Clinical Medicine of China 2011;27(4):398-401
Objective To explore the value of serum procalcitonin (PCT) for early evaluation of the severity in bacterial infection, and compare it with C-reactive protein(CRP), white blood cell(WBC) and temperature to understand their strengths and weaknesses. Methods A sample of 139 impatients with bacterial infection from July 2009 to June 2010 in our hospital were divided into four groups, including local bacterial infection group(Group A, n = 50), sepsis group(Group B, n = 51), severe sepsis group(Group C, n = 21) and septic shock group (Group D, n = 17) according to ACCP/SCCM consensus conference and the clinical manifestation. Twenty noninfectious patients during the same time were recruited as control(Group E, n = 20)PCT, CRP,WBC and temperature were evaluated in all patients before treatment and PCT were measured 5 days after treatment. Results During the early period of the bacterial infection, PCT was highest in Group D ([42.36±48.49] × 10-6 g/L),which was higher than that of (4.66 ±2.46) × 10-6 g/L in Group C,( 1.08 ± 0. 68) × 10 -6 g/L in Group B and (0. 09 ± 0. 05) × 10 -6 g/L in Group A(Ps < 0. 05), PCT in the four infectious groups were all higher than the control group([0. 05 ± 0. 01 ] × 10-6 g/L)(Ps < 0. 05). No significant difference in CRP between Group D and Group C, neither did between Group C and Group B (Ps >0. 05). No significant difference was found in WBC between Group D and Group C,neither did between Group D and Group B (Ps > 0. 05). Temperature in Group D was not significantly different to Group C(P > 0. 05). After 5 days' treatment, PCT decreased in the four infectious groups than before. Conclusion The level of PCT can be used to evaluate the severity of infection during the early period of bacterial infection, which is better than CRP,WBC and temperature. PCT decreases after effective treatment, which can help monitor the patients' condition.

BACKGROUND:The tensile and stress-relaxation mechanical properties of sciatic nerve injury after repaired with autologous nerve and poly(lactic acid-glycolic acid) (PLGA) scaffold are rarely reported. OBJECTIVE:To analyze the tensile and stress relaxation characteristics of sciatic nerve injury after the transplantation of autologous nerve and PLGA scaffold. METHODS:Sixty sciatic nerves were extracted from the fresh cadavers dead within 24 hours, processed into 35 mm samples, and were then randomly divided into three groups. The nerve samples in control group received no intervention;the nerves in artificial and autologous groups were modeled into 20 mm defects, followed by repaired with PLGA scaffold and autologous nerve, respectively. Afterwards, the tension and stress-relaxation tests were performed in each group. RESULTS AND CONCLUSION:The stress in each group descended fast at the first 600 seconds, then descended slowly and was closed to the horizontal level until 7200 seconds, and the stress-relaxation curves in each group were in logarithmic decrease. The order of the elastic limit load, elastic limit stress, maximum load, maximum stress, elastic limit strain and maximum strain during tension was as follows:control group>artificial group>autologous group (P<0.05). Our results indicate that the PLGA scaffold holds good tension and stress-relaxation properties, which meets the mechanical requirements of the biomaterials used for sciatic nerve repair.

