1.Application of hand-held autorefractor in physical examination on candidates of pilots

AIM:To compare the difference between diopter of refration measured after cycloplegic mydriasis with retinoscopy and hand-held autorefractor so as to explore the feasibility of using hand - held autorefractor for refraction screening in cadet candidates.
METHODS:Respectively using hand-held autorefractor and streak retinoscope to do optometry examination in 317 cadet candidates students ( 634 eyes ) after mydriasis. Main index of refraction values ( spherical, cylindrical mirror degree and axial) were analyzed by difference test and correlation analysis. And the elimination rate was comparatively analyzed.
RESULTS: Diopters of sphere and cyclic refraction measured by hand-held autorefration were significantly higher than those by retinoscopy ( P< 0. 05 ), but the correction analysis showed that there was high correlation (r=0. 733, 0. 694), axial of refration measured by these two methods were non - significant ( P > 0. 05 ). The disqualifying rate using autorefraction was much higher than that using retinoscopy. It was easy to be wrongly eliminated.
CONCLUSION:Because of the high correlation between diopters of autorefraction measured by retinascopy and autorefractor, autorefraction has application value in refraction screening in cadet candidates with normal vision.
2.Prognostic factors of 152 cases with Ⅲ/Ⅳ stage nasopharyngeal carcinoma

Purpose:To compare the survival rates of hyperf ractionated radiotherapy plus concurrent chemotherapy with hyperfractionated rad iotherapy alone in the treatment of stage Ⅲ/Ⅳ nasopharyngeal carcinoma (NPC) a nd analyze the prognostic factors. Methods:Between December 1992 and December 1995, 144 NPC patien ts were randomized into hyperfractionated radiotherapy plus concurrent chemother apy (R +C) and hyperfractionated radiotherapy alone (R alone).Radiotherapy were similar in the two groups: 1.2 Gy/f, twice a day. Chemotherapy was given to R + C patients before and during the course of radiotherapy. Results:The 5-year overall survival rates of the R + C and R alone groups were 63.3% and 50.7%,repectively . The factors influencing the resu lts were N stage,chemotherapy and the peripheral blood hemoglobin concentration . Conclusions:Hyperfractionated radiotherapy plus concurrent chem otherapy can improve the survival rate, especially for the patients with N2-N3 stage.The negative prognostic factors for Ⅲ/Ⅳ NPC are N2-N3 stage, radiothera py alone and the reduction of hemoglobin during treatment.
3.Study of the osteogenic ability of mesenchymal stem cells in vivo

[Objective]To investigate the osteogenic ability in vivo of mesenchymal stem cells(MSCs).[Method]Ten-fifteen ml bone marrow aspirates were harvested from the iliac crest of sheep and enriched for MSCs by density gradient centrifugation over a Percoll cushion(1.073 g/ml).After cultured and proliferated,the tissue-engineered bones were constructed with these cells and seeded onto porous 13-tricalcium phosphate ceramics(13-TCP).Then,the constructs were implanted into left metatarsus defect(21 mm in length)of 8 sheep as an experimental group.Porous ?-TCP composed with bone marrow was implanted into defects of same size and position in 8 sheep as a control group.Sheep were sacrificed in the 6th,12th,and 24th week postoperatively,and the implanted samples were examined by radiograph,histology,and biomechanical analysis.[Result]New bone tissue was observed either radiographically or histologically at the defect area of experimental group as early as the 6th week postoperatively;but was not observed in the control group.Because of the new bone formed in a direct manner without progression through a cartilaginous process intermediately,osteoid tissue,woven bone and lamellar bone,occurred earlier in the experimental group than in the control group,in which the bone defects were repaired in a "creep substitution" manner.At the 24th week,radiographs and biomechanical tests revealed an almost complete repair of the defect in experimental group,but only a partial repair in the control group.[Conclusion]MSCs have good reproductive activity not only in vitro,but also in vivo.The new bone formed in a direct manner without progression through a cartilaginous process intermediately when MSCs were transplanted in vivo.The results demonstrated that MSCs was an excellent seed cells for bone tissue engineering.
4.Early treatment for Grade ⅢB tibial fractures

Objective To seek an effective method for the treatment of grade ⅢB tibial fractures.Method From Oct. 1985 to Oct.1999, 26 cases of grade ⅢB tibial fractures were treated by early soft tissue coverage(including 12 cases of free muscle flaps and 14 cases of local muscle flaps), early fracture fixation(including 21 cases of external fixation and 5 cases of non-reamed intramedullary nail) and early bone grafting(15 cases).Results The follow up duration were from 10 to 32 months with an average of 15.5 months. The success rates of free and local muscle flaps were respectively 91.7 %(11/12) and 92.9 %(13/14)?Infection occurred in 5 cases (3 cases of local superficial infection and 2 cases of osteomyelitis). The time for bone union were from 17 to 66 weeks with an average of 42 weeks. Conclusion "San-Zao"therapy,especially early soft tissue coverage and early fracture fixation is on effective management for grade ⅢB tibial fractures with the advantages of promoting wound healing and bone union and decreasing the possibility of infection.
5.Establishment of a modified rat laryngeal transplantation model

