1.The influence and significances of ghrelin on growth and cardiac function in infants with congenital heart disease

The ghrelin is an endogenous ligand of the growth hormone (GH) secretagogue receptor,which can regulate the secretion of GH,stimulate the appetite,adjust the energy metabolism,and improve the cardiovascular function.Children with congenital heart disease (CHD) usually suffered from malnutrition and growth retardation,which were connected with insufficiency of energy intake,poor absorption,and increase of nutritional requirement,anatomy and the changes of hemodynamics.Many infants with CHD gradually present ventricular dysfunction and growth restriction in their early childhood,which consequently lead to severe complications.Ghrelin is valuable in the early detection of potential CHD,plays an important role in improving the state of malnutrition and protecting the cardiac function,and provides an important reference to explore the new drug therapy for CHD.This review summarized the effects and significances of ghrelin on growth and cardiac function in infants with CHD.
2.The Transmission of the Ishinpo Versions

Objective] To study the transmission of the Ishinpo versions ,understand the prevailing version of Ishinpo. [Method] With the research methods of the Chinese traditional history and philology,collect and sort historical cited references of the Ishinpo, by paying attention to three main emendation in the transmission history of Ishinpo. [Result] Ishinpo of Nakarai is the golden version for document research. The prevailing Ansei Ishinpo version of contemporary China is based on Nakarai version,which consists of twenty seven scrolls of Hei era and three scrolls of Edo Era. Ishinpo Versions have been divided into Emperor Nakarai versions,Uji versions and Family tradition versions. There are big differences in utilization and completeness of the three version system. The Emperor Nakarai versions are relatively good, of which the Nakarai version preserves best. The Uji versions are widespread, of which the RenHe Temple version is better preserved but lacuna. There are lots of lacunas in the family tradition versions, owing to circulateed copies. [Conclusion] In the three main emendation, of Ishinpo, mostly, the Nakarai version is the source edition, the Uji version is primary edition, and the family tradition version is the secondary edition. The paper, which is useful in the transmission of the Ishinpo and thus historical process of medical exchange between China and Japan, has established the system of Ishinpo versions, defined the golden version and the prevailing version.
3.Diabetic constipation treated with acupoint embedding therapy and forlax: a randomized controlled trial.

OBJECTIVETo compare the difference among the combined method of oral administration of forlaxand acupoint embedding therapy, the simple acupoint embedding therapy and the simple oral administration of for-lax in the clinical efficacy on diabetic constipation.
METHODSOne hundred and fifty patients were randomized intoa comprehensive group, an acupoint embedding group and a forlax group, 50 cases in each one. In the acupointembedding group, the embedding therapy was applied to bilateral Tianshu (ST 25), Daheng (SP 15), Shangjuxu(ST 37) and Dachangshu (BL 25), once a week. In the forlax group, forlax (polyethylene glycol) was prescribedfor oral administration, once a day, 10 g each time. In the comprehensive group, the acupoint embedding therapyand forlax were combined and the methods were the same as the first two groups. The treatment for 4 weeks wasas one session, and 2 sessions were required in the three groups. Separately, in 4 weeks, 8 weeks of treatment and2 months after treatment, the constipation symptom scores were compared among the three groups. At the end of2 sessions of treatment, the clinical efficacy and adverse reactions were compared among the three groups. In2 months after treatment, the recurrence rate was compared among the three groups.
RESULTSThe total effectiverate was 98. 0% (49/50) in the comprehensive group, better than 86. 0% (43/50) in the acupoint embeddinggroup and 78. 0% (11/50) in the forlax group (both P<0. 01). In the 4 weeks and 8 weeks of treatment, the con-stipation symptom scores were reduced significantly as compared with those before treatment in the three groups(all P<0. 05). The results in the comprehensive group were lower than those in the other two groups (all P<0. 05). In the 4 weeks of treatment, the scores were not different significantly between the acupoint embedding group and the forlax group (P>0.05). In 8 weeks of treatment and 2 months after treatment, the scores in the acupoint embedding group were better tan those in the forlax group (all p<0.05). There were 2 cases of drug adverse reaction in the comprehensive group, 6 cases in the forlax group and 0 case in the acupoint embedding group. The recurrence rate was 8.1% (4/49) in the comprehensive group, lower than 32.6% (14/43) in the acupoint embedding group and 59.0% (23/39) in the forlax group (both P<0.01).
CONCLUSIONthe combined therapy of acupoint embedding and forlax achieves the better clinical efficacy on diabetic constipation and constipation symptom scores as compared with the simple acupoint embedding therapy and the oral administration of forlax the short-term efficacy of the simple acupoint embedding therapy is not different significantly from the simple forlax medication, but the long-term efficacy and safety are better than those of simple forlax medicaiton.

