1.Toxic effects of fosthiazate feeding for 90 days in rats

Objective To study the toxicity of fosthiazate feeding for 90 days in rats, and to determine the maximal non-effective dose of fosthiazate , in order to provide the reference dose for safety in production and chronic toxicity experiment.Methods A total of 80 SD rats ( half female and half male ) were randomly divided into 4 groups, respectively:0.8 mg/kg· bw· d group, 4.0 mg/kg· bw· d group, 20.0 mg/kg· bw· d group, and normal control group .The rats were sacrificed to determine the indices including serum biochemical parameters , body weight , routine urine test and organ coefficients after the end of the experiment , and the results were statistically analyzed .Results In the high dose group, the body weight gain was slowed in male and female rats .The TG and CHE in the high dose group of male rats and the TP, ALB, CREA, GLU, and CHE in the high dose group of female rats were significantly lower than those of normal control group.The ALP in the high dose group of female rats was higher than that of the normal control group .The positive rates of BIL, SG, and PRO in both male and female rats had significant differences compared with those of normal control group.The organ coefficients of brain , lung, kidney, adrenal, and testis of male rats, and the organ coefficients of brain , lung, and kidney of female rats in the high dose group were significantly higher than those of the normal control group .The ovaries and uterus in the female rats of high dose group were significantly lower than those of normal control group ( P<0.05,P<0.01).Conclusions The oral dose of fosthiazate at 4.0 mg/kg· bw· d fed for 90 days and above cause toxic effects on rats , and its maximal non-effect dose of long-term intake of low-dose fosthiazate on rats is 4 mg/kg· bw· d.

3.Primary intensive insulin therapy improves islet ?-cell function in type 2 diabetes——A dream or reality?

Hyperglycaemia attenuates?-cell insulin secretion in response to plasma glucose elevation and reduces insulin-mediated glucose transport. The long-standing severe hyperglycaemia may irreversibly damage?-cells. However, in this regard, there are increasing evidences showing that short-term intensive insulin therapy may significantly improve the response to endogenous insulin by normalizing blood glucose, so as to induce a "honeymoon period" for several years in a considerable number of subjects with newly diagnosed type 2 diabetes. Thus, a question has been raised: whether or not insulin therapy should be taken as a part of the strategy of the primary treatment for type 2 diabetes? A number of experts believe that it is an important therapeutic strategy, however, others hold that insulin therapy may yield more cost than benefit and suggest that to halt this cycle of progressive metabolic deterioration by any means may transiently restore metabolic re-compensation in type 2 diabetes. This issue was discussed in the fourth meeting of China Group on Insulin Secretion and in 66th ADA scientific session this year. Here we offer our consideration on this question for your reference.
4.The diagnostic value of endoscopy assisted with laparoscopy in patient with small intestinal bleeding

Objective To study the diagnostic value of endoscopy assisted by laparoscopy in patients with small intestinal bleeding. Methods In case of small intestinal bleeding it is hard to define the accurate etiological factor by routine examination. From 1994 to 2002 there were 14 patients with chronic me-lena or persisted melena complicated with chronic anemia and 2 cases of acute alimentary tract hematorrhea complicated with obvious hypovolemia. Results Endoscopies assisted by laparoscopy were accomplished in all of the 16 cases. Lesions were detected in 5 cases with laparoscopy directly, and in 11 cases the pathological changes were discovered by endoscopies assisted with laparoscopy . The pathologic results were leiomyo-sarcoma 1 cases, leiomyoma 4 cases, adenoma of small intestine , hemangioma , vascular malformation and capillary telangiectasia 2 cases each, and Meckels diverticulitis with hemorrhage 3 cases. Conclusion The diagnostic value of endoscopy assisted with laparoscopy is superior to the routine examination in detecting the small intestinal bleeding. This technique has the advantage of simple, less-traumatic and high sensitivity. It is worthy to popularize in clinical practice.
5.The significance of lactate dehydrogenase in the diagnosis and treatment of children with refractory Mycoplasma pneumoniae pneumonia