OBJECTIVE:To investigate clinical efficacy and safety of goserelin in the treatment of adenomyosis complicated with severe dysmenorrheal before placing levonorgestrel intrauterine system (LIS). METHODS:A total of 140 adenomyosis pa-tients with severe dysmenorrheal were selected from Sanya Family Planning Technology Service Center during Jan. 2014-Jan. 2016, and then divided into control group and observation group according to lottery method, with 70 cases in each group. Both groups re-ceived LIS within first 7 d of menstruation, and observation group was given Goserelin acetate sustained-release implant 10.8 mg subcutaneously via anterior abdominal wall one week before placing. Therapeutic efficacy was evaluated 6 months later. The im-provement of dysmenorrhea was compared between 2 groups. The uterine volume,COX dysmenorrhea score (CMSS),menstrual volume, hemoglobin (Hb) and CA125 content were compared between 2 groups before and after treatment. The occurrence of ADR was also compared. RESULTS:The rate of dysmenorrheal improvement in observation group was 95.71%,which was signifi-cantly higher than 81.43%of control group, with statistical significance(P<0.05). Before treatment, there was no statistical signif-icance in above indexes between 2 groups (P>0.05). Compare to before treatment,CMSS score, menstrual volume and CA125 content of 2 groups were decreased significantly after treatment, while Hb content was increased significantly;the observation group was significantly better than the control group, with statistical significance(P<0.05). There was no statistical significance in uterine volume between 2 groups before and after treatment (P>0.05). There was no statistical significance in the incidence of ADR(10.00% vs. 14.29%)between 2 groups(P>0.05). CONCLUSIONS:Goserelin in the treatment of adenomyosis complicated with severe dysmenorrheal before placing LIS can effectively relieve dysmenorrheal, reduce menstrual volume and improve Hb and CA125 levels, but do not increase the risk of ADR.
8.Non-rigid medical image registration based on mutual information and thin-plate spline.
Chinese Journal of Medical Instrumentation 2009;33(1):11-14
To get precise and complete details, the contrast in different images is needed in medical diagnosis and computer assisted treatment. The image registration is the basis of contrast, but the regular rigid registration does not satisfy the clinic requirements. A non-rigid medical image registration method based on mutual information and thin-plate spline was present. Firstly, registering two images globally based on mutual information; secondly, dividing reference image and global-registered image into blocks and registering them; then getting the thin-plate spline transformation according to the shift of blocks' center; finally, applying the transformation to the global-registered image. The results show that the method is more precise than the global rigid registration based on mutual information and it reduces the complexity of getting control points and satisfy the clinic requirements better by getting control points of the thin-plate transformation automatically.

Objective To study the value of CT angiography(CTA) and three dimensional contrast enhanced magnetic resonance angiography(3D CE-MRA) in the diagnosis of arteriosclerotic occlusive disease (ASO) of lower limb and evaluate arterial stenosis. Methods Thirty-eight patients clinically suspected ASO were studied with CTA and3D CE-MRA,twenty-two patients underwent DSA at the same time. The degree of seriousness of the stenosis shown by various imaging techniques were analyzed respectively. Results Using the DSA images as the standard reference, the sensitivity and specificity of CTA were 97.3% ,99. 1%, and MRA both were 100.0% in detecting the segments occluded. With regard the detection of segments that had more than moderate stenosis ,the sensitivity and specificity of CT were 99.3% ,97.6% ,and MRA were 97.9% ,96.4%. The degree of seriousness of the stenosis shown by MRA was overestimated than CTA on the segments that had more than moderate stenosis(x2 =5.80,P <0. 05). Conclusion CTA and 3D CE-MRA were reliable method in diagnosis of ASO.

Objective To investigate the apoptotic effect of the transmembrane form vaccine of human blood group A mimotope on malignant melanoma cell line B16. Methods B16 cells were transfected with different recombinant plasmid through Lipofectamine 2000 and incubated with different concentration of monoclonal anti-A antibody at 2.5 μg/ml, 5 μg/ml,10 μg/ml and 20 μg/ml. Apoptosis rate of cells was determined with Annexin Ⅴ/PI double staining by flow cytometry. Results Apoptosis rate to P/F-M-pIRES group B16 cells was 74.74% when anti-A monoclonal antibody concentration was 10 μg/ml; apoptosis rate of plasmids carrying peptide/Fas fusion gene such as P/F-M-pIRES group and P/F-pIRES group were significantly higher than M-pIRES group and pIRES group. The apoptosis rate was statistically significantly different between different recombinated plasmid groups (F=669.707,P<0.01). The apoptosis rate was statistically significantly different between different antibody groups (F=106.596,P<0.01). The interaction between recombinated plasmid groups and antibody groups was statistically significant (F=34.806,P<0.01). Conclusions The transmembrane form vaccine of human blood group A mimotope could induce B16 cell apoptosis in vitro. This vaccine may be a promising candidate for potential malignant melanoma therapy.
