1.Risk analysis of lymph node metastasis in 285 patients with superficial esophageal squamous cell carcinoma

Objective To identify risk factors of lymph node metastasis in superficial esophageal squamous cell carcinoma (ESCC),and to provide evidence for treatment choice under endoscope.Methods From January 2007 to December 2011,285 patients with pathologically diagnosed ESCC who received surgery and had clear record of lymph nodes resection were enrolled.The clinical pathological data of these patients were analyzed,including age,gender,smoking and drinking history,history of cancer,family history of cancer,location,tumor size,presence of esophageal,depth of infiltration,differentiation,and vascular cancer embolus.Univariate analysis (chi square test or Fisher exact probability method) and multivariate Logistic regression analysis were performed for risk factors of lymph node metastasis assessment.According to the rates of lymph node metastasis,patients were divided into three groups as follows:low risk,high risk and extremely high risk of lymph node metastasis.KaplanMeier method was used to calculate the average survival time and cumulative five years survival rate.Results Among the 285 patients with ESCC,40 (14.0 %) patients with lymph node metastasis.The results of univariate analysis showed that location (x2 =9.333),tumor length (Fisher exact probability method),depth of infiltration (x2 =9.327),differentiation degree (Fisher exact probability method) vascular cancer embolus (Fisher exact probability method) were significantly associated with lymph node metastasis (all P<0.05).The results of multivariate analysis indicated that tumor length over 5 cm,invasion to submucosal layer and vascular cancer embolus were independent risk factors of lymph node metastasis,and the odd ratio was 17.408(95% confidence interval (CI) 1.557 to 194.686),3.471(95%CI 1.440 to 8.365) and 6.256(95%CI 1.787 to 21.910),respectively.The lymph node metastasis rates of patients in low risk,high risk and extremely high risk group were 5.2%(6/115),15.8% (24/152) and 10/18,respectively;the average survival times were (69.9 ± 2.4),(63.8 ± 2.1) and (51.7 ± 1.7) months,respectively.The cumulative five years survival rates were 59 %,51 % and 31%,respectively,and the difference was statistically significant (x2 6.816,P=0.033).Conclusions The risk of lymph node metastasis is high in ESCC patients with tumor length over 5 cm,invasion to submucosal layer and vascular cancer embolus,and the prognosis is poor.Lymph node metastasis should be considered when endoscopic therapy is chosen.
2.Therapeutic Effect of He-Mu-Shu Point Combination Acupuncture for Patients with Functional Diarrhea and Its Influence of Anxiety

Objective To observe the efficacy of electroacupuncture(EA) on the acupoint combination of He-(sea) points, front-mu points, and shu-points(abbrev. He-Mu-Shu) for the treatment of functional diarrhea, and to explore the effect of He-Mu-Shu point combination EA on the anxiety of patients. Methods A randomized controlled clinical trial was performed in 62 functional diarrhea patients. All of the patients were randomly divided into He-Mu-Shu group(32 cases) and medicine group(30 cases). He-Mu-Shu group was treated with EA on unilateral Quchi, Shangjuxu, Tianshu, Dachangshu point alternatively, and medicine group was treated with oral use of Loperamide hydrochloride capsules. Both groups were treated for 4 continuous weeks. Defecation frequency, stool property and anxiety scores were observed before treatment and 2, 4 weeks after treatment. Results After treatment, weekly defecation frequency, stool property and anxiety scores were improved in both groups, and the differences were statistically significant compared with those before treatment(P < 0.05). He-Mu-Shu point combination EA had better effect on improving weekly defecation frequency, stool property and anxiety scores of functional diarrhea patients than Loperamide hydrochloride capsules, and the differences were statistically significant (P < 0.05). Conclusion The clinical efficacy and anxiety-relief action of He-Mu-Shu point combination EA on functional diarrhea are superior to oral use of Loperamide hydrochloride capsules.
3.Variability of plantar response in normal population

Background: There is lack of normality data in the literature on the plantar response. Objective: To
determine the variability of plantar response in normal population and factors that might contribute to
the variability. Methods: We conducted a study of plantar response in healthy subjects aged 19-21 in
two phases. First phase was plantar examination with hard strike. Subjects with big toe extension in
the fi rst phase were re-examined with light and hard strike in the second phase. Results: 100 subjects
recruited in fi rst phase, 19 subjects in second phase. Out of 600 attempts (3 attempts for each foot) with
hard strike, responses of the big toe were fl exion (48.3%), extension (11.3%), no movement (39.8%);
other toes were fl exion (55.0%), extension (13.0%), no movement (32.0%); ankle was dorsifl exion
(30.8%), no movement (59.3%); knee were fl exion (15.7%), no movement (84.3%); hip were fl exion
(15.2%), no movement (84.8%). Of those with big toe extension, signifi cantly less had persistent big
toe extension with light strike. More (41%) among those with extensor big toe has withdrawal response
(with fl exion of the hip and knee) as compared to those with fl exor big toe (18.5%, p<0.05). Interfoot
asymmetry was seen in 27% (kappa 0.54). Plantar responses of sequential strikes were mostly
consistent, with only 3.5% inter-attempt variability.
Conclusion: Plantar response with extensor great toe is seen in about 10% of normal adults, less with
lighter strike. Withdrawal response can be a partial explanation to the big toe extensor response in
normal adults.
4.Investigation on medical cost of 72 patients with panic disorder

