2.Clinical significance of the variation of the serum inflammatory factor in the elder patients with femoral neck fracture during perioperative period

Objective To study the clinical significance and change law of the serum inflammatory factor in the elder patients with femoral neck fracture during perioperative period. Methods 62 patients with femoral neck fracture were selected as research object,and the serum IL-1,IL-6,IL-10,TNF-α,CRP and BGP of the patients at preoperative day and after the treatment at the first,third,seventh and fourteenth day were detected with ELISA. Results The serum IL-1 ,IL-6,IL-10,TNF-α and CRP went up first and then down,the levels were highest at the third day, and they were compared to the levels at preoperative day( t = 3.34,3.53,3.44,3.69, all P < 0. 05 ), the levels at the fourteenth day returned to normal levels,but the serum BGP showed a rising trend,the level was highest at the fourteenth day, and it was compared to the level at preoperative day( t = 2. 87, P < 0. 05 ). Conclusion The changes of the serum inflammatory factor in the elder patients with femoral neck fracture during periopearative period showed regular pattern, and it was meaningful for understanding the developing and prognosis.
3.Mediation of norepinephrinergic nervous systemcn coldstress-induced suppression of natural killer cytotoxicity

Aim To investigate the role of locus coeruleus(LC)-norepinephrinergic system and sympathetic nervous system in immunosuppression under cold stress, using 6-hydroxydopamine(6-OHDA) as chemical sympathectomy.Methods Rats were maintained in cold chamber at 4℃ for 4 h.The 51Cr release assay from YAC-1 cells was used to determine the splenic NK cell activity, the double staining of ABC method was employed to observe the immunoreactive expression of Fos, arginine-vasopressin(AVP), oxytocin(OT) and tyrosine hydroxylase(TH), and conventional radioimmunoassay was used to measure plasma corticosterone (CORT) concentrations.Results Central sympathectomy with intracerebroventricular(i.c.v.) injection of 6-OHDA r educed significantly the elevation of plasma corticosterone level, the expressio n of Fos in hypothalamic paraventricular nucleus(PVN) and in locus coeruleus(LC ), as well as the suppression of NK activity induced by cold stress at 4℃ for 4 h. Peripheral sympathectomy with intraperitoneal (i.p.) injection of 6-OHDA al so reversed the cold stress-induced suppression of NK cytotoxicity, but without significant effect on Fos expression in brain. Double staining showed that amon g the Fos-positive neurons in PVN only a few co-expressed Fos and AVP or Fos a nd OT, while in LC the majority of Fos-positive neurons co-expressed Fos and T H.Conclusion The mechanism of suppression of NK activity induce d by cold stress may be mediated through HPA axis activated partially by central LC-norepinephrinergic system and through the peripheral sympathetic nervous system.
4.RATES OF ANTEROGRADE-AND RETROGRADE-TRANSPORT AND DURATION OF PRESERVATION OF HRP IN THE CENTRAL NERVOUS SYSTEM

It has been found by Ju('81)that HRP could be anterogradely transported forlong distances.Based on this fact the rate of anterograde transport was investigatedin the present study,and,at the same time,the rate of retrograde transport and thetime of disappearance of HRP from the labeled sites were observed.Twenty tworabbits,weighing 2~2.2 kg,were used.The HRP was injected into the lower lumbarspinal cord unilaterally.The animals were sacrificed after 12 hours to 7 days andwere processed with benzidine and o-dianisidine.The anterogade labeling in the dorsalaccessory olivary nucleus and the retrograde labeling in the red nucleus and thenucleus raphe pallidus were chosen for study.It was quite unexpected to find that the anterograde and retrograde labelingsparalled each other in time as well as in intensity.They appeared at the same timeand waxed and waned practically in full accord.Labeling of the brainstem firstappeared at 18 hour's survival,reached its peak at the 2nd day,and then graduallyfaded away till at the 7th day when only traces of labeling remained.In the casewhich showed the earliest labeling the distance between the injection site and theobex,which was at about the same level as the most densely labeled parts of thedorsal accessory olivary nucleus and nucleus raphe pallidus,was measured to be 265mm and the rate of transport,the same for anterograde and retrograde transports,was calculated to be more than 350mm per day.Counting from the time of theearliest labeling,HRP was found to remain at the labeled site for as long as 5~6 days.The period of net accumulation of HRP at the labeled sites was about 1 day.Thus,the best survival period would be roughly the time required for HRP to travel thefiber tract plus 1 day.Neurons of the red nucleus and the nucleus raphe pallidus differ greatly intheir morphology,physiology as well as biochemistry.The results in them however,were similar.This,in connection with observations on other nuclei,both anterogr-adely and retrogradely labeled,suggests that the rate of transport and the duration ofpreservation of HRP in the central nervous system are basically the same in differentneuronal systems.
5.Efficacy of external fixators in treatment of unstable pelvic fractures

Objective To investigate the efficacy of external fixators in the treatment of unstable pelvic fractures( Tile B and Tile C ). Methods The different results of 78 patients who had been treated with or without external fixator were compared. Results In 38 cases who were treated without external fixator, the cure rate for hemorrhagic shock was 76%, the mortality 10.6%, and the average ISS score 11.6. In 40 cases who were treated with external fixator, the cure rate for hemorrhagic shock was 90%, the mortality 2.5%, and the average ISS score 9.8. Conclusion The treatment of unstable pelvic fracture with external fixator is simple and reliable, and can reduce the mortality significantly.

