1.Effects of gastric mucosa washing with povidone-iodine solution on bacteremia after endoscopic ultrasonography-guide fine needle aspiration

Objective To study the occurrence of bacteremia and whether washing gastric mucosa with povidone-iodine could reduce its occurrence after endoscopic ultrasonography (EUS)-guided fine needle aspiration (FNA). Methods A total of 20 healthy adult dogs were randomly assigned into experimental group and control group, with 10 in each. The animals in experimental group underwent gastric mucosa washing with 10 ml 0. 5% povidone-iodine solution before FNA, while those in control group with 10ml normal saline. Blood samples were collected for culture before EUS, between EUS and FNA, and 5, 15 and 30 minutes after FNA. Results There were 3 cases of positive bacteremia in experimental group and l in control group,which were not significantly different ( P = 0. 582). Conclusion Pre-washing gastric mucosa with 0. 5% povidone-iodine solution cannot reduce bacterial infection after EUS-guided FNA.
2.Value of FFA and OCT examination in the diagnosis and treatment of uveitis

AIM: To study the clinical value of fundus fluorescein angiography (FFA) and optical coherence tomography (OCT) in the diagnosis and treatment of uveitis.METHODS: From April 2014 to April 2016 in our hospital for treatment of patients with uveitis were 140 cases,including 104 cases of patients with binocular incidence in 208 eyes and 36 patients with unilateral disease.All of patients after history,slit lamp,direct ophthalmoscopy and B-scan examination was the diagnosis of uveitis.Based on the anatomy of uveitis was divided into: 72 cases of patients with anterior uveitis in 132 eyes,24 cases of patients with intermediate uveitis in 48 eyes,posterior uveitis in 28 eyes of 40 patients,16 patients with panuveitis in 24 eyes.FFA and OCT were used to detect the pathological changes of the fundus of the patients.RESULTS: The detection of anterior uveitis was 46.2%,the detection rate of 43.8% intermediate uveitis,posterior uveitis detection rate was 45.0%,the detection rate of 54.2% panuveitis.Patients with anterior uveitis diseased were mainly in peripheral fundus,inflammation,posterior uveitis and panuveitis patients were mainly located in the posterior pole position,and were accompanied by vascular leakage.CONCLUSION: FFA objectively reflects the effect of uveitis on the retina,helping doctors to understand the extent of their choroidal,optic nerve and retinal lesions,and thus the use of appropriate methods for treatment;OCT for patients with an objective and direct means of evaluation.
3.Correlative factors study on alexithymia of patients with asthma

Objective To investigate factors related to alexithymia of patients with asthma.Methods 52 patients with asthma were assessed with TAS-26,POMS-SF and EPQ-RSC.Results Compared with normal model control , patients with asthma showed alexithymia(t=2.15~5.29,P<0.05~0.01). There was negative correlation between the total score of TAS,Ⅰ,Ⅱ, Ⅲ , Ⅳ and E(r=-0.372~-0.451,P<0.01), between Ⅲ and N (r=-0.37,P<0.01).There was opposite correlation between the total score of TAS,Ⅰ, Ⅱand N (r=0.312~0.415,P<0.01),and between Ⅱ, Ⅳ and P(r=0.531~0.621,P<0.01). There was close relation between alexithymia and mood state. There was opposite correlation between the total score of TAS,Ⅰ,Ⅱ, Ⅳ and the total score of POMS, TA,DD,AH,FI , CB(r=0.449~0.779,P<0.01). There was negative correlation between the total score of TAS,Ⅰ,Ⅱand VA (r=-0.418~-0.544,P<0.05~0.01),and between Ⅳ and VA(r=-0.320,P<0.05).Conclusion Patients with asthma have alexithymia.There was close relation between alexithymia,personality traits and mood state.
4.Transmission straight wire appliance treats Angle Ⅱ 1 malocclusion:report of 13 cases

Objective To observe the clinical effects of transmission straight wire appliance on the treatment of AngleⅡ1 malocclusion and explore its mechanism.Methods Thirteen AngleⅡ1 malocclusion patients (6 males and 7 females) were treated with the appliance by extracting 4 first premolars or 2 first maxillary premolars.The results of cephalometric measurements before and after treatment were statistically analyzed.Results The average treatment time of 13 cases was 15.6 months.The overbite's treatment cost 4.2 months averagely.Overbite,overjet and molar relationship were turned to normal (only one adult case had complete distal in the molar relationship by extracting 2 first maxillary premolar) and lateral appearances were improved after treatment.The results of cephalometric analysis showed that the changes of SNB,ANB,U1-NA angle,L1-NB angle,U1-LI angle,U1-NA length,L1-NB length,L6-MP length,L1C-MP length,overbite,and overjet were of high statistical significance (P0.05).Conclusion Transmission straight wire appliance can quickly and efficiently correct AngleⅡ1 malocclusion.

