Objective To elucidate the possibility of diagnosing BOO with urodynamic data with indefinite P F findings in patients with BPH. Methods Some urodynamic data besides the P F fin dings were studied in BPH patients,and these were compared between the BOO group and the non BOO group. 2.Complications of Visceral and Vascular Injury in Laparoscopic Surgery for Gynecologic Diseases

Objective To study the incidence of complications of visceral and vascular injury in laparoscopic surgery for gynecologic diseases,and to discuss the ways to decrease the incidence. Methods The data of 2684 patients who received laparoscopic surgery from Januray 2003 to December 2005 in the Department of Obstetrics and Gynecology were reviewed retrospectively.The incidence and treatment of complications of visceral and vascular injury were observed. Results The total incidence rate of complications was 2.53%(n=68),and that of the visceral and vascular injury was 0.37%(n=10).Four cases of injury were related with trocar punctures(injury of omental blood vessel,n=2;postperitoneal vessel,n=2),three took place during the operation (severe bleeding,n=1;bladder injury,n=2),and the other three were observed in the postoperative stage(ureter injury,n=2;intestinal fistula,n=1). Conclusion The complications in laparoscopic surgery for gynecologic diseases are increased with the extension and difficulty of operation,and are closely related with the experience of the surgeons.Proper candidates for the surgery and established operative technique are the key factors in decreasing the incidence.
3.Study on Early Changes of Myocardial Combination Enzyme in Dogs with Gunshot Wound in Seawater.

Objective To investigate the changes of myocardial combination enzyme in the early stages of gunshot wound of dogs in seawater for exploring early treatment protocol. Methods Fourteen conventional Beagles models undergoing gunshot wound in seawater were used along with another 3 dogs receiving the wound on land to serve as control. After the dogs were rescued from the seawater, the origin of blood coming from central venous pressure tube was measured (33.75?25.29) min before and after injured for indexes such as alkaline aminotransferase (ALT) , aspartate aminotransferase (AST) , creatine kinase (CK) , creatine kinase isoenzymes - MB (CK -MB) , lactate dehydrogenase (LDH) and adenosine monophosphate (AMY) in the early stages of wound, followed by statistical analysis of the results. Results There was an increase in blood CK (P0.05). As compared with the control, the experimental group in seawater was observed to have significant difference in blood AMY (P 0.05). But it was noted that CK, CK - MB , LDH , AST and ALT contents tended to increase. Conclusion The functions of respiratory, heart, liver and kidney were injured and restrained in Beagle dogs undergoing the firearms wound in natural seawater. The immediate diagnosis and treatment of the organism as well as the out - of - wound - tract injury shoud do while the firearms primal wound tract were treated in the early stages of gunshot wound.
4.Therapeutic effect of the co-administrated metformin and rosiglitazone in treatment of type 2 diabetes mellitus

Objective To study the therapeutic effect of co-administrated metformin and rosiglitazone in the treatment of type 2 diabetes mellitus(T2DM).Methods 53 patients with T2DM were divided into 2 groups in random:co-administrated glipizide and metformin(group A)and co-administrated metformin and rosiglitazone(group B).All cases were treated for 6 months.Blood pressure(BP),blood glucose(BG),body mass index(BMI),hemoglobin A1c(HbA1c),insulin,and blood lipid were determined and the HOMA-insulin resistance index(HOMA-IR)was calculated.Results Both treatments were effective on hyperglucose,hypertension,hyperlipidemia and high BMI(P<0.05-0.01),while group B could lower the level of insulin(P<0.05)and HOMA-IR(P<0.01).Conclusions Treatment of co-administrated metformin and rosiglitazone can improve the abnormal carbohydrate and lipid metabolism,higher levels of BP and BMI,and ameliorate insulin resistance.
5.Clinical Observation on General Anxiety Disorder Treated by Acupuncture plus Herbal Medicine

