1.Treatment of Tumorous Airway Obstruction by Placing Net Stents Following High-frequency Electrocautery via Fiberoptic Bronchoscope

Objective To observe the short-term efficacy of treating the tumorous airway obstruction by placing net stents following high-frequency electrocautery via fiberoptic bronchoscope.Methods 14 patients of lung cancer with airway obstruction had undergone the tumor burning excision by high-frequency electrocautery via fiberoptic bronchoscope and placed net stents , then to observe the clinical efficiency.Results 14 patients of lung cancer with airway obstruction all revealed better efficacies, obviously improving Karnofsky scores and pulmonary function, and no evident complication was found.Conclusion Placing net stents following high-frequency electrocautery via fiberoptic bronchoscope can cure efficiently the tumorous airway obstruction, and have popular value.
2.Application of MRI and Three-dimensional Ultrasound Diagnosis of Congenital Malformations in the Fetus

Obiective To discuss the diagnosis value, limitations and mutual complement of MRI and TDUI in the fetus. Methods, TDUI and MRI was performed in 24 cases pregnant women that suspected fetus congenital malformation by Ultrasound examination, the examination results were compared by MR1 and TDUI with the clinical results.Results The 9 cases congenital malformation of 24 cases pregnant women were found by MRI. The 9 cases were confirmed and 3 cases were more sufficient by MRI. The one case examined by MRI was normal other than Ultrasound examination. Conclusion MRI is superior to Ultrasound in diagnosing malformation of nervous system. The Ultrasound and the TDUI in particular is better in finding the surface malformation than MRI. In clinical application, MRI and Ultrasound must be connected to make up the limitation and have a definite diagnosis.
3.Imaging features and intervention therapy of esophageal fistula

Objective: To discuss the imaging features of esophageal fistula and the value of intervention.Methods: We retrospectively analyzed the complete clinical data of 12 cases of esophageal fistula treated from June 2000 to May 2008,of whom,2 developed esophago-thoracis fistula and esophago-mediastinum fistula respectively after conservative radiotherapy,and 10 developed esophageal fistula after surgery for esophageal cancer.All the patients received iodine-based oral contrast,spiral CT scanning and intraperitoneal stent implantation for fistula closure.Results: Iodine-based oral contrast showed that the contrast agent overflowed into the chest in 8 cases,into the airway in 3 and into the mediastinum in 1.Spiral CT scanning revealed 8 cases of pleural fistula and 2 cases of remnant stomach-airway fistula,but failed to display the other 2 because of the small size.Intraperitoneal stent implantation was successfully accomplished in all the cases,which helped to close the fistula,take in food,stop bucking,control lung,mediastinum and other infections,and improve the patients' quality of life.Conclusion: Esophageal fistula can be effectively diagnosed based on the findings of iodine-based oral contrast and spiral CT scanning.As a novel technique,intraperitoneal stent implantation for fistula closure,simple,safe and with obvious immediate effect,is well worth promoting.
4.Dynamic changes of aspartate transaminase level in gingival crevicular fluid on tooth movement in new bone area

Objective To observe the dynamic changes of aspartate transaminase(AST ) in gingival crevicular fluid on tooth movement in new bone area after distraction osteogenesis at different time .Methods The distraction osteogenesis surgical proce‐dure was performed in 8 beagle dogs without periodontal disease and normal teeth ,experimental teeth were transplanted into the bone regeneration area after 2 weeks and after 6 weeks .Comparative analysis AST of each time (1 ,2 ,3 ,7 ,14 ,28 d after distraction) dynamic changes in gingival crevicular fluid .Results The AST level of gingival crevicular fluid in experimental tooth was rising for the first three days ,and the group of two weeks were significantly higher than 6 weeks ;AST levels after 7 d showed a trend of de‐cline ,down to the lowest point at 21 d ,and gradually restored ,AST levels reached a higher level again in the 28 d .Conclusion The AST level of experimental teeth increased significantly after 2 weeks than after 6 weeks ,but over time the AST level change is not linear ,this change has certain guiding significance for the clinical research in the future .
5.Clinical evaluation of IVP,CTU and MRU in the diagnosis of urinary system stones