Objective To investigate the medical cost of 72 patients with panic disorder and efficacy of paroxetine(SSRIs)in the treatment.Methods The course,medical cost,types and times of main examination of all 72 patients were reviewed and analyzed.The effect of paroxetine on the patients was studied in terms of anxiety and depression scale.Results The 72 patients had long course,high medical cost and received many unnecessary medical examinations.Paroxerine was effective on patients with panic disorder in light of anxiety and depression scale.Con- clusion Patients with panic disorder had long course and spent much unnecessany money before final diagnosis. Paroxetine was useful and safe in the treatment of panic disorder.
5.Selection of Different Occluders in Transcatheter Closure of Congenital Ventricular Septal Defect in Children

Objective To discuss the methods of selecting different shapes occluder and to evaluate the feasibility,safety and efficacy of them in transcatheter closures of congenital ventricular septal defect (VSD) in children.Methods Transcatheter closures were performed in 226 children with congenital VSD,age ranging from 2 to 14 years(mean 5.62 years) under the guidance of transthoracic echocardiography(TTE) and fluoroscopy.There were 14 patients with intracristal VSD,209 patients with perimembranous VSD and 3 patients with muscular VSD.Left ventriculography and transthoracic echocardiography were performed repeatedly after the procedure to assess the effect of occlusion.The echocardiography and electrocardiography were scheduled before discharge,1,6 and 12 months for the follow-up.Results The occluders were deployed successfully in 211 patients.The successful rate was 93.4%.Thin waist shape occluders,were deployed in 7 patients;equal side shape occluders,were deployed in 191 patients;eccentric shape occluders were deployed in 12 patients,and muscular defect occluders were deployed in 1 patient.There were no complications encountered during or after closure.Conclusions It is very important in transcatheter closure of congenital in children to select different shape occluder according to pathologic characteristics.In general,equal side shape occluder is suita-ble for a large number of defect and it is easy for deployment.In some conditions,the other shape occluder may be necessary.
6.Effects of B.adolescentis and L.acidophilus in treating experimental ulcerative colitis in mice and their potential mechanisms

Objective To investigate the effects of B.adolescentis and L.acidophilus in treatment of dextran sulphate sodium (DSS)-induced ulcerative colitis(UC) in mice and their potential mechanisms. Methods Seventy-five BABL/C mice were randomly divided into control,saline (NS),B.adolescentis BF0624 treatment (B),L.acidophilus LT0637 treatment (L) and salicylazosulpha-pyridine treatment (S) groups.Except control group,the other four groups were received DSS to induce ulcerative colitis. The weight-loss,fecal trait and bleeding were recorded every day.Colonic length and histological scores were evaluated on day 3,5 and 7.The gene and protein expression of hot shock protein (HSP)70, glucocorticoid receptor (GR),interleukin (IL)-10 and tumor necrosis factor (TNF)-α were detected by Western blot and reverse polymerase chain reaction (RT-PCR) respectively.Results B.adolescentis BF0624 and L.acidophilus LT0637 could relieve the inflammatory reaction of the experimental UC.The DAI scores were 1.84±0.4 in L group on day 3,which was lower than that in NS group (2.8±1.0).The DAI scores in all treatment groups were decreased on day 5.Compared with NS group[(8.1±0.6)cm ], the colon length on day 8 were (9.0±0.6)cm in B group,(9.35±0.6)era in L group and (8.8±1.1)cm in S group (P<0.05).The colonic mucosa was improved pathohistologically in L group (6.0±1.0) on day 8.The expression of HSP70 and IL-10 in B and L groups were up-regulated and the expression of TNF-α was down-regulated.Conclusions Both B.adolescentis BF0624 and L.acidophilus LT0637 were effective in treatment of acute ulcerative colitis.The potential mechanism of two probiotics may be related with up-regulation of HSPT0 and IL-10 expressions and down-regulation of TNF-α expression.
7.Establishment of an anti-CD45 monoclonal antibody that inhibits T-cell proliferation

Objective To identify molecules that modulate T-cell functions and serve the studies on T-cell media-ted autoimmune diseases.Methods Bone marrow-derived dentritic cells were collected from BALB/c mice to immunize Wistar rats, and to establish many hybridoma cell lines.Many hybridoma cell lines which could modulate T-cell functions were obtained.One of the cell lines, most actively inhibiting T-cell proliferation, was further studied.Results The anti-CD45 mAb recognized CD45 and significantly suppressed T-cell proliferation in proliferation assays.Conclusions Our re-sults indicate that the anti-CD45 mAb can effectively suppress T-cell proliferation, and is promising to be used in the pre-vention and treatment of T-cell mediated autoimmune diseases in the future.
8.Effect of downregulation of FBI-1 on proliferation of human breast carcinoma cell line and its mechanism

