2.Ultrasound measurement of detrusor wall thickness in equivocal bladder outlet obstruction

Objective To estimate the application of ultrasound measurement of detrusor wall thickness (DWT) in the assessment of curative effect after operation. Methods Detrusor thickness was measured by linear ultrasound (7. 5 MHz) either at a filling volume of 50% of cystometric capacity or at 250 ml filling in 86 patients, who were diagnosed equivocal BOO, during a pressure-flow study. All patients accepted transurethral resection of the prostate. At 3 months post-surgery, the patients were divided into two groups according to curative effect after operation. The volume of the prostate, age and DWT were compared between the two groups. Results There was no difference in either age or volume of the prostate between the two groups. DWT was significantly higher (P<0.01) in the more curative effect group (37 cases, DWT 2. 5±0.3 mm) compared to the less curative effect group (49 cases, 2.2±0. 3 mm). As a predictor of curative effect, DWT of 2. 8 mm or greater had a positive predictive value of 100%, a negative predictive value of 62%, specificity of 100% and sensitivity of 19%. Receiver operating characteristic analysis (ROC) revealed that DWT had a high predictive value for curative effect post-surgery with an AUC of 0. 84±0. 04. Conclusions In patients with equivocal BOO, ultrasonographically assessed detrusor thickness may have a predictive value for curative effect post-surgery. However, this cutoff value needs to be validated in a larger study population.
3.Antagonistic Effects of Extracts from Cultural Mycelium of Cordyceps sinensis on Arrhythmia

AIM The purpose is to study the anti-arrhythmic effects of extracts from cultured Cordyceps sinensis. METHODS Aconitine and barium chloride were used to induce experimental arrhythmia models in rats. RESULTS The results showed that PE was more potent than other ingredients. Pretreatment with PE by oral administration significantly antagonized arrhythmia induced by aconitine. PE significantly prolonged the onset of arrhythmia, decreased the duration and diminished the degree of severity. And in barium chloride-induced arrhythmia model, PE had some anti-arrhythmia effect,which could diminish the degree of severity effectively to some extent. CONCLUSION PE is an active component to antagonize arrhythmia induced by aconitine and barium chloride.

6.Clinical Observation of Spraying Mannatide and 5-fluorouracil in Anocelia during Surgery of Lung Cancer

OBJECTIVE:To observe the efficacy and safety of spraying mannatide and 5-fluorouracil in anocelia during sur-gery of lung cancer based on the adjuvant chemotherapy. METHODS:Totally 114 patients with lung cancer radical surgery were randomly divided into observation group(59 cases)and control group(55 cases). There were spraying mannatide(40-80 mg)and 5-fluorouracil(500 mg)in the observation group;and nothing drugs were sprayed in control group. The patients higher than phaseⅡ were treated by adjuvant chemotherapy after surgery for continuous 2 cycles,21 d as a cycle. The clinical data was compared, including amount of bleeding and drainage,hospital stay,complications,toxicity,KPS score,body weight changes,immune func-tion,survival rate and recurrence rates between 2 groups. RESULTS:There were no significant differences among the amount of bleeding and drainage,hospital stay and complications in the 2 groups (P>0.05). The nausea,vomiting,leucopenia,the KPS score,improvement of total effective rate,and improvement rate of body mass,immune function,survival rate in 1 and 2 year (s),recurrence rate and time within 2 years in observation group were significantly better than control group,with significant dif-ferences(P<0.01 or P<0.05). CONCLUSIONS:Spraying mannatide and 5-fluorouracil can reduce the toxicity in anocelia during surgery of lung cancer based on the adjuvant chemotherapy,improve the immune function and survival rate,reduce recurrence rate and prolong the recurrence time.
7.The significance of the characteristics of intra-thoracic lymph node metastasis for radiotherapy range in small cell lung cancer

Objective To explore the reasonable radiotherapy range by analyzing the patterns and characteristics of intra-thoracic lymph node metastasis in small cell lung cancer (SCLC).Methods One hundred and fifty patients with limited-stage SCLC who received radical resection of primary tumor and systemic intra-thoracic lymph node dissection were included in the study.All the lymph nodes in each area were recorded and examined pathologically to analyze the patterns and characteristics of intra-thoracic lymph node metastasis.Results A total of 2372 lymph nodes were found in 631 areas,and a total of 413 positive lymph nodes (17.4%) were found in 188 lymph node areas (29.8% ).Intra-thoracic lymph node metastasis were found in 88 patients,with a positive rate of 58.7%.The frequencies of metastasis in the area 11,10,7,5,4 were much higher than those in the other areas,and central located lesions and the higher T-stage lung tumors were more likely to develop intra-thoracic lymph node metastasis (x2 =15.32,39.72;P =0.000,0.000,respectively).Tumors located in the right upper lobe and right middle/lower lobe had a higher tendency of metastasis to the areas 4,7,10 and 4,7,10,11,respectively.Tumors located in the left upper lobe and left lower lobe had a higher tendency of metastasis to the areas 4,5,6,10 and 4,7,9,10,11,respectively.Mediastinal lymph node metastasis (N2 ) were found in 72 patients,among whom 29 patients (40.3% ) had skipping N2 metastasis without hilar metastasis.Tumors located in the upper lobe had a tendency of skipping metastasis to the upper mediastinum,while tumors located in the middle/lower lobe had a tendency of skipping metastasis to the upper and lower mediastinum.Conclusions The lymph node metastases in SCLC follow the lymphatic drainage routes,that is,from intrapulmonary to the hilar and then to the mediastinum,but with some skipping metastases.Tumors located in different lobes have different high risk lymph node areas for metastasis,and elective irradiation to these lymph node areas maybe increase radiotherapy gain ratio in SCLC.
8.Relationship between serum carcinoembryonic antigen and EGFR mutation in recurrent non-small cell lung cancer

