1.Clinical analysis of cognitive function and depressive states in patients with age-related macular degeneration

AIM: To investigate the difference of cognitive impairment and depression between age-related macular degeneration ( AMD ) group and the control group patients. ·METHODS: A prospective case-control study was performed from November 2014 to August 2016 in the hospital for AMD patients and sex-matched control group. The Mini-Mental State Examination ( MMSE) and the Geriatric Depression Scale ( GDS ) score of each patient were collected for statistical analyzing. ·RESULTS: There were total 84 cases ( 168 eyes ) included in the study. The difference of visual acuity between the two group was statistically significant ( F=8. 953, P=0. 004) by baseline data analyzing. There were no significant differences in MMSE scores between the two groups according to educational status ( P>0. 05 ) , while the prevalence of cognitive impairment in each group was statistically significant (x2 =4. 14, P=0. 042). The difference of GDS scores, prevalence of total and mild depression between two groups were both statistically significant (F=5. 852, P=0. 018; x2=6. 372, P=0. 012; x2 = 5. 674, P = 0. 017 ). However, there was no statistically significant difference in the prevalence of moderate to severe depression (x2=0. 672, P=0. 412). ·CONCLUSION: AMD patients have a higher prevalence of depression. Although MMSE score differences were not statistically significant in subgroup analysis by educational levels, AMD patients are more likely to have cognitive impairment overall.

Objective To investigate the inhibition effect of plasma-mediated small interfering RNA (siRNA) on the expression of monocyte chemotacite protein-1 (MCP-1) gene in human renal tubular epithelial cells (HKC).Methods Three pairs of siRNAs directed human's MCP-1 mRNA 67,116,142 targets were designed and synthesized.Eukaryotic expression vector special for MCP-1,pRNAT-MCP-1-Ⅰ、pRNAT-MCP-1-Ⅱ、pRNAT-MCP-1-Ⅲ were constructed and transfected into HKC by lipofectamine.At 24 hour after transfection,the expression of MCP-1 in the levels of mRNA was detected by Real Time RT-PCR,and the expression of MCP-1 in the levels of protein was detected by Western blot.Results Transfection efficiency of siRNA expression vector was 60%;the expression of MCP-1 in the levels of mRNA and protein of three pairs of plasma-mediated siRNA group were markedly decreased compared with normal control group(P

A young female complained repeated nasal discharge for over three months with discomfort of right cheek, and oral antibiotics had less effect. She has a history of "root canal therapy" five years before. Physical examination found purulent secretion in the right middle nasal meatus, and light tenderness in the right side of the maxillary sinus area. The CT scan of paranasal sinus shown possible fungal infection of right maxillary sinus. Finally the nasal endoscopic surgery confirmed the fungus ball of right maxillary sinus with foreign body (the root canal filling material).

6.Effects of neuropeptide Y gene transfection on seizure in rat and the expression of phosphorylated Tau protein in the rat hippocampus

Objective To discuss the effect of gene transfection of rAAV2/1-NPY-EGFP on KA-induced rat seizures,EEG and the expression of hippocampal phosphorylated Tau protein.Methods Altogether 72 healthy male Wistar adult rats were randomly divided into three groups:control group,KA group and NPY group(n=24).The epileptic models were established by the injection of KA 2 μl (0.4 μg/μl) five times to the right side of the hippocampus CA3 area every three days.rAAV2/1-NPY-EGFP group,in which 10 μl of rAAV2/1-NPY-EGFP (titer 5× 1011 v.g./ml) was injected to the lateral ventricle in successful rats chronic model,while KA group was injected with an equal dose of saline.The control group was injected with an equal dose of saline both in the hippocampal CA3 area and the lateral ventricle.The seizure situation,the onset latency and EEG were observed at 2 weeks and 4 weeks after vector injection.Then the expression of phosphorylated Tau protein in hippocampus were detected with Western blotting.Results (1) Scale and latency of each seizure onset in rats of rAAV2/1-NPY-EGFP group (12.13 ± 8.06) had no significant difference at 2 weeks (P> 0.05) compared with KA group (12.10± 8.07).The scale of seizure in rats of rAAV2/1-NPY-EGFP group(6.06±3.78) significantly reduced at 4 weeks(P <0.05).Latency of seizure onset (79.06±8.83min) significantly increased at 4 weeks(P<0.05),EEG epileptic discharge frequency and wave amplitude decreased (P< 0.05) at 4 weeks.The control group had no seizures.(2)Compared with the control group,the expression of phosphorylated Tau protein in KA group and NPY group significantly increased(P<0.05) at 2 weeks and 4 weeks,and the expression of phosphorylated Tau protein in the NPY group (1.15±0.16 RQ value) at 4 weeks significantly decreased (P<0.05) compared with the KA group(1.87± 0.23 RQ value).Conclusion rAAV2/1-NPY-EGFP gene transfection significantly reduces scale of seizure onset and prolongs latency of seizure onset in KA-induced rat model.rAAV2/1-NPY-EGFP gene transfection may play anti-epileptic and neuroprotective effects through inhibiting the expression of phosphorylated Tau protein in hippocampus of KA-induced epileptic rat model.
7.Effects of ischemic time factors on reduction of myocardial ischemia-reperfusion injury by sulfentanil postconditioning in rats

