[Objective]To investigate the treating effect of bicondylar tibial plateau fractures with lateral supporting plate and compression screw system.[Method]Forty-six cases of bicondylar tibial plateau fractures were treated with internal fixation from the year of 2000 to 2004 in our hospital.All of them were treated with open reduction and internal fixation by lateral supporting plata and compression screw system.Orthopaedic evaluation at latest follow-up included a knee functional outcome and radiographic examination.[Result]Forty-six cases were followed up for 12 to 18 months(averaged,14.4 months).All fractures healed.According to Rasmussen scale,84.8% excellent and good results were obtained.[Conclusion]Most of all bicondylar tibial plateau fractures were stabilized by applying lateral supporting plate and compression screw system.This system help reduce complication rates resulting from open reduction and internal fixation.

As a result tentative diagnosis of Mirizzi's syndrome was established in 54 cases accounting for 0 56% of all LC patients.Results Laparoscopic procedure was successful in 46 out of 50 type Ⅰ cases. Four cases suffering from iatrogenic bile duct injury received primary repairment under laparoscopy. Fistulae repairment was successful laparoscopically in 3 out of 4 cases of type Ⅱ Mirizzi syndrome.Conclusion Type Ⅰ and type Ⅱ Mirizzi syndromes can be safely and successfully managed under laparoscopy in the hand of careful and skilled operators.

Objective To observe the outcome of Nimodipine and Salvia Miltiorrhiza Composite in treating newborn hypoxic ischemic encephalopathy(HIE ) . Methods 128 HIE patients were randomly divided into four groups;-Salviae Miltiorrhiza Compsite treated group (n=32) , Nimodipine treated group (n 30) , Nimodipine plus Salviae Miltiorrhiza Composite treated group ( n =35) , and control group ( n =31) , in which the symptoms and signs , the reactions following treatment were observed .Results The total effective rates of Salviae Miltiorrhize Composite treated group , Nimodipine treated group , Nimodipine plus Salviae Miltiorrhiza Composite treated group were 81.3% (26/32) , 80% (24/30) and 97.1% (34/35) respectively , and the markedly effective rates were 53.1% (17/32) , 53.3% (16/30) and 88.6% (31/35) respectively . The total effective rates and the markedly effective rates of control group were 51.6% (16/31) and 6.5% (2/31) respectively ,the treated groups were higher than that of control group ( P

AIM:To study the effect of fractalkine(CX3CL1,Fkn) on the expression and secretion of matrix metalloproteinase-2(MMP-2) in cultured human monocyte line U937 cells.METHODS:The cultured U937 cells were incubated with recombinant human Fkn,the supernatant of human renal mesangial cells(HRMC) and Fkn neutralizing antibodies for 24 h.The mRNA expression of MMP-2 was analyzed by RT-PCR.The production of MMP-2 in the supernatant was analyzed by gelatin zymography.RESULTS:The level of MMP-2 mRNA and protein in the cells incubated with recombinant human Fkn decreased compared to control group.Similarly,the level of MMP-2 mRNA in the cells incubated with the supernatant of HRMC reduced compared to control group.However,the level of MMP-2 mRNA and protein in the cells incubated with the supernatant of HRMC adding Fkn neutralizing antibodies increased compared to that incubated with the supernatant of HRMC.CONCLUSION:Fkn inhibits the expression and secretion of MMP-2 in cultured U937 cells.HRMC might mediate the expression and secretion of MMP-2 in U937 cells through Fkn.

Objective To investigate the mechanism of neural protection of mouse nerve growth factor combined with sub-hypothermia in the treatment of patients with severe traumatic brain injury. Methods 90 cases of severe traumatic brain injury were randomly divided into study group and control group with 45 cases of each group, the control group were given routine treatment; the study group were given on the basis of routine treatment of mouse nerve growth factor combined with sub-hypothermia treatment, with 2 weeks treatment, the clinical indicators and corresponding nerve injury, inflammation, oxidative stress indexes, clinical effect and complications were compared after 2 weeks treatment. Results Compared with before treatment or control group, scores of Glasgow coma scale (GCS) and Glasgow outcome scale (GOS) and montreal cognitive assessment (MoCA) in study group after the treatment increased, National Institute of Health stroke scale (NIHSS) score decreased(P<0.05), neuronspecific enolase (NSE), myelin basic protein (MBP) and S100 beta levels decreased(P<0.05), the serum tumor necrosis factor-α (TNF-α),interleukin-6 (IL-6), IL-10 levels decreased (P<0.05), the malondialdehyde (MDA) decreased, the glutathione peroxidase (GPx) and oxidation resistance (AOA) levels increased (P<0.05). The control group efficiency was 73.33%, the study group efficiency was 91.11%, there was significant difference (P<0.05). All patients were followed up, no case off, there was no significant difference in adverse drug reaction rate between two groups. Conclusion Mouse nerve growth factor and sub-hypothermia has the significant neural protection for patients with severe traumatic brain injury, and its mechanism may be related to reduce nerve injury indicators and improve inflammatory factor and oxidative stress response.

ObjectiveTo investigate the characteristics of growth and secretion in primarily cultured human pheochromocytoma cells.MethodsThe human pheochromocytoma tissues were digested successively,and the tumor cells were cultured.The growth curves of three tumors were plotted by cell count.At the same time,the catecholomine in the medium was measured with high performance liquid chromatography.ResultsThe number of primarily cultured human pheochromocytoma cells increased gradually from first to sixth day after plating,and then the cell number began to decrease from seventh day.The levels of norepinephrine(NE) and epinephrine(E) in the medium of cultured cells significantly decreased on first and second day,and the levels of NE and E had no significant difference on the second to eighth day.The levels of dopamine in the medium had no significant difference on the first to eighth day.ConclusionThe primarily cultured pheochromocytoma cells differentiate obviously after plating,but have no significant proliferation.The levels of catecholomine in the medium are decreased significantly on the first and second day.

