1.Clinical value of procalcitonin in diagnosis of severe acute pancreatitis complicated with infection

Objective To investigate the value of serum procalcitonin(PCT)level in diagnosis of severe acute pancreatitis(SAP)complicated with infection.Methods 65 cases of SAP who were treated as research subjects and divided into infection group with 47 cases and non-infected group with 1 8 cases according to whether complicated with infection or not.The serum PCT,C-reactive protein(CRP),white blood cell count(WBC),temperature(T)and acute physiology and chronic health standards score(APACHE-Ⅱ)of the two groups were compared and analyzed.Results The serum PCT level of SAP co-infection group was(4.58±1.20)μg/L which was significandy higher than non-infected group,the difference between groups was statistically significant(P<0.05);but the CRP,WBC,T and APACHEⅡscore compared with non-infected group had no significant difference(P>0.05).Conchrsion The significant increase of serum PCT level in patients with SAP maybe relate to whether complicated with infection or not.and it was a better diagnosis and observation index for SAP complicated with infection and had important clinical significance.
2.Correlation study of arterial blood lactate levels and acute physiology and chronic health evaluation Ⅱ in critically ill patients

Objective To investigate the correlation between arterial blood lactate levels and acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ ) in critically ill patients.Methods Seventy-one patients with critical illness who treated in intensive care unit were included as research subjects and divided into death group with 16 cases and survival group with 55 cases according to the prognosis.The correlation between arterial blood lactate levels and APACHE Ⅱ was analyzed.Results With the increase of APACHE Ⅱ ,arterial blood lactate levels and mortality all increased significantly.The blood lactate levels reached (8.20±3.52) mmol/L and mortality was 64.7%(11/17) when APACHE Ⅱ >30 scores,and there were significant differences between different segment of APACHE Ⅱ (P <0.05).Linear correlation analysis showed:arterial blood lactate levels were positively correlated to APACHE Ⅱ significantly (r = 0.563,P <0.05).The APACHE Ⅱ and arterial blood lactate levels in the death group were (29.7 ± 6.9) scores and (7.6 ±4.1) mmol/L respectively,which were significantly higher than those in the survival group [(18.3 ± 7.0) scores and(3.0 ± 1.7) mmol/L].The differences between two groups were statistically significant (P<0.05).Conclusion The arterial blood lactate levels of critically ill patients are positively related to APACHE Ⅱ ,and its level has an important clinical significance in judging the severity and the prognosis of the illness.
4.Simultaneous Content Determination of Gatifloxacin Dexamethasone Acetate Ear Drops by HPLC

Objective:To developed an HPLC method for the simultaneous content determination of gatifloxacin dexamethasone ac-etate ear drops. Methods:The promising chromatographic separation was achieved on a Phenomenex C18 (250 mm × 4. 6 mm, 5 μm) column. Using gradient elution method, the optimized mobile phase consisted of methanol as the solvent A, and 0. 05% triethylamine solution (adjusting pH to 3. 0 with acetic acid) as the solvent B. The flow rate was 1. 0 ml·min-1, the column temperature was 30℃and the UV detection was carried out at 240 nm. Results: The compounds were separated well, the linearity between the peak areas and the concentrations was observed within the range of 1. 2-60. 0 mg·L-1 for gatifloxacin, and 0. 8-40. 0 mg·L-1 for dexamethasone acetate. The mean recovery of gatifloxacin and dexamethasone acetate was 101. 31% ( RSD =1. 17%) and 99. 71% ( RSD =1. 01%) , respectively. Conclusion: The method is specific and stable in the determination of gatifloxacin dexamethasone acetate ear drops.

8.Pathologic and clinical features of central neurocytoma:Analysis of 15 cases

Objective:Central neurocytomas are uncommon tumors ofthe central nervous system.In order to get better recognition ofcentral neurocytoma and diminish misdiagnosis,15 cases of central neurocytoma were analyzed by retrospective study.Methods:All cases of central neurocytoma were analyzed for their clinical symptoms,pathologic changes,immunohistochemical staining,prognosis and differential diagnosis.Clinical follow-up was available for 11 patients.Results:There were 8 males and 7 females whose ages ranged from 10 to 64 years(median 32.93 years).The most common presenting symptoms were those related to increased intracranial pressure(ICP), including headache(100%),papilledema(93%)and vomiting(80%).All tumors were located in the ventricular system.The tumors were composed of uniform cells with round nuclei and a fine chromatin pattern and small cells with perinuclear halo in some areas can be seen.In particular,a fine fibrillary matrix(neuropi)lin the anuclear areas can be seen.Nuclear atypia and vascular proliferation showed in two cases,respectively.Focal necrosis could be seen in one case.Immunohistochemical findings included expression of synaptophysin (15/15),neuron specific enolase(12/15)and glial fibrillaryacidic protein(3/15).While MIB-1 proliferation indexranged from0.8% ~12.5%,and were more than 2%in 3 of 15 cases assessed.Follow-up information was available for eleven patients.Conclusion:Central neurocytomas have a favorable prognosis in general,but the clinical course of some cases could be aggressive.Increase of GFAP positivity,proliferation index and vascular proliferation might suggest a more malignant process.
9.A study on the effect of Genistein on migration of endothelial cells induced by human ovarian carcinoma cell line SKOV_3

Objective:To investigate the effect of Genistein on migration of human endothelial cells ECV-304 induced by human ovarian serous cystadenocarcinoma cell line SKOV_3,and to explore the mechanism of anti-angiogenesis of Genistein.Methods:Millicell chamber and coculture method were used to observe the influence on migration of ECV-304 induced by SKOV_3 cells or its conditioned media,and the effect of Genistein on migration of ECV-304 induced by SKOV_3 cells or its conditioned media without serum.The expression of angiogenesis associated protein VEGF,bFGF and TGF?-1 were determined using immunocytochemical method.Results:Either SKOV_3 cells or its conditioned media could induce the committed chemotactic migration of ECV-304.The chemotactic migrations of ECV-304 induced by SKOV_3 or its conditioned media were significantly inhibited by Genistein in a dose-dependent manner.20?mol/L Genistein could down-regulate the expression of VEGF and bFGF;up-regulate the expression of TGF?-1.Conclusion:Migrations of ECV-304 induced by human ovarian carcinoma cell line SKOV_3 or its conditioned media were apparently inhibited by Genistein.It suggests that this inhibitory effect of Genistein is completed by down-regulating the expression of vessel growth-promoting factor VEGF and bFGF,and up-regulating the expression of negative regulator TGF?-1.This may be one of the mechanisms of anti-angiogenesis of Genistein.
10.A follow-up investigation on nonalcoholic fatty liver disease in medical staff at a hospital during 2006-2008

A retrospective survey wag performed among medical staff in a hospital during 2006-2008 with physical examinations and reviews of medical records.Nonalcoholic fatty liver disease(NAFLD)related factors were compared and analyzed for them.Overall detection rate of NAFLD was 13.9%(230/1655)in 2006 and 18.9%(344/1817)in 2008,with statistically significant difference(χ2=15.549,P<0. 01 ). Among people surveyed over 30 years old, detection rate of NAFLD was 9.4%, 17. 8% and 21.7% for those aged 30 -39, 40 -49 and 50 years and over, respectively in 2006, while 14. 4%, 28. 2% and 30. 5% , respectively in 2008. Detection rate presented a increasing trend in those of younger age. Relative risk for fatty liver was 1. 859 for those with obesity and overweight. Prevalence of NAFLD in medical staff increases year by year. Medical staff should attend for their own health and take measures to prevent them from its potential risk factors and pay more attention to behavioral intervention for NAFLD.
