2.Clinical research on application of adjustable sutures in glaucoma filtering operation

AIM:To discuss the application effect of adjustable sutures in glaucoma filtering operation after trabecular resection.
METHODS: Seventy-eight cases ( 101 eyes ) suffered from glaucoma were randomly divided into two groups, observation group and control group. Thirty-nine cases ( 51 eyes ) in the observation group underwent trabeculectomy with adjustable sutures, the control group (39 cases, 50 eyes) only adopted trabeculectomy.RESULTS: Compared Preoperative IOP in two groups, the difference was not statistically significant (P>0. 05). After 6mo, IOP were decreased compared with preoperative in two groups, and that in observation group was lower than control group, the difference was statistically significant ( P < 0. 05 ). Six months after operation, there were 1 eye with shallow anterior chamber Ⅰ and 2 eyes with non- functional bleb in observation group, and the complication rate was 5. 9%. While there were of 6 eyes with shallow anterior chamber, in which 4 eyes at gradeⅠ, each one at gradeII and Ⅲ. Non-functional blebs in 5 eyes and a scleral flap adhesion complications rate in the control group was 24. 0%, significantly higher than that in the observation group, the difference was statistically significant ( P<0.01).
CONCLUSION: The adjustable sutures combined with trabeculectomy for glaucoma can significantly reduce the postoperative complications. The curative effect is exact and clinically applicable.
3.Clinical therapeutic effect of rehabilitative therapy on dysphagia after cerebral stroke

Objective:To explore the clinical therapeutic effect of comprehensive rehabilitative treatment on dyspha-gia after acute cerebral stroke .Methods :A total of 78 patients with dysphagia after acute cerebral stroke ,who visi-ted to our hospital from Jun 2011 to Jun 2012 ,were selected ,randomly and equally divided into Vitalstim electrical stimulus plus swallowing training group (rehabilitation treatment group ) and traditional swallowing training group (routine training group) .Patients received Kubota's water drinking test before and after treatment .Therapeutic effects were observed ,compared and statistically analyzed .Results:Total effective rate of rehabilitation treatment group was significantly higher than that of routine training group (92.3% vs .74.3% ,χ2 = 4.191 , P= 0.041 );mean declining amplitude of swallowing function score in rehabilitation group was significantly higher than that of routine training group [ (2.0 ± 0.5) scores vs .(1.1 ± 0.6) scores , t=5.284 , P=0.032] .Conclusion:Comprehen-sive rehabilitation training can significantly improve dysphagia caused by cerebral stroke .

Epigenetics has become a hot topic in biomedical research in last a few years.Epigenetics refers to as epigenome,and it is interpreted as epigenetic changes (DNA methylation,histone modifications and non-coding RNAs such as microRNAs) in the regulation of genes expression on chromatin-based pathways rather than a single gene.Epigenetic factors such as DNA methylation,histone modification and miRNA serve as an interface for environmental stimuli and interacts each other to regulate gene expression and control phenotypes of cell.All of those epigenetic factors are essential in the keeping homeostasis of human body and contribute to many biologic conditions.In addition,the application of epigenetic drugs has been come into clinical trials especially in the treatment of cancers,therefore the understanding the mechanism of epigenetics in the development of human diseases and the therapeutic potential of epigenetic regulators in the treatment of human disease will bring us into the epigenetics era in biomedical research.
6.Risk factors of contrast-induced nephropathy in the elderly patients with diabetes