Objective:To develop a modified rat laryngeal transplantation model to improve the survival rate of recipients.Methods: Eighty Class Ⅱ isogeneic F344 rats,weighing 250-350 g,were randomly divided into 2 groups(n=40).The control group adopted the Strome model.The allograft used for the experimental group preserved ascending pharyngeal artery,which formed a complex allograft together with tongue base,larynx and pharyngolarynx.End-to-end anastomosis was performed between both allograft common carotid arteries and anterior neck veins.The patency rate and survival rate of the allograft of the 2 groups were compared.Results: The artery,vein patency rate and survival rate were 30%,15%,and 30% in control group,and 75%,65%,and 80% in experimental group,respectively(P
6.On-line detecting of transformer winding deformation based on parameter identification of leakage inductance

Transformers are required to demonstrate the ability to withstand short circuit currents. Over currents caused by short circuit can give rise to windings deformation. In this paper, a novel method is proposed to monitor the state of transformer windings, which is achieved through on-line detecting the leakage inductance of the windings. Specifically, the mathematical model is established for on-line identifying the leakage inductance of the windings by applying least square algorithm (LSA) to the equivalent circuit equations. The effect of measurement and model inaccuracy on the identification error is analyzed, and the corrected model is also given to decrease these adverse effect on the results. Finally, dynamic test is carried out to verify our method. The test results clearly show that our method is very accurate even under the fluctuation of load or power factor. Therefore, our method can be effectively used to on-line detect the windings deformation.
7.Change of tear film function in seasonal allergic conjunctivitis between acute exacerbation and non-onset phase

Objective To investigate the change of tear film function in patients with seasonal allergic conjunctivitis (SAC) between acute exacerbation and non-onset phase.Methods This was a prospective controlled study.Functional assessment of tear film was performed in 30 eyes of 15 patients with SAC (SAC group) between acute exacerbation and non-onset phase and 15 healthy controls (control group).The tear film function included tear film break-up-time (BUT),Schirmer I test (SIt) and tear film interferometer imager measurement.Results BUT was significantly decreased in SAC group on acuteexacerbation compared with that on non-onset phase and control group [(6.97 ± 1.56) s vs.(11.27 ± 1.39),(12.00 ± 1.11) s],and there was significant difference (U =20.50,P =0.000;U =1.00,P =0.000).Moreover,BUT in SAC group on non-onset phase was similar as control group(U =322.00,P > 0.05).There was no significant difference in SIt among SAC group on acute exacerbation and non-onset phase and control group(P > 0.05).In tear film interferometer imager measurement,80.0%(24/30) was Ⅰ-Ⅱ grade,20.0%(6/30) was Ⅲ grade in control group,20.0%(6/30) was Ⅰ-Ⅱ grade,80.0%(24/30) was Ⅲ-Ⅴ grade in SAC group on acute exacerbation,60.0%(18/30) was I-Ⅱ grade,40.0%(12/30) was Ⅲ-Ⅴ grade in SAC group on non-onset phase,and there was significant difference between SAC group on acute exacerbaiion and SAC group on non-onset phase,control group (x2 =19.27,P =0.000; x2 =8.40,P =0.004),and there was no significant difference between SAC group on non-onset phase and control group (x2 =1.98,P>0.05).Conclusion SAC can cause the instability of tear film during the acute exacerbation,whereas this instability can be recovered within the non-onset phase of S A C,which is close to the normal control
8.The clinical efficacy evaluation of Asian proximal femoral nail antirotation in treatment of intertrochanteric fractures in the elderly