Objective To investigate the significance of lactate dehydrogenase(LDH) in the diagnosis and treatment of children with refractory Mycoplasma pneumoniae pneumonia(RMPP).Methods This was a study of children with Mycoplasma pneumoniae pneumonia(MPP) admitted to hospital from June 2013 to June 2016.All patients were treated with erythromycin.Based on the definition of RMPP,subjects were divided into usual MPP group and RMPP group.The children in RMPP group were treated with erythromycin combined with methylprednisolone.The indicators including LDH,WBC counts,C-reactive protein,erythrocyte sedimentation rate(ESR),alanine aminotransferase(ALT),aspartate amino ransferase(AST) and creatine kinase were analyzed between RMPP group and usual MPP group.The differences of the above indicators between the effective and ineffective group before and after methylprednisolone treatment were also compared.Results In total,253 subjects were enrolled,including 161 in the usual MPP group and 92 in the RMPP group.The average age in RMPP group was older than that in the usual MPP group.The indicators(including LDH,AST,ALT,and ESR) of RMPP group were higher than those in the usual MPP group.Logistic regression showed that LDH and ESR were the significant factors in predicting RMPP.The odds ratio(OR) of LDH was 1.029 with 95%CI 1.020-1.037,and the OR of ESR was 1.063 with 95%CI 1.009-1.120.Receiver operating characteristic curve analysis showed that the area under the curve of LDH(400.50U/L) was the largest(0.959,95%CI 0.936-0.983).The administration of methylprednisolone to patients in the RMPP group resulted in the rapid improvement of symptoms and decrease in serum LDH and ESR levels in effective group(P<0.05).Conclusion Serum LDH may be used as a biomarker to predict RMPP at the early stage,also may be an important marker for the evaluation of therapeutic efficacy in RMPP.
6.Application of microcurrent detector to intraoperative diagnosis of 101 cases of tumor

Objective To investigate the relationship of tumor malignancy with Na+ microcurrent measured by microcurrent detector in order to develop a novel approach for rapid intraoperative diagnosis of tumor.Methods 101 cases of various tumors were studied.Na+ microcurrent of tumor tissue or surrounding normal tissue was measured by Na+ microcurrent detector.All tumor tissues were sampled for histopathological examination as well.Results In 97 cases(96%),the Na+ microcurrent level in tumor tissue was significantly different from that in surrounding normal tissue.It was greater than 30?A in tumor tissue with the maximum of 60-70?A,and averagely 20?A in surrounding normal tissue.The consistency of Na+ microcurrent detection and pathological examination was 96%.Conclusion the measurement of Na+ microcurrent by microcurrent detector is rapid,convenient and accurate,which may become a novel approach for rapid intraoperative diagnosis of tumor malignancy.
7.Growth hormone with glutamine accelerate remnant intestinal adaptation after massive small bowel resection in rat

Objectives: In order to investigate effect and mechanism of solely or combined treatment of glutamine (Gln) and growth hormone (GH) on remnant intestinal adaptation after massive small bowel resection, we observed the weight of the rats, and analyzed morphology, nutrients absorptive function and gene expression of the remnant intestine. Methods: All rats undertaken 85% small bowel resection and placement of intravenous catheter and gastric tube were randomly assigned to 4 groups :①glycine (SB); ②GH+ glycine (SB/GH); ③Gln (SB/Gln); ④Gln+GH (SB/GH/Gln). All rats received isocaloric, isonitrogenous and isovolumic nutrition support. All rats were weighed daily after operation. On the POD12, crypt depth, villus height and mucosa thickness of remnant small bowel were analyzed; the \[ 14 C\] glucose and \ palmitic acid absorptive test were in vivo assayed; plasma insulin like growth factor 1(IGF 1) was measured; IGF 1, sodium glucose cotransport 1(SGLT1) and intestinal fatty acid binding protein(I FABP) mRNA were semiquatified analyzed by RT PCR. Results: The body weight, plasma IGF 1, villous height and mucosal thickness,\[ 14 C\] glucose and \ palmitic acid absorption both in SB/GH group and SB/Gln group had significantly increased than those in SB group, while those in SB/GH/Gln were significantly higher than those in any other group. The mRNA expression of IGF 1, SGLT1 and I FABP in SB/GH and SB/GH/Gln groups were significantly higher than those in SB and SB/Gln groups. Conclusions: Combined treatment of GH and Gln can enhance the intestinal trophic effect after massive small bowel resection. Gln can enlarge the mucosal absorptive surface in remnant small bowel. GH can not only enlarge the mucosal absorptive surface, but also enhance absorptive functional gene expression of enterocyte via mediation by local expression of IGF 1 .
8.Risk factors for development of early acute renal failure after liver transplantation in patients with normal renal function