To investigate the efficacy of the formula composed of the herbal drugs to soothe the liver, regulate qi, clarify the heart and eliminate vexation plus acupuncture for general anxiety disorder. Methods: Forty cases in the treatment group were treated with acupuncture plus herbal formula and 38 cases in the control group were treated with oral Doxepin. The assessment was conducted by Self-rating Anxiety Scale (SAS) and the Treatment Emergent Symptom Scale (TESS) before, during and after treatment. Results: The total effective rate was respectively 82.50% in the treatment group and 84.21% in the control group, with a significant difference in SAS assessment (P<0.01) between the two groups before and after the treatment.There were no significant differences in the curative and remarkable effective rate, total effective rate and SAS assessment between the two groups (P>0.05), but the side effect was higher in the control group than in the treatment group (P<0.01). Conclusion: Acupuncture plus herbal formula can have a precise effective effect for extensive anxiety neurosis with mild toxic side effects.
6.Protective effect of naloxone on mitochondrial membranal potential of hypoxic myocardial cells and apoptosis

BACKGROUND: Studies confirm that ischemia/reperfusion (I/R) injury can induce myocardial apoptosis. The loss of mitochondrial membrane potential (MMP) after reperfusion is the inevitable pathway of apoptosis. Protection of MMP may reduce apoptosis.OBJECTIVE: To observe the effect of naloxone on MMP of hypoxic myocardial cells and apoptosis in neonatal rats, and investigate the protective effect of naloxone on hypoxic myocardial cells.DESIGN: Observation and controlled trial.SETTING: Department of Cardiology, the General Hospital of Chinese PLA.MATERIALS: Detection of apoptosis of myocardial cells was carried out in the Laboratory of Pathophysiology, General Hospital of Chinese PLA in December 2004. Ten neonatal rats were used, and detection of MMP of myocardial cells was carried out in the same laboratory in March 2006 and 20 rats were used. All the involved rats were provided by the Animal Center of the General Hospital of Chinese PLA on the day of birth, were involved in this trial. Reagents: Naloxone hydrochloride injection (0.4 g/L, Beijing Sihuan Pharmaceutical Factory, Batch No. 0206272); the lowest and essential medium (Dulbecco, DEME,GIBCO company); phosphate buffer solution and fetal bovine serum (PBS and FBS, SIGMA Company).METHODS: The neonate rats were cut open along the median line of chest bone after local sterilization with iodine tincture on the day of birth. 1/3 ventricular myocardium before cardiac apex was harvested. On the 4th day of the culture,culture flasks of cells in good growth status ( > 106 cells/bottle) were selected and divided into 3 groups: control group (normal culture, n =3 bottles), hypoxia group (hypoxia/reoxygenation, n =15 bottles) and naloxone group (hypoxia/reoxygenation, and treated by naloxone, n =15 bottles). Three time points were set in hypoxia group and naloxone group according to different time of hypoxia and reoxygenation: hypoxia 2 hours/reoxygenation 0 hour; hypoxia 2 hours/reoxygenation 2 hours; hypoxia 2 hours/reoxygenation 4 hours, 5 bottles at each time point. In the hypoxia group, DEME medium, which was pre-filled with 0.95 volume fraction of N2 and 0.05 volume fraction of CO2, containing 0.01 volume fraction of FBS, was used and 0.95 volume fraction of N2 and 0.05 volume fraction of CO2 was also filled to replace the air in the culture flasks. The culture flasks were enveloped for incubation at 37 ℃. The cells in the hypoxia group were incubated at normal condition (0.95 volume fraction of air and 0.05 volume fraction of CO2) at set time. In the naxolone group, hypoxia/reoxygenation treatment was the same as above, and naloxone hydrochloride was added at the sametime and the final concentration of naloxone in culture flasks was 5 μmol/L (The volume for naloxone ≤ 0.5% of the total medium). In the control group, hypoxia/reoxygenation and naloxone treatment were not given, but the same volume of normal saline was added, and this time served as time point of hypoxia 0 hour/reoxygenation 0 hour. After intervention,myocardial MMP changes and apoptosis were detected with fluorescent staining-flow cytometer at different time after hypoxia/reoxygenation.MAIN OUTCOME MEASURES: ① MMP changes of hypoxia group and naloxone group at different time points; ② Comparison of survival, apoptosis and necrosis of cells at hypoxia 2 hours/reoxygenation 4 hours in each group.RESULTS: ① After hypoxia, MMPs of the cells in hypoxia group and naloxone group were decreased. MMP of the cells in the naloxone group was higher than that in the hypoxia group at each time point (P < 0.01). The great amplitude of decrease of MMP occurred in hypoxia period, but not in reoxygenation period. ② At hypoxia 2 hours/reoxygenation 4 hours, the apoptotic and necrotic rates in the hypoxia group were significantly higher than those in the naloxone group [(9.88±0.98)% vs. (2.41±0.52)%; (5.10±0.29)% vs. (3.56±0.56)%, both P < 0.01]. The apoptotic rate was significantly higher than the necrotic rate in the hypoxia group (P < 0.05).CONCLUSION: Early application of naloxone can significantly alleviate and postpone the decrease of myocardial MMP after I/R, and reduce apoptosis and necrosis.
7.Expression of liver X receptor in the liver tissues of rat with multiple organ dysfunction syndrome induced by wound infection

