1.The diagnosis value of using NBI-ME in serum CEA positive microvascular parting observation of gastric cancer and colorectal cancer

Objective To study clinical diagnosis value of using magnifying endoscopy with narrow-band imaging(NBI-ME) in the CEA positive cases of microvascular parting to gastric cancer and colorectal cancer .Methods 873 cases in our hospital from 2012~2013 ,who had serum CEA positive were choosen ,the 114 patients who first diagnosis for suspected gastric cancer ,35 pa-tients who first diagnosis forsuspected colorectal cancer were included in this study .Using NBI-ME to check and the pathology bi-opsy .NBI-M E check key observation microvascular properties and classification :rules ,slightly irregular ,irregular ,disappeared ,the microvascular parting situation compared with pathological findings .Results 13 patients diagnosed with cancer by pathology ,the positive rate was 8 .72% ;intraepithelial neoplasia in 10 cases ,the positive rate was 6 .71% .No cases of cancer were regular and slightly irregular type ,all cancer cases in the disappearance .The sensitivity of type of disappeared was 100 .00% ,and the sensitivity of irregular type of canceration possibility intestinal large atrophy and high grade intraepithelial neoplasia was 87 .50% ,the differ-ence was statistically significant(P<0 .05) .Conclusion Using NBI-ME microvascular classification diagnosis could help observa-tion CEA positive cases in the screening of gastric cancer and colorectal cancer .
2.Correlation between interleukin-1 and the obesity of polycystic ovary syndrome

Objective To investigate the correlation between interleukin-1β(IL-1β),interleukin-1 receptor antagonist(IL-1 ra)and the obesity of polycystic ovary syndrome(PCOS).Methods(1)From Oct.2006 to Jan.2007,118 PCOS patients were enrolled in this study in Peking University Third Hospital,which were divided into 56 patients in obese PCOS group and 62 patients in non-obese PCOS group according to the WHO International Obesity Task Force Asia-Pacific criteria[body mass index(BMI)25 kg/m2].The polymorphism of IL-1β gene promoter region,exon-5 and intron 2 of IL-1 ra gene were detected by PCR.(2)Twenty-nine obese PCOS patients and 31 non-obese PCOS patients were selected randomizedly serum levels of IL-1β,IL-1 ra were measured by ELISA,in the mean time,serum levels of fasting glucose,fasting insulin and the total white blood cell,hypersensitive C-reactive protein levels were measured.Results(1)Genetic test:the frequency of TT genotype and T allele of IL-1β promoter region(-511)in obese PCOS patients were significantly higher than those in non-obese patients(44.6% vs.11.3%,63.4% vs.39.5%,all P <0.05).The frequency of IL-1 ra Ⅰ / Ⅴ genotype and Ⅴ allele of IL-1 ra gene were 19.6% and 9.8% in obese PCOS patients,which were significantly higher than those in non-obese group(3.2% and 1.6%,P <0.05).(2)Serological test:serum level of IL-1β and IL-1ra of(149 ±36)and(284 ±97)ng/L in obese PCOS group which were significantly higher than those in non-obese PCOS group[(96 ± 42)and(208 ± 84)ng/L,P < 0.05].Fasting blood glucose,fasting insulin and hypersensitive C-reactive protein and white blood cell count were(5.1 ± 0.7)mmol/L,(17 ± 9)mU/L,(1.5 ± 0.6)mg/L and(7.0 ± 2.3)× 109/L in obese PCOS group,which were significantly higher than in non-obese PCOS group[(4.9 ±0.5)mmol/L,(11 ±8)mU/L,(0.9 ±0.4)mg/L and(5.9 ±1.3)× 109/L,P<0.05].(3)The correlation between interleukin and BMI: serum levels of IL-1β(r =0.673)and IL-1 ra(r =0.557)were positively correlated with BMI in PCOS patients(P < 0.05).Conclusions Inflammatory factors IL-1β and IL-1ra had correlation with obesity of PCOS patients,PCOS patients who carried T allele of IL-1β gene promoter region(-511)and Ⅴ allele of IL-1ra gene were high risk of obesity.
3.Protein expression profilings of polycystic ovary syndrome