Objective To observe and analyze cupping therapy for chronic wound healing.Methods Thirty-nine patients with chronic wounds were collected and randomly divided into cupping therapy group (n =20) and control group (n =19).The control group was treated with route dressing change once every other day,while the cupping therapy group was added cupping therapy.Compared the two groups of patients in general view,positive ratio of wound germiculture,area percentage of wound healing and pain score (VAS score).Results The pus of wounds was mostly drained out and the fresh tissue fluid leakage when patients were treated with cupping therapy.After three days of treatment ,the positive ratio of wound germiculture of the cupping group (6 5 % )was lower than that of the control group(79%),but the difference was not significant.After one-week treatment,the positive ratio of wound germiculture of the cupping group(40%)was significantly lower than that of the control group(73%)(χ2 =4.496, P =0.034).And the VAS score of the cupping group (2.20 ±1.00)was significantly lower than that of the control group (4.16 ±0.96)(t =-12.929,P =0.001).After two-week treatment,the area percentage of wound of the cup-ping group (80.68%)was significantly lower than that of the control group (92.28%)(t =-13.675,P =0.000). And 4 cases of the cupping group cured,while no patient was cured in the control group.Conclusion Cupping thera-py based on route dressing change has positive therapeutic effect on chronic wounds.And the advantages of lower cost and easier operations would make it suitable for middle-and low-income individuals and primary hospitals.

Objective To investigate the surgical treatment and clinical outcome of patients with intracranial posterior circulation poor-grade aneurysms. Methods The clinical data of 35 patients with intracranial posterior circulation poor-grade aneurysms were collected. Seventeen patients were World Federation of Neurosurgical Societies (WFNS) grade IV and 18 patients were WFNS grade V. Twenty- nine patients were definitively treated with open microsurgery treatment(2 patients) and endovascular treatment (27 patients). Six patients received conservative treatment, including 4 patients only receiving external ventricular drainage to decrease intracranial pressure and 2 patients only receiving medical treatment. The nerve function were evaluated by Glasgow outcome scale (GOS) at discharge from hospital and 1 and 12 months after operation. Results In the patients who received conservative treatment, 5 patients were dead in hospital, and 1 patient got good outcome after 12 months. In the patients who received surgical treatment, at discharge from hospital good outcome were achieved in 15 patients (51.7%), poor outcome in 9 patients (31.0%), and death happened in 5 patients (17.2%). At 1 month after operation, good outcomes were achieved in 14 patients (48.3%), poor outcome in 9 patients (31.0%), and death happened in 6 patients (20.7%). At 12 months after operation, good outcome were achieved in 18 patients (62.1%), poor outcome in 5 patients (17.2%), and death happened in 6 patients (20.7%). Conclusions Patients with posterior circulation poor-grade aneurysms have better prognosis after positive surgical treatment.

Objective The study aimed to evaluate the application effect of clinical pathways in laparoscopic cholecystectomy patients by using the meta-analysis.Methods Published randomized controlled trials (RCT) about laparoscopic cholecystectomy patients were searched and screened in China National Knowledge Infrastructure (CNKI),China Scientific Journal Database by VIP,Wanfang Database under present standards.The quality of the included studies was evaluated by certain standards.The Review Manager 5.2 software was used for analysis.Results Totally 29 studies including 5 570 cases were eligible to the criteria (2 753 in the experimental group and 2 853 in the control group) altogether.The meta-analysis showed that the hospitalization time and hospitalization costs in the clinical nursing pathway group were significantly less than those of the control group (SMD=-1.69 and-3.75),the satisfaction degree and the mastering of health knowledge in the clinical nursing pathway group were significantly higher than those of the control group (RR=1.16 and 1.26),the differece had statistical significance.Conclusions Application effect of clinical nursing pathways is superior to the traditional nursing method in laparoscopic cholecystectomy patients.

Objective To investigate the expression of autophagy and the effect of complement C5a/C5aR pathway on autophagy induced by renal ischemia reperfusion injury (IRI).Methods MaleWT and C5aR gene knockout (BALB/C background) mice were selected.The model of renal IRI was established by occluding bilateral renal pedicles with microaneurysm clamps.Mice were divided into wild type BALB/C (WT) group and C5aR gene knock out (C5aRKO) group.The pathology of kidney was assessed by HE staining.The levels of BUN and KIM-1 were detected 24 h after reperfusion.The expression of the autophagy-associated protein (LC3 Ⅱ/LC3 Ⅰ and P62) was measured by Western blotting and immunofluorescence.In vitro,human renal tubular epithelial cells (HK2) were cultured.The expression of LC3 in HK2 cells was investigated by immunofluorescence and Western blotting after being treated with recombinant C5a or C5a combined with C5aR antagonist (C5aRA).Results As compared with WT group,the severity of kidney injury was obviously reduced in C5aRKO group (P＜0.05).After ischemia-reperfusion,the expression of autophagy-related protein LC3 gradually increased with the reperfusion time prolonged.The level of autophagy induced by ischemia-reperfusion was significantly reduced in C5aRKO group as compared with WT group (P＜0.05).In addition,the expression of autophagy-related protein LC3 Ⅱ in HK2 cells was increased with the augment of C5a stimulation concentration in vitro.Blockage of C5aR pathway by C5aRA led to a significant decrease in autophagy (P ＜ 0.05).Conclusion Complement C5a/C5aR pathway promotes renal IRI-induced autophagy.
