined with the positive ion mode for quantification in four sections.Results The retention time,the characteristic fragment ions,and the relative abundance ratio of the molecular ion peak could be used to identify these nine compounds sensitively.The liner calibration curve of morphine,codeine,heroin,ketamine,6-monoacetylmorphine,MDA,MDMA,amphetamine and methamphetamine were obtained in the concentration range of 5.00×10-3~5.00 (r=0.9934),1.00× 10-2~ 10.00 (r=0.9905),1.00× 10-2~ 10.00 (r=0.9929),2.50× 10-3 ~2.50 (r=0.9960),5.00× 10-3 ~ 5.00 (r=0.9925),5.00× 10-4 ~ 5.00 (r=0.9910),5.00× 10-4 ~ 5.00 (r=0.9924),5.00× 10-4 ~ 5.00 (r=0.9920) and 5.00×10-4~5.00 μg·mL-1 (r=0.9900) respectively.The lowest detection limit was 1.00,1.00,1.00,0.50,0.50,0.10,0.10,0.10 and 0.10 ng· mL-1 respectively.The relative standard deviation (RSD) of the inter-day and intra-day were less than 18%.The relative recovery was more than 60%,and the RSD were less than 15%.Conclusion The method is accurate,sensitive and suitable for identification and quantification of 9 drugs of opiates,ketamine and amphetamines in human plasma.
7.Research on quality parameters of scutellariae radix (formula particles) using on-line NIR in pilot with different extraction process.

The on-line monitor for the changes in the content of baicalin in Scutellariae Radix formula particles during the extraction process was conducted by using near infrared spectroscopy (NIR). High performance liquid chromatography (HPLC) was used as a reference method. Kennard-Stone (KS) was used to divide sample sets, so as to compare different pretreatment methods. The synergy interval partial least squares (SiPLS) was used to screen out modeling wave band to establish partial least-squares models. The relative error method was applied to predict forecast set samples of Scutellariae Radix in three extraction phases. The results showed that the model established by Savitzky-Golay smoothing with 11 points (SG11 points) was the best, with the root mean square with cross validation (RMSECV), root mean square error of correction (RMSEC) and root mean square error of prediction (RMSEP) of baicalin of 0.092 7, 0.134 4 and 0.114 8, respectively, the determination coefficient R2 of greater than 0.99, and the relative error of baicalin content of less than 5%. This indicates that the on-line near infrared reflectance spectroscopy could be applied in on-line monitor and quality control of the extraction process of Scutellariae Radix formula particles.

9.Early endovascular embolization treatment for the ruptured posterior communicating artery aneurysm:analysis of the mid-term and long-term effects

Objective To evaluate the mid-term and long-term effects of early endovascular emboli-zation in treating ruptured posterior communicating artery aneurysm. Methods Early endovascular embolization therapy was carried out in 36 patients with ruptured posterior communicating artery aneurysm (36 aneurysms in total), and the effects of embolization therapy were evaluated by using Raymond grading method. The unified imaging review program was formulated, and the mid-term and long-term effects were evaluated at half, 1, 2, 3 and 5 years after the treatment. Results The technical success rate was 100% in 36 patients (36 aneurysms in total). Raymond grading evaluation showed that gradeⅠwas seen in 32 patients, gradeⅡ in 2 patients, and grade Ⅲ in 2 patients. Recurrence of aneurysm was found in 5 patients (13.89%) at half (n=1), 2 (n=1), 3 (n=1) and 5 years (n=2) after the treatment. Re-rupture of the recurrent aneurysm was seen in one patient (2.78%). No death occurred during the following-up period. Conclusion Early endovascular embolization for ruptured posterior communicating artery aneurysm can effectively improve the prognosis of subarachnoid hemorrhage, and regular imaging examination can promptly detect the recurrence of aneurysm. Effective interventions are helpful for preventing deterioration and rupture of aneurysm.