Objective: To screen out serum protein profiling of polycystic ovary syndrome(PCOS) by surface - enhancedlaser desorption/ ionization time of flight mass spectrometry (SELDI-TOF MS) for discovering the discriminatory proteins. Methods: Thirty one women with PCOS and thirty healthy women were detected by Weak Cation Exchange(WCX2)chip. ProteinChip reader and Biomarker Wizard software from Ciphergen Inc were combined with a bioinformatics method (support vector machines, SVM ) to analyze protein fingerprinting. Results: There were 4 proteins which were obviously different between the PCOS group and the control. Three of them were up-regulated, and one down-regulated. To set up a analysis model by SVM using the 4 proteins could successfully distinguish between PCOS and the normal control. The corresponding sensitivity, specificity and positive predict value were 86.7%, 83.3%, 87.2%, resectively. Conclusion: Using ProteinChip technology can screen out serum discriminatory proteins quickly and efficiently. Combined with SVM, an optimal fingerprinting model has been set up, which can easily predict PCOS. In disease state of PCOS, there are significant variations which consist of four proteins. To investigate those four discriminatory proteins, especially the protein m/z 6 628 may be of benefit to pathogenic study and the development of biomarkers for PCOS.
4.Endometrial blood flow distribution measured by color Doppler energy imaging in the prediction of pregnancy outcomes during in vitro fertilization and embryos transfer cycles

Objective To discuss the role of endometrial blood flow screening measured by color Doppler energy(CDE) in the prediction of outcomes in patients receiving in vitro fertilization and embryos transfer(IVF-ET).Methods A total of 182 women below the age of 38 years old undergoing IVF-ET in this center from November 2005 to December 2005 were recruited in this prospective study.The endometrial thickness,echo pattern,and blood flow status were detected by transvaginal ultrasonography on the day of hCG injection(8 hours before injection).The patients were divided into three groups: Group A,the endometrial blood flow could not be detected;Group B,only sub-endometrial blood flow could be detected;and Group C,both endometrial and sub-endometrial blood flow could be detected.All the patients were re-divided into three groups according to their outcomes of IVF-ET: Group 1,no pregnancy;Group 2,intrauterine pregnancy with live fetus and Group 3,other conditions(including biochemical pregnancy,embryonic diapause,ectopic pregnancy,and spontaneous abortion) were encountered.Results The clinical pregnancy rate with live fetus and the implantation rate were significantly higher in the Group C(62.2% and 65.6%) than in the Group A(0 and 0) and the Group B(17.1% and 20.7%),respectively(P

Objective To study the expression of leptin and long form of leptin receptors (Ob R L) messenger ribonucleic acid (mRNA) and protein in endometrium during mid luteal phase of patients with polycystic ovary syndrome (PCOS) Methods The expession of leptin and Ob R L mRNA in endometrium during mid luteal phase from 15 normal controls and 14 PCOS related anovulatory patients who underwent ovulation induction therapy were studied by in situ hybridization, and levels of leptin and Ob R L protein were investigated by immunohistochemical methods Results The expression of Ob R L mRNA and protein in endometrial glands of most patients with PCOS were weak or moderate positive,whereas the levels of Ob R L mRNA and protein were moderate or strong positive in those of the controls,there was significant difference between the two groups; leptin mRNA and protein expression in enodmetrium of patients with PCOS and controls were not significantly different between the groups Conclusion The expression of Ob R L decreased in endometrium of mid luteal phase in patients with PCOS during induced ovulation,which may be related to low pregnancy rate of patients with PCOS.
7.Research progress of sequencing method for animal mitochondrial genome

Mitochondria, the power house of cells, are important organelles in eukaryotic cells. Having their own unique and complete DNA (mtDNA) and genetic system, mitochondria play an essential role in cellular energy metabolism, intracel?lular signaling and apoptotic pathways, as well as many other biological functions, which are closely related with cellular met?abolic network. A disruption of mitochondrial genes can therefore result in mitochondrial dysfunction and human diseases, thus they have been widely used in molecular biology, development biology, genetics, forensic identification and clinical diag?nosis. Consequently, sequencing mitochondrial genome has shown great significance in mitochondrial structure and function research. In this review, research progress in mitochondrial genome sequencing method is summarized, mainly focusing on Sanger sequencing, long-PCR and next-generation sequencing. Also rolling circle amplification and indirect sequencing of mtDNA are reviewed. The ambiguities caused by numts in indirect sequencing are mentioned and resolved.
8.EML4-ALK and EGFR mutation status and survival analysis in Uygur with stageⅣNSCLC