Objective To study the expression of liver X receptor (LXR) during the progress of multiple organ dysfunction syndrome (MODS) caused by wound infection, and its role in the lipid metabolism during MODS. Method The MODS models caused by wound infection were produced. One hundred Wistar rats were randomly divided into wound group (group T, n = 30) and MODS caused by wound infection group (group M, n = SO). The closed multi-fracture and extensive soft-tissue injury was produced in rats using jaw. After 12 hours, a thirty percent total body surface area Ⅲ (TBSA Ⅲ ) bum was induced in group M,and the rats were contaminated by pseudomonas aeruginosa. The levels of endotoxin ( LPS), alanine transaminase (ALT), free fatty acid ( FFA), TG, HDL, VLDL were determined before injuries and post injuries 24, 48, 96, 120 hours, and at each time point in each group, only 10 rats were determined. And at the same tune, real-time quantitative reverse transcription polymerase chain reaction technique was used to measure the gene expression of LXR. Chi-square test was used. Results The blood ALT and LPS levels in rats were increased slowly, the levels of FFA reached peak at 96 hours post injuries (P < 0.05), the level of TG increased first and then decreased to the lowest at 96 hours, HDL dropped slowly, and VLDL decreased first and then increased. The gene expression of LXR increased slightly at 24 hours and then gradually decreased in group T, and the gene expression of LXR decreased more obviously in group M, and there was significant difference compared with group T. Conclusions ALT increased slowly, and was positively correlated with IPS post injuries. This indicated that infection aggravated the liver injury. The gene expression of LXRα increased in the early stage and then decreased in the late stage, LPS and LXRα were negatively correlated, which may cause the change of LXRα. The decreased expression of LXRα during MODS may contribute to dysfunction of cholesterol transport and fatty acid synthesis.
8.Application of island flap pedicled with dorsal cutaneous branches of digital internal artery for severe thermos injury of fingers

Objective To investigate the therapeutic effect of island flap pedicled with dorsal cutaneous branches of digital inter-nal artery for severe thermos injury of fingers .Methods From May 2008 to May 2012 ,15 fingers in 12 cases with severe thermos injury were treated with island flap pedicled with dorsal cutaneous branches of digital internal artery .The defect size ranged from 1 .5 cm × 2 .0 cm to 2 .2 cm × 3 .0 cm ,4 fingers with anterograde flap repair ,11 fingers with retrograde flap repair .Results Partial necrosis happened at the distal end of 2 island flaps ,All the other flaps survived completely .The patients were followed up for 6-12 months with satisfactory cosmetic and functional results ,cicatricial contracture around the skin graft in 3 fingers ,The two-points discrimination distance was 9-11 mm in flaps .Conclusion These flaps have the advantages of an extended skin paddle and a versa-tile pivot point on the phalanx ,and they allow coverage of wide and distal defects .
9.Analysis of skin microbiota diversity in patients with pemphigus based on 16S rDNA sequences