Objective To evaluate the effects of ischemic time factors on reduction of myocardial ischemia-reperfusion injury by sulfentanil postconditioning in rats .Methods Healthy adult male Sprague-Dawley rats ,weighing 230-250 g ,were heparinized and anesthetized with intraperitoneal 5% chloral hydrate 8 ml/kg .The hearts were excised and perfused in a Langendorff apparatus with K-H solution .After 30 min of stabilization ,48 isolated rat hearts were randomly assigned into 6 groups (n=8 each) using a random number table .In I/R1-3 groups ,the hearts were subjected to 30 ,45 ,and 60 min of myocardial ischemia ,respectively ,followed by 90 min of reperfusion . In SP1-3 groups , the hearts were subjected to 30 , 45 , and 60 min of myocardial ischemia , respectively ,followed by 90 min of reperfusion ,and sulfentanil 100 nmol/L was added to K-H solution at 15 min of reperfusion for postconditioning . Left ventricular end-diastolic pressure (LVEDP ) , left ventricular developed pressure (LVDP) ,+dp/dtmax ,-dp/dtmax ,HR and coronary flow (CF) were measured at the end of equilibration and 15 ,30 ,60 and 90 min of reperfusion .Creatine kinase (CK) and lactate dehydrogenase (LDH) activities in coronary effluent were measured at 5 min of reperfusion .Myocardial infarct size was determined in 5 hearts chosen randomly at 90 min of reperfusion .Results LVEDP was significantly higher ,and LVDP , ± dp/dtmax and CF were lower at each time point of reperfusion than at the end of equilibration in I/R1-3 groups ( P<0.05) .Compared with I/R1 and I/R2 groups ,LVEDP was significantly increased ,and LVDP and ± dp/dtmax were decreased at 30 ,60 and 90 min of reperfusion ,HR and CF were decreased ,and CK and LDH activities in coronary effluent and infarct size were increased at 15 and 30 min of reperfusion in group I/R3 ,and LVEDP was significantly decreased ,LVDP , ± dp/dtmax and CF were increased ,CK and LDH activities in coronary effluent and infarct size were decreased at 30 , 60 and 90 min of reperfusion in groups SP1 and SP2 ( P< 0.05 ) . Compared with group I/R3 , LVDP was significantly decreased at 30 ,60 and 90 min of reperfusion ( P<0.05) ,and no significant change was found in CK and LDH activities in coronary effluent and infarct size in group SP3 ( P>0.05 ) .Conclusion Sulfentanil postconditioning can attenuate myocardial ischemia-reperfusion injury when the rats are subjected to 30 or 45 min of ischemia ,however ,it provides no myocardial protection when the rats are subjected to 60 min of ischemia .
8.The complications of traumatic optic neuropathy

Objective:To investigate the complications of traumatic optic neuropathy and to call the surgeons' attention to precaution. Method:Retrospectively analysis of 122 cases patients with traumatic optic neuropathy and 3 cases were analyzed in detail including 1 case with purulent meningitis, another 1 case with internal carotid artery pseudoaneurysm and the other 1 case with internal carotid artery cavernous sinus fistula. Result:Most of the patients had the complications of orbital fracture, maxillofacial fracture, ocular and craniocerebral injury. A few of patients had other injuries all over the body. The case with purulent meningitis was cured with antibiotics. The case with internal carotid artery pseudoaneurysm was cured with neurosurgery. The visual acuity of the both cases were improved. The case with internal carotid artery cavernous sinus fistula died of severe hemorrhea. Conclusion: The patient with traumatic optic neuropathy has the possibility of severe cranial disorders,orbital fracture, maxillofacial fracture and injuries of viscera or limbs. It should be paid mare attention and treated accordingly.
9.Single-fiber conduction study of motor nerve in patients with diabetic peripheral neuropathy

