Objective To explore the influencing factors of therapeutic effect and prognosis in patients with angioimmunoblastic T cell lymphoma (AITL). Methods The clinical data of 48 patients with AITL were collected in order to evaluate its therapeutic effect and the influencing factors of prognosis. Results In the 48 patients with AITL, complete remission (CR) was in 15 cases, partial remission (PR) was in 16 cases, and the total effective rate was 64.58%(31/48). The recent total effective rate in patients of international prognostic index (IPI) score ≤2 scores was significantly higher than that in patients of IPI score >2 scores: 84.00% (21/25) vs. 43.48% (10/23), and there was statistical difference (P<0.05). The 1-year, 2-year and 3-year overall survival rates were 75.00%(36/48), 52.08%(25/48) and 35.42%(17/48), and the median survival time was 24.5 months. The patients of Ann Arbor stage Ⅰ-Ⅱ, IPI score ≤ 2 scores, Ki-67<50% and using CHOP combined with asparaginase (ASP) regimen had a higher 3-year overall survival rate compared with the patients of Ann Arbor stage Ann Arbor stage Ⅲ -Ⅳ, IPI score >2 scores, Ki-67 ≥ 50% and using CHOP regimen: 8/13 vs. 25.71%(9/35), 52.00% (13/25) vs. 17.39% (4/23), 55.00% (11/20) vs. 21.43% (6/28) and 48.28% (14/29) vs. 3/19, and there were statistical differences (P<0.05). Conclusions AITL is a kind of disease with poor prognosis. IPI score is the important influencing factor of recent therapeutic effect. The Ann Arbor stage, IPI score, Ki-67 levels and using contained ASP chemotherapy are the important factors of prognosis in patients with AITL.

Objective Through analysis the amount and the types for the bacterial in the conjunctival sac between the warm meibomian gland massage and routine preoperative preparation for the inner eye operations,to compare the bacterial state before and after the nursing intervention,as to analysis of preoperative administration of the routine preoperative preparation on the effectiveness of prevention of postoperative intraocular inflammation,and so on peri operation period nursing intervention is effective in reducing or preventing endophthalmitis.Methods 40 patients(40 eyes) undergoing eye operation during March to December of 2013 in our hospital,they were divided into the experimental group and the control group,with preoperative warm meibomian gland massage the experimental group based in the implementation of internal eye operation on routine preparation.Based on all the patients in group (V1),on the morning of surgery (V2) before operation,after disinfection(V3),before the end of operation(V4),after the next morning(V5) and after 1 weeks of operation (V6) of the 5 conjunctival sac sampling cultured conjunctival sac fungi and bacteria population analysis.Results Two groups of cultured conjunctival sac and there were significant differences between the V1 and V6 for both groups in the amount and the tapes of the bacterial.Conclusions The inner eye operation patients with warm meibomian gland massage can effectively reduce or inhibit the bacterial amount after operation in the conjunctival sac,effectively reduce the occurrence of postoperative endophthalmitis of hidden dangers,this procedure should be schduled in the standerand SOP for the peri--operative preparation for the inner eye operations.

Background Glaucoma is a kind of psychosomatic disorder.Investigating the psychological and behavioral characteristics of glaucoma patients has important implication for the prevention and treatment of the disease.Objective Present study aimed to characterize the personality profile of initial diagnosed glaucomatous patients.Methods Eighty-seven patients with primary open angle glaucoma (POAG),ninety-two patients with primary angle-closure glaucoma (PACG) and eighty-nine normal controls (normal control group) were enrolled in this study from January 2008 to May 2014 in Peking University People 's Hospital.Clinical and demographic information which could relate to personality type was collected by filling in Symptom Checklist 90 (SCL-90),Life Event Scale (LES),Eysenck Personality Questionnaire (EPQ),Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS) voluntarily under the informed consent.The psychological,personality and behavior characteristics of the individuals were analyzed and compared among the different populations.Results In SCL-90,the scores of compulsivity,interpersonal sensitivity,depression,anxiety,hostility,phobic anxiety,paranoid ideation and psychoticism inpatients with primary glaucoma were significant different among the three groups (all at P＜0.01),and the scores were significantly higher in the patients of the POAG group and PACG group than those in the normal control group,and the scores were increased in the PACG group compared with the POAG group (all at P＜0.05).In LES,the scores of family events,social intercourse and other events,life events,stress response were significantly different among the three groups (P =0.032,0.002,0.001,0.000),and these scores were significant higher in the POAG group and PACG group compared with the normal control group,and the PACG group also had a significantly greater frequency of clinically abnormal score than the POAG group (all at P＜0.05).EPQ data showed that the scores of psychology and nervousness in the POAG group and the PACG group were higher than those in the normal control group (all at P＜0.05),but no significant difference were found in extroversion and stability of the personality among the three groups (P=0.321,0.315).The total scores of SAS and SDS were considerably higher in the POAG group and the PACG group compared with the normal control group or the PACG group compared with the POAG group (all at P＜0.05).The incidences of anxiety and depression were elevated in the POAG group and the PACG group in comparison with the normal control group (x2 =15.520,P =0.000 ; x2 =14.870,P =0.000).Conclusions The personality and behavioral factors are associated with the pathogenesis,development and therapy of primary glaucoma.The anxiety,depression and stronger stress response are found in glaucoma patients,especially PACG patients.

