Objective To discuss the feasibility and clinical value of percutaneous transluminal stenting (PTS) for the treatment of central venous obstruction (CVO) in hemodialysis patients with arteriovenous fistula (AVF).Methods The clinical data of 10 hemodialysis patients with AVF complicated by CVO were retrospectively analyzed.Clinically, all patients presented as swollen hand syndrome.Preoperative or intraoperative digital subtraction angiography (DSA) was performed to determine the obstruction site, and based on the disease condition the appropriate surgical approach was employed. For patients having thrombus formation,catheter-directed thrombolysis (CDT) was carried out first. For patients having severe stenosis or occlusion of veins,pre-expansion with small diameter balloon was employed before PTS.For the remaining patients,PTS was directly performed. All patients were regularly followed up after operation. Results DSA showed that brachiocephalic vein occlusion and/or occlusion or stenosis of subclavian vein, internal jugular vein and superior vena cava were observed in 5 patients who had history of internal jugular vein catheterization, while localized severe stenosis of medial segment of AVF-side subclavian vein was detected in the other 5 patients who had no history of internal jugular vein catheterization. The technical success rate of PTS was 100% (10/10).A total of 19 stents were implanted in the 10 patients.Seven months after the treatment, one patient developed in-stent re-stenosis,and PTS had to be carried out again.Primary patcncy rates at 6 months and 12 months after the treatment were 100% (8/8) and 75% (3/4) respectively. Conclusion In hemodialysis patients with AVF,CVO is mainly characterized by obstructive or severely stenotic lesions.PTS carries higher success rate with satisfactory short-term and mid-term effect, its complications are slight and mild, and the technique is safe and feasible. Therefore,PTS can be used as the preferred treatment method.

AIM: According to the functions of Lumbago Suppositiory (Radix Aconiti, Ramulus Cinnamoni, Moschus, etc) (LS), we observed the connection among anti-inflammatory, analgesia and promoting blood circulation and removing blood stasis. METHODS: Rat chemical radiculoneuritis model was set up to observe influence of LS on inflammation, pain threshold, gait and PLA_2 in serum, content of adrencorticoids and adrenalin. Cold accumulation type of blood stasis symptom model was built up to observe the influence of LS on blood rheological property, effect of LS on mouse's micro-circulation in ears was also observed. RESULTS: Lumbago Suppository inhibited inflammation of rat radiculoneuritis model obviously and had recovery of functions for gait and pain threshold. And it also could reduce the content of PLA_2 in serum and raise the level of adrenocorticoids and adrenalin. It also obviously changed "blood stasis symptom”, rat blood rheological property and auricle micro-circulation of mouse. CONCLUSION: Lumbago Suppository's functions of anti-inflammatory and analgesia have strong relation ships with promoting blood circulation and removing blood stasis.

BACKGROUND: Diabetic retinopathy is a commonly chronic-vascular complication in a course of diabetes mellitus (DM), and its relation with apoptosis and changes of ion level needs to be proved.OBJECTIVE: To observe apoptosis and changes of Na+ and K+ contents in retinal tissue of DM rats.DESIGN: Randomized controlled study.SETTING: Department of Ophthalmology, Liuzhou Municipal People's Hospital.MATERIALS: The experiment was completed at the Department of Ophthalmology, Liuzhou Municipal People's Hospital. A total of forty adult male Wistar rats, aged 20 months, weighing 320-350 g, were provided by Animal Center of Medical College Affiliated to Sun Yat-sen University.METHODS: Forth Wistar rats were randomly divided into control group and experimental group with 20 in each group. Streptozotocin was used to induce DM in the experimental group. Apoptosis and changes of Na+ and K+ contents in retinal tissue were measured on the 4th, 6th, 12th and 16th weeks after DM onset. Rats in the control group were injected only with the same volume of citrate buffer solution. Then, correlations on the aspect of fasting blood glucose (FBG), hemoglobin glycosylation (HbAlc), Na+ and K+ contents and DM course were analyzed between the two groups.MAIN OUTCOME MEASURES: ① Na+ and K+ contents; ② FBG concentra tion and HbAlc level; ③ changes of apoptosis; ④ correlations among markers.RESULTS: Apoptosis could be detected in retinal tissue in the experimental group at 4 weeks after DM onset, and with course elongating, level of apoptosis was aggravated gradually. Na+ concentration was increased in retinal tissue, but K+ concentration was decreased (P < 0.01 or 0.05). Levels of apoptosis in retinal tissue in the experimental group were positive correlation with FBG concentration, HbAlc level, Na+ content and DM course (r=7.584, 7.844, 7.369, 6.246; P < 0.01); however, they were negative correlation with K+ content in retinal tissue (r=7.658, P < 0.01).CONCLUSION: There are apoptosis and abnormal Na+ and K+ contents in retinal tissue of DM rats. Moreover, these changes may be one of pathological bases of diabetic retinopathy.