Objective To assess the function of motor nerve fiber in patients with diabetic peripheral neuropathy (DPN) by single-fiber conduction studies.Methods According to the diagnostic standard of DPN issued in Toronto meeting in 2009,on the basis of the result of peroneal nerve conventional conduction study,a total of 65 patients with DPN in the Department of Endocrinology and the Department of Neurology of Tianjin Third Central Hospital from October 2012 to October 2013 were enrolled into the study,from whom 33 had abnormal sensory conduction (sensory-diabetic peripheral neuropathy group,S-DPN group),32 had abnormal sensory motor conduction (sensory motor-diabetic peripheral neuropathy group,SM-DPN group).Single-fiber conduction velocity (SF-CV) and single-fiber distal motor latency (SF-DML)were detected in all subjects.The obtained results were compared with the data from 34 healthy volunteers (control group).The relationship of SF-CV,SF-DML and the duration of diabetes mellitus,fasting glucose,HbA1 c was also studied in DPN patients.Results The SF-CV ((43.1 ± 3.6) m/s) was decreased in S-DPN group compared with control group ((47.5 ± 3.3) m/s,t =5.077,P < 0.01).There were no significant differences in SF-DML ((3.6 ± 0.7) ms),motor nerve conduction velocity (MCV (49.5 ± 2.6)m/s) and DML ((3.4 ± 0.6) ms) in S-DPN group compared with that of control group ((3.4 ± 0.5) ms,(50.9 ± 3.5) m/s,(3.2 ± 0.5) ms,respectively).SM-DPN group had lower SF-CV ((35.2 ± 3.6)m/s,t =9.119,14.219),MCV ((40.9 ± 3.2) m/s,t =11.131,13.025) and increased SF-DML ((4.5±0.7) ms,t=5.692,7.231),DML ((4.2 ±0.7) ms,t=5.561,6.975) compared with the other two groups (P <0.01).SF-CV in DPN patients was negatively related to the diabetic duration (r =-0.340,P =0.006),while SF-DML had no correlation with duration of DM,fasting blood glucose and HbAlc.Conclusions Detection of SF-CV is easy to find early motor nerve dysfunction in DPN patients.SF-CV is decreased with the increasing duration of diabetes.
10.Analysis of laparoscopic cervical cerclage before pregnancy after pregnancy

Objective To analyze the pregnancy outcome of laparoscopic cervical cerclage before pregnancy, discuss the clinical value of the treatment of cervical incompetence.Methods 40 cases with cervical insufficiency during 2012.1 -2016.3 before pregnancy underwent laparoscopic ligation.All patients had a history of abortion or premature birth,and the average gestational age was 23.7 weeks.Preoperative cervical length was less than 2.5cm. The operation condition and the pregnancy outcome after operation were observed.Results The average time of oper-ation was 35.6(40.2 ±10.4)min and the amount of bleeding was 10.3(15.7 ±3.2)mL.Postoperative pregnancy cervical average length was 3.61 (4.35 ±0.52)cm.Pregnancy outcome:37 weeks of gestation and above delivery 20 cases (50.0%).34 -37 weeks 16 cases (40.0%),28 -34 weeks in 3 cases,received cesarean section,the aver-age gestational was 35.6 weeks.Compared with the preoperative abortion,the gestational week of delivery was pro-longed by 12.1 weeks.Neonatal survival in 38 cases (95.0%),1 case of gestational 28 +3 weeks premature death. Neonatal birth weight was 2 800(3 079 ±500)g;1 case (2.5%)was difficult to avoid abortion at 23 weeks of gesta-tion.Conclusion Laparoscopic pre -pregnancy cervical cerclage suture in cervix mouth,postoperative can maintain effective cervical length,prolong gestational age,improve the pregnancy outcome,can be used as effective treatment for the pregnant cerclage failure or cervical length less than 2.5cm of cervical insufficiency patients.