Objective To determine the risk factors for development of early acute renal failure (ARF) after orthotopic liver transplantation (OLT) in patients with normal renal function.Methods Sixty ASA Ⅱ or Ⅲ patients aged 28-64 yr weighing 35-88 kg undergoing OLT were studied.Their preoperative serum Cr and BUN were within normal range.Early ARF was defined as serum Cr≥132 μmol/L and/or BUN≥18 mmol/L within 24 h after operation.The patients were divided into 2 groups: ARF group and non-ARF group.Arterial blood samples and urine specimens were collected before induction of anesthesia for determination of blood β2-micreglobulin(β2-MG) and urinary β2-MG and N-acetyl-β-D-glucurenidnse (NAG). Factors including preoperative liver function,preoperative blood and urinary β2-MG,the amount of urine output and bank blood infused during operation,MAP during anhepatic and neohepatic phase,the amount of vnsoactive drugs and diuretics used during operation,hypotension and arrbythmia during operation were recorded.The risk factors were identified by logistic regression analysis.Results Logistic analysis indicated that serum β2-MG higher than normal value before operation and persistent hypotensien during operation were closely correlated with development of early ARF after OLT.Conclusion Serum β2-MG higher than the normal value before operation.and persistent hypotension during operation are the risk factors for early ARF after OLT.
9.Effects of bisphosphonate on dental mineralization protein 1 expression in condylar cartilage of osteoporotic rats

Objective To examine the distribution and expression of dentin matrix protein1 ( DMP1 ) in the condylar cartilage and subchondral bone of osteoporosis rats. Methods Female Sprague-Dawley rats aged 6 months (n=30)wererandomlydividedinto3groups. TheShamgroupunderwentshamoperationonly(n=10),theOVX group ( n = 10 ) received a bilateral ovariectomy first and then saline solution treatment subcutaneously for 3 months. The RIS group ( n=10 ) also received a bilateral ovariectomy and then with risedronate treatment ( 2. 4μg/kg) subcutaneously for 3 months. Three months after the operation, the animals were sacrificed. Toluidine blue staining showed the structure changes of rat condylar cartilage region. The changes of osteoclasts in the bony subcondylar region were evaluated by tartrate-resistant acid phosphatase ( TRAP) staining. The expression of DMP1 was analyzed immunohistochemically and then performed by semi-quantitative imaging analyses. Results Toluidine blue staining showed a thickened hypertrophic layers of condylar cartilage in RIS group. The results of TRAP staining indicated that the number of osteoclasts was significantly greater in OVX group than RIS group (P<0. 05). Immunohistochemistry showed that DMP1 localized mainly in the chondrogenic layers and osteocytes, bony subcondylar region in three groups. The expression levels of DMP1 proteins statistically decreased in OVX group than the other two groups(both P<0. 05). Conclusion Bisphophonates may reduce the the number of osteoclasts in the condyle from osteoporosis rats, with increasing of the expression of DMP1, which may influences condylar cartilage biomineralization.
10.Effects of bisphosphonates on the expression of Bcl-2, Bax and Caspase-3 in condylar cartilage of osteoporosis rats

RESULTS AND CONCLUSION:(1)The number of apoptotic cels in rat condylar cartilage and subcondylar region: the sham operation group < the treatment group < the model group (alP< 0.05). (2)Expression of Bcl-2: The trend of the model group was lower than that in the sham operation group, although there was no statisticaly significant difference between the two groups; Bcl-2 expression in the treatment group was statisticaly higher compared to the model group (P< 0.05).(3)Expression of Bax and Caspase-3: The expression levels of Bax and Caspase-3 were higher in the model group than in the sham operation group (alP< 0.05), while Bax and Caspase-3 expression was lower in the treatment group than that in the model group (alP< 0.05). The results suggested that bisphophonates can regulate apoptosis in condylar cartilage from osteoporosis rats by changing the expression of Bcl-2, Bax and Caspase-3.