Objective:To explore the clinical application in urinary system stones of the inspection technologies intravenous pyelography (IVP),computer tomography urography (CTU) and magnetic resonance urography (MRU). Methods:We collected 100 cases of urinary system stones on which 80 were carried out to IVP,60 to CTU and 35 to MRU,25 patients were implemented all 3 examinations. CTU and MRU are reconstructed with multi-level reorganization (MPR),curved projection reproduction (CPR),volume reproduction (VR) and the maximum intensity projection (MIP). Results:Stones were found in 73 patients of the 80 people by IVP,the diagnosis rate is 91.3 %. Stones were found in all the 60 patients except 1 by CTU with images direct and accurate,the diagnosis rate was 98.1 %. By the MRU,stones were found in 20 of the 35 patients at a display rate of 57.1 %,with images displayed in a low stone location signal,filling defect and the hydrocele circs of renal pelvis and ureter. Conclusion:IVP is a simple method,easy-to-patient acceptance and has a high detection rate of stones; CTU has a good image quality and the detection rate is higher than the stone IVP. By combining with the original cross-sectional image,the rate of diagnosis is up to 98%; as a non-invasive and no contrast agent approach,MRU is also very important in the diagnosis of stones.
6.Discussion of Dual-Energy Imaging with the Dual-source Computer Tomography

Objective To investigate the principle and its clinical initial application of the dual-energy imaging with the dual-source CT and increase the realization of it. Methods According to the research results of the dual-energy imaging with the dual-source CT and the course of development of the dual-energy imaging to analyze and summarize the basis radiological physics, the principles, the correlation parameters and the clinical initial application of the dual -energy imaging with the dual -source CT. Results The dual -energy imaging with the dual -source CT is different from other common dual-energy imaging, it has its own special feature. Conclusion According to the analysis of the dual-energy imaging with the dual-source CT, greatly increase the realization of the dual-energy imaging with the dual-source CT.
7.Clinical analysis of percutaneous fluoroscopic gastrostomy

Objective:Percutaneous fluoroscopic gastrostomy(PFG) is an alterative method for the patients who were not suitable for percutaneous endoscopic gastrostomy(PEG) and stent.This is a study to report the clinical experience of PFG in 25 patients. Methods:Twenty-five patients could not have oral intake due to obstruction of the oesophagus and depended on long-term parenteral nutrition.They contraindicated for percutaneous endoscopic gastrostomy.Results:The procedures all were successful completed without complication.The patients could use home enteral nutrition and get rid of parenteral nutrition.The patients' quality of life was improved significantly.Conclusion:PFG is a useful,convenient and less complication method for patients who are contraindication for PEG.
8.CT and PET/CT in the diagnosis of pleural mesothelioma

Objective:To study the CT and PET/CT features of pleural mesothelioma and the value of CT and PET/CT imaging in the diagnosis of the disease.Methods:We retrospectively analyzed the spiral CT images of 23 patients and PET/CT images of 7 patients with pleural mesothelioma,all histologically confirmed.Results:Of the 23 cases detected by spiral CT,7 were benign and 16 malignant;local nodular pleural thickening was found in 9 cases,7 benign and 2 malignant;diffuse pleural thickening was observed in 14 cases,all malignant.Of the 7 cases diagnosed by PET/CT,5 were excluded from surgery.Conclusion:CT imaging helps determine the location,shape,extent of the pleural lesion and its relation to the surrounding tissues,while PET/CT is better in the differentiation of pleural mesothelioma and staging of malignancy,and therefore plays an important guiding role in the treatment of the disease.
9.Clinical Observation of Bifidobacteria Triple Viable Bacteria Combined with Infliximab in the Treatment of Moderate and Severe Ulcerative Colitis