6.Usefulness of nasal continuous positive airway pressure in acute congestive heart failure infants with pulmonary artery hypertension and congenital heart disease

Objective To describe the early use of nasal continuous positive airway pressure (nCPAP) ventilation for infants presenting acute congestive heart failure (CHF) complicated by congenital heart disease (CHD) and pulmonary artery hypertension (PH).Methods Sixty infants with CHD treated for acute CHF were randomly divided into the nCPAP group (n =32) and the non-nCPAP group (n =28).Data were analyzed,which included lactic acid value (Lac) by arterial blood gas analysis,calculation of oxygenation index [pa (O2)/FiO2],detection of serum N-terminal pro-brain natriuretic peptide of type B (NT-proBNP) level,determination of left ventricular end diastolic volume index (LVEDVI),left ventricular ejection fraction (LVEF) and pulmonary artery systolic pressure (PASP) by noninvasive bedside ultrasonic cardiogram (UCG).Results 1.Comparison of arterial blood gas between the 2 groups:1 d after treatment,there were significant differences in pa (O2)/FiO2 and Lac between the 2 groups (t =4.743,5.402,all P < 0.05).A significant difference was found in the pa (O2)/FiO2 between the nCPAP group and the non-nCPAP group 3-7 d after treatment(t =6.366,P < 0.05).The level of Lac had no significant difference between the 2 groups 3-7 d after treatment(t =1.812,P > 0.05).2.Comparison of index of heart function between the 2 groups:after 3-7 d treatment,LVEDVI,LVEF,and PASP were statistically different between the 2 groups (t =2.052,2.704,2.019,all P <0.05).3.Comparison of serum indexes between the 2 groups:3-7 d after treatment,serum NT-proBNP level was improved dramatically compared with the non-nCPAP group(t =9.869,P <0.05).4.Comparison of clinical prognosis between the 2 groups:the differences in needing endobronchial intubation rate,mechanical ventilation time,time in PICU and mortality rate were all statistically significant between the 2 groups (x2 =5.505,P =0.019; t =4.788,P =0.000;t =5.068,P =0.000 ;x2 =4.284 ;P =0.038).Conclusions The early use of noninvasive nCPAP for eligible patients with acute CHF complicated by CHD and PH seems to improve their prognosis by improving pa (O2)/FiO2,reducing left ventricular and right ventricular afterload and improving the left ventricular function.
8.Expression and clinical significance of ROC1 in women with epithelial ovarian cancer

Objective To investigate the expression and clinical significance of ROC1 in ovarian cancer tissues ,to provide new ideas for the gene therapy of ovarian cancer .Methods 261 women with ovarian cancer underwent resection from December 2005 to December 2007 in the Affiliated Hospital of Zunyi Medical College and with 5 years follow-up were enrolled .ROC1 mRNA transcription and protein expression level of ovarian cancer tissue and surrounding normal tissue were tested ,and later the correla-tion between the results and patient′s prognosis was statistically analyzed .Results ROC1 in ovarian cancer tissue was over-ex-pressed ,but almost no expression in surrounding normal ovarian carcinoma ;ROC1 expression levels had a negative correlation with the survival rate of the patients .Conclusion ROC1 in ovarian cancer tissue was over-expressed ,but almost no expression in sur-rounding normal ovarian carcinoma ;ROC1 expression levels had a negative correlation with the survival rate of the patients .
9.Early systemic nursing intervention for the incidence of constipation in depressed patients during treatment with amitriptyline