Objective To investigate the relationship of epidermal growth factor receptor (EGFR) mutation with clinical features of baselines as well as serum CEA level in patients with recurrent non-small cell lung cancer (NSCLC). Methods A total of 54 patients with first recurrence of advanced lung cancer who had received chemotherapy were included in this study. ADx-ARMS was performed to detect EGFR gene mutations in surgical specimens taken from the primary tumor. Serum CEA level was measured by the electrochemical luminescence method. Results The mutation rate of EGFR was significantly higher in females than in males (χ2= 11.868, P =0.006), with a total mutation rate of 60.8%in 106 patients. The rate was higher in adenocarcinoma than in other histological types(χ2=6.002,P=0.014), and significantly higher in non-smokers than in smokers (χ2= 8.502,P=0.004) and in the patients with serum CEA level over or equal to 5.0 ng/mL than those with CEA level less than 5.0 ng/mL (χ2=22.543,P=0.000). A multivariate analysis revealed that a higher serum CEA level at the time of disease recurrence was associated with EGFR gene mutations (P = 0.002). Conculsions Serum CEA level is closely associated with the presence of EGFR gene mutations in patients with first recurrence of advanced NSCLC. A higher serum CEA level at the time of disease recurrence is independently associated with EGFR gene mutations. CEA level can be used as a potential indicator to determine EGFR mutation.
9.Clinical significance of circulating tumor cells detection in small cell lung cancer patients

Objective To investigate the clinical significance of circulating tumor cells (CTCs) in pa-tients with small-cell lung cancer patients (SCLC). Methods 42 SCLC patients, 10 patients with benign lung disease(hospitalized from June 2012 to June 2013), and 10 healthy volunteers from the First Affiliated Hospital of Shihezi University Medical College were studied and their clinical information was collected. Negative immuno-magnetic separation method and immunocytochemistry method were applied to collect the Pan-CK +/CD45 - of CTCs, in order to analyze the relationship between CTCs and clinical manifestation. Results CTCs were found in 32 cases of SCLC group, with a positive rate of 76.19%. CTCs was not found in peripheral blood in control group. Difference in the two groups is significant (P = 0.000). The presence of CTCs was correlated positively with the patients′ PS score , clinical stage , M stage , the number of metastatic sites , NSE value and LDH value (P < 0.05). Conclusion The CTCs number is correlated with patients′ PS score, clinical stage, M stage, the number of metastatic sites, NSE value and LDH value, respectively. CTCs detection is useful for more accurate risk assessment of patients.
10.The relationship between circulating tumor cells and clinicopathologic characteristics of primary lung cancer patients

Objective To investigate the relationship between the presence of circulating tumor cells (CTC) and the clinicopatho-logic characteristics of primary lung cancer patients .Methods Samples of peripheral blood(7 .5 mL) from 65 cases of patients with lung cancer(lung cancer group) and 10 cases of patients with lung benign diseases and 10 healthy volunteers (control group) ,and then those CTC were enriched by negative immunomagnetic separation with anti-CD45 antibody and then dectected by immunocyto-chemistry with anti-pan-cytokeratin antibody .The relationship between the positive rate of CTC and clinicopathologic characteristics of lung cancer patients were analyzed .Results The positive rate of CTC in lung cancer patients was 69 .23% (45/65) ,while CTC was not detected in any patients with lung benign disease or healthy volunteers .The positive rate of CTC in patients at TNM stageⅠ - Ⅱ ,Ⅲ and Ⅳ was 23 .08% (3/13) ,50 .00% (6/12) and 90 .00% (36/40) ,respectively .There was significant difference in posi-tive rate of CTC between patients at different TNM stages (P= 0 .000) .The positive rate of CTC in metastatic (stage M1 ) lung cancer patients(83 .33% ,35/42) was significantly higher than that in non-metastatic(stage M0 ) lung cancer patients(43 .48% ,10/23)(P=0 .002) .With the increasing number(0 ,1 ,2 ,3) of metastatic sites ,the positive rate of CTC increased (53 .48% ,83 .33% , 87 .50% ,75 .00% ) ,and there was significant difference in different positive rates of CTC (P=0 .024) .There was statistically sig-nificant difference in the positive rate of CTC between Cyfra 21-1 normal group(53 .33% ,16/30) and high value group(92 .00% ,23/25)(P=0 .021) ,the same with LDH normal group(55 .00% ,22/40) and high value group(82 .86% ,29/35)(P=0 .004) .Conclu-sion CTC were presented in some lung cancer patients .The presence of CTC in peripheral blood is correlated positively with the patients′clinical stage ,M stage ,the number of metastatic sites ,Cyfra21-1 value and LDH value .CTC detection is useful for more accurate risk assessment of patients with lung cancer .