BACKGROUND:Studies demonstrated that matrix metalloproteinase-1(MMP-1) and tissue inhibitor of metalloproteinases-1(TIMP-1) exhibit important role in corneal defect of rats with alkali burns.OBJECTIVE:To observe the promoting effect of recombinant human epidermal growth factor(rhEGF) on rats' corneas following alkali burns,in addition,to explore the effect and significance of rhEGF on the expression of MMP-1 and TIMP-1 in alkali burned rats.DESIGN,TIME AND SETTING:A randomized controlled animal experiment was performed at the Laboratory Animal Center,Liaoning Medical College from March 2008 to May 2009.MATERIALS:Sixty healthy SD rats,weighting 180-210 g,with 6-8 weeks old,half male and half female.All rats were confirmed that no anterior segment disease before the experiment with slit lamp microscope examination.METHODS:Sixty rats were randomly divided into experimental and control groups,with 30 animals in each group.After the preparation of corneal alkali burn models,cornea of rats in the experimental and control groups were given rhEGF eye drops,and physiological saline eye drops respectively,fourth a day,consecutive for 21 days.Six rats of each group were killed at days 1,4,7,14 and 21 after operation.MAIN OUTCOME MEASURES:①Changes of cornea after alkali burned was observed by slit lamp microscope.②The expressions of MMP-1 and TIMP-1 in cornea were detected with immunohistochemistry and computer image analysis system at different time points.RESULTS:The slit lamp microscope results showed that the area of corneal ulcer in the experimental group was obvious smaller than the control group at the corresponding time points.The expression of MMP-1 gradually increased and reached a peak level at 14 days after surgery,then began to decrease.TIMP-1 expressed in the early phrase,but later,its activity gradually decreased,reached the lowest level at 14 days,then began to increase,and peaked at 21 days.The level of MMP-1 in the experimental group was greatly lower than that in the control group at each time points.However,the level of TIMP-1 of the experimental group was greatly higher compared with the control group.CONCLUSION:The rhEGF eye drops can improve effectively the healing of corneal epithelial defect by adjusting the proportion of matrix metalloproteinase and its tissue inhibitor.

Objective To evaluate the effect of ulinastatin on hepatic ischemia-reperfusion injury in patients with hepatic alveolar echinococcosis in plateau region.Methods Sixty patients with hepatic alveolar echinococcosis in plateau region,weighing 45-75 kg,aged 30-55 yr,of American Society of Anesthesiologists physical status Ⅰ or Ⅱ,scheduled for elective partial hepatectomy,were divided into control group (C group) and ulinastatin group (U group) using a random number table,with 30 patients in each group.Ulinastatin 12 000 U/kg (in 100 ml of normal saline) was infused intravenously at 20 min before occlusion of hepatic portal in group U,and normal saline 100 ml was given instead in group C.Before operation,at 10 min after occlusion of hepatic portal,at 20 min and 1 h after unclamping hepatic portal and at 12 h after operation,blood samples were collected from the right internal jugular vein for determination of the levels of serum alanine aminotransferase,aspartate aminotransferase and malondialdehyde.Results Compared with group C,the levels of serum alanine aminotransferase,aspartate aminotransferase and malondialdehyde were significantly decreased at 20 min and 1 h after unclamping hepatic portal and 12 h after operation in group U (P＜0.05).Conclusion Ulinastatin can attenuate hepatic ischemia-reperfusion injury in patients with hepatic alveolar echinococcosis in plateau region.

The total volatile components were extracted from safflower by ultrasonic-assisted solvent extraction (USE) and their chemical constituents were analyzed by gas chromatographymass spectrometry (GC-MS) to provide scientific basis for the quality control of safflower.Five different solvents (diethyl ether,ethanol,ethyl acetate,dichloromethane and acetone) were used and compared in terms of number of volatile components extracted and the peak areas of these components in TIC.The results showed that USE could be used as an efficient and rapid method for extracting the volatile components from safflower.It also could be found that the number of components in the TIC of ethyl acetate extract was more than that in the TIC of other solvent ones.Meanwhile,the volatile components of safflower from Xinjiang Autonomous Region and Henan Province of China were different in chemical components and relative contents.It could be concluded that both the extraction solvents and geographical origin of safflower are responsible for these differences.The experimental results also indicated that USE/GC-MS is a simple,rapid and effective method to analyze the volatile oil components of safflower.

An inductively coupled plasma mass spectrometry (ICP-MS) or inductively coupled plasma atomic emission spectrometry (ICP-AES) was developed to determine 19 elements in safflower,a traditional Chinese medicinal herb from Xinjiang Autonomous Region and Henan Province of China.Totally 19 elements in safflower included heavy metals,i.e.As,Cd,Cu,Hg and Pb,and wholesome elements,i.e.AI,Ca,Co,Cr,Fe,Mg,Mn,Mo,Ni,P,Se,Sr,V and Zn.The results showed that the concentrations of heavy metals in safflower samples were both low,all of which met the national hygiene standards except for Pb in Xinjiang sample.Meanwhile,the distribution tendency of elements in the two samples was similar,which indicated that the plant might absorb given elements in a proportional way.The method can be used for the quality control of elements in safflower,and it provides a way for the determination of the contents of safflower from Xinjiang and Henan.

Radiotherapy is an effective mean of treatment of gastric cancer. Combination of radiotherapy and chemotherapy in gastric cancer preoperative, postoperative and palliatie treatments can increase the local control rate and the survival rate of gastric cancer patients. With the development of three-dimensional conformal and intensity modulated radiotherapy, the radiotherapy is being more extensively applied in the comprehensive treatment of gastric cancer.