Objective To investigate the effect of early systemic nursing intervention applied by ward nurse on constipation among patients with depression. Methods A total of 144 participants were divided into the intervention group (72 cases) andthe control group (72 cases) by random number table. Except for a period of 4 weeks routine nursing and 4 weeks follow-up observation which were applied for control subjects, systemic nursing interventions werealso conducted inintervention patients. The incidence of constipation and constipation symptom scoreswere measured at the ends of the 4th and 8th week of treatment. Results The incidences of constipation and constipation symptom scores of the 4th and 8th weekend in the intervention group were 17.6% (12/68), 6.59 ± 4.08, 23.5% (16/68) and 6.81 ± 3.99 respectively. While in the control group they were 34.9%(22/63), 8.84 ± 4.45, 44.4%(28/63) and 10.52 ± 3.86 respectively. These differences between two groups at the 4th and 8th weekend were all significant (χ2=5.077, t=3.023, P < 0.05 and χ2=6.413, t=5.411, P < 0.05, respectively). Compared with the 4th weekend, in the 8th weekend, constipation rate and scores of constipation symptom were increased in thecontrol group (P < 0.05), but not in theintervention group (t=-1.965, P > 0.05). Conclusions In patients with depression, early systemic nursing intervention can effectively ameliorate the constipation caused by amitriptyline.
10.Effect of Fagopyrum cymosum (Trev.) Meisn alcohol extract on defecation and isolated colon of diarrhea-IBS rats and its mechanism.

OBJECTIVETo explore the intervention of Fagopyrum cymosum (Trev.) Meisn alcohol extract (FAE) on defecation function and motor functions of isolated colons of diarrhea-predominant irritable bowel syndrome (D-IBS) rats and to study its underlying mechanism.
METHODSThe D-IBS rat model was established by neonatal pups maternal separation (NMS) combined with intracolonic infusion of acetic acid (AA). Adult IBS rats were randomly divided into the pre-intervention control group (n = 10, with no gastrogavage), the normal saline control group (n = 10, administered with normal saline by gastrogavage), the pre-treatment model group (n = 8,with no gastrogavage),the normal saline model group (n = 8, administered with normal saline by gastrogavage), the low dose FAE group (n = 8, administered with 6 g/kg FAE by gastrogavage), the high dose FAE group (n = 8, administered with 24 g/kg FAE by gastrogavage), and the Pinaverium Bromide group (n = 8, administered with 0.02 g/kg Pinaverium Bromide by gastrogavage). All medication was performed once daily for 2 weeks. The abdominal withdrawal reflex (AWR) was employed to evaluate the visceral hypersensitivity; their loose and watery stool grade was assessed by Bristol scores for stool consistency; and their fresh feces weight was calculated. In vitro effect of different concentrations of FAE and Pinaverium Bromide (0.02 μg/mL) on spontaneous contraction and spasmodic contraction induced by acetylcholine (Ach) in rats' isolated colon were observed and the influence on the intestinal calcium channel was evaluated.
RESULTSCompared with the pre-intervention control group, the pain pressure threshold and the maximum tolerance pressure decreased significantly in the pre-intervention model group (P < 0.05), and the loose and watery stool grade and fresh feces weight increased drastically (P < 0.01). Compared with the normal saline control group, the pain pressure threshold and the maximum tolerance pressure decreased significantly in the normal saline model group (P < 0.05), and the loose and watery stool grade and fresh feces weight increased markedly (P < 0.01). Compared with the normal saline model group, the pain pressure threshold of 24 g/kg FAE and Pinaverium Bromide group significantly increased (P < 0.05). The loose and watery stool grade and fresh feces weight decreased obviously in the low dose FAE group, the high dose FAE group, and the Pinaverium Bromide group (P < 0.05). FAE (30, 100, 300, 1,000, and 3,000 μg/mL) and Pinaverium Bromide could significantly inhibit spontaneous contraction of isolated intestines (P < 0.05, P < 0.01), and FAE (30, 100, and 300 x 10(-6) g/mL) could remarkably inhibit their spasmodic contraction and contractile tension induced by Ach and Ca2+ respectively (P < 0.05, P < 0.01) in a concentration-dependent manner. Pinaverium Bromide also could significantly inhibit Ach and Ca2+ induced contraction.
CONCLUSIONEffective components of FAE improved the defecation function and inhibited enterospasm induced intestinal hyperactivity in IBS model rats via antagonizing calcium channel competitively and inhibiting colonic motility dose-dependently.