Objective To investigate the relationship between the echinoderm microtubule associated protein like 4-anaplastic lymphoma kinase (EML4-ALK) and epithelial growth factor receptor (EGFR) mutation status and overall survival (OS) in Uygur patients with stageⅣnon-small cell lung cancer (NSCLC) who did not accept tyrosine kinase inhibitor treat-ment. Methods Totally 97 tissue samples were collected from Uygur patients with stageⅣNSCLC who did not accept tyro-sine kinase inhibitor treatment. EML4-ALK fusion gene and EGFR mutation status were detected by using FISH and ARMS methods. The survival rates were analysed. Results In 97 tissue samples, EML4-ALK fusion genes were found in 6 (6.2%) samples, EGFR mutations were found in 26 (26.8%) samples. The survival analysis showed that there was no significant dif-ference in OS between EML4-ALK fusion gene group and no EML4-ALK fusion gene group (P=0.941). There was no signifi-cant difference in OS between EGFR mutation group and wild-type EGFR group (P=0.607). The values of median OS were 17.7 months, 17.3 months and 16.2 months for EGFR mutant group, EML4-ALK positive group and EML4-ALK negative+EGFR wild-type group, and thre was no significant difference between them (P=0.915). Conclusion Excluding the thera-peutic influence in TKIs, EML4-ALK fusion gene and EGFR mutation status of tumor tissue can not be used as an indepen-dent factor in assessing the prognosis in Uygur patients with stageⅣNSCLC.
9.The impact of residual platelet activity and CYP2C19 polymorphism on the prognosis of acute coronary syndrome patients

Objective To evaluate the correlation between RPA or the polymorphism of CYP 2C19 and the incidence of ischemic events and the influence on the clinical prognosis .Methods A case-control study was used.A total of 202 patients [male 66%,(63 ±11) years] with ACS on aspirin and clopidogrel treatment were recruited , whose RPA were measured by whole blood aggregometry ( WBA ) , and their CYP2C19 polymorphism were also tested .Their clinical ischemic events were recorded in the mean follow-up period of 16 months.The RPA cut-off values for antiplatelet low-responsiveness were defined by the receiver operator characteristic curve ( ROC); the relationships of clinical outcomes with RPA and CYP 2C19 were assessed by the Kaplan-Meier survival analysis.Results CYP2C19*2 (681G>A) present in 52.5% of recruited patients and*3 (636G>A) present in12.9%.RPA induced by adenosine diphosphate ( ADP) showed significant difference among CYP 2C19*2 or *3 heterozygotes, CYP2C19*2 or *3 homozygotes and noncarriers (χ2 =9.318, P=0.009);whereas, RPA induced by arachidonic acid (AA) (χ2 =2.441, P=0.295) and the incidence of ischemic events (χ2 =0.513, P=0.774) were not.During follow-up, 18 (9%) patients experienced clinical ischemic episodes , and their RPA were higher than patients without ischemic episodes [(8.6 ±4.8) Ωvs (5.2 ±3.7) Ω, P =0.013; (8.6 ±6.8) Ωvs (1.6 ±3.7) Ω, P <0.001].Moreover, employing 6.5 Ω(induced by ADP) and 2.5 Ω(induced by AA) as cutoff values,RPA showed optimal negative predictive values (97%, 96%) and poor positive predictive values (16%,29%).Survival analysis showed, statistically, patients with clopidogrel low-responsiveness had higher riskof ischemic episodes than patients with clopidogrel responsiveness (HR =2.86, χ2 =11.27,P =0.0008);however, patients with aspirin low responsiveness (HR =1.77, χ2 =1.74, P =0.19) or patients withCYP2C19*2 or *3 (HR =0.89, χ2 =0.12, P =0.73) did not.Conclusion Clopidogrel lowresponsiveness is associated with the occurrence of clinical ischemic events ; however, patients withCYP2C19 function reduced genetypes do not show higher risk of ischemic episodes though it presented slighlyhigher RPA.
10.Evaluation of HR-HPV testing in diagnosis of cervical intraepithelial neoplasia and cervical cancer

Objective To evaluate the role of high-risk human papillomavirus (HR-HPV) testing in diagnosis of high-grade cervical intraepithelial neoplasia (CIN2/3+) and cervical cancer.Methods Total 3426 women with known cytologic abnormality tests were subjected to detect high-risk HPV DNA by hybrid capture II and to have established diagnoses by cervical coloposcopy-directed biopsy test.Results Among 3426 women,45 were cervical cancer,670 were CIN 2/3,and 2711 were CIN 1 or other benign lesions based on the cervical histologic results. Compared with the women with CIN 1 or other benign lesions,the women with CIN 2/3+ had higher positive rates of HR-HPV. Higher load of HR-HPV was more common among women with cervical lesions than those without. Fifty-seven women who were transiently HR-HPV positive did not develop CIN2/3+ during follow-up.Conclusions Our data suggest that a high viral load could be used as a short-term marker of CIN 2/3+,and women repeatedly tested positive for HR-HPV are at risk of developing CIN2/3+. Detecting HR-HPV and follow up the positive women would be useful in diagnosis of CIN2/3+.