Objective:To analyze the skin microbiota diversity in patients with pemphigus vulgaris (PV) using 16S rDNA sequencing.Methods:Ten patients with PV and 10 healthy controls were collected from the Department of Dermatology, the Third People′s Hospital of Hangzhou. Skin swabs were collected from perilesional skin (PV group) and contralateral non-lesional skin (PVn group) of the patients with PV, as well as from the corresponding body sites of the healthy controls (normal control group) . The 16S rDNA amplicon sequencing technology was used for gene sequencing and classification in all microbiota samples, and Usearch software for data cluster analysis to obtain operational taxonomic units (OTUs) and assess species abundance at the phylum, class, order, family and genus levels. Observed species index, Shannon index and Simpson index were used to estimate α diversity, and principal coordinate analysis (PCoA) was performed to analyze β diversity. Linear discriminant analysis effect size (LEfSe) analysis was conducted to identify differentially abundant species in each group. PICRUSt software was used for gene function prediction. Wilcoxon rank sum test was used as nonparametric test for comparisons between 2 groups, and Kruskal Waills test as nonparametric test for comparisons among 3 groups.Results:There were 2 002, 1 869, 1 751, 1 611 and 1 120 OTUs at phylum, class, order, family and genus levels respectively. Cluster analysis showed that skin microbiome in the 3 groups mainly consisted of Firmicutes, Actinobacteria, Bacteroidetes and Proteobacteria at the phylum level. At the genus level, Staphylococcus was the most abundant in the PV group and PVn group, and Corynebacterium was the most abundant in the normal control group. The observed species index, Shannon index and Simpson index all significantly differed among the 3 groups (all P< 0.05) , and the Shannon index and Simpson index were significantly lower in the PV group (3.24±1.30, 0.70±0.19, respectively) than in the normal control group ( P< 0.05) . PCoA analysis showed no significant difference in β diversity among the 3 groups ( P=0.054) . Rank sum test showed that the abundance of 32 species significantly differed among the 3 groups ( P< 0.05) . Among them, high relative abundance was observed in the class Bacilli enriched in the PV group, as well as the genera Micrococcus and Brevundimonas enriched in the normal control group. According to the disease duration, the patients with PV were divided into long-course PV group with disease duration of ≥ 3 months, and short-course PV group with disease duration of < 3 months. Clostridiales, Oscillibacter, Sphingomonas were enriched in the long-course PV group, and Gammaproteobacteria was enriched in the short-course PV group. Gene function prediction analysis showed that the genes related to infectious diseases were enriched in the pemphigus group. Conclusion:The 16S rDNA-based microbiota profiling suggested differences in the diversity and composition of skin microbiota between patients with PV and healthy individuals.
10.Clinical Analysis of Intrauterine Occupational Disease in 71 Cases

Objective To diagnose the intrauterine occupational disease by vaginal ultrasonography or hysteroscopy, and to evaluate the diagnostic values of these two methods. Methods One hundred and fifty patients were detected by vaginal ultrasonography and hysteroscopy from Jan 2004 to Dec 2004, and a total of 71 cases were confirmed intrauterine occupational disease. Tissue hysteroscopy or bioposy were performed during the hysterosocopy in order to analyse the tissue pathology. Results In these 71 patients, 47 were no less than 46 years old. Most of them had abnormal uterine bleeding.Thirty-three cases (46.48%) were suggested by vaginal ultrasonography, and 49 (69.01%) by hysteroscopy. Thirty-eight cases (53.52%) were in line with the pathological result.There were significant differences between vaginal ulterasonography and hysteroscopy in diagnosing the intrauterine occupational disease (P
