1.Analysis of Distribution and Drug Resistance of Pathogens in Patients with Hospital Acquired Pneumonia from Neurosurgery Department of Our Hospital during 2011-2015

OBJECTIVE:To explore the distribution and drug resistance of pathogens in patients with hospital-acquired pneumo-nia(HAP)from neurosurgery department of the First Affiliated Hospital of Xinjiang Medical University(called“our hospital”for short),so as to provide reference for clinical use of antibiotics in the clinic. METHODS:The pathogen culture and drug sensitivity test of 652 HAP patients were analyzed retrospectively in neurosurgery department of our hospital during May 2011 to May 2015. RESULTS:Of 652 cases,pathogens were detected in 608 cases,with positive rate of 93.3%. Among 608 strains of pathogen, there were 443 strains of Gram-negative bacteria(71.2%)as Klebsiella pneumonia,Pseudomonas aeruginosa and Escherichia co-li;108 strains of Gram-positive bacteria (17.8%) as Staphylococcus aureus and Streptococcus pneumoniae;67 strains of fungus (11.0%)as Candida albicans. There were 67 strains of ESBLs K. pneumonia,43 strains of ESBLs E. coli and 8 strains of methicil-lin-resistant coagulase negative Staphylococci. The drugs sensitive to Gram-negative bacteria included imipenem,piperacillin sodi-um and tazobactam sodium,cefepime,etc.;those sensitive to Gram-positive bacteria included vancomycin,etc. CONCLUSIONS:Main pathogen of HAP patients in neurosurgery department of our hospital are Gram-negative bacteria,and they were highly resis-tant to antibiotics;the highly sensitive drugs to HAP include imipenem and vancomycin,etc.;the antibiotics should be used ratio-nally according to etiological analysis.
2. Correlation analysis between hemostatic activity and commercial grades of Panax notoginseng

Objective: To explore the correlation between the hemostatic activity and commercial grades of Panax notoginseng. Methods: After treatment with different commercial grades of P. notoginseng, the mice blooding time (BT) and clotting time (CT) were measured. At the same time the prothrombin time (PT), activation part thrombin time (APTT), fibrinogen (FIB), and platelet count (PLT) were also detected by fully automated blood cell analyzer. The contents of dencichine were determined by HPLC. Results: Mice BT, CT, PT, and APTT were decreased and PLT was increased significantly in all the commercial grades of P. notoginseng. Among all the commercial grades, the countless Tou Sanqi Powder has the best hemostatic activity. There is no correlation between the commercial grades and homostatic activity. Different contents of the extracted dencichine were observed among different commercial grades of P. notoginseng. The highest content of dencichine was 0.98% in countless Tou Sanqi and the lowest was 0.52% in 80 Tou Sanqi. There is no statistical correlation between the dencichine and hemostatic activity or dencichine and commercial grades of P. notoginseng. Conclusion: There is no statistical correlation between the commercial grades of P. notoginseng and haemostatic activity or dencichine contents. The dencichine in countless Tou Sanqi which is the cheapest of all displays the better hemostaic activity than those in 13 or 20 Tou Sanqi whose cost is almost as ten times as countless Tou Sanqi's.
3.Spindle and chromosome configuration of human in vitro matured oocytes after slow freezing-fast thawing

Objective To evaluate the development of immature oocytes after freezing-thawing by conventional cryopreservation method for mature oocytes.Methods Immature oocytes were collected from stimulated ovaries of intracytoplasmic sperm injection(ICSI)cycles.Immature oocytes were in vitro matured directly or after slow freezing-fast thawing and immunostained for tubulin and chromatin and at last visualized by confocal microscopy.Results No statistical difference was found in maturity rate between freezing groups and the controls.There was a statistically significant increase in abnormalities of chromosome(23.7% vs. 50%)and spindle(28.9% vs.53.9%)in the GV freezing group compared with the GV control(P
4.Evaluation on ischemia-modified albumin, D-dimer and cTn Ⅰ in the early diagnosis of acute coronary syndrome