Objective To analyze of the relationship between level of anxiety and depression before surgery and anus exhausting time for patients after gynecological abdominal surgery.Methods A descriptive study was conducted with a survey of 100 cases of patients with gynecological abdominal surgery where the level of anxiety and depression before surgery and post-anus exhausting time were analyzed to explore the correlation between them.Results In the 88 valid questionnaires,the incidences of stress and anxiety before surgery were respectively 38.64%(34/84) and 19.32%(17/88) and the average anus exhausting time after surgery was (44.55 ± 13.90) hours;the anus exhausting time of anxious patients was significantly longer than that of non-anxious patients [(48.35±12.84) hours vs.(42.50±14.13) hours,t=-2.12,P<0.05] and bivariate Logistic regression analysis suggested that preoperative level of anxiety had a significant effect on anus exhausting time after surgery.Conclusions Stress,anxiety and other negative emotions can affect anus exhausting time after surgery and level of anxiety could be used as one predictor of the delay of anus exhausting time after surgery.

This study examined the effects of TRAIL-endostatin-based gene-radiotherapy on cellular growth, apoptosis and cell cycle progression in human vascular endothelial cells ECV304 in vitro. The expression of TRAIL and endostatin protein in ECV304 cells was detected by ELISA after the transfection of recombinant plasmid pshuttle-Egr1-shTRAIL-shES and X-ray irradiation. Then MTT assay was used for determining the cellular proliferation, and flow cytometry (FCM) plus Annexin V and propidium iodide (PI) double-staining or PI single-staining were employed for the detection of apoptosis and cell cycle progression. The results showed that expression of TRAIL and endostatin protein exhibited a time- and dose-dependent change in ECV304 cells after pshuttle-Egr1-shTRAIL-shES transfection in conjunction with irradiation. In the TRAIL-endostatin-based single- or double-gene-radiotherapy, the cell viability declined in a time- and dose-dependent manner, the percentage of cells at G(2)/M phase and apoptotic rate was increased, and the percentage of cells at G(0)/G(1) phase was lowered as compared with those receiving radiotherapy alone. Moreover, TRAIL-endostatin-based double-gene-radiotherapy demonstrated better effects on growth inhibition, promotion of apoptosis and induction of cell cycle arrest in ECV304 cells than single-gene-radiotherapy.
10.Effect of N-acetylcysteine on the liver function and mRNA and protein expressions of NF-KB in brain-dead BA-Ma mini pigs

Objective To investigate the effects of N-acetylcysteine (NAC) pretreatment on the liver function and mRNA and protein expressions of nuclear factor-KB (NF-KB) in brain-dead BA-Ma mini pigs. Methods The brain-dead model was established by increasing intracranial pressure by a modi-fied, slow and intermittent way. A total of 15 BA-Ma mini pigs were randomly and equally divided into three groups (five in each group), ie, control group (Group C) : treated only with opening and closing abdomen after anesthesia; group without NAC treatment (group B): brain-dead models without use of NAC; NAC treatment group (Group N): 1 and 12 hours after establishment of brain-dead models, 200 mg/kg NAC was added into 100 ml normal saline and intravenously transfused. Levels of ALT and AST in serum as well as TNF-α, IL-1β and IL-6 were determined at 3,6,12, 18,24 hours after brain death. The changes of liver tissues were observed by HE staining under a light microscope, the uhrastruc-rural changes of liver tissues observed under electron microscope, the expression of NF-KB detected by immnohistochemistry and change of NF-KB mRNA by RT-PCR. Results (1) Compared with Group C, serum ALT and AST began to increase at 12 hours after brain death, but IL-1β, IL-6 and TNF-α be-gan to increase three hours after brain death in Groups B and N. mRNA and protein expressions of NF-KB in Groups B and N began to increase six hours after brain death, when Group B increased more sharply than Group N, with statistical difference (P<0.05). (2) At 12 hours after brain death, injury of liver cells in Group B was severer than that in Group N. Conclusion NAC can inhibit the mRNA and pro-tein expressions of NF-KB, decrease the release of inflammatory factors and hence protect the hepatic structure and function during brain death.