Background and purpose: Factor that binds to the inducer of short transcripts of human immuno-deficiency virus-1 (FBI-1) in a variety of malignant tumors showed high expression levels, which may be closely related to tumor proliferation and differentiation, angiogenesis, metastasis, but its relationship with breast cancer has not been fully elucidated. The purpose of this study was to investigate the expression of FBI-1 in breast cancer cells, and to study the effect of FBI-1 gene expression on the proliferation of breast cancer cells and its possible mechanism. Methods:Real-time fluorescent quantitative polymerase chain reaction (RTFQ-PCR) and Western blot analysis were applied to detect FBI-1 expression in normal human mammary epithelial cell line MCF-10A and breast cancer cell MCF-7. RNA interference method was used to down-regulate FBI-1 expression in MCF-7 cells. The cell proliferation was measured by CCK-8 kit and colony formation assay. RTFQ-PCR and Western blot were used to detect the expression of FBI-1 and NF-κBp65 in MCF-7 cells before and after the interference of FBI-1 expression. Results: The expression of FBI-1 was higher in breast cancer cells than that in normal human mammary epithelial cells (P<0.05). The effects of FBI-1 down-regulation inhibited proliferation in MCF-7 cells (P<0.05). At the same time, after inhibition of FBI-1, the NF-κBp65 mRNA and protein expression levels were significantly decreased (P<0.05). Conclusion: FBI-1 is highly expressed in breast cancer cells. Down-regulated FBI-1 expression can inhibit the proliferation of breast cancer cells,and its mechanism may be related to the inhibition of NF-κB signaling pathway.
9.Effect of Short-time Group Psychological Guidance on Mental Quality of College Students

Objective:To evaluate the effect of short-time group psychological guidance on mental quality of college students. Methods:Designed the content of one day's psychological coach.600 college students received psychological coach successively.60 students were randomly chosen as the intervened group,who were treated with one-day group psychological guidance,while 60 students,who never attended the group psychological guidance,were chosen as control.Assessed and compared the mental quality of the intervened group and the control group with the Faith in People Scale (FPS),Personal Evaluation Inventory (PEI),Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale (SDS) on the day before or 1 and 12 weeks after coach.Results:There were no significant differences of the scale of FPS,PEI,SAS,and SDS between two groups before coach and there were significant differences between two groups after coach.Conclusion:Short-time group guidance is helpful to developing college students' ability in interpersonal contact and self-confidence and improving depression and anxiety.
10.Application effect of family integrated care combined with intelligent health education platform in health education of elderly patients with stroke

Objective:To explore the effect of family integrated care (FIC) combined with intelligent health education platform in health education for elderly patients with stroke.Methods:Eighty patients with stroke were included from the Department of Neurology of the Second Affiliated Hospital of Chongqing Medical University from June 2019 to May 2020. They were divided into the experimental group and the control group ( n=40, respectively) by the random number table method. Patients in the control group received routine FIC in the Department of Neurology. Additional intelligent health education platform was provided for the experimental group to carry out intensive education. The modified Barthel index scale, self-care compliance scale and hospital anxiety and depression scale (HADS) were used before and after the intervention to evaluate patient′s self-care ability, self-care compliance and the degree of anxiety and depression. Results:Finally, 39 patients in the experimental group (1 case lost to followed-up) and 37 patients in the control group (3 cases lost to followed-up) completed the study. After the intervention, the scores of the modified Barthel index and self-care compliance scale of the experimental group were significantly higher than those of the control group [(65.9±12.6) vs (60.1±11.9) points, (78.2±13.6) vs (71.4±14.6) points], the HADS score was significantly lower than the control group [(15.18±2.46) vs (19.46±2.40) points] (all P<0.05). The scores of the modified Barthel index scale and self-care compliance scale of the experimental group after the intervention were significantly higher than those before the intervention [(65.9±12.6) vs (40.8±12.5) points, (78.2±13.6) vs (54.6±15.9) points] (all P<0.05), the HADS score was significantly lower than before the intervention [(15.18±2.46) vs (21.74±3.52) points] (all P<0.05). Similarly,the scores of the modified Barthel index and self-care compliance scale of the control group after the intervention were significantly higher than before the intervention [(60.1±11.9) vs (41.6±9.72) points, (71.4±14.6) vs (54.3±14.8) points], the HADS score was significantly lower than before the intervention [(19.46±2.40) vs (21.38±3.09) points] (all P<0.05). Conclusion:FIC combined with intelligent health education platform can significantly improve the self-care ability and self-nursing compliance of elderly stroke patients, and markedly improve the degree of depression and anxiety.