ObjectiveTo investigate the early diagnosis value of IMA and D-dimer and cTn Ⅰ in ACS.MethodsAll the 113 blood samples of patients with chest pain in the emergency department of the Third Hospital of Hebei Medical University were selected.Thirty healthy people were selected as the control group.Patients were divided into two groups:one of 52 cases was within 3 hours after onset of chest pain.the other of 61 cases was between 3 to 6 hours.According to the final clinical diagnosis,31 cases were divided into non-ischemic chest pain group (NICP) and 82 cases were divided into the ACS group.The ACS group was divided into the UA group (51 cases),NSTEMI group (18 cases) and STEMI group (13 cases).IMA was measured with albumin-cobalt binding test,and D-dimer was measured with the automated blood coagulation analyzer,and cTn Ⅰ was measured with the automatic biochemical analyzer.Levels of IMA,D-dimer,cTn Ⅰ were compared in the different groups and their sensitivity,specificity and accuracy to the early diagnosis of ACS were evaluated by Four format diagnositic test.ResultsThe levels of IMA in ACS group,which include the UA group,NSTEMI group and STEMI group were (0.722 ± 0.181 ),(0.601 ± 0.122),(0.631 ± 0.153 ) ABSU respectively.The levels of D-dimer were ( 0.485 ± 0.124 ),( 0.571 ± 0.181 ),(0.748 ± 0.094 ) mg/L respectively.The levels of cTn Ⅰ were ( 0.076 ± 0.027 ),( 0.059 ± 0.038 ),(0.065 ± 0.015 )μg/L respectively.Concentrations of IMA,D-dimer and cTn Ⅰ in ACS group were significantly higher than those of the NICP group [IMA (0.338 ± 0.065 ) ABSU,D-dimer (0.368 ± 0.078 )mg/L,cTn Ⅰ (0.022 ±0.007) μg/L] and the controls group [IMA (0.292 ±0.058) ABSU,D-dimer (0.267 ±0.052) mg/L,cTn Ⅰ (0.029 ± 0.016) μg/L].There were significant differences between the ACS group and the NICP group and the control group,F value was 3.613,3.289 and 3.521 respectivily,and P <0.05.The levels of IMA in ACS group within 3 hours and between 3 to 6 hours,which is (0.665 ±0.104 ),(0.520 ± 0.073 ) ABSU,were significantly higher than that of the controls (0.292 ± 0.058 ) ABSU ( F value was 3.58,P < 0.05 ).The levels of D-dimer and the cTn Ⅰ in the group between 3 to 6 hours [which were (0.634 ±0.213 ) mg/L and (0.079 ±0.032) μg/L] were significantly higher than those of the controls [(0.267 ±0.052) mg/L and (0.029 ±0.016) μg/L] (q was4.24 and 3.46,P <0.05).The sensitivity,specificity and accuracy was 83.36%,70.97% and 81.42% of the IMA in a separate diagnosis of ACS,but the sensitivity,specificity and accuracy were 97.56%,58.06% and 86.73% of the IMA,D-dimer and cTn Ⅰ with the affiliation diagnosis.ConclusionThe serum IMA is a more sensitive indicator of ACS in early myocardial ischemia than cTn Ⅰ and plasma D-dimer.Serum IMA in combination with D-dimer and cTn Ⅰ could improve the sensitivity and specitivity,and had value to guide cinical diagnosis of ACS in the early stage.

10.Risk factors for traumatic lumbar punctures in children with acute lymphoblastic leukaemia

Objective To investigate the risk factors for traumatic lumbar punctures in children with acute lymphoblastic leukaemia. Methods 132 children with acute lymphoblastic leukemia totally received 2634 lumbar punctures. The basic data on age, sex, body mass index (BMI), platelet count, interval between two punctures, and presence or absence of ultrasound-guided procedure were collected and analyzed. The risk factors for traumatic lumbar puncture were identified by logistic regression. Results The risk for traumatic lumbar puncture was higher in children younger than 1 year, and it was relatively lower in those aged 1 to 10 years. The risk for traumatic lumbar puncture was slightly higher in children with a BMI index of more than 95. The longer the interval between two punctures, the lower the risk. If lumbar puncture was guided under ultrasound or radiographic images, the risk was much smaller. Conclusions Age of younger than 1 year, BMI index of more than 95, shorter interval between two punctures, and direct puncture can increase the risk for traumatic lumbar puncture.