4.Improving islet yield and function by pancreatic ductal preservation

Objective To determine whether pancreatic ductal preservation(pancreatic ductal injection of sparing TIUW solution at the time of pancreas procurement)can improve islet yield and function after cold preservation of pancreas.Methods Pancreases were classified into five groups:fresh pancreases(group1 ,n= 10);preserved for 6 hr in TIUW solution without and with pancreatic ductal preservation by TIUW solution(group2, n=10 and group3, n = 10);preserved for 24 hr in TIUW solution without and with pancreatic ductal preservation by TIUW solution(group4 ,n= 10 and group5,n= 10).Dithizon(DTZ)staining was used to identify islet morphous and yield, trypan blue uptake(TBU)was used to assess the nonviability of islets, the insulin secretory response to glucose was used to evaluate islet function in vitro, islet transplantation was used to determine islet function in vivo.Results Islet yields per pancreas after purification in group 1 to 5 were 590± 127, 272 ± 50,454 ± 65, 253± 56, 447 ± 66(islet equivalent ± SD), respectively.Percentage of nonviable islets in grouplto5were(5.7±4.2)%,(18.3±6.5)%,(11.7±4.2)%,(26.3±5.6)%,(15.0±5.3)%.Stimulation index in group 1 to 5 were 7.32±2.32, 4.81±1.17, 7.56±2.44, 2.88±1.00, 5.71±1.90.Cure rates in group 1 to 5 were 100%, 0%, 100%, 0%, 70%.In this study, the differences were significant between control groups and experimental groups(P<0.05, group 2 vs.group3 and group4 vs.group5).Conclusion The pancreatic ductal preservation can improve islet yield and function.
5.Thirty Cases Clinical Study on the Treatment of Premature Ovarian Failure with the BuShen NingXin YiJing Pill

Objective The purposes of the present study are to investigate the effects and mechanisms of Bu Shen Ning Xin Yi Jing Pill on premature ovarian failure (POF) through a clinical study. Methods Sixty women with POF were administered and randomized into the treatment group and the control group. Every group had thirty cases. Patients in the treatment and control groups were requested to have the Bu Shen Ning Xin Yi Jing Pill and the Liu Wei Di Huang Pill, respectively. Six-month duration was considered as a therapeutic course. The serum levels of follicle stimulating hormone (FSH) and estradiol (E2) were determined by the chemistry luminescence method. Differences between the treatment and control groups were investigated.Results Improved menstruation were revealed in both groups and the treatment group showed significant improvement than the control group (P<0.01). In the treatment group, serum FSH levels sharply dropped after treatment, while the serum E2 levels significantly elevated compared with those of prior-treatment (P<0.01) . The total effective rate in the treatment group was 70% (21 cases), whereas 40% in the control group (12 cases). The difference had statistical significance (P<0.01) .Conclusion The BuShen NingXin YiJing Pill is effective in the treatment of premature ovarian failure via significant improvements in the clinical symptoms and balance of the endocrine hormones.
6.Alternation of blood pressure and behavioral performance in rat offspring following maternal inflammation during pregnancy

Objective To explore the effect of prenatal inflammation on rat offspring's blood pressure and behavioral performance.Methods Twenty-four time-mated Sprague-Dawley(SD)rats were randomly divided into three groups,i.e.control,zymosan and LPS.On their gestation days 8,10,12,the pregnant rats received i.p.injections of sterile saline,0.79 mg/kg zymosan or 0.79 mg/kg LPS respectively.When the offspring were sixty weeks old,eight pups were chosen randomly for the later study.Their serum TNF-? level was detected by ELISA using tail blood,arterial blood pressure was measured by tail-cuff method,and their behavioral performance was detected in Morris water maze test.At the end of the experiment,the brains were rapidly taken from the sacrificed offspring,cut into sections and stained with H&E method to investigate the structural changes of hippocampus.Results Compared with control group,all offspring from the inflammation groups showed increased serum level of TNF-?,increased systemic arterial blood pressure,long latency in finding the safe platform and decreased number of neurons in CA1 region of hippocampus.Conclusion Prenatal exposure to LPS or zymosan results in hypertension and disturbance of learning and memory.
7.17 cases of mucosal antral cyst with main symptoms of eye pain.

