Objective To investigate the clinical characteristics of childhood intracranial tumor.Methods The age and sex distribution,location of tumor,clinical manifestation,imaging examination and pathological examination,diagnosis and treatment of 98 children under 14 years old admitted into Children′s Hospital of Chongqing Medical University from Jun.2003 to Sep.2008 were analyzed.Results There were 63 male and 35 female in the study.The gender ratio(male to female) was 1.8:1.The age range was 4 months to 14 years old,75 cases(76.5%) visited a doctor in the course of disease from 1 to 90 days.The tumors of 60 cases(61.2%) were located in posterior cranial fossa,while 14 cases(14.3%) in cerebral hemisphere.The most common intracranial tumors were medulloblastom(30 cases,30.6%),astrocytic tumor(26 cases,26.5%),ependymal tumor(17 cases,17.3%),craniopharyngioma(10 cases,10.2%) and germ cell tumor(4 cases,4.1%).The main clinical manifestations were vomiting(61 cases,62.2%),headache(58 cases,59.2%),instability of gait or incoordination(38 cases,38.8%),29 cases(29.6%) of the tumors were misdiagnosed.Obstructive hydrencephalus was found in 78 cases(79.6%) by imaging examination,positive rate of tumor′s diagnosis was 95.9% and 100% by brain CT and MRI scaning,respectively.Conclusions Intracranial tumors in childhood are mostly located in posterior cranial fossa with atypical clinical manifestations and short course of diseases.It is common to be misdiagnosed at early stage.Brain CT and MRI examinations are helpful for early diagnosis,which should be considered for the children when unreasonable increased intracranial pressure and localizing neurological sings exist.

Objective:To investigate the association of pretherapeutic neutrophil, lymphocyte, monocyte, and serum albumin levels with nasopharyngeal carcinoma (NPC) prognosis in 401 patients. Methods:This study involved 401 NPC patients recently diagnosed and admitted in Sun Yat-sen University Cancer Center, China. Laboratory variables, such as immunology-related parameters and albumin level prior to treatment, were analyzed. The overall survival and disease-free survival rates were determined using the Kaplan-Meier method and were then compared using the log-rank test. Univariate and multivariate analyses were performed using the Cox proportional hazards model to identify prognostic factors. Results:Patients with neutrophil counts>5.20×109/L (n=172) had significantly lower overall survival and disease-free survival rates compared with those with neutrophil counts≤5.20×109/L (n=229) (P=0.014, P=0.009). In the 401 NPC cases, patients with high albumin levels (>43.00 g/L;n=253) had significantly higher overall survival (P=0.000) and disease-free survival (P=0.006) rates compared with those with low albumin levels (≤43.00 g/L;n=148). Multivariate analysis showed that the neutrophil level is an in-dependent risk factor for disease-free survival, whereas the albumin level is a strong independent prognostic factor for overall survival. Conclusion:The pretherapeutic neutrophil and albumin levels are independent prognostic factors for survival in NPC patients.

Objective To evaluate the adjunctive therapeutic effect of intravenous immunoglobulin (IVIG) therapy in the pulmonary infection after kidney transplantation.Methods In 14 cases of pulmonary infection after kidney transplantation, 8 and 6 cases were subjected to high dose (7 to 10 days, group A) or low dose (3 to 7 days, group B) of IVIG therapy besides the standard specific anti-pathogen therapy. In 12 cases of pulmonary infection after kidney transplantation, only standard specific anti-pathogen therapy was given (group C). The incidence and mortality of severe pulmonary infection, levels of serum IgG, IgA, IgM and T lymphocyte subsets among the three groups were observed.Results The incidence of severe pulmonary infection was 0, 66.7 % and 66.7 % respectively in groups A, B and C with the mortality being 0, 16.7 % and 25.0 % respectively. The level of serum IgG was significantly increased in group A after treatment as compared with that before treatment and in group B ( P 0.05 ).Conclusion As an adjunctive therapy, high dose of IVIG at early stage of pulmonary infection can reduce the incidence of severe pulmonary infection and mortality after kidney transplantation.

Objective To investigate the clinical efficacy of hemodialysis (HD) combined with hemoperfusion (HP) versus HD combined with hemodiafiltration(HDF) in treating uremic pruritus .Methods The randomized controlled trials(RCTs) on HD combined HP versus HDF in treating uremic pruritus were retrieved from the electronic databases of Cochrane library ,PubMed , Science ,CNKI ,Chinese Biological Medicine ,Wanfang and CNKI by computer .The domestic medical journals ,monographs ,meeting documents and academic dissertations were manually retrieved .The retrieval time limit was from their establishment to October 2015 .Meanwhile the obtained reference literature indexes were consulted .The two researchers independently screened the collected literatures according to the formulated inclusion and exclusion standards .The visual analogue scale (VAS) ,skin itchy effect judg‐ment criteria ,Dirk R Kuypers score ,PTH ,Ca2+ and P3 - were adopted as the evaluation standards .The Review Manager 5 .3 soft‐ware was used to conduct the meta‐analysis .Results Nine RCTs were finally included ,involving 405 patients .The meta analysis showed that HD+ HP had better effect than HD+ HDF in treating uremic pruritus and clearing blood BUN ,HD+ HDF had better effect than HD+ HP for maintaining Scr and P3 - clearing effect ,while no statistically significant difference was found in terms of PTH decrease and Ca2+ influence (P>0 .05) .Conclusion The dialysis mode of HD+ HP is more effective in treating uremic pruri‐tus .

Objective To study the gene mutation and clinical characteristics of hereditary spinocerebellar ataxia type 7 (SCA7).Methods The regions of SCA7 gene containing CAG repeat were amplified by means of polymerase chain reaction (PCR) and polyacrylamide gelelectrophoresis (PAGE) technique in 112 patients with autosomal dominant SCA from 92 families, 16 sporadic SCA patients, 71 family members and 60 healthy controls. The abnormal allele fragments were sequenced by ABI377 DNA sequencing machine. The correlation between clinical manifestations and CAG repeat size in SCA7 gene product was analyzed. Results 2 patients of 6 members in 1 SCA7 family carried a 71-repeat allele. The main clinic features included ataxia, hypopsia, axanthocyanopsia and retinal pigmental degeneration. Alleles from 7 to 9 repeats were seen in the other 4 healthy members. CAG repeats from 6 to 21 were found in other 126 SCA patients, 71 family members and 60 healthy controls. Conclusions Expanded triplet repeats in SCA7 gene contributes to the pathologic phenotype, and molecular genetic analysis is effective for the diagnosis of SCA7. Retinal pigmental degeneration is an important characteristic of SCA7.

Objective To explore the roles of insulin in the early treatment of acute brain injury and its administration methods.Methods 253 patients were randomly divided into the intensive insulin therapy group and the conventional therapy group.Infection rates,the short-term effect (APACHE Ⅱ assessment),and long-term efficacy (GOS prognosis) was compared between two groups.Results The results of the strengthen treatment group in the rate of infection (25.95％ vs 39.34％,x2 =5.17,P ＜0.05),the short-term effect (11.33 ± 7.66 vs 16.49 ± 14.97,u =3.42,P ＜ 0.05) and the long-term efficacy (40.46％,55.73％,3.82％ vs 25.41％,68.85％,5.74％,x2 =7.62,P ＜0.05) were significantly better than the conventional therapy group with the statistically significant differences (P ＜ 0.05).Conclusions The hypoglycemic effect,neuroprotective effect,regulatory role,and nutrition role of insulin occurred in the early treatment of acute brain injury.After acute brain injury,patients with hyperglycemia should be treated early with an enough volume,continuous,and uniform insulin.