Objective To investigate the clinical efficacy of Asian proximal femur intramedullary nail antirotation (PFNA-Ⅱ) in the treatment of intertrochanteric fractures in elderly people.Methods From July 2009 to December 2011,127 elderly patients with intertrochanteric fracture were treated by PFNA-Ⅱ.They were 49 cases of male and 78 cases of female,with a mean age of (74.3±15) years.127 elderly patients were classified by modified Evans:type Ⅰ (n=37),typeⅡ (n=42),type Ⅲ (n=26),type Ⅳ (n=15),type V (n=7).We made the statistics on the intraoperative bleeding volume,operation time,incision length,X-light exposure times,and made evaluation of the efficacy of postoperative follow-up and complications by the Harris hip score.Results The average operation time was 42.5 min (35-90) min,the average intraoperative bleeding volume was 107.5 ml (65-410)ml,X-light exposure times were (2.5±1.4) times (2-4) times and the average total incision length was (6.5± 1.8) cm (5.5-11.0) cm.No serious complications or original complications were found during perioperative period.All patients received a 6 to 24 month follow-up (mean 12.5 months in average).The X ray examination showed that the average neck shaft angle was (134±13)° (120°-150°).The average fracture healing time was (14.0±2.5) weeks (11 to 19 weeks).The average hip Harris score was (86.5± 19.5) points (65-100 points).Among them,29 cases (22.83%) were excellent,76 cases (59.8%) were good,20 cases (15.7%) were moderate,2 cases (1.6%) were poor.Totally,excellent and good rate was 82.7%.No complications including hip varus deformity,screw cut-out and femoral shaft fractures were found.There were 14 patients (11.0%) with thigh pain and 5 patients (3.9%) with inner thigh pain.The heavier pain in 4 patients were improved by physical therapy.Conclusions Asian PFNA-Ⅱ has advantages including simple operation,fewer complications,and better clinical efficacy for the treatment of intertrochanteric fractures,but its longterm efficacy and complications still need a large sample and multi-center observation.
9.Ultrasound-guided parathyroid homogenate injection in anterior fascia of the flexor carpus vs. intramuscular transplantation for secondary hyperparathyroidism

Objectives:To evaluate forearm intramuscular autograft Tx vs. forearm subcutaneous ultrasound-guided homogenate injection transplantation. Methods:One hundred-ten end-stage chronic renal failure patients who after total parathyroidectomy and autologous forearm Tx from Mar 2011 to Mar 2020 were enrolled for retrospective analysis.The control group received autologous parathyroid forearm intramuscular autograft Tx, and the observation group had the anterior fascia of the flexor carpus ultrasound-guided homogenate injection Tx. We evaluated the effects of transplantation between the two groups.Results:There were no significant differences in the levels of iPTH, blood calcium, blood phosphorus, and calcium-phosphorus product between the two groups of patients 30 minutes , 1 month , and 3 months after surgery.The two-dimensional ultrasound simulated ellipse dispersion volume in the observation group is much smaller than that in the control group [(2.51±0.41) cm 3vs. (1.91±0.55) cm 3, t=6.519, <0.001] The linear regression equation of the actual volume of the three-dimensional ultrasound in the observation group and the iPTH value in the observation group after 3 months is Y1=47.728+ 122.337X, r1=0.892, P<0.05. The linear regression equation of the two-dimensional ultrasound simulated ellipse diffusion volume in the observation group and the iPTH value in the observation group at 3 months after operation was Y2=121.408+ 81.736X, r2=0.883, P<0.05. Conclusion:Compared with intramuscular Tx, the recovery levels of iPTH, calcium, phosphorus and calcium-phosphorus product after the anterior fascia of the flexor carpus ultrasound-guided homogenate parathyroid glands injection Tx are similar, but its operation is more simple and diffusion volume is smaller.
10.Effect of nerve invasion on long term survival of middle and distal cholangiocarcinoma

Objective To analyze the long-term survival of patients with middle and distal cholangiocarcinoma,and to explore the influence of nerve invasion on the survival time.Methods We retrospectively analyzed the clinical data of 161 patients with middle or distal cholangiocarcinoma in General Hospital of Fushun Mining Bureau between January 2012 and June 2016.All the patients were divided into nerve group and control group according to the nerve invasion.And then the perioperative and prognosis data of the two groups were analyzed and compared.Results In our research,all patients were successfully completed surgery,the incidence of neural invasion was 47.2% (76/161),more common of the nerve invasion in distal cholangiocarcinoma and phase Ⅱ cholangiocarcinoma.The perioperative mortality rate was 3.7% (6/161),and the incidence of complications was 27.3% (44/161).The 1 years,2 years and 3 years survival rates of middle and distal cholangiocarcinoma were 78.7%,42.1%,20.0%,the average survival time was(23.8 ±1.2) months.The 1 years,2 years and 3 years survival rates of nerve group and control group were 67.1%,29.8%,9.4% and 89.0%,52.6%,26.8%,the average survival time were (18.6 ± 1.4) months and (27.3 ± 1.6) months.The prognosis of nerve group was significantly poor than the control group (P < 0.05).Conclusions Nerve invasion is a common way of invasion and metastasis of cholangiocarcinoma.And it has a poor prognosis in cholangiocarcinoma patients with nerve invasion.