OBJECTIVE:
To draw the attention to mucosal antral cyst, thus reduce the probability of missed diagnosis and missed treatment by retrospectively analyzing 17 cases of eye pain patients with no obvious nasal symptoms in our department from January 2003 to September 2011.
METHOD:
All patients were performed ophthalmologic examination including eye position, eyesight, refraction, visual field, intraocular pressure and fundus examination, and the results showed that all of them had no eye disease. After excluding the deviation of nasal septum, concha bullosa and Haller gas room, CT scanning of paranasal sinus showed the maxillary sinus cyst. The patients were operated by nasal endoscope through joint path of middle meatus or inferior meatus and given antibiotics after operation. The nasal cavity packing was removed 24 hours postoperatively and the patients were discharged from hospital a week later. All the patients continued using intranasal corticosteroids and were followed up for half a year to observe whether the eye pain disappeared or recurred, when various processing was given to the surgical cavity with the help of nasal endoscope.
RESULT:
Among the 17 cases, 14 cases of eye pain disappeared after the operation, and the remaining 3 patients got symptomatic relief without recurrence during the follow-up period.
CONCLUSION
Mucosal antral cyst can cause eye pain. After excluding eye disease, once we diagnosed patients for mucosal antral cyst and the routine application of corticosteroids treatment was proved to be invalid, we should take operation treatment as early as possible, regardless of the size and location of the cysts.

8.The treatment and clinical therapeutic effect analysis in 59 cases of advanced supraglottic laryngeal squamous cell carcinoma.

OBJECTIVE:
To compare the therapeutic effect of postoperative radiotherapy with surgery alone in advanced supraglottic laryngeal squamous cell carcinoma (LSCC).
METHOD:
According to the treatment methods, 59 patients with advanced supraglottic LSCC were divided into surgery plus radiotherapy group (S+R group) (33 cases) and surgical group (S group) (26 cases). In S+R group, total laryngectomy was performed on 27 patients and partial laryngectomy on 6 patients, 27 of whom underwent lateral neck dissections; while in S group, total laryngectomy was performed on 23 patients and partial laryngectomy on 3 patients, 24 of whom underwent lateral neck dissections.
RESULT:
The 3-year overall survival rate of S+R and S group were 62.6% and 62.6% respectively, and the 5-year overall survival rate of S+R and S group were 43.8% and 40.5% respectively. But there was no statistically significant difference of survival rate between two groups by Log-rank test (P > 0.05). In S+R group, recurrence happened in 5 cases and the recurrent rate was 15.1%; in S group, recurrence happened in 10 patients and the recurrent rate was 38.5%, statistically significant difference was found between the two recurrent rates (P < 0.05).
CONCLUSION
Surgery combined with postoperative radiotherapy could not improve the 3-year-or 5-year overall survival rate, but could significantly reduced tumor recurrence rate.

9.Central lymph node metastasis in cNO papillary thyroid carcinoma.

OBJECTIVE:
This study was to evaluate the patterns of central lymph nodes metastasis, by analyzing the results of surgery in clinical NO (cNO) papillary thyroid carcinoma (PTC).
METHOD:
We retrospectively studied the effect of surgical treatment among 123 cN0 PTC patients. All the patients underwent central lymph node dissection; 47 patients underwent ipsilateral neck dissection and 9 patients underwent bilateral neck dissection. RE- SULT: Seventy-eight cases (63.4%)were found positive central lymph node, which included 34 cases (27.6%) bilateral positive central lymph node. Central lymph node metastases correlated with age < 45 years, extrathyroidal extension, surrounding tissue invasion and tumor size grade (P < 0.05), were the independent risk factors of central lymph node metastasis.
CONCLUSION
Age < 45 years, extrathyroidal extension and surrounding tissue invasion were the independent risk factors of central lymph node metastasis. For cNo patients with PTC, primary excision as well as central neck dissection was recommended; according to the results of intraoperative frozen and situation, further process were made.

10.T lymphocyte subsets and the cytokines in the newborns with infectious diseases

Objective To study the T Lymphocyte Subsets and the cytokines in the newborns with infectious diseases. Methods Twenty cases of neonatal bacterial pneumonia (bacterial group), 15 cases of rotavirus enteritis (virus group) and 20 newborns with jaundice (control group) were recruited in this study. The peripheral CD4+T Cells and lymphocyte subsets were assessed by flow cytometry. Results The IL-4 level was significantly different among different groups (F=3.39, P=0.041). The levels of IL-17 and IFN-γdid not differ signiifcantly among different groups (F=0.28 and 1.24 respectively, P>0.05). The IL-4 level was higher in bacterial group than that in virus group and control group (P<0.05). The percentages of CD3+, CD4+, CD19+and NK cells and the ratio of CD4+/CD8+ had significant difference among different groups (F=3.30-26.69, P<0.05). The percentages of CD3+and CD4+cells and the ratio of CD4+/CD8+were lower in bacterial group and virus group than those in control group (P<0.05). The percentage of CD19+cells was higher in bacterial group and virus group than that in control group (P<0.05). The percentage of NK cells was lower in virus group than that in bacterial group and control group (P<0.05). Conclusions The pattern of cytokines level is different in newborns with infection caused by different pathogens. Newborns with infectious diseases have immune dysfunction and Th2-dominated imbalance. The low percentages of T lymphocyte subsets indicate the depressed cellular immunity after infection, which may result in atypical symptom and prolonged disease course.