Objective To understand the clinical feature of the elderly patients with diabetes during coronary angiography,and analyze the risk factors of contrast-induced nephropathy (CIN).Methods The clinical data of 269 elderly patients who had undergone coronary angiography and percutaneous coronary intervention(PCI) from January 2007 to December 2009 in our hospital were analyzed retrospectively.The patients were divided into two groups:CIN group and non-CIN group.The possible risk factors for CIN,such as glycemic control,diabetic complication,renal function,volume of contrast medium,inflammatory state,therapy of perioperative period,past medical history were analyzed and compared between the two groups. Results In 269 elderly patients with diabetes,the incidence of CIN was 9.3 % (25/269).According to estimated glomerular filtration rate (e-GFR),the patients were divided into four subgroup:≥90 ml/min,89-60 ml/min,59-30 ml/min,<29 ml/min.The incidences of CIN for the subgroups were 2.2%(1/45),4.4%(6/135),17.3%(14/81) and 50 % (4/8),respectively.Multivariate logistic gradual regressive analysis showed that loop diuretic use (OR> 6.07),preoperative e-GFR(<60 ml/min) (OR>3.27),volume of contrast medium (≥200ml) (OR>3.26),chronic kidney disease(CKD) (OR>2.80) (P=0.001,0.024,0.015,0.048) were indepen-dent risk factors for CIN (P<0.05). Conclusions Loop diuretic use,preoperative GFR (<60 ml/min),volume of contrast medium (≥200 ml) and CKD are independent risk factors of CIN.
7.Research progress of polyamidoamine dendrimer in targeting drug delivery system.

Targeting drug delivery system (TDDS) is one of the most concerned research fields in cancer treatment because it can bind selectively and react with the target diseased sites at the cellular or sub-cellular level, making distribution and release of drugs in a controlled manner, thus enhance therapeutic effects and reduce toxic and side-effects on normal cells. Polyamidoamine dendrimer (PAMAMD) is a kind of newly developed polymer in nanometer degree. Hyper-branched, monodispersity, three-dimensional structure and host-guest entrapment ability make it used as drug carrier, gene delivery system and imaging agent. Various targeting ligands, which have high affinity to specific organs, tissues or cells in human body, can be linked to surface functional groups of PAMAMD. And drugs and theoretical gene are carried by encapsulation or chemical conjugation. Finally, PAMAMD targeting drug delivery system can carry drugs and theoretical gene to diseased sites and then release them for targeted therapy. The PAMAMD-based conjugates have small size, ligh permeability and retention effect (EPR), low toxicity and so on. The research progress of PAMAMD modified by different ligands in targeting drug delivery system is reviewed, and research direction of the PAMAMD targeting delivery system in the future is also suggested.
8.Research progress of the relationship between choroidal thickness and idiopathic macular hole

Idiopathic macular hole is a full - thickness defect of retinal tissue involving the anatomic fovea and affecting central visual acuity and quality of life in elder patients. Recent evidence showed that the alterations of choroidal blood flow and choroidal thickness are associated with the formation of macular holes. Enhanced depth imaging spectral-domain optical coherence tomography (EDI SD-OCT) enables in vivo measurement of choroidal thickness and may provide new insight into the understanding of pathogenesis of idiopathic macular hole. In this article, we reviewed current studies on the relationship between choroidal thickness measured by optical coherence tomography and the pathogenesis of idiopathic macular hole.

Objective To study the effects of Danlong Xingnao Tablet on neuronal apoptosis and expression of TNF-? after focal cerebral ischemia reperfusion in rat hippocampus. Methods The model of middle cerebral arteries occlusion (MCAO) were performed with intraluminal filament blockade. 10.Antitumor efficacy of lymphocytes from hepatic porta lymph node stimulated by staphylococcal enterotoxin A

ObjectiveTo evaluate the antitumor effects of lymphocytes from hepatic porta lymph node co cultured with staphylococcal enterotoxin A(SEA)?MethodsLymphocytes isolated from lymph node of 5 HCC patients were co cultured with SEA. The proliferation and cytotoxicity of the lymphocytes were observed. Flow cytometer was used to detect the expression of CD3?CD4 and CD8 on the surface of the lymphocytes. TNF ? and IFN ? released into the culture medium were measured by the method of ELISA. The results were compared with that yielded from TILs of the individual patients. ResultsCultured in the presence of SEA, there was a sharp rise of T cells, especially CD8 + cells. Cytotoxic activity was increased. TNF ? and IFN ? were predominantly produced in the first week.ConclusionsLymphocytes from lymph node activated by SEA were armed with an increased antitumor efficacy.