OBJECTIVE:To investigate therapeutic efficacy of bifidobacteria triple viable bacteria combined with infliximab in the treatment of moderate and severe ulcerative colitis(UC),and its effects on immune function. METHODS:Seventy-two UC pa-tients were selected from gastroenterology department of the First Affiliated Hospital of Chongqing Medical University during Jun. 2009 to Jan. 2015. They was divided into single group and combination group according to therapy plan,with 36 cases in each group. Single group was given infliximab 5 mg/kg,ivgtt,on 0,2nd,6th,8th week. Combination group was additionally given bi-fidobacteria triple viable bacteria 420 mg,tid. Both groups were treated for consercutive 2 months. Clinical efficacies were com-pared between 2 groups as well as the levels of humoral immunity and T cell function. The occurrence of ADR was observed in 2 groups. RESULTS:Clinical response rate of combination group was 86.11%,which was significantly higher than 52.78% of single group,with statistical significance (P<0.05). Before treatment,there was no statistical significance in humoral immunity and T cell function between 2 groups(P>0.05). After treatment,the levels of CD4+ and CD4+/CD8+ were significantly increased in both 2 groups,while the levels of CD8+,Th1 and Th2 were significantly decreased;the combination group was significantly better than the control group,with statistical significance (P<0.05);there was no statistical significance in CD3+ between 2 groups before and after treatment(P>0.05). After treatment,the levels of IgA and IgG were increased significantly in 2 groups,and the level of C3 was decreased significantly;the combination group was significantly better than the control group,with statistical significance (P<0.05);there was no statistical significance in IgM content and C4 level between 2 groups before and after treatment (P>0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:Bifidobacteria triple viable bacteria combined with infliximab shows significant therapeutic efficacy in the treatment of moderate and severe UC, and can significantly improve humoral immunity function and the recovery of T cell level,with good safety.
10.Effect of glucagon-like peptide-1 agonist on insulin signaling pathway in skeletal muscle of mice with insulin resistance

Objective To study the effect of glucagon-like peptide-1 agonist(exendin-4)on insulin signaling pathway in skeletal muscle of mice with insulin resistance(IR). Methods Thirty male KM mice were divided into normal control group(NC group, n=10)and IR group(n=20). The mice in NC group and IR group were fed with routine diet and high-fat diet for 16 weeks, respectively. After IR models were successfully established, 10 mice in IR group were treated with exendin-4(Ex group, 3μg·kg-1·d-1)for 4 weeks. NC group and IR group were given the same dose of normal saline. Body weight, fasting blood glucose(FBG), and fasting insulin(FINS)were measured before and after the intervention, and homeostasis model assessment for insulin resistance ( HOMA-IR ) was calculated . The muscle glycogen content were measured by Periodic Acid-Schiff Stain ( PAS ) . The expressions of insulinreceptor(IRC), insulin receptor substrate-1(IRS-1), and phosphatidylinositol 3-kinase(PI3K)in the skeletal muscle were measured by Western blot and immunohistochemistry. Results Before intervention, the body weight, FBG, and HOMA-IR of IR group and Ex group were higher compared with NC group(all P<0. 05), but FINS did not rise significantly(P>0. 05). Compared with before intervention, the body weight of three groups after intervention were all increased, but increased more significantly in the IR group. After intervention, the FBG and HOMA-IR in Ex group decreased, compared with IR group and Ex group of non-intervention (all P<0. 05), but FINS did not decrease significantly(P>0. 05). The skeletal muscle cells in NC control were colored extensively and more colored granules, while IR group were less colored, and Ex group were between NC group and IR group. Compared with NC group, the expressions of IRC, IRS-1, and PI3K in the skeletal muscle of IR group were significantly decreased(P<0. 05), not in Ex group(P>0. 05). Compared with IR group, the expressions of IRC, IRS-1 and PI3K in Ex group were higher(P<0. 05). Conclusions Exendin-4 improves insulin resistance by up-regulating the expressions of IRC, IRS-1, and PI3K in the skeletal muscle and promoting the synthesis of muscle glycogen.
